2Uncommon presentation of a common disease-Bouveret's syndrome:A case report and systematic literature review显示文摘AIM To investigate and summarise the current evidence surrounding management of Bouveret's syndrome(BS).METHODS A MEDLINE search was performed for the BS. The search was conducted independently by two clinicians(Yahya ALHabbal and Matthew Ng) in April 2016. A case of BS is also described.RESULTS A total of 315 articles, published from 1967 to 2016, were found. For a clinically meaningful clinical review, articles published before 01/01/1990 and were excluded, leaving 235 unique articles to review. Twenty-seven articles were not available(neither by direct communication nor through inter-library transfer). These were also excluded. The final number of articles reviewed was 208. There were 161 case reports, 13 reviews, 23 images(radiological and clinical images), and 11 letters to editor. Female to male ratio was 1.82. Mean age was 74 years. Treatment modalities included laparotomy in the majority of cases, laparoscopic surgery, endoscopic surgery and shockwave lithotripsy.CONCLUSION There is limited evidence in the literature about the appropriate approach. 3Traumatic brain injury:A case-based review显示文摘BACKGROUND:Traumatic brain injuries are common and costly to hospital systems.Most of the guidelines on management of traumatic brain injuries are taken from the Brain Trauma Foundation Guidelines.This is a review of the current literature discussing the evolving practice of traumatic brain injury.DATA SOURCES:A literature search using multiple databases was performed for articles published through September 2012 with concentration on meta-analyses,systematic reviews,and randomized controlled trials.RESULTS:The focus of care should be to minimize secondary brain injury by surgically decompressing certain hematomas,maintain systolic blood pressure above 90 mmHg,oxygen saturations above 93%,euthermia,intracranial pressures below 20 mmHg,and cerebral perfusion pressure between 60-80 mmHg.CONCLUSION:Much is still unknown about the management of traumatic brain injury.The current practice guidelines have not yet been sufficiently validated,however equipoise is a major issue when conducting randomized control trials among patients with traumatic brain injury.Liza Victoria S Escobedo Joseph Habboushe Haytham Kaafarani George Velmahos Kaushal Shah Jarone Lee 2013World Journal of Emergency Medicine2013,4,4:4

5Repeated occurrence of palaeo-wildfires during deposition of the Bahariya Formation(early Cenomanian) of Egypt显示文摘The Upper Cretaceous(early Cenomanian)Bahariya Formation of Egypt has an outstanding reputation for its wealth of vertebrate remains,including a variety of iconic dinosaurs,like the carnivorous Spinosaurus and Carcharodontosaurus,as well as the herbivorous Aegyptosaurus and Paralititan.Besides these dinosaur fossils,the Bahariya Formation yielded also a wealth of invertebrate and plant remains,but even today many aspects concerning the continental palaeoenvironments reflected in these deposits(including the occurrence of palaeo-wildfires)have not been studied in detail.So far six distinct macro-charcoal bearing levels could be identified within the type section of the Bahariya Formation at Gabal El Dist profile,one of the most prolific outcrops of this formation in terms of fossil occurrence located in the north of the Bahariya Oasis,Western Desert,Egypt.Most of the charcoal investigated by means of SEM originates from ferns,pointing to a considerable proportion of this plant group within the palaeo-ecosystems that experienced fires.Gymnosperms and(putative)angiosperms have less frequently been identified.The collected data present evidence that the landscapes at the northern shores of Gondwana repeatedly experienced palaeo-wildfires,adding extra proof to previous statements that the Late Cretaceous was a fiery world on a global scale.Haytham El Atfy Tarek Anan Andre Jasper Dieter Uhl 2019Journal of Palaeogeography2019,8,4:2
6胸腔镜内乳淋巴结清扫:一种针对乳腺癌内乳淋巴结的分期技术(英文)显示文摘Objective: The aim of our study was to investigate the feasibility and safety of thoracoscopic internal mammary lymphadenectomy as a method to refine and thereby improve nodal staging in breast cancer. Methods: During the period from June 2004 to May 2007, 50 patients with operable breast cancer underwent modified radical mastectomy (MRM) or breast conserving surgery (BCS), followed by thoracoscopic internal mammary lymphadenectomy, using 3 ports through the skin incision of the MRM or the BCS. Metal clips were used to mark precise site of lymphadenectomy. Results: of total number of 50 patients, the mean age of patients was 44 years (range, 27-60 years). 40 (80%) had medio-central tumor, 10 (20%) had lateral tumor. 35 (70%) had clinically involved axillary nodes. 16 out of 50 patients received neo-adjuvant CTH. 44 patients underwent MRM and 6 patients underwent BCS. No intra-operative complications occurred. Atelectasis was the only postoperative complication that was encountered, which occurred in 12 cases, and was treated conservatively. The average chest drainage period was 1.2 day (range, 1-2 days). The total number of IMN metastasis was 18 patients (36%). The risk of IMN metastasis was higher; in younger patients (P = 0.03), in medio-central tumors (P = 0.03), in bigger tumors (P = 0.05), with heavier metastasis of axillary LNs (P = 0.001). But a correlation with the histological pattern of the 1ry tumor didn't exist (P = 1). Knowing the IMN status helped in proper staging of patients, 7 patients showed evident stage migration after adding the IMN analysis to that of primary tumor and axillary LN. During the follow up period (the median, 22 months; range, 7 to 42 months), no patient had pleural dissemination or port-site metastasis. Conclusion: Thoracoscopic IMN lymphadenectomy is a safe procedure, which can be done serious additional complications or cosmetic compromise.
