1骨科创伤患者深静脉血栓危险因素的研究显示文摘目的研究骨科创伤患者深静脉血栓(deep vein thrombosis,DVT)的发生率,探讨骨科创伤患者DVT发生的危险因素。方法以2003年11月至2004年10月天津医院收治的新鲜四肢及骨盆骨折患者为研究对象,通过对不同年龄组患者的骨折类型、受伤机制、肿胀程度、既往DVT病史、手术及治疗方法等资料进行统计,并以3次彩色超声多普勒检查为诊断依据。需要手术的患者入院24h内进行第1次静脉多普勒检查,第2次和第3次检查分别在术前2d及术后7d;非手术患者3次检查分别在入院后24h、第5及14天。将需要分析的数据输入临床报告系统数据库1.1版,进行统计学分析及DVT发生率的流行病学分析。结果统计的547例患者中,平均年龄为39.6岁,DVT发生率为12.4%。以股骨干骨折DVT发生率为最高达30.6%,其次为髋部骨折15.7%,膝关节周围骨折14.5%,胫腓骨骨折10.8%。3个部位以上的多部位骨折患者DVT发生率为50%,单部位骨折患者DVT的发生率低于10%。超过半数的DVT(56.7%)发生于受伤3d之内。随着年龄增加DVT发生的风险也随之增大,年龄超过60岁患者的发病风险为184%。吸烟患者发生DVT的危险几乎是非吸烟患者的2倍。结论DVT在骨科创伤患者中具有一定的发生率,其年龄、病史、骨折部位、多发骨折及受伤距手术时间等均与DVT发生相关,须引起骨科临床医生的足够重视,并采取预防措施以降低肺栓塞的发生。陆芸 马宝通 郭若霖 张建国 吴英华 庞贵根 辛景义 叶伟胜 邹玉安 王毅 董强 王学谦 Kerry H Paul C John F 2007中华骨科杂志2007,27,9:244

3Antigen presentation by dendritic cells and their instruction of CD4+ T helper cell responses显示文摘Dendritic cells are powerful antigen-presenting cells that are essential for the priming of T cell responses.In addition to providing T-cell-receptor ligands and co-stimulatory molecules for naive T cell activation and expansion,dendritic cells are thought to also provide signals for the differentiation of CD4+T cells into effector T cell populations.The mechanisms by which dendritic cells are able to adapt and respond to the great variety of infectious stimuli they are confronted with,and prime an appropriate CD4+T cell response,are only partly understood.It is known that in the steady-state dendritic cells are highly heterogenous both in phenotype and transcriptional profile,and that this variability is dependent on developmental lineage,maturation stage,and the tissue environment in which dendritic cells are located.Exposure to infectious agents interfaces with this pre-existing heterogeneity by providing ligands for pattern-recognition and toll-like receptors that are variably expressed on different dendritic cell subsets,and elicit production of cytokines and chemokines to support innate cell activation and drive T cell differentiation.Here we review current information on dendritic cell biology,their heterogeneity,and the properties of different dendritic cell subsets.We then consider the signals required for the development of different types of Th immune responses,and the cellular and molecular evidence implicating different subsets of dendritic cells in providing such signals.We outline how dendritic cell subsets tailor their response according to the infectious agent,and how such transcriptional plasticity enables them to drive different types of immune responses.Kerry L.Hilligan Franca Ronchese 2020Cellular & Molecular Immunology2020,17,6:19

6Shifting equilibriums in Alzheimer’s disease: the complex roles of microglia in neuroinflammation, neuronal survival and neurogenesis显示文摘Alzheimer's disease is the leading cause of dementia.Its increased prevalence in developed countries,due to the sharp rise in ageing populations,presents one of the costliest challenges to modern medicine.In order to find disease-modifying therapies to confront this challenge,a more complete understanding of the pathogenesis of Alzheimer's disease is necessary.Recent studies have revealed increasing evidence for the roles played by microglia,the resident innate immune system cells of the brain.Reflecting the well-established roles of microglia in reacting to pathogens and inflammatory stimuli,there is now a growing literature describing both protective and detrimental effects for individual cytokines and chemokines produced by microglia in Alzheimer's disease.A smaller but increasing number of studies have also addressed the divergent roles played by microglial neurotrophic and neurogenic factors,and how their perturbation may play a key role in the pathogenesis of Alzheimer's disease.Here we review recent findings on the roles played by microglia in neuroinflammation,neuronal survival and neurogenesis in Alzheimer's disease.In each case,landmark studies have provided evidence for the divergent ways in which microglia can either promote neuronal function and survival,or perturb neuronal function,leading to cell death.In many cases,the secreted molecules of microglia can lead to divergent effects depending on the magnitude and context of microglial activation.This suggests that microglial functions must be maintained in a fine equilibrium,in order to support healthy neuronal function,and that the cellular microenvironment in the Alzheimer's disease brain disrupts this fine balance,leading to neurodegeneration.Thus,an understanding of microglial homeostasis,both in health and across the trajectory of the disease state,will improve our understanding of the pathogenic mechanisms underlying Alzheimer's disease,and will hopefully lead to the development of microglial-based therapeutic strategies to restore equilibrium in the Alzheimer's disease brain.Sophie C. 10Effects of intermittent fasting on health markers in those with type 2 diabetes:A pilot study显示文摘AIM To determine the short-term biochemical effects and clinical tolerability of intermittent fasting(IF) in adults with type 2 diabetes mellitus(T2DM).METHODS We describe a three-phase observational study(baseline 2 wk, intervention 2 wk, follow-up 2 wk) designed to determine the clinical, biochemical, and tolerability of IF in community-dwelling volunteer adults with T2DM. Biochemical, anthropometric, and physical activity measurements(using the Yale Physical Activity Survey) were taken at the end of each phase. Participants reported morning, afternoon and evening self-monitored blood glucose(SMBG) and fasting duration on a daily basis throughout all study stages, in addition to completing a remote food photography diary three times within each study phase. Fasting blood samples were collected on the final days of each study phase. RESULTS At baseline, the ten participants had a confirmed diagnosis of T2DM and were all taking metformin, and on average were obese [mean body mass index(BMI) 36.90 kg/m^2]. We report here that a short-term periodof IF in a small group of individuals with T2DM led to significant group decreases in weight(-1.395 kg, P = 0.009), BMI(-0.517, P = 0.013), and at-target morning glucose(SMBG). Although not a study requirement, all participants preferentially chose eating hours starting in the midafternoon. There was a significant increase(P < 0.001) in daily hours fasted in the IF phase(+5.22 h), although few attained the 18-20 h fasting goal(mean 16.82 ± 1.18). The increased fasting duration improved at-goal(< 7.0 mmol/L) morning SMBG to 34.1%, from a baseline of 13.8%. Ordinal Logistic Regression models revealed a positive relationship between the increase in hours fasted and fasting glucose reaching target values(χ~2 likelihood ratio = 8.36, P = 0.004) but not for afternoon or evening SMBG(all P > 0.1). Postprandial SMBGs were also improved during the IF phase, with 60.5% readings below 9.05 mmol/L, compared to 52.6% at baseline, and with less glucose variation. Neither insulin resistance(HOMAIR), nor inflammatory markers(C-reactive protein) normalized during the IF phase. IF led to an overall spontaneous decrease in caloric intake as measured by food photography(Remote Food Photography Method). The data demonstrated discernable trends during IF for lower energy, carbohydrate, and fat intake when compared to baseline. Physical activity, collected by a standardized measurement tool(Yale Physical Activity Survey), increased during the intervention phase and subsequently decreased in the follow-up phase. IF was well tolerated in the majority of individuals with 6/10 participants stating they would continue with the IF regimen after the completion of the study, in a full or modified capacity(i.e., every other day or reduced fasting hours).CONCLUSION The results from this pilot study indicate that shortterm daily IF may be a safe, tolerable, dietary intervention in T2DM patients that may improve key outcomes including body weight, fasting glucose and postprandial variability. 11Implications of discoveries from genome-wide association studies in current cardiovascular practice显示文摘Genome-wide association studies(GWAS)have identified several genetic variants associated with coronary heart disease(CHD),and variations in plasma lipoproteins and blood pressure(BP).Loci corresponding to CDKN2A/CDKN2B/ANRIL,MTHFD1L,CELSR2,PSRC1 and SORT1 genes have been associated with CHD,and TMEM57,DOCK7,CELSR2,APOB,ABCG5,HMGCR,TRIB1,FADS2/S3,LDLR,NCAN and TOMM40-APOE with total cholesterol.Similarly,CELSR2-PSRC1-SORT1,PCSK9,APOB,HMGCR,NCAN-CILP2-PBX4,LDLR,TOMM40-APOE,and APOC1-APOE are associated with variations in low-density lipoprotein cholesterol levels.Altogether,forty,forty three and twenty loci have been associated with high-density lipoprotein cholesterol,triglycerides and BP phenotypes,respectively.Some of these identified loci are common for all the traits,some do not map to functional genes,and some are located in genes that encode for proteins not previously known to be involved in the biological pathway of the trait.GWAS have been successful at identifying new and unexpected genetic loci common to diseases and traits,thus rapidly providing key novel insights into disease biology.Since genotype information is fixed,with minimum biological variability,it is useful in early life risk prediction.However,these variants explain only a small proportion of the observed variance of these traits.Therefore,the utility of genetic determinants in assessing risk at later stages of life has limited immediate clinical impact.The future application of genetic screening will be in identifying risk groups early in life to direct targeted preventive measures.Panniyammakal Jeemon Kerry Pettigrew Christopher Sainsbury Dorairaj Prabhakaran Sandosh Padmanabhan 2011World Journal of Cardiology2011,3,7:6

13Liver transplantation for alcoholic liver disease显示文摘Alcoholic liver disease(ALD) is the second commonest indication for liver transplantation after viral hepatitis in the United States and Europe.Controversies surround the indications and allocation of scarce and expensive resource for this so called self inflicted disease.Controversies stem from the apprehension that alcoholic recipients are likely to relapse and cause damage to the graft.There is a need to select those candidates with lower risk for relapse with the available predictive factors and scores.Substance abuse specialist and psychiatrists are mandatory in the pre-transplant evaluation and in the post-transplant follow-up.There is conflicting evidence to support a fixed period of pretransplant abstinence,although most units do follow this.Alcoholic hepatitis(AH) continues to be a contraindication for transplantation,however there is a need for further research in this f ield as a subset of patients with AH who do not respond to medical treatment,have high early mortality and could benefit from transplantation.One year,3-year,and 5-year survival post-transplant is similar for both ALD and non-ALD recipients.The incidence of post-transplant rejection and retransplantation is also similar to other recipients.ALD with viral hepatitis especially hepatitis C virus leads to a more aggressive liver disease with early presentation for transplantation.ALD patients are more prone to develop de-novo malignancy;this is attributed to the long term effect of alcohol,tobacco combined with immunosuppression.Post-transplant surveillance is important to detect early relapse to alcoholism,presence of de-novo malignancy and treat the same adequately.Vibha Varma Kerry Webb Darius F Mirza 2010World Journal of Gastroenterology2010,16,35:6

16Endoscopic balloon catheter dilatation via retrograde or static technique is safe and effective for cricopharyngeal dysfunction显示文摘AIM To evaluate the safety and efficacy of upper esophageal sphincter(UES) dilatation for cricopharyngeal(CP) dysfunction. To determine if:(1) indication for dilatation; or(2) technique of dilatation correlated with symptom improvement.METHODS All balloon dilatations performed at our institution from over a 3-year period were retrospectively analyzed for demographics, indication and dilatation site. All dilatations involving the UES underwent further review to determine efficacy, complications, and factors that predict success. Dilatation technique was separated into static(stationary balloon distention) and retrograde(brusque pull-back of a fully distended balloon across the UES).RESULTS Four hundred and eighty-eight dilatations were reviewed. Thirty-one patients were identified who underwent UES dilatation. Median age was 63 years(range 27-81)and 55% of patients were male. Indications included dysphagia(28 patients), globus sensation with evidence of UES dysfunction(2 patients) and obstruction to echocardiography probe with cricopharyngeal(CP)bar(1 patient). There was evidence of concurrent oropharyngeal dysfunction in 16 patients(52%) and a small Zenker's diverticula(≤ 2 cm) in 7 patients(23%). Dilator size ranged from 15 mm to 20 mm. Of the 31 patients, 11 had dilatation of other esophageal segments concurrently with UES dilatation and 20 had UES dilatation alone.Follow-up was available for 24 patients for a median of 2.5 mo(interquartile range 1-10 mo), of whom 19 reported symptomatic improvement(79%). For patients undergoing UES dilatation alone, follow-up was available for 15 patients, 12 of whom reported improvement(80%).Nineteen patients underwent retrograde dilatation(84%response) while 5 patients had static dilatation(60%response); however, there was no significant difference in symptom improvement between the techniques(P = 0.5).Successful symptom resolution was also not significantly affected by dilator size, oropharyngeal dysfunction,Zenker's diverticulum, age or gender(P > 0.05). The only complication noted was uvular edema and a shallow ulcer after static dilatation in one patient, which resolved spontaneously and did not require hospital admission.CONCLUSION UES dilatation with a through-the-scope balloon by either static or retrograde technique is safe and effective for the treatment of dysphagia due to CP dysfunction. 19Current cancer therapies and their influence on glucose control显示文摘This review focuses on the development of hyperglycemia arising from widely used cancer therapies spanning four drug classes.These groups of medications were selected due to their significant association with new onset hyperglycemia,or of potentially severe clinical consequences when present.These classes include glucocorticoids that are frequently used in addition to chemotherapy treatments,and the antimetabolite class of 5-fluorouracil-related drugs.Both of these classes have been in use in cancer therapy since the 1950s.Also considered are the phosphatidyl inositol-3-kinase(PI3K)/AKT/mammalian target of rapamycin(mTOR)-inhibitors that provide cancer response advantages by disrupting cell growth,proliferation and survival signaling pathways,and have been in clinical use as early as 2007.The final class to be reviewed are the monoclonal antibodies selected to function as immune checkpoint inhibitors(ICIs).These were first used in 2011 for advanced melanoma and are rapidly becoming widely utilized in many solid tumors.For each drug class,the literature has been reviewed to answer relevant questions about these medications related specifically to the characteristics of the hyperglycemia that develops with use.The incidence of new glucose elevations in euglycemic individuals,as well as glycemic changes in those with established diabetes has been considered,as has the expected onset of hyperglycemia from their first use.This comparison emphasizes that some classes exhibit very immediate impacts on glucose levels,whereas other classes can have lengthy delays of up to 1 year.A comparison of the spectrum of severity of hyperglycemic consequences stresses that the appearance of diabetic ketoacidosis is rare for all classes except for the ICIs.There are distinct differences in the reversibility of glucose elevations after treatment is stopped,as the mTOR inhibitors and ICI classes have persistent hyperglycemia long term.These four highlighted drug categories differ in their underlying mechanisms driving hyperglycemia,with clinical presentations ranging from potent yet transient insulin resistant states[type 2 diabetes mellitus(T2DM)-like]to rare permanent insulin-deficient causes of hyperglycemia.Knowledge of the relative incidence of new onset hyperglycemia and the underlying causes are critical to appreciate how and when to best screen and treat patients taking any of these cancer drug therapies.Carly Yim Kerry Mansell Nassrein Hussein Terra Arnason 2021World Journal of Diabetes2021,12,7:5
