1Characterization of functional biliary pain and dyspeptic symptoms in patients with sphincter of Oddi dysfunction:Effect of papillotomy显示文摘AIM: To characterize functional biliary pain and other gastrointestinal (GI) symptoms in postcholecystectomy syndrome (PCS) patients with and without sphincter of Oddi dysfunction (SOD) proved by endoscopic sphincter of Oddi manometry (ESOM), and to assess the post- endoscopic sphincterotomy (EST) outcome. METHODS: We prospectively investigated 85 cholecystectomized patients referred for ERCP because of PCS and suspected SOD. On admission, all patients completed our questionnaire. Physical examination, laboratory tests, abdominal ultrasound, quantitative hepatobiliary scintigraphy (QHBS), and ERCP were performed in all patients. Based on clinical and ERCP findings 15 patients had unexpected bile duct stone disease and 15 patients had SOD biliary typeⅠ. ESOM demonstrated an elevated basal pressure in 25 patients with SOD biliary-type Ⅲ. In the remaining 30 cholecystectomized patients without SOD, the liver function tests, ERCP, QHBS and ESOM were all normal. As a control group, 30 ‘asymptomatic’ cholecystectomized volunteers (attended to our hospital for general cardiovascular screening) completed our questionnaire, which is consisted of 50 separate questions on GI symptoms and abdominal pain characteristics. Severity of the abdominal pain (frequency and intensity) was assessed with a visual analogue scale (VAS). In 40 of 80 patients having definite SOD (i.e. patients with SOD biliary typeⅠand those with elevated SO basal pressure on ESOM), an EST was performed just after ERCP. In these patients repeated questionnaires were filled at each follow-up visit (at 3 and 6 mo) and a second lookQHBS was performed 3 mo after the EST to assess the functional response to EST. RESULTS: The analysis of characteristics of the abdominal pain demonstrated that patients with common bile duct stone and definite SOD had a significantly higher score of symptomatic agreement with previously determined biliary-like pain features than patient groups of PCS without SOD and controls. In contrary, no significant differences were found when the pain severity scores were compared in different groups of PCS patients. In patients with definite SOD, EST induced a significant acceleration of the transpapillary bile flow; and based on the comparison of VASs obtained from the pre- and post-EST questionnaires, the severity scores of abdominal pain were significantly improved, however, only 15 of 35 (43%) patients became completely pain free. Post-EST severity of abdominal pain by VASs was significantly higher in patients with predominant dyspepsia at initial presentation as compared to those without dyspeptic symptoms. 2Variation of head and facial morphological characteristics with increased age of Han in Southern China显示文摘We investigated 13940 (6735 male, 7250 female) adult head and facial physical attributes from 19 different Han ethnic groups in 10 southern-China provinces, and calculated 12 head and facial indexes. Indexes were used to analyze the variation of head and facial morphological characteristics with increased age. Results showed that as age increases: (1) Head breadth, minimum frontal breadth, face breadth, interocular breadth, external biocular breadth, lip height, lip thickness, head circumference, auricular height, length-breadth head index, length-height head index, and lip-index values decline significantly in a linear fashion. 3Critical slope gradient for compulsory abandonment of farmland on the hilly Loess Plateau显示文摘Scientific evidence for the upper limit of farmland is proposed. Shallow gully erosion is one of erosion types distributed extensively on sloping farmland in the hilly regions of the Loess Plateau. Field observation and aerial photos interpretation, as well as laboratory experiments show that the shallow gully erosion occurring on the steeper farmland in the hilly regions of the Loess Plateau is an important factor leading to intensive erosion on slope, because of its extensive distribution and intensive runoff collection. The data on the formation, development and distribution of shallow gullies on sloping farmland indicate that critical slope gradient for shallow gully initiation ranges from 15 to 20 degrees with an average of 18.2 degrees. Therefore, it is suggested that critical slope gradient for compulsory abandonment of farmland on the hilly Loess Plateau should be kept within the critical slope gradient for shallow gully initiation to prevent shallow gully formation in order to control soil loss more effectively. 4Field test of best management practice pollutant removal efficiencies in Shenzhen,China显示文摘This paper presents a study on the use of best management practices(BMPs)for controlling nonpoint pollution in the Xikeng Reservoir watershed located in Shenzhen,China.A BMP treatment train design,including a pond,a wetland,and a buffer strip placed in series was implemented at the reservoir location.A separate grass swale was also constructed at the site.Low impact development(LID)BMPs,namely a planter box and bioboxes,were used at the parking lot of the reservoir’s Administration Building.Samples were collected during storm events and were analyzed for total suspended solids(TSS),biochemical oxygen demand(BOD5),ammonia nitrogen(NH3-N),and total phosphorus(TP).The removal efficiencies of both BMP systems were evaluated using the Efficiency Ratio(ER)method based on the event mean concentration(EMC)data.In summary,the pond/wetland treatment train removed 70%–90%of TSS,20%–50%of BOD5,and 30%–70%of TP and NH3-N.The swale removed 50%–90%of TSS,30%–55%of BOD5,–10%–35%of NH3-N,and 25%–70%of TP.For the planter box and biobox,the ranges of removal rates were 70%–90%,20%–50%,and 30%–70%for TSS,BOD5,and ammonia and phosphorus,respectively.Ru ZHANG Wenbin ZHOU Richard FIELD Anthony TAFURI Shaw LYU Keli JIN 2009Frontiers of Environmental Science & Engineering2009,3,3:12
5Endoscopic transluminal pancreatic necrosectomy using a self-expanding metal stent and high-flow water-jet system显示文摘Walled-off pancreatic necrosis and a pancreatic abscess are the most severe complications of acute pancreatitis. Surgery in such critically ill patients is often associated with significant morbidity and mortality within the first few weeks after the onset of symptoms. Minimal invasive approaches with high success and low mortality rates are therefore of considerable interest. Endoscopic therapy has the potential to offer safe and effective alternative treatment. We report here on 3 consecutive patients with infected walled-off pancreatic necrosis and 1 patient with a pancreatic abscess who underwent direct endoscopic necrosectomy 19-21 d after the onset of acute pancreatitis. The infected pancreatic necrosis or abscess was punctured transluminally with a cystostome and, after balloon dilatation, a non-covered self-expanding biliary metal stent was placed into the necrotic cavity. Following stent deployment, a nasobiliary pigtail catheter was placed into the cavity to ensure continuous irrigation. After 5-7 d, the metal stent was removed endoscopically and the necrotic cavity was entered with a therapeutic gastroscope. Endoscopic debridement was performed via the simultaneous application of a high-flow water-jet system; using a flush knife, a Dormia basket, and hot biopsy forceps. The transluminal endotherapy was repeated 2-5 times daily during the next 10 d. Supportive care included parenteral antibiotics and jejunal feeding. All patients improved dramatically and with resolution of their septic conditions; 3 patients were completely cured without any further complications or the need for surgery. One patient died from a complication of prolonged ventilation severe bilateral pneumonia, not related to the endoscopic procedure. No procedure related complications were observed. 6Relation of the IGF/IGF1R system to autophagy in colitis and colorectal cancer显示文摘Metabolic syndrome(Met S), as a chronic inflammatory disorder has a potential role in the development of inflammatory and cancerous complications of the colonic tissue. The interaction of DNA damage and inflammation is affected by the insulin-like growth factor 1 receptor(IGF1 R) signaling pathway. The IGF1 R pathway has been reported to regulate autophagy, as well, but sometimes through a bidirectional context. Targeting the IGF1 R-autophagy crosstalk could represent a promising strategy for the development of new antiinflammatory and anticancer therapies, and may help for subjects suffering from Met S who are at increased risk of colorectal cancer. However, therapeutic responses to targeted therapies are often shortlived, since a signaling crosstalk of IGF1 R with other receptor tyrosine kinases or autophagy exists, leading to acquired cellular resistance to therapy. 8RILL EROSION PROCESS ON STEEP SLOPE LAND OF THE LOESS PLATEAU显示文摘Rill erosion processes and principle of preventing rill erosion of different applied measures are studied by rainfall simulation and field investigation. Research results show that rill erosion processes can be schematically divided into five stages, that is, concentrated flow forming process; little waterfalls in the concentrated flow forming Process; little waterfalls developing into rill head cut process; rill head cuts developing to form discontinuous rill process and discontinuous rills connecting to form continuous rill process. Rill head cuts backward erosion, rill wall collapsing and runoff washing rill bed are the main patterns of rill development. 9Synthesis, isolation and characterization of methyl levulinate from cellulose catalyzed by extremely low concentration acid显示文摘A direct synthesis of methyl levulinate from cellulose alcoholysis in methanol medium under mild condition(180 210 C)catalyzed by extremely low concentration sulfuric acid(0.01 mol/L)and the product isolation were developed in this study.Effects of different process variables towards the catalytic performance were performed as a function of reaction time.The results indicated that sulfuric acid concentration,temperature and initial cellulose concentration had significant effects on the synthesis of methyl levulinate.An optimized yield of around 50%was achieved at 210 C for 120 min with sulfuric acid concentration of 0.01 mol/L and initial cellulose concentration below 100 g/L.The resulting product mixture was isolated by a distillation technique that combines an atmospheric distillation with a vacuum distillation where n-dodecane was added to help distill the heavy fraction.The light fraction including mainly methanol could be reused as the reaction medium without any substantial change in the yield of methyl levulinate.The chemical composition and structural of lower heavy fraction were characterized by GC/MS,FTIR,1H-NMR and13C-NMR techniques.Methyl levulinate was found to be a major ingredient of lower heavy fraction with the content over 96%.This pathway is efficient,environmentally benign and economical for the production of pure levulinate esters from cellulose.Hui Li Lincai Peng Lu Lin Keli Chen Heng Zhang 2013Journal of Energy Chemistry2013,22,6:9

11Effect of Altitude and Latitude on Surface Air Temperature across the Qinghai-Tibet Plateau显示文摘The correlation between mean surface air temperature and altitude is analyzed in this paper based on the annual and monthly mean surface air temperature data from 106 weather stations over the period 1961-2003 across the Qinghai-Tibet Plateau.The results show that temperature variations not only depend on altitude but also latitude,and there is a gradual decrease in temperature with the increasing altitude and latitude.The overall trend for the vertical temperature lapse rate for the whole plateau is approximately linear.Three methods,namely multivariate composite analysis,simple correlation and traditional stepwise regression,were applied to analyze these three correlations.The results assessed with the first method are well matched to those with the latter two methods.The apparent mean annual near-surface lapse rate is-4.8 °C /km and the latitudinal effect is-0.87 °C /?latitude.In summer,the altitude influences the temperature variations more significantly with a July lapse rate of-4.3°C /km and the effect of latitude is only-0.28°C /latitude.In winter,the reverse happens.The temperature decrease is mainly due to the increase in latitude.The mean January lapse rate is-5.0°C /km,while the effect of latitude is-1.51°C /?latitude.Comparative analysis for pairs of adjacent stations shows that at a small spatial scale the difference in altitude is the dominant factor affecting differences in mean annual near-surface air temperature,aided to some extent by differences of latitude.In contrast,the lapse rate in a small area is greater than the overall mean value for the Qinghai-Tibet Plateau (5 to 13°C /km).An increasing trend has been detected for the surface lapse rate with increases in altitude.The temperature difference has obvious seasonal variations,and the trends for the southern group of stations (south of 33? latitude) and for the more northerly group are opposite,mainly because of the differences in seasonal variation at low altitudes.For yearly changes,the temperature for high-altitude stations occurs earlier clearly.Temperature datasets at high altitude stations are well-correlated,and those in Nanjing were lagged for 1 year but less for contemporaneous correlations.The slope of linear trendline of temperature change for available years is clearly related to altitude,and the amplitude of temperature variation is enlarged by high altitude.The change effect in near-surface lapse rate at the varying altitude is approximately 1.0°C /km on the rate of warming over a hundred-year period.WANG Keli SUN Jia CHENG Guodong JIANG Hao 2011Journal of Mountain Science2011,8,6:7
12Novel Surface Design of Deployable Reflector Antenna Based on Polar Scissor Structures显示文摘Space-deployable mechanisms can be used as supporting structures for large-diameter antennas in space engineering.This study proposes a novel method for constructing the surface design of space reflector antennas based on polar scissor units.The concurrency and deployability equations of the space scissor unit with definite surface constraints are derived using the rod and vector methods.Constraint equations of the spatial transformation for space n-edge polar scissor units are summarized.A new closed-loop deployable structure,called the polar scissor deployable antenna(PSDA),is designed by combining planar polar scissor units with spatial polar scissor units.The overconstrained problem is solved by releasing the curve constraint that locates at the end-point of the planar scissor mechanism.Kinematics simulation and error analysis are performed.The results show that the PSDA can effectively fit the paraboloid of revolution.Finally,deployment experiments verify the validity and feasibility of the proposed design method,which provides a new idea for the construction of large space-reflector antennas.Pengyuan Zhao Jinguo Liu Chenchen Wu Yangmin Li Keli Chen 2020Chinese Journal of Mechanical Engineering2020,33,5:7
13The westerly fluctuation and water vapor transport over the Qilian-Heihe valley显示文摘The westerly fluctuation and the atmospheric water vapor transport over the Qilian-Heihe valley are analyzed and the results show that, in the water vapor transport stream field from Jun to September, this valley is in the westerly stream and the water vapor comes from westerlies water transport via the Black Sea and the Caspian Sea. The net water vapor transport is less net import and different from most areas of the northwest China. The interannual changes in water vapor transport over the valley arise from the westerly fluctuation, and have a positive relationship to the interannual changes in westerly wind speed. The cold air actions from the Mongol low pressure are the primary system that controls the westerly water vapor transport. Its action chain is that, the Mongol low pressure is strengthened → the circulation meridionality will be increased → the cold air will move southwards → the westerly will be stronger → the wind convergence of direction and speed will be stronger → the water vapor convergence transport will be increased → the local water vapor content will be increased. The interannual changes in atmospheric water vapor transport over the valley rely mainly on the convergence transport, but the effect of advection transport is less. 15Effectiveness of Catch-Up Vaccinations after COVID-19 Containment--China,2020显示文摘Introduction:In the early stages of the coronavirus disease 2019(COVID-19)epidemic in China,the highest level of public health emergency response(Level 1 Response)was launched in all province-level administrative divisions(PLADs)across the country.Provision of vaccination services was selectively suspended due to the physical distancing and gathering restrictions required of Level 1 Response.The purpose of this paper is to evaluate the impact of COVID-19 on vaccination services and the effectiveness of selective vaccination service suspension and catch-up vaccinations in China.Methods:Vaccine doses administered,routine vaccination coverage levels,catch-up vaccine doses administered,and coverage levels after the catch-up campaign were determined from individual vaccination records in sampled clinics,standard routine immunization administrative coverage reports,and catch-up vaccination activity reports submitted by PLADs between October 2019 and October 2020.Results:National Immunization Program(NIP)and non-NIP vaccine doses administered in sampled clinics decreased by 80% and 90%,respectively,compared with doses administered before the COVID-19 pandemic.Coverage with the birth dose of hepatitis B vaccine(HepB1)and the Bacillus Calmette-Guérin vaccine(BCG)—two vaccinations recommended to continue throughout the epidemic due to their timecritical nature—were maintained at over 85%,while coverage of other NIP vaccines decreased to below 60%by February 2020.Catch-up vaccination coverage of the 29 PLADs,excluding Xinjiang and Xizang(Tibet),began in April 2020 and exceeded 90%;Hubei catch-up coverage was 95% by October 2020.Conclusions and Implications for Public Health Practice:The COVID-19 pandemic had a great impact on China’s vaccination services.During the epidemic and time of local transmission of the coronavirus,HepB1,BCG,rabies vaccine for postexposure prophylaxis,and tetanus antitoxin(TAT)for wound prophylaxis were maintained at high levels.Of the 69 million vaccine doses postponed during the time of local transmission of the coronavirus,94% were administered in the catch-up campaign.China’s pandemic immunization practices may provide evidence for other countries’immunization programs.Jing Wu Wenzhou Yu Lei Cao Lingsheng Cao Lance Rodewald Jiakai Ye Yifan Song Li Li Xiaoxue Liu Ning Wen Fuzhen Wang Lixin Hao Yixing Li Hui Zheng Keli Li Chao Ma Dan Wu Yanmin Liu Guomin Zhang Zhijie An Huaqing Wang Zundong Yin 2020China CDC weekly2020,2,50:6
16Vapor-phase growth of high-quality wafer-scale two-dimensional materials显示文摘Emerging two-dimensional(2D)materials have stimulated tremendous scientific and industrial interests due to their diverse and tunable physical,chemical,and mechanical properties.The scalable production of high-quality wafer-scale 2D materials has become significantly essential to bring us closer to practical industrial applications,particularly in electronic devices.Vapor-phase growth provides attractive opportunities for the synthesis of large-area and high-quality 2D materials.In this review,we will emphasize vapor-phase growth strategies from three aspects,including suppressing nucleation,seamless stitching,and evolutionary selection growth.We discuss the general understanding of the related fundamental mechanism and specific parameter optimization from precursors and substrate design to the adjusting of growth parameters(temperature and pressure).Meanwhile,we present other strategies to produce various kinds of wafer-scale 2D materials.Finally,we conclude the current challenges and future directions in this developing field.This work may inspire researchers to better design routes in the synthesis of wafer-scale 2D materials with high quality.Xin Tong Keli Liu Mengqi Zeng Lei Fu 2019InfoMat2019,1,4:5
17Diabetic gastroenteropathy:An underdiagnosed complication显示文摘This article is an extensive review that provides an update on the pathophysiology,symptoms,diagnosis,and treatment of diabetic gastroenteropathy.There is no reported prevalence,but it has been described that patients with type 1 diabetes have a cumulative incidence at 10 years of 5.2%,and type 2 patients,1%.Also,in the group of type 1 diabetes,it has been observed that women are more likely to present this condition(5.8%vs 3.5%).Many factors are associate with its development(e.g.,hyperglycemia,vagal dysfunction,loss of expression of neural nitric oxide synthase in the myenteric plexus,alterations in the Cajal interstitial cell network,and oxidative stress).Gastrointestinal discomfort could be perceived 70% higher in diabetic patients,describing that 25%of diabetic patients experience gastrointestinal symptoms.Diabetic enteropathy could affect any portion of the gastrointestinal tract,but esophageal alterations were described in more than 60% of diabetic patients,also 60% of them present constipation,and 20%,diarrhea.Gastric emptying scintigraphy is useful to evaluate gastroparesis,therefore,gastric retention of more than 60%at 2 h has a sensitivity of 100% and specificity of 20% for diagnosis;however,other studies such as breath tests,with a sensitivity of 89% and a specificity of 80%,or the endoscopic capsule contribute to the diagnosis.There is no cure;however,management must be multidisciplinary,focused on slowing the progression of diabetic gastroenteropathy,reducing symptoms,and restoring function;that includes nutritional recommendation,maintain glucose levels kept below 180 mg/dL,use of prokinetics,anti-emetics;nowadays,it has been special interest in surgical treatment,such as pyloroplasty,also gastric electrical stimulation appears to be another alternative.Marcio JoséConcepción Zavaleta Jhean Gabriel Gonzáles Yovera Diego Martín Moreno Marreros Luciana del Pilar Rafael Robles Kely Roxana Palomino Taype Karen Nohelly Soto Gálvez Luis Fernando Arriola Torres Julia Cristina Coronado Arroyo Luis Alberto Concepción Urteaga 2021World Journal of Diabetes2021,12,6:4

20Icariin upregulates phosphorylated cyclic adenosine monophosphate response element binding protein levels in the hippocampus of the senescence-accelerated mouse显示文摘At 8 weeks after intragastric administration of icariin to senescence-accelerated mice (P8 strain), Morris water maze results showed that escape latency was shortened, and the number of platform crossings was increased. Immunohistochemical staining and western blot assay detected significantly increased levels of cyclic adenosine monophosphate response element binding protein.
