1Efficacy of premedication with activated Dimethicone or N-acetylcysteine in improving visibility during upper endoscopy显示文摘AIM:To assess the efficacy of N-acetylcysteine(NAC) and activated Dimethicone in improving endoscopic mucosal visibility.METHODS:A total of 148 patients were randomly allocated into four groups to receive one of the following premedications:group A:100 mL water alone;group B:activated Dimethicone plus water(up to 100 mL);group C:NAC plus water(up to 100 mL);and group D:activated Dimethicone and NAC plus water(up to 100 mL).A single endoscopist blinded to the patients group assessed the gastric mucosal visibility scores(range 1-4) at four sites.The sum of the scores from the four sites was considered as the total mucosal visibility score(TMVS).RESULTS:The patients in group B showed a significantly lower TMVS than those of groups A and C(P < 0.001).The TMVS in patients of group D was significantly lower than that of groups A and C(P < 0.001).The TMVS did not significantly differ between groups B and D(P > 0.05).The difference between TMVS of groups C and A was not significant(P > 0.05).CONCLUSION:Premedication with activated Dimethicone 20 min prior to the upper endoscopy leads to the best visibility.NAC does not improve visualization by itself.Seyed Mohammad Kazem Hosseini Asl Gholam Reza Sivandzadeh 2011World Journal of Gastroenterology2011,17,37:15

3Cyclic vomiting syndrome in children: Experience with 181 cases from southern Iran显示文摘AIM: To evaluate the clinical presentation, response to prophylactic therapy and outcome of children with cyclic vomiting syndrome (CVS) in Shiraz, Iran.METHODS: During a period of 11 years (March 1994 to March 2005), 181 consecutive children with a final diagnosis of CVS were evaluated, treated and followed in our center. Patients were randomized to receive either amitriptyline or propranolol as prophylactic treatments. RESULTS: There were 88 boys and 93 girls with mean age of onset of symptoms of 4.9 ± 3.3 years (range, neonatal period to 14 years), the mean age at final diagnosis was 6.9 years (range, 1.5 to 14), and the mean duration between the onset of the first attack and the final diagnosis of CVS was 2 ± 1.81 years (range, 1/6 to 8). The mean duration of each attack was 4.26 days (range, from few hours to 10 d) and the mean interval between the attacks was 1.8 mo (range, 1 wk to 12 mo). The time of onset of the attacks was midnight to early morning in about 70% of cases. Amitriptyline was effective in 46 out of 81 (56%) patients (P < 0.001). Propranolol appeared to have a superior action and was effective in 74 out of 83 (92%) patients (P < 0.0001).CONCLUSION: There is a significant lag time between the onset of clinical symptoms and the final diagnosis of CVS in our area. In patients with typical clinical presentations of CVS, who are examined by an experienced physician, invasive workup is not necessary. 4Oral manifestation in inflammatory bowel disease:A review显示文摘Inflammatory bowel diseases(IBDs),including Crohn’s disease(CD)and ulcerative colitis,not only affect the intestinal tract but also have an extraintestinal involvement within the oral cavity.These oral manifestations may assist in the diagnosis and the monitoring of disease activity,whilst ignoring them may lead to an inaccurate diagnosis and useless and expensive workups.Indurated tag-like lesions,cobblestoning,and mucogingivitis are the most common specific oral findings encountered in CD cases.Aphthous stomatitis and pyostomatitis vegetans are among non-specific oral manifestations of IBD.In differential diagnosis,side effects of drugs,infections,nutritional deficiencies,and other inflammatory conditions should also be considered.Treatment usually involves managing the underlying intestinal disease.In severe cases with local symptoms,topical and/or systemic steroids and immunosuppressive drugs might be used.Kamran B Lankarani Gholam Reza Sivandzadeh Shima Hassanpour 2013World Journal of Gastroenterology2013,19,46:10
5不同屈膝角度与g值全身振动刺激对下肢肌肉影响的研究显示文摘目的:探讨在不同半蹲屈膝角度情况下接受不同重力加速度(g)的全身振动训练(Wholebodyvibration,WBV),下肢股内侧肌(Vastusmedium,VM)与股二头肌(Bicepesfemoris,BF)激活表现以及两者共同收缩变化.方法:46名健康男性大学生,以表面肌电仪监测惯用腿VM与BF肌电反应.先以3种角度(90、60、45°)半蹲姿势进行无振动刺激(NonVibration,NV)测试,再以相同3种屈膝角度接受WBV,振动刺激为随机采用0.98、2.52、3.72、5.61等4种g值.统计学显著性水平为p〈0.05,以配对样本T检验比较NV与WBV的肌电信号.因变量为NV与WBV的标准化肌电比例(%MVC),自变量为g值与屈膝角度.以双因素方差分析比较不同g值、角度的肌电反应差异以及共同收缩比例,主效应检验g值与屈膝角度对肌电信号的影响,交互作用检验不同g值振动刺激下屈膝角度差异对肌电信号的影响.结果:BF%MVC在屈膝60°搭配(0.98~3.72)g与45°搭配(0.98~5.61)g的振动刺激情况下,显著高于NV,而VM在屈膝45°搭配0.98和2.52g时有显著提升肌肉激活的效果.结论:BF与VM的激活程度受半蹲屈膝角度影响较大,而屈膝45°搭配0.98、2.52等2种g值对于BF与VM有较佳激活效果.张帆 王长生 袁艳 祝捷 谷松 Seyed R.Seyedi Gholam R.Zourmand 2014北京体育大学学报2014,37,11:10
6GIS-based spatial analysis of urban traffic accidents: Case study in Mashhad, Iran显示文摘There is a growing concern in traffic accident rate in recent years. Using Mashhad city(Iran second populous city) traffic accident records as case study, this paper applied the combination of geo-information technology and spatial-statistical analysis to bring out the influence of spatial factors in their formation. The aim of the study is to examine 4 clustering analyses to have a better understanding of traffic accidents patterns in complex urban network. In order to deploy the clustering technique in urban roads, 9331 point features for inner city traffic accidents during 12 months have been registered according to their x and y location in geographic information system(GIS). The mentioned areas were analyzed by kernel density estimation(KDE) using ARCMAP and two other analyses using SANET 4th edition software so that the results of network analysis can be compared with traditional KDE method. In addition, this research introduces five classifications for determining the eventfulness of the under study area based on standard deviation and to make priority in creating security in the area.The nearest neighbor and K-function output analysis consist of four curves and regarding the fact that for all fatal, injury and property damage only crashes, the observed value curve is above the 5% confidence interval. Accidents in the study region are more clustered than expected by random chance. 9Extractive oxidative desulfurization of model oil/crude oil using KSF montmorillonite-supported 12-tungstophosphoric acid显示文摘12-Tungstophosphoric acid(PW) supported on KSF montmorillonite, PW/KSF, was used as catalyst for deep oxidative desulfurization(ODS) of mixed thiophenic compounds in model oil and crude oil under mild conditions using hydrogen peroxide(H_2O_2) as an oxidizing agent. A one-factor-at-a-time method was applied for optimizing the parameters such as temperature, reaction time, amount of catalyst, type of extractant and oxidant-tosulfur compounds(S-compounds) molar ratio. The corresponding products can be easily removed from the model oil by using ethanol as the best extractant. The results showed high catalytic activity of PW/KSF in the oxidative removal of dibenzothiophene(DBT) and mixed thiophenic model oil under atmospheric pressure at 75 ℃ in a biphasic system. 12Analysis of dynamic wave model for flood routing in natural rivers显示文摘Flooding is a common natural disaster that causes enormous economic, social, and human losses. Of various flood routing methods, the dynamic wave model is one of the best approaches for the prediction of the characteristics of floods during their propagations in natural rivers because all of the terms of the momentum equation are considered in the model. However, no significant research has been conducted on how the model sensitivity affects the accuracy of the downstream hydrograph. In this study, a comprehensive analysis of the input parameters of the dynamic wave model was performed through field applications in natural rivers and routing experiments in artificial channels using the graphical multi-parametric sensitivity analysis (GMPSA). The results indicate that the effects of input parameter errors on the output results are more significant in special situations, such as lower values of Manning's roughness coefficient and/or a steeper bed slope on the characteristics of a design hydrograph, larger values of the skewness factor and/or time to peak on the channel characteristics, larger values of Manning's roughness coefficient and/or the bed slope on the space step, and lower values of Manning's roughness coefficient and/or a steeper bed slope on the time step and weighting factor.Reza BARATI Sajjad RAHIMI Gholam Hossein AKBARI 2012Water Science and Engineering2012,5,3:3

14Detection of leptospiral antibodies by microscopic agglutination test in north-east of Iran显示文摘Objective:To detect leptospiral antibodies by microscopic agglutination test(MAT) in northeast of Iran.Methods:This study was conducted to evaluate prevalence of human leptospiral infections by MAT.using six current reference strains of Leptospira interrogans in north-east of Iran.A total of 285 serum samples were collected from three north-east provinces of Iran,from December,2009 to June,2010.Results:Antibodies were detected at least against one serovar of Leptospira interrogans in 45 sera(15.79%) among 283 samples at a dilution 1:100 or greater.Positive titers against more than one serovar were detected in 24 sera of the positive samples.Therefore, there were 75 positive reactions against different serovar of Leptospira interrogans.Positive titers were recorded against serovar icterohaemorrhagiae(31 samples),hardjo(26 samples), grippotyphosa(7 samples),pomona(5 samples),canicola(4 samples) and ballum(2.sample). 19A Cu-Cr alloy with nano and microscale Cr particles produced in a water-cooled copper mold显示文摘Microstructures have profound effects on the hardness and strength of Cu-Cr alloys. The microstructures of a Cu-Cr alloy cast in a water-cooled copper mold were studied in the present work. The scanning electron microscopy (SEM) results show that there are the copper matrix saturated with chromium, spherical precipitates of chromium separated from liquid phase during cooling before the initiation of so- lidification, and a eutectic phase in grain boundary areas. To investigate the effect of age-hardening treatment on the microstructures and properties of the material, some samples were subsequently age-hardened in a salt bath and investigated by transmission electron microscopy (TEM). The results show that coherent precipitates with the diameter of 11 nm are detectable in the samples before and after the age-hardening stage.
