| 1 | Protein-loosing enteropathy associated with rotavirus infection in an infant显示文摘Rotavirus is an acute enteric pathogen in infants and children. We reported a rare case of a 6-mo-old infant with protein-loosing enteropathy (PLE) caused by rotavirus gastroenteritis, and evaluated the immunological profile in peripheral blood lymphocytes. Laboratory examinations showed lymphopenia, hypoproteinemia, hypoalbuminemia, hypogammaglobulinemia, and elevation of alpha-1-antitrypsin (α1-AT) clearance. Lymphocytes subpopulation study revealed the reversal of CD4+/CD8+ ratio with the selective decrease of CD4- positive lymphocytes. Moreover, the excessive increase of T cells producing IFN-gamma (IFN-γ) was found, which plays an important role in the protection against viral infection. | 2 | Study protocol of the Asian XELIRI ProjecT(AXEPT):a multinational,randomized,non-inferiority,phase Ⅲ trial of second-line chemotherapy for metastatic colorectal cancer, comparing the eicacy and safety of XELIRI with or without bevacizumab versus FOLFIRI w显示文摘Background: Capecitabine and irinotecan combination therapy(XELIRI) has been examined at various dose levels to treat metastatic colorectal cancer(m CRC). Recently, in the Association of Medical Oncology of the German Cancer Society(AIO) 0604 trial, tri?weekly XELIRI plus bevacizumab, with reduced doses of irinotecan(200 mg/m^2 on day 1) and capecitabine(1600 mg/m^2 on days 1–14), repeated every 3 weeks, has shown favorable tolerability and eicacy which were comparable to those of capecitabine and oxaliplatin(XELOX) plus bevacizumab. The doses of capecit?abine and irinotecan in the AIO trial are considered optimal. In a phase I/II study, XELIRI plus bevacizumab(BIX) as second?line chemotherapy was well tolerated and had promising eicacy in Japanese patients.Methods: The Asian XELIRI Projec T(AXEPT) is an East Asian collaborative, open?labelled, randomized, phase Ⅲ clinical trial which was designed to demonstrate the non?inferiority of XELIRI with or without bevacizumab versus standard FOLFIRI(5?fluorouracil, leucovorin, and irinotecan combination) with or without bevacizumab as second?line chemo?therapy for patients with m CRC. Patients with 20 years of age or older, histologically conirmed m CRC, Eastern Coop?erative Oncology Group performance status 0–2, adequate organ function, and disease progression or intolerance of the irst?line regimen will be eligible. Patients will be randomized(1:1) to receive standard FOLFIRI with or with?out bevacizumab(5 mg/kg on day 1), repeated every 2 weeks(FOLIRI arm) or XELIRI with or without bevacizumab(7.5 mg/kg on day 1), repeated every 3 weeks(XELIRI arm). A total of 464 events were estimated as necessary to show non?inferiority with a power of 80% at a one?sided α of 0.025, requiring a target sample size of 600 patients. | 5 | Effect of branched-chain amino acids in patients receiving intervention for hepatocellular carcinoma显示文摘AIM:To investigate the usefulness of branched-chain amino acids(BCAA)before transarterial chemoembolization(TACE)or radiofrequency ablation(RFA).METHODS:We investigated the usefulness of preintervention with BCAAs by comparing patients treated with BCAAs at 12.45 g/d orally for at least 2 wk before TACE or RFA and those not receiving such pretreatment.A total of 270 patients with hepatocellular carcinoma complicated by cirrhosis were included in the study.Mean changes from baseline(Δ)in serum albumin(Alb),C-reactive protein(CRP),and transaminase levels,as well as peak body temperature were also determined and compared at days 2,5,and 10 after the start of TACE or RFA.RESULTS:In patients who underwent TACE or RFA,BCAA pre-intervention significantly suppressed the development of post-intervention hypoalbuminemia and reduced inflammatory reactions during the subsequent clinical course.After TACE,theΔAlb peaked on day 2,remained constantly lower in BCAA-treated patients,compared to the control group,and was-0.13±0.42g/dL in BCAA-treated patients and-0.33±0.51 g/dL in untreated patients on day 10.TheΔCRP was also significantly lower in BCAA-treated patients on days2,5 and 10 after TACE.Like the trends noted after TACE,a similar tendency was noted as to theΔAlb andΔCRP after RFA.The changes in serum Alb level were inversely correlated with CRP changes;therefore,a possible involvement of the anti-inflammatory effect of BCAAs was inferred as a factor contributory to the suppression of decrease in serum Alb level.CONCLUSION:Pre-intervention with BCAAs may hasten the recovery of serum Alb level and mitigate postoperative complications in patients undergoing TACE or RFA. | Tomoaki Ishihara Motoh Iwasa Hideaki Tanaka Masahiko Kaito Jiro Ikoma Toshiya Shibata Yoshiyuki Takei | 2014 | World Journal of Gastroenterology2014，20，10： | 2 |

The goal of this experiment is to study the reaction rate of 18Ne(,p)21Na reaction, which mightbe a key breakout reaction from the hot CNO cycle to rp-process in X-ray burst and nova. Yet its reaction rate ispoorly known.Explosive hydrogen burning is thought to be the main source of energy generation and a source of nucleosynthesisin X-ray burst and nova[1;2]. For example, XRBs are characterized by a sudden increase of X-ray emission withinonly a few seconds to a total energy output of about 1040 ergs, which is observed to repeat with some regularity.The recurrence time for single bursts can range from hours to days at the typical temperature of 0.4~2 GK. Thebursts have been interpreted as being generated by thermonuclear runaway on the surface of a neutron star thataccretes H- and He-rich material from a less evolved companion star in a close binary system. | 7 | Atomically phase-matched second-harmonic generation in a 2D crystal显示文摘Second-harmonic generation(SHG)has found extensive applications from hand-held laser pointers to spectroscopic and microscopic techniques.Recently,some cleavable van der Waals(vdW)crystals have shown SHG arising from a single atomic layer,where the SH light elucidated important information such as the grain boundaries and electronic structure in these ultra-thin materials.However,despite the inversion asymmetry of the single layer,the typical crystal stacking restores inversion symmetry for even numbers of layers leading to an oscillatory SH response,drastically reducing the applicability of vdW crystals such as molybdenum disulfide(MoS_(2)).Here,we probe the SHG generated from the noncentrosymmetric 3R crystal phase of MoS_(2).We experimentally observed quadratic dependence of second-harmonic intensity on layer number as a result of atomically phase-matched nonlinear dipoles in layers of the 3R crystal that constructively interfere.By studying the layer evolution of the A and B excitonic transitions in 3R-MoS_(2) using SHG spectroscopy,we also found distinct electronic structure differences arising from the crystal structure and the dramatic effect of symmetry and layer stacking on the nonlinear properties of these atomic crystals.The constructive nature of the SHG in this 2D crystal provides a platform to reliably develop atomically flat and controllably thin nonlinear media. | Mervin Zhao Ziliang Ye Ryuji Suzuki Yu Ye Hanyu Zhu Jun Xiao Yuan Wang Yoshihiro Iwasa Xiang Zhang | 2016 | Light(Science & Applications)2016，5，1： | 2 |
