| 1 | Early plasmapheresis and rituximab for acute humoral rejection after ABO-compatible liver transplantation显示文摘Acute humoral rejection (AHR) is uncommon after ABO-compatible liver transplantation. Herein, we report two cases of AHR treated with plasmapheresis and rituximab in two ABO-compatible liver-transplant patients with preformed anti-human leukocyte antigen donor-specif ic antibodies. Patient 1 experienced a biopsy-proven AHR at day 10 post-transplant. She was treated by steroid pulses, and OKT3. Because of persisting signs of biopsy-proven AHR at day 26, she was treated by plasmapheresis and rituximab. Liver enzyme levels did not improve, and she died on day 41. Patient 2 experienced a biopsy-proven AHR on day 10 post-transplant. She was treated by steroid pulses, plasmapheresis, and rituximab. Liver enzymes returned to within normal range 18 d after diagnosis. Liver biopsies, at 3 and 9 mo post-transplant, showed complete resolution of AHR. We conclude that plasmapheresis should be started as soon as AHR is diagnosed, and be associated with a B-cell depleting agent. | 2 | Energy Dissipation of P- and S-Waves in Fluid-Saturated Rocks: An Overview Focusing on Hydraulically Connected Fractures显示文摘An important characteristic of fractured rocks is their high seismic attenuation, which so far has been mainly attributed to wave-induced fluid flow(WIFF) between the fractures and the embedding matrix. The influence of fracture connectivity on seismic attenuation has, however, recently, only been investigated. Numerical compressibility and shear tests based on Biot's quasi-static poro-elastic equations illustrate that an important manifestation of WIFF arises in the presence of fracture connectivity. The corresponding energy loss, which can be significant for both P- and S-waves, is mainly due to fluid flow within the connected fractures and is sensitive to the permeabilities as well as the lengths and intersection angles of the fractures. | 3 | MicroRNAs as emerging biomarkers and therapeutic targets for pancreatic cancer显示文摘Despite tremendous efforts from scientists and clinicians worldwide, pancreatic adenocarcinoma(PDAC) remains a deadly disease due to the lack of early diagnostic tools and reliable therapeutic approaches. Consequently, a majority of patients(80%) display an advanced disease that results in a low resection rate leading to an overall median survival of less than 6 months. Accordingly, robust markers for the early diagnosis and prognosis of pancreatic cancer, or markers indicative of survival and/or metastatic disease are des-perately needed to help alleviate the dismal prognosis of this cancer. In addition, the discovery of new therapeutic targets is mandatory to design effective treatments. | 4 | Predictive factors of histological response of colorectal liver metastases after neoadjuvant chemotherapy显示文摘BACKGROUND Colorectal cancer is the third most common cancer in men and the second most common in women worldwide. Almost a third of the patients has or will develop liver metastases. Neoadjuvant chemotherapy(NAC) has recently become nearly systematic prior to surgery of colorectal livers metastases(CRLMs). The response to NAC is evaluated by radiological imaging according to morphological criteria.More recently, the response to NAC has been evaluated based on histological criteria of the resected specimen. The most often used score is the tumor regression grade(TRG), which considers the necrosis, fibrosis, and number of viable tumor cells.AIM To analyze the predictive factors of the histological response, according to the TRG, on CRLM surgery performed after NAC.METHODSFrom January 2006 to December 2013, 150 patients who had underwent surgery for CRLMs after NAC were included. The patients were separated into two groups based on their histological response, according to Rubbia-Brandt TRG.Based on their TRG, each patient was either assigned to the responder(R) group(TRG 1, 2, and 3) or to the non-responder(NR) group(TRG 4 and 5). All of the histology slides were re-evaluated in a blind manner by the same specialized pathologist. Univariate and multivariate analyses were performed.RESULTS Seventy-four patients were classified as responders and 76 as non-responders.The postoperative mortality rate was 0.7%, with a complication rate of 38%.Multivariate analysis identified five predictive factors of histological response.Three were predictive of non-response: More than seven NAC sessions, the absence of a radiological response after NAC, and a repeat hepatectomy(P <0.005). Two were predictive of a good response: A rectal origin of the primary tumor and a liver-first strategy(P < 0.005). The overall survival was 57% at 3 yr and 36% at 5 yr. | 5 | Protic ionic liquids/poly(vinylidene fluoride) composite membranes for fuel cell application显示文摘Poly(vinylidene fluoride), PVDF, membranes have attracted considerable attention as polymer electrolytes for fuel cells. This study explores the effect of solvent on the spherulite size and the crystallinity of the polymeric membranes. Based on Hansen solubility parameters theory, the mixture of DMC and DMSO was selected among a dozen of solvents for the preparation of PVDF membranes by thermally induced phase separation. The addition of two protic ionic liquids(PILs), bis(2-ethyl hexyl) ammonium hydrogen phosphate [EHNH_2][H_2PO_4], and imidazolium hexanoate [Im][Hex] to PVDF membranes at concentrations(10% < wP IL< 50%) has been investigated by SEM, FTIR, DSC, TGA, EIS, and DMA. The inclusion of ionic liquids into the polymer matrix influences structural parameters(degree of crystallinity and electroactive phases), thermal stability, proton conductivity and mechanical properties of the membranes. The membranes become transparent regardless type of ionic liquid employed. A small amount of ionic liquids increases the degree of crystallinity and facilitates the production of polar β and γ crystals. The proton conductivity mechanism(Grotthuss) is dependent on the ionic liquid structure(due to its selforganization in water) and the content in the PVDF membrane, as well as the membrane water uptake.Different behavior has been observed for the two ionic liquids, which stresses the challenge on selecting an appropriate cation and anion combination. The obtained composite membranes exhibited excellent mechanical performance and reduced elastic modulus, with respect to the pure polymer matrix. | 6 | Role of endoscopic ultrasound in the molecular diagnosis of pancreatic cancer显示文摘Pancreatic ductal adenocarcinoma remains one of the most deadly types of tumor. Endoscopic ultrasoundguided fine-needle aspiration(EUS-FNA) is a safe, cost-effective, and accurate technique for evaluating and staging pancreatic tumors. However, EUS-FNA may be inconclusive or doubtful in up to 20% of cases. This review underlines the clinical interest of the molecular analysis of samples obtained by EUS-FNA in assessing diagnosis or prognosis of pancreatic cancer, especially in locally advanced tumors. On EUS-FNA materials DNA, mRNA and miRNA can be extracted, amplified, quantified and subjected to methylation assay. Kras mutation assay, improves diagnosis of pancreatic cancer. When facing to clinical and radiological presentations of pseudo-tumorous chronic pancreatitis, wildtype Kras is evocative of benignity. Conversely, in front of a pancreatic mass suspected of malignancy, a mutated Kras is highly evocative of pancreatic adenocarci-noma. This strategy can reduce false-negative diagnoses, avoids the delay of making decisions and reduces loss of surgical resectability. Similar approaches are conducted using analysis of miRNA expression as well as Mucin or markers of invasion(S100P, S100A6, PLAT or PLAU).
