1Radiation-induced sarcomas of the head and neck显示文摘With improved outcomes associated with radiotherapy, radiation-induced sarcomas(RIS) are increasingly seen in long-term survivors of head and neck cancers, with an estimated risk of up to 0.3%. They exhibit no subsite predilection within the head and neck and can arise in any irradiated tissue of mesenchymal origin. Common histologic subtypes of RIS parallel their de novo counterparts and include osteosarcoma, chondrosarcoma, malignant fibrous histiocytoma/sarcoma nitricoxide synthase, and fibrosarcoma. While imaging features of RIS are not pathognomonic, large size, extensive local invasion with bony destruction, marked enhancement within a prior radiotherapy field, and an appropriate latency period are suggestive of a diagnosis of RIS. RIS development may be influenced by factors such as radiation dose, age at initial exposure, exposure to chemotherapeutic agents and genetic tendency. Precise pathogenetic mechanisms of RIS are poorly understood and both directly mutagenizing effects of radiotherapy as well as changes in microenvironments are thought to play a role. Management of RIS is challenging, entailing surgery in irradiated tissue and a limited scope for further radiotherapy and chemotherapy. 3Management of post-operative fistula in head and neck surgery: Sweeping it under the carpet?显示文摘The most dreaded complication in head and necksurgery is the development of fistula. Fistulas are common and devastating. The prevalence and the risk factors that contribute to fistula formation after head and neck procedures were discussed briefly. The main goal of this manuscript is to discuss current management of head and neck fistula. We believed that the best management strategy for head and neck fistulas is prevention. We recommend a holistic preventive approach during the perioperative period. The roles of different types of wound products and hyperbaric oxygen therapy were also discussed and highlighted. We also discussed the operative repair of fistulas, which relies on the tenet of providing wellvascularized tissue to an area of poor wound healing. Most often, the surgeon's preference and range of operative skills dictate the timing and the type of repair. 4Genetic alterations in head and neck squamous cell carcinoma:The next-gen sequencing era显示文摘Head and neck squamous cell carcinoma is the sixth most common cancer in the world with approximately650000 new cases diagnosed annually.Next-generation molecular techniques and results from phase 2 of the Cancer Genome Atlas becoming available have drastically improved our current knowledge on the genetics basis of head and neck squamous cell carcinoma.New insights and new perspectives on the mutational landscape implicated in head and neck squamous cell carcinoma provide improved tools for prognostication.More importantly,depend on the patient's tumor subtypes and prognosis,deescalated or more aggressive therapy maybe chosen to achieve greater potency while minimizing the toxicity of therapy.This paper aims to review our current knowledge on the genetic mutations and altered molecular pathways in head and neck squamous cell carcinoma.Some of the most common mutations in head and neck squamous cell carcinoma reported by the cancer genome atlas including TP53,NOTCH1,Rb,CDKN2 A,Ras,PIK3 CA and EGFR are described here.Additionally,the emerging role of epigenetics and the role of human papilloma virus in head and neck squamous cell carcinoma are also discussed in this review.The molecular pathways,clinical applications,actionable molecular targets and potential therapeutic strategies are highlighted and discussed in details.Thien Khanh Nguyen N Gopalakrishna Iyer 2013World Journal of Medical Genetics2013,3,4:0
