| 3 | Probiotics for gastrointestinal disorders: Proposed recommendations for children of the Asia-Pacific region显示文摘Recommendations for probiotics are available in several regions. This paper proposes recommendations for probiotics in pediatric gastrointestinal diseases in the Asia-Pacific region. Epidemiology and clinical patterns of intestinal diseases in Asia-Pacific countries were discussed. Evidence-based recommendations and randomized controlled trials in the region were revised. Cultural aspects,health management issues and economic factors were also considered. Final recommendations were approved by applying the Likert scale and rated using the GRADE system. Saccharomyces boulardii CNCM I-745(Sb) and Lactobacillus rhamnosus GG(LGG) were strongly recommended as adjunct treatment to oral rehydration therapy for gastroenteritis. Lactobacillus reuteri could also be considered. Probiotics may be considered for prevention of(with the indicated strains): antibiotic-associated diarrhea(LGG or Sb); Clostridium difficile-induced diarrhea(Sb); nosocomial diarrhea(LGG); infantile colic(L reuteri) and as adjunct treatment of Helicobacter pylori(Sb and others). Specific probiotics with a history of safe use in preterm and term infants may be considered in infants for prevention of necrotizing enterocolitis. There is insufficient evidence for recommendations in other conditions. Despite a diversity of epidemiological,socioeconomical and health system conditions,similar recommendations apply well to Asia pacific countries. | 5 | Poor performance of the modified early warning score for predicting mortality in critically ill patients presenting to an emergency department显示文摘BACKGROUND:This study was undertaken to validate the use of the modified early warning score(MEWS) as a predictor of patient mortality and intensive care unit(ICU)/ high dependency(HD)admission in an Asian population.METHODS:The MEWS was applied to a retrospective cohort of 1 024 critically ill patients presenting to a large Asian tertiary emergency department(ED) between November 2006 and December2007.Individual MEWS was calculated based on vital signs parameters on arrival at ED.Outcomes of mortality and ICU/HD admission were obtained from hospital records.The ability of the composite MEWS and its individual components to predict mortality within 30 days from ED visit was assessed.Sensitivity,specificity,positive and negative predictive values were derived and compared with values from other cohorts.A MEWS of ≥4 was chosen as the cut-off value for poor prognosis based on previous studies.RESULTS:A total of 311(30.4%) critically ill patients were presented with a MEWS ≥4.Their mean age was 61.4 years(SD 18.1) with a male to female ratio of 1.10.Of the 311 patients,53(17%)died within 30 days,64(20.6%) were admitted to ICU and 86(27.7%) were admitted to HD.The area under the receiver operating characteristic curve was 0.71 with a sensitivity of 53.0%and a specificity of 72.1%in addition to a positive predictive value(PPV) of 17.0%and a negative predictive value(NPV)of 93.4%(MEWS cut-off of ≥4) for predicting mortality.CONCLUSION:The composite MEWS did not perform well in predicting poor patient outcomes for critically ill patients presenting to an ED. | Le Onn Ho Huihua Li Nur Shahidah Zhi Xiong Koh Papia Sultana Marcus Eng Hock Ong | 2013 | World Journal of Emergency Medicine2013，4，4： | 12 |

| 7 | Panaxdiol Saponins Component Promotes Hematopoiesis and Modulates T Lymphocyte Dysregulation in Aplastic Anemia Model Mice显示文摘Objective: To investigate the potential efficacy of panaxadiol saponins component(PDS-C) in the treatment of aplastic anemia(AA) model mice. Methods: Totally 70 mice were divided into 7 groups as follows: normal, model, low-, medium-, high-dose PDS-C(20, 40, 80 mg/kg, namely L-, M-, H-PDS-C), cyclosporine(40 mg/kg), and andriol(25 mg/kg) groups, respectively. An immune-mediated AA mouse model was established in BALB/c mice by exposing to 5.0 Gy total body irradiation at 1.0 Gy/min, and injecting with lymphocytes from DBA mice. On day 4 after establishment of AA model, all drugs were intragastrically administered daily for 15 days, respectively, while the mice in the normal and model groups were administered with saline solution. After treatment, the peripheral blood counts, bone marrow pathological examination, colony forming assay of bone marrow culture, T lymphocyte subpopulation analysis, as well as T-bet, GATA-3 and Fox P3 proteins were detected by flow cytometry and Western blot. Results: The peripheral blood of white blood cell(WBC), platelet, neutrophil counts and hemoglobin(Hb) concentration were significantly decreased in the model group compared with the normal group(all P<0.01). In response to 3 dose PDS-C treatment, the WBC, platelet, neutrophil counts were significantly increased at a dose-dependent manner compared with the model group(all P<0.01). The myelosuppression status of AA was significantly reduced in M-, H-PDS-C groups, and hematopoietic cell quantity of bone marrow was more abundant than the model group. The colony numbers of myeloid, erythroid and megakaryocytic progenitor cells in the model group were less than those of the normal mice in bone marrow culture, while, PDS-C therapy enhanced proliferation of hematopoietic progenitor cells by significantly increasing colony numbers(all P<0.01). Furthermore,PDS-C therapy increased peripheral blood CD3^+ and CD3^+CD4^+ cells and reduced CD3^+CD8^+ cells(P<0.05 or P<0.01). Meanwhile, PDS-C treatment at medium-and high doses groups also increased CD4^+CD25^+Fox P3^+ cells, downregulated T-bet protein expression, and upregulated GATA-3 and Fox P3 protein expressions in spleen cells(P<0.05). | 8 | The Future of Gas to Liquids as a Gas Monetisation Option显示文摘The paper introduces gas to liquids (GTL) as a monetising option from a technology, marketing and project perspective. GTL is complementary to LNG and pipelines. At the same time, using natural gas as a source for fuels in the form of GTL helps countries around the world to diversify their energy supplies. Furthermore, gas-based products are inherently cleaner than oil products. Shell's proprietary GTL technology or SMDS (Shell Middle Distillates Synthesis), is discussed in some detail. The paper also covers the challenges for successful implementation of GTL projects and why Shell is well positioned to take a lead in the industry on the basis of its long standing and broad experience in GTL research,plant operations, marketing and excellent track record in mega projects in the last thirty years. Shell's commitment to GTL is best demonstrated by the recent signing of a Heads of Agreement with Qatar Petroleum for the construction of the world's largest GTL plant. A key success factor is Shell's experience with marketing quantities of high quality GTL products from its 12,500 barrels per day plant at Bintulu,Malaysia since 1993. Further marketing opportunities will arise when new GTL capacity comes on-stream in the middle east when more quantities will become available to bulk users. Amongst the most interesting fuel beyond oil' providing energy security to host countries. Shell is actively engaging with a number of regulators, automotive companies and governments worldwide including China, to demonstrate the performance of GTL and its cost effectiveness in reducing local emissions. They arise most commonly in the upper esophagus and may, rarely, originate in the hypopharynx. They can vary significantly in size. Even though they are benign, they may be lethal due to either bleeding or, rarely, asphyxiation if a large polyp is regurgitated. Patients commonly present with dysphagia or hematemesis. The polyps may not be well visualized on endoscopy and imaging plays a vital role in aiding diagnosis as well as providing important information for preoperative planning, such as the location of the pedicle, the vascularity of the polyp and the tissue elements of the mass. They can also be recurrent in rare cases, especially if the resection margins of the base are involved. | 14 | Prehospital system delay in patients with ST-segment elevation myocardial infarction in Singapore显示文摘BACKGROUND: Timely reperfusion in ST-segment elevation myocardial infarction(STEMI)improves outcomes. System delay is that between first medical contact and reperfusion therapy,comprising prehospital and hospital components. This study aimed to characterize prehospital system delay in Singapore.METHODS: A retrospective chart review was performed for 462 consecutive STEMI patients presenting to a tertiary hospital from December 2006 to April 2008. Patients with cardiac arrest secondarily presented were excluded. For those who received emergency medical services(EMS),ambulance records were reviewed. Time intervals in the hospital were collected prospectively. The patients were divided into two equal groups of high/low prehospital system delay using visual binning technique.RESULTS: Of 462 patients, 76 received EMS and 52 of the 76 patients were analyzed. The median system delay was 125.5 minutes and the median prehospital system delay was 33.5minutes(interquartile range [IQR]=27.0, 42.0). Delay between call-received-by-ambulance and ambulance-dispatched was 2.48 minutes(IQR=1.47, 16.55); between ambulance-dispatch and arrival-at-patient-location was 8.07 minutes(IQR=1.30, 22.13); between arrival-at- and departurefrom-patient-location was 13.12 minutes(IQR=3.12, 32.2); and between leaving-patient-location to ED-registration was 9.90 minutes(IQR=1.62, 32.92). Comparing patients with prehospital system delay of less than 35.5 minutes versus more showed that the median delay between ambulancedispatch and arrival-at-patient-location was shorter(5.75 vs. 9.37 minutes, P<0.01). The median delay between arrival-at-patient-location and leaving-patient-location was also shorter(10.78 vs.14.37 minutes, P<0.01).CONCLUSION: Prehospital system delay in our patients was suboptimal. | 16 | Use of Farmers’ Indicators to Evaluate the Sustainability of Cropping Systems on Sloping Land in Yunnan Province, China显示文摘Diversity in the biophysical and socio-economic attributes of agricultural systems makes them uniquely niche based. Farmers are expert in local biophysical and socio-economic situations and can contribute in developing pragmatic indicators of agro-environmental development. During evaluation of an agricultural research project in Yunnan, China, local farmers were capable of evaluating the effects of modified technologies on existing cropping systems and discussed their attitudes to the interventions using their own indicators. Farmers’ response can be grouped into seven major aspects: i) effects on income, ii) effects on production resources, iii) effects on crop management, iv) existing local knowledge about the technology, v) availability of inputs, vi) access to information, and vii) socio-economic conditions of farming households. Farmers concluded that environmental conditions in the experimental catchment in comparison to an adjacent untreated catchment were better in terms of soil and water losses, vegetation cover and natural resources, infrastructures and catchment management, use of environmentally-friendly technologies, and crop productivity. Success in soil and water conservation programmes depends on the efforts of the farmers and other local users and their greater involvement helps to identify more pragmatic indicators. Furthermore, it increases ownership of the programme, enhances interactions with the project scientists, increases farmers’ awareness of agro-environmental problems and their possible consequences. These development will enable scientists to develop better targeted interventions and increase the likelihood of adoption of tested technologies by local communities. | 19 | Serous pancreatic neoplasia, data and review显示文摘AIM To describe the imaging features of serous neoplasms of the pancreas using ultrasound, endoscopic ultrasound, computed tomography and magnetic resonance imaging.METHODS This multicenter international collaboration enhances a literature review to date, reporting features of 287 histologically confirmed cases of serous pancreatic cystic neoplasms(SPN). RESULTS Female predominance is seen with most SPN presenting asymptomatically in the 5th through 7th decade. Mean lesion size was 38.7 mm, 98% were single, 44.2% cystic, 46% mixed cystic and solid, and 94% hypoechoic on B-mode ultrasound. | 20 | Review of Fresnel incoherent correlation holography with linear and non-linear correlations显示文摘Fresnel incoherent correlation holography(FINCH)is a well-established incoherent imaging technique.In FINCH,three selfinterference holograms are recorded with calculated phase differences between the two interfering,differently modulated object waves and projected into a complex hologram.The object is reconstructed without the twin image and bias terms by a numerical Fresnel back propagation of the complex hologram.A modified approach to implement FINCH by a single camera shot by pre-calibrating the system involving recording of the point spread function library and reconstruction by a nonlinear cross correlation has been introduced recently.The expression of the imaging characteristics from the modulation functions in original FINCH and the modified approach by pre-calibration in spatial and polarization multiplexing schemes are reviewed.The study reveals that a reconstructing function completely independent of the function of the phase mask is required for the faithful expression of the characteristics of the modulating function in image reconstruction.In the polarization multiplexing method by non-linear cross correlation,a partial expression was observed,while in the spatial multiplexing method by non-linear cross correlation,the imaging characteristics converged towards a uniform behavior. | Vijayakumar Anand Tomas Katkus Soon Hock Ng Saulius Juodkazis | 2021 | Chinese Optics Letters2021，19，2： | 3 |
