| 1 | Cardiac remodeling and physical training post myocardial infarction显示文摘After myocardial infarction(MI), the heart undergoes extensive myocardial remodeling through the accumulation of fibrous tissue in both the infarcted and noninfarcted myocardium, which distorts tissue structure, increases tissue stiffness, and accounts for ventricular dysfunction. There is growing clinical consensus that exercise training may beneficially alter the course of post-MI myocardial remodeling and improve cardiac function. This review summarizes the present state of knowledge regarding the effect of post-MI exercise training on infarcted hearts. Due to the degree of difficulty to study a viable human heart at both protein and molecular levels, most of the detailed studies have been performed by using animal models. Although there are some negative reports indicating that post-MI exercise may further cause deterioration of the wounded hearts, a growing body of research from both human and animal experiments demonstrates that post-MI exercise may beneficially alter the course of wound healing and improve cardiac function. Furthermore, the improved function is likely due to exercise training-induced mitigation of reninangiotensin-aldosterone system, improved balance between matrix metalloproteinase-1 and tissue inhibitor of matrix metalloproteinase-1, favorable myosin heavy chain isoform switch, diminished oxidative stress, enhanced antioxidant capacity, improved mitochondrial calcium handling, and boosted myocardial angiogenesis. | 2 | Bone morphogenetic protein-2 gene controls tooth root development in coordination with formation of the periodontium显示文摘Formation of the periodontium begins following onset of tooth-root formation in a coordinated manner after birth. Dental follicle progenitor cells are thought to form the cementum, alveolar bone and Sharpey's fibers of the periodontal ligament (PDL). However, little is known about the regulatory morphogens that control differentiation and function of these progenitor cells, as well as the progenitor cells involved in crown and root formation. We investigated the role of bone morphogenetic protein-2 (Bmp2) in these processes by the conditional removal of the Bmp2 gene using the Sp7-Cre-EGFP mouse model. Sp7-Cre-EGFP first becomes active at E18 in the first molar, with robust Cre activity at postnatal day 0 (P0), followed by Cre activity in the second molar, which occurs after P0. There is robust Cre activity in the periodontium and third molars by 2 weeks of age. When the Bmp2 gene is removed from Sp7+ (Osterix+) cells, major defects are noted in root, cellular cementum and periodontium formation. First, there are major cell autonomous defects in root-odontoblast terminal differentiation. Second, there are major alterations in formation of the PDLs and cellular cementum, correlated with decreased nuclear factor IC (Nfic), periostin and α-SMA+ cells. Third, there is a failure to produce vascular endothelial growth factor A (VEGF-A) in the periodontium and the pulp leading to decreased formation of the microvascular and associated candidate stem cells in the Bmp2-cKOSp7-Cre-EGFP . Fourth, ameloblast function and enamel formation are indirectly altered in the Bmp2-cKOSp7-Cre-EGFP . | 5 | Anesthesia and sedation in pediatric gastrointestinal endoscopic procedures: A review显示文摘Gastrointestinal(GI) endoscopic procedure has become an essential modality for evaluation and treatment of GI diseases.Intravenous(IV) sedation and General Anesthesia(GA) have both been employed to minimize discomfort and provide amnesia.Both these procedures require,at the very least,monitoring of the level of consciousness,pulmonary ventilation,oxygenation and hemodynamics.Although GI endoscopy is considered safe,the procedure has a potential for complications.Increased awareness of the complications associated with sedation during GI endoscopy in children,and involving the anesthesiologists in caring for these children,may be optimal for safety.Belonging to a younger age group,having a higher ASA class and undergoing IV sedation were identified as risk factors for developing complications.Reported adverse events included inadequate sedation,low oxygen saturation,airway obstruction,apnea needing bag mask ventilation,excitement and agitation,hemorrhage and perforation.A complication rate of 1.2% was associated with procedures performed under GA,as compared to 3.7% of complications associated with IV sedation.IV sedation was seen to be independently associated with a cardiopulmonary complication rate 5.3% times higher when compared to GA.GA can therefore be considered safer and more effective in providing comfort and amnesia. | Abdul Q Dar Zahoor A Shah | 2010 | World Journal of Gastrointestinal Endoscopy2010,2,7: | 8 |

| 8 | 标准辅助化疗基础上增加塞来昔布或安慰剂对Ⅲ期结肠癌患者无复发生存的疗效对比研究:CALGB/SWOG 80702(Alliance)随机对照研究显示文摘背景:既往观察性和随机对照研究已表明阿司匹林和环氧化酶-2(COX-2)抑制剂可降低肠道息肉和肠癌的发生风险。COX-2抑制剂塞来昔布治疗非转移性结肠癌的效果尚未明确。目的:本研究旨在明确FOLFOX辅助化疗基础上增加塞来昔布是否能提高Ⅲ期结肠癌患者的无复发生存率。方法:本研究为在美国、加拿大的654个中心开展的采用2×2析因设计的Ⅲ期临床研究,从2010年6月至2015年11月共纳入2526例Ⅲ期结肠癌患者,随访时间截止为2020年8月10日。以化疗时长和是否接受塞来昔布治疗作为2个随机因素,入组患者被随机分配接受3个月或6个月的FOLFOX辅助化疗(每两周1次)及随机接受联合3年塞来昔布(400 mg QD,n=1263)或安慰剂(n=1261)治疗。主要终点测量指标为3年无复发生存率(DFS),次要终点测量指标包括总生存、不良反应及心血管特异性不良事件。本次仅报道以是否接受塞来昔布治疗的随机分组研究结果。结果:入组患者平均年龄(61±11)岁,1134例(44.9%)为女性,最终2524例被纳入初步分析。患者治疗方案依从性(即接受塞来昔布或安慰剂治疗2.75年以上或持续治疗至复发、死亡或不可接受的不良事件)在塞来昔布组和安慰剂组分别为70.8%和69.9%。经过6年中位随访期,塞来昔布组和安慰剂组各有337例和363例患者出现疾病复发或死亡,3年无复发生率分别为76.3%和73.4%(疾病复发或死亡风险比HR=0.89,95%CI:0.76~1.03,P=0.12)。塞来昔布治疗对DFS的影响不会因辅助化疗时长(3个月或6个月)而存在差异(交互作用P=0.61)。两组5年生存率分别为84.3%和81.6%(死亡风险比HR=0.86,95%CI:0.72~1.04,P=0.13)。任何级别的高血压病发生率在塞来昔布组和安慰剂组分别为14.6%和10.9%,而2级或以上肌酐水平升高的发生率在两组分别为1.7%和0.5%。结论:对于Ⅲ期结肠癌患者,在标准辅助化疗的基础上联合3年塞来昔布治疗与安慰剂比较,对患者无复发生存率的改善不明显。 | 邱萌 MEYERHARDT J A SHI Q FUCHS C S | 2021 | 结直肠肛门外科2021,27,3: | 6 |
