| 1 | Validation of the Rockall scoring system for outcomes from non-variceal upper gastrointestinal bleeding in a Canadian setting显示文摘AIM: To validate the Rockall scoring system for predicting outcomes of rebleeding, and the need for a surgical procedure and death. METHODS: We used data extracted from the Registry of Upper Gastrointestinal Bleeding and Endoscopy including information of 1869 patients with non-variceal upper gastrointestinal bleeding treated in Canadian hospitals. Risk scores were calculated and used to classify patients based on outcomes. For each outcome, we used χ2 goodness-of-fit tests to assess the degree of calibration, and built receiver operating characteristic curves and calculated the area under the curve (AUC) to evaluate the discriminative ability of the scoring system. RESULTS: For rebleeding, the χ2 goodness-of-fit test indicated an acceptable fit for the model [χ2 (8) = 12.83, P = 0.12]. For surgical procedures [χ2 (8) = 5.3, P = 0.73] and death [χ2 (8) = 3.78, P = 0.88], the tests showed solid correspondence between observed proportions and predicted probabilities. The AUC was 0.59 (95% CI: 0.55-0.62) for the outcome of rebleeding and 0.60 (95% CI: 0.54-0.67) for surgical procedures, representing apoor discriminative ability of the scoring system. For the outcome of death, the AUC was 0.73 (95% CI: 0.69-0.78), indicating an acceptable discriminative ability. | 2 | AngioJet流体溶血栓吸引术对急性心肌梗死血栓性病变的疗效显示文摘目的 AngioJet流体溶血栓吸引术是通过流体击碎并吸引冠状动脉内血栓而将其移出体外的一种新的冠状动脉介入治疗技术。本研究以TIMI心肌灌注级别作为疗效指标评价该技术对急性心肌梗死血栓性病变的疗效。方法 选择澳大利亚皇家珀斯医院 2 0 0 0年 1月至 2 0 0 1年 1月所有冠状动脉造影明确显示血栓性病变的急性心肌梗死患者共 2 8例作为研究对象。其中 1 3例患者于支架置入术前进行了AngioJet流体溶血栓吸引术 (AngioJet组 ) ,而另 1 5例患者只行直接支架置入术(支架组 )。重点比较两组患者之间治疗前后心表面冠状动脉TIMI血流及TIMI心肌灌注级别 (后者与预后有强烈的相关性 )的变化。结果 支架组术后心表面冠状动脉TIMI血流及TIMI心肌灌注级别均明显优于AngioJet组 (分别为 2 60± 0 0 9vs 2 2 3± 0 1 6 ,P =0 0 4 2及 2 50± 0 1 1vs 1 92± 0 2 3 ,P=0 0 2 4 )。AngioJet组冠状动脉穿孔的例数显著多于支架组 (分别为 3例及 0例 ,P =0 0 4 9)。结论对于急性心肌梗死血栓性病变 ,支架置入术治疗可获得良好疗效 ;支架置入前采用AngioJet流体溶血栓吸引术并不增加疗效 。 | 刘海波 高润霖 Richard A Clugston Jamie M Rankin Geoffrey Mews Geoffrey DCope | 2002 | 中华心血管病杂志2002，30，12： | 6 |
| 3 | Mycophenolate mofetil for maintenance of remission in steroid-dependent autoimmune pancreatitis显示文摘Systemic corticosteroids represent the standard treatment for autoimmune pancreatitis with IgG4-associated cholangitis.For steroid-dependent disease,azathioprine has been used for maintenance of remission.Mycophenolate mofetil has been used for transplant immunosuppression and more recently for autoimmune hepatitis;however,there are no case reports to date on the use of mycophenolate mofetil in adult patients with autoimmune pancreatitis.A patient with IgG4-mediated autoimmune pancreatitis and IgG4-associated cholangitis refractory to steroids and intolerant of azathioprine was treated with mycophenolate mofetil,which inhibits de novo guanosine synthesis and blockade of both B and T lymphocyte production.Introduction of mycophenolate mofetil and uptitration to 1000 mg by mouth twice daily over a treatment period of 4 mo was associated with improvement in the patient's energy level and blood glucose control and was not associated with any adverse events.The patient was managed without a biliary stent.However,there was a return of symptoms,jaundice,increase in transaminases,and hyperbilirubinemia when the prednisone dose reached 11 mg per day.In the first report of mycophenolate mofetil use in an adult patient with IgG4-associated autoimmune pancreatitis and IgG4-associated cholangitis,the introduction of mycophenolate mofetil was safe and well-tolerated without adverse events,but it did not enable discontinuation of the steroids.Mycophenolate mofetil and other immunomodulatory therapies should continue to be studied for maintenance of remission in the large subset of patients with refractory or recurrent autoimmune pancreatitis. | Jamie B Sodikoff Steven A Keilin Sheila J Bharmal Melinda M Lewis Gottumukkala S Raju Field F Willingham | 2012 | World Journal of Gastroenterology2012，18，18： | 6 |
