| 1 | Transcranial direct current stimulation enhances theory of mind in Parkinson’s disease patients with mild cognitive impairment:a randomized,double-blind,sham-controlled study显示文摘Background:Parkinson’s Disease(PD)with mild cognitive impairment(MCI)(PD-MCI)represents one of the most dreaded complications for patients with PD and is associated with a higher risk of developing dementia.Although transcranial direct current stimulation(tDCS)has been demonstrated to improve motor and non-motor symptoms in PD,to date,no study has investigated the effects of tDCS on Theory of Mind(ToM),i.e.,the ability to understand and predict other people’s behaviours,in PD-MCI.Methods:In this randomized,double-blind,sham-controlled study,we applied active tDCS over the medial frontal cortex(MFC)to modulate ToM performance in twenty patients with PD-MCI.Twenty matched healthy controls(HC)were also enrolled and were asked to perform the ToM task without receiving tDCS.Results:In the patients with PD-MCI,i)ToM performance was worse than that in the HC,ii)ToM abilities were poorer in those with fronto-executive difficulties,and iii)tDCS over the MFC led to significant shortening of latency for ToM tasks.Conclusions:We show for the first time that active tDCS over the MFC enhances ToM in patients with PD-MCI,and suggest that non-invasive brain stimulation could be used to ameliorate ToM deficits observed in these patients. | Mauro Adenzato Rosa Manenti Ivan Enrici Elena Gobbi Michela Brambilla Antonella Alberici Maria Sofia Cotelli Alessandro Padovani Barbara Borroni Maria Cotelli | 2019 | Translational Neurodegeneration2019，8，1： | 9 |
| 2 | Acute heart failure in elderly patients:a review of invasive and non-invasive management显示文摘Acute heart failure(AHF)is a major cause of unplanned hospitalisations in the elderly and is associated with high mortality.Its prevalence has grown in the last years due to population aging and longer life expectancy of chronic heart failure patients.Although international societies have provided guidelines for the management of AHF in the general population,scientific evidence for geriatric patients is often lacking,as these are underrepresented in clinical trials.Elderly have a different risk profile with more comorbidities,disability,and frailty,leading to increased morbidity,longer recovery time,higher readmission rates,and higher mortality.Furthermore,therapeutic options are often limited,due to unfeasibility of invasive strategies,mechanical circulatory support and cardiac transplantation.Thus,the in-hospital management of AHF should be tailored to each patient's clinical situation,cardiopulmonary condition and geriatric assessment.Palliative care should be considered in some cases,in order to avoid unnecessary diagnostics and/or treatments.After discharge,a strict follow-up through outpatient clinic or telemedicine is can improve quality of life and reduce rehospitalisation rates.The aim of this review is to offer an insight on current literature and provide a clinically oriented,patient-tailored approach regarding assessment,treatment and follow-up of elderly patients admitted for AHF. | Gregorio Tersalvi Alessio Gasperetti Marco Schiavone Jeroen Dauw Cecilia Gobbi Marialessia Denora Joel Daniel Krul Giacomo Maria Cioffi Gianfranco Mitacchione Giovanni B.Forleo | 2021 | Journal of Geriatric Cardiology2021，18，7： | 4 |
| 3 | Epilepsy triggered by mefloquine in an adult traveler to Uganda显示文摘We report a case of a traveler who visited Uganda for 8 d, and took mefloquine one tablet/week for malaria prophylaxis. After the second dose, he suffered from two episodes of loss of consciousness with seizures, therefore mefloquine was discontinued. During the flight back after full recovery, seizures reoccurred while he was on board, he was disembarked in Addis Ababa and then transferred to Nairobi. After repatriation to Italy, he experienced four other similar episodes. The patient was still on full dose anticonvulsant therapy one year and a half after, as any attempt at reduced dose was unsuccessful. Currently, three agents(mefloquine, atovaquone/proguanil, and doxycycline) are recommended for malaria chemoprophylaxis, with similar efficacy but different adverse event profiles, regimens, and prices.
