1Endoclipping treatment of life-threatening rectal bleeding after prostate biopsy显示文摘Rectal bleeding is frequently seen in patients undergoing transrectal ultrasound(TRUS)-guided multiple biopsy of the prostate,but is usually mild and stops spontaneously.We report what is believed to be the first case of life-threatening rectal bleeding following this procedure,which was successfully treated by endoscopic intervention through placement of three clips on the sites of bleeding.This case emphasizes endoscopic intervention associated with endoclipping as a safe and effective method to achieve hemostasis in massive rectal bleeding after prostate biopsy.Additionally,current data on the complications of the TRUS-guided multiple biopsy of the prostate and the options for treating fulminant rectal bleeding, a consequence of this procedure,are described.Panagiotis Katsinelos Jannis Kountouras Georgios Dimitriadis Grigoris Chatzimavroudis Christos Zavos Ioannis Pilpilidis George Paroutoglou George Germanidis Kostas Mimidis 2009World Journal of Gastroenterology2009,15,9:4
2An unusual case of prolonged post-endoscopic retrograde cholangiopancreatography jaundice显示文摘Despite the effectiveness of endoscopic retrograde cholangiopancreatography(ERCP) for the treatment of choledocholithiasis, various complications have been described. We herein report the first case of prolonged postERCP jaundice due to toxicity of the contrast agent Iobitridol(~174;XENETIX, Guerbet, Roissy Cd G Cedex, France) in a patient who underwent ERCP with sphincterectomy and common bile duct stone removal. While clinical improvement and normalization of aminotransferases and cholestatic enzymes after the procedure, an unexplained increase of direct bilirubin was noticed. A second ERCP was performed one week later, excluding possible remaining choledocholithiasis. Nevertheless, serum direct bilirubin increased further up to 15 mg/d L. Other potential causes of direct hyperbilirubinemia were ruled out and patient's liver biopsy was compatible with drug-induced liver toxicity. Additionally, the cause-result time connection between the use of Iobitridol and bilirubin increase indicated the possibility of a toxic effect related to the repeated use of the particular contrast agent. 3Transforming urinary stone disease management by artificial intelligence-based methods:A comprehensive review显示文摘Objective:To provide a comprehensive review on the existing research and evidence regarding artificial intelligence(AI)applications in the assessment and management of urinary stone disease.Methods:A comprehensive literature review was performed using PubMed,Scopus,and Google Scholar databases to identify publications about innovative concepts or supporting applications of AI in the improvement of every medical procedure relating to stone disease.The terms“endourology”,“artificial intelligence”,“machine learning”,and“urolithiasis”were used for searching eligible reports,while review articles,articles referring to automated procedures without AI application,and editorial comments were excluded from the final set of publications.The search was conducted from January 2000 to September 2023 and included manuscripts in the English language.Results: A total of 69 studies were identified.The main subjects were related to the detection of urinary stones,the prediction of the outcome of conservative or operative management,the optimization of operative procedures,and the elucidation of the relation of urinary stone chemistry with various factors.Conclusion: AI represents a useful tool that provides urologists with numerous amenities,which explains the fact that it has gained ground in the pursuit of stone disease management perfection.The effectiveness of diagnosis and therapy can be increased by using it as an alternative or adjunct to the already existing data.However,little is known concerning the potential of this vast field.Electronic patient records,containing big data,offer AI the opportunity to develop and analyze more precise and efficient diagnostic and treatment algorithms.Nevertheless,the existing applications are not generalizable in real-life practice,and high-quality studies are needed to establish the integration of AI in the management of urinary stone disease.Anastasios Anastasiadis Antonios Koudonas Georgios Langas Stavros Tsiakaras Dimitrios Memmos Ioannis Mykoniatis Evangelos NSymeonidis Dimitrios Tsiptsios Eliophotos Savvides Ioannis Vakalopoulos Georgios Dimitriadis Jean de la Rosette 2023Asian Journal of Urology2023,10,3:0
4New endoscopes and add-on devices to improve colonoscopy performance显示文摘Colonoscopy is the gold standard for colorectal cancer prevention; however, it is still an imperfect modality. Precancerous lesions can be lost during screening examinations, thus increasing the risk of interval cancer. A variety of factors either patient-, or endoscopist dependent or even the procedure itself may contribute to loss of lesions. Sophisticated modalities including advanced technology endoscopes and add-on devices have been developed in an effort to eliminate colonoscopy's drawbacks and maximize its ability to detect potentially culprit polyps. Novel colonoscopes aim to widen the field of view. They incorporate more than one cameras enabling simultaneous image transmission. In that way the field of view can expand up to 330°. On the other hand a plethora of add-on devices attachable on the standard colonoscope promise to detect lesions in the proximal aspect of colonic folds either by offering a retrograde view of the lumen or by straightening the haustral folds during withdrawal.
