1Relationship between intubation rate and continuous positive airway pressure therapy in the prehospital setting显示文摘BACKGROUND: To determine whether the prehospital use of continuous positive airway pressure(CPAP) therapy is associated with a reduced rate of endotracheal intubation in patients with an acute respiratory disorder brought to the emergency department(ED).METHODS: We reviewed medical records of patients with acute respiratory distress who had been treated with CPAP in the Mobile Intensive Care Unit(MICU) from January 2010 to December 2011. These records were compared with those of patients who received standardized care without CPAP in the MICU from January 2004 to December 2004. Categorical variables were summarized as frequencies and compared between groups using Fisher's exact test or the Chi-square test. Continuous variables were summarized as medians(interquartile range), and comparison between the groups was made using Wilcoxon's rank-sum test. The relationship between CPAP and intubation rate was determined using multivariable logistic regression analysis of propensity scores. The results were presented as odds ratio(OR), 95% confidence interval(CI), and P value for test effect. The adequacy of the model was calibrated using Hosmer and Lemeshow's goodness-of-fit test. P<0.05 was considered statistically significant.RESULTS: The records of 785 patients were reviewed. Of the 215 patients treated with CPAP in the MICU, 13% were intubated after admission. In contrast, of the 570 patients who did not receive CPAP, 28% were intubated after ED admission. Unadjusted logistic regression analysis showed that patients who had been treated with CPAP were less likely to be intubated than those without CPAP treatment(OR=0.37, 95% CI, 0.24–0.57, P<0.0001). With propensity scores adjusted, multivariate logistic regression analysis showed that CPAP treatment was associated with a 62% reduction of intubation(OR=0.384, 95%CI, 0.25–0.60, P≤0.0001).CONCLUSIONS: In patients with acute respiratory disorder, there was a relationship between CPAP therapy and the decreased intubation rate. 2Carbon storage in biomass,litter,and soil of different native and introduced fast-growing tree plantations in the South Caspian Sea显示文摘Replantation of degraded forest using rapidgrowth trees can play a significant role in global carbon budget by storing large quantities of carbon in live biomass,forest floor,and soil organic matter.We assessed the potential of 20-year old stands of three rapid-growth tree species,including Alnus subcordata,Populus deltoides and Taxodium distichum,for carbon(C) storage at ecosystem level.In September 2013,48 replicate plots(16 m × 16 m) in 8 stands of three plantations were established.36 trees were felled down and fresh biomass of different components was weighed in the field.Biomass equations were fitted using data based on the 36 felled trees.The biomass of understory vegetation and litter were measured by harvesting all the components.The C fraction of understory,litter,and soil were measured.The ecosystem C storage was as follows: A.subcordata(626.5 Mg ha^(-1)) [ P.deltoides(542.9Mg ha^(-1)) [ T.distichum(486.8 Mg ha^(-1))(P \ 0.001),of which78.1–87.4% was in the soil.P.deltoides plantation reached the highest tree biomass(206.6 Mg ha^(-1)),followed by A.subcordata(134.5 Mg ha^(-1)) and T.distichum(123.3 Mg ha^(-1)).The highest soil C was stored in theplantation of A.subcordata(555.5 Mg ha^(-1)).The C storage and sequestration of the plantations after 20 years were considerable(25–30 Mg ha^(-1) year^(-1)) and broadleaves species had higher potential.Native species had a higher soil C storage while the potential of introduced species for live biomass production was higher.Jamshid Eslamdoust Hormoz Sohrabi 2018Journal of Forestry Research2018,29,2:3

10Refractive surgery: the most cost-saving technique in refractive errors correction显示文摘AIM: To compare the lifetime and annual economic burden of spectacles, contact lenses, and refractive surgery in correction of refractive errors.METHODS: This is a cross-sectional study with convenience sampling which 120 patients were interviewed in a tertiary referral hospital in the Iranian health care system. The bottom-up based cost of illness approach was estimated using a face-to-face interview to assess the direct and indirect cost of different refractive errors correction of any correction technologies.RESULTS: Correction with spectacle imposes a total direct cost of US dollar(US$) 342.5(±8.41) per year and US$9373.5(±230.1) per lifetime to each patient. These figures for the contact lenses were obtained US$198.3(±0.12) and US$5203.1(±256.3) and for refractive surgery were obtained US$19.1(±1.2) and US$568.1(±64.6),respectively. Overall, based on age-adjusted prevalence rates, astigmatism had the highest share of refractive errors economic burden with a lifetime direct cost of slightly less than US$5.49 billion, while hyperopia and myopia imposed less than US$5.24 and 4.2 billion on patients, respectively. The annually imposed cost on each individual Iranian patient with refractive errors is US$308.5.CONCLUSION: Based on 18 mo post refractive surgery course observation, which is generalized to whole life,refractive surgery significantly imposed much less cost compared with spectacles and contact lenses. Refractive errors among Iranians result in considerable economic burden. 15Above- and below-ground biomass and carbon stocks of different tree plantations in central Iran显示文摘In arid and semi-arid lands using industrial wastewater for irrigating tree plantations offers a great opportunity to fulfill the purpose of Clean Development Mechanism by sequestering carbon in living tissues as well as in soil. Selection of tree for plantation has a great effect on the goal achievements, especially when the managers deal with afforestation projects rather than reforestation projects. The objective of this study was to quantify the above- and below-ground biomass accumulation and carbon storages of the 17-year-old monoculture plantations of mulberry(Morus alba L.), black locust(Robinia pseudoacacia L.), Eldar pine(Pinus eldarica Medw.) and Arizona cypress(Cupressus arizonica Greene) planted in central Iran. To assess the potential carbon storage, we destructively measured individual above- and below-ground tree biomass and developed and scaled models at stand level. Furthermore, carbon content at three soil depths(0–15, 15–30, 30–45 cm), the litter and the understory were assessed in sample plots. The results showed that the total amount of carbon stored by Eldar pine(36.8 Mg/hm2) was higher than those stored by the trees in the other three plantations, which were 23.7, 10.0, and 9.6 Mg/hm2 for Arizona cypress, mulberry and black locust plantations, respectively. For all the species, the above-ground biomass accumulations were higher than those of the below-ground. The root mass fractions of the deciduous were larger than those of the coniferous.
