1Predictors of esophageal varices and first variceal bleeding in liver cirrhosis patients显示文摘AIM To assess 'predictors' of esophageal varices(EV) and variceal bleeding using non-invasive markers in Albanian patients diagnosed with liver cirrhosis.METHODS One hundred thirty-nine newly diagnosed cirrhotic patients without variceal bleeding were included in this analysis. Model for end-stage liver disease(MELD), aspartate aminotransferase(AST) to alanine aminotransferase(ALT) ratio(AST/ALT), AST to platelet ratio index(APRI), platelet count to spleen diameter(PC/SD), fibrosis-4-index(FIB-4), fibrosis index(FI) and King's Score were measured for all participants. All patients underwent endoscopic assessment within two days of hospitalization. The major end point was the first esophageal variceal bleeding(EVB) event. The diagnostic performance of 'predictors' for the presence of EV and EVB were assessed by sensitivity and specificity values obtained from the receiver operating characteristics procedure.RESULTS FIB-4 was the only strong and significant 'predictor' of esophageal varices(multivariable-adjusted OR = 1.57 for one unit increment; 95%CI: 1.15-2.14). Furthermore, a cut-off value of 3.23 for FIB-4 was a significant predictor of esophageal varices, with a sensitivity of 72%, a specificity of 58% and a proportion of area under the curve(AUC) of 66%(P = 0.01). During the follow-up(median: 31.5 mo; interquartile range: 11-59 mo), 34 patients(24%) experienced a first EVB. FIB-4 was a poor predictor of EVB(the AUC was only 51%) for a cut-off value of 5.02. Furthermore, the AUC of AST/ALT, APRI, PC/SD, FI, MELD and King's Score ranged from 45% to 55%. None of the non-invasive markers turned out to be a useful predictor of EVB.CONCLUSION Despite the low diagnostic accuracy, FIB-4 appears the most efficient non-invasive liver fibrosis marker which can be used as an initial screening tool for cirrhotic patients.Bledar Kraja Iris Mone Ilir Akshija Adea Kocollari Skerdi Prifti Genc Burazeri 2017World Journal of Gastroenterology2017,23,26:40

4Spontaneous combustion liability between coal seams: A thermogravimetric study显示文摘The spontaneous combustion liability of coal can be determined by using different experimental techniques.These techniques are well-known in their application,but no certain test method has become a standard to prove the reliability of all of them.A general characterisation which included proximate and ultimate analyses,petrographic properties and spontaneous combustion tests(thermogravimetric analysis(TGA)and the Wits-Ehac tests)were conducted on fourteen coal and four coal-shale samples.The spontaneous combustion liability of these samples collected between coal seams(above and below)were predicted using the TGA and the Wits-Ehac tests.Six different heating rates(3,6,9,15,20 and 25C/min)were selected based on the deviation coefficient to obtain different derivative slopes and a liability index termed the TGspc index.This study found that coal and coal-shale undergo spontaneous combustion between coal seams when exposed to oxygen in the air.Their intrinsic properties and proneness towards spontaneous combustion differ considerably from one seam to the other.The Wits-Ehac test results agreed with the TGspc results to a certain extent and revealed the incidents of spontaneous combustion in the coal mines.Moshood Onifade Bekir Genc Samson Bada 2020International Journal of Mining Science and Technology2020,30,5:6
5Evaluation of line brattice length in an empty heading to improve air flow rate at the face using CFD显示文摘The effectiveness of line brattice(LB) ventilation system depends on the associated system variables.However, the effect of these variables on the air flow rates close to the face of the heading is not extensively studied. In this paper, the effect of the LB length in relation to the LB-wall distance on the air flow rate reaching the face is analysed. Scenarios were developed using four LB lengths, two LB-wall distances and two heading depths. These scenarios were simulated with a validated CFD model. The air flow rates and patterns at various locations inside the heading were analysed. This helped to find the minimum LB-face distance that should be maintained for each LB-wall distance to maximise the air flow rate at the face. 18Oxidative stress in portal hypertension-induced rats with particular emphasis on nitric oxide and trace metals显示文摘AIM: To investigate the oxidative-stress-related changes in rats with portal hypertension with particular emphasis on nitric oxide (NO) and trace metals. METHODS: Cirrhosis was induced by partial portal vein ligation (PVL) in Wistar rats. The lipid peroxidation marker (malondialdehyde, MDA), antioxidant defense enzymes including superoxide dismutase (SOD), catalase (CAT), glutathione (GSH), and agents known to have antioxidant features including nitric oxide (NO), zinc (Zn), copper (Cu) were determined both in serum and in liver tissue at 4 wk after surgery in PVL and sham-operated rats. Portal pressure of all experimental animals was measured. MDA was detected by thiobarbituric acid reactivity assay.SOD activity was determined by inhibition of nitroblue tetrazolium reduction with xanthine/xanthine oxidase used asa superoxide generator. CAT activity was determined by the breakdown of hydrogen peroxide. GSH concentrations were measured by using metaphosphoric acid for protein precipitation and 5'-5'-dithio-bis-2-nitrobenzoic acid for color development. NO was detected by the Griess method after reduction of nitrate to nitrite with nitrate reductase, and the concentrations of Zn and Cu were measured by a Shimadzu 680 AA atomic absorption spectrometer. Histopathological confirmation was done under light microscope. Statistical analyses were done by Student's t-test, and significance of the difference was tested by the unpaired Mann-Whitney test. P<0.05 was considered statistically significant.RESULTS: Histopathological studies confirmed PVL induced cirrhotic changes. There was a statistically significant difference in portal pressure between PVL and control groups (P<0.001). The results showed significant increases in the levels of MDA and NO in both tissue and serum (P<0.05 and P<0.001, respectively in tissue; P<0.001 for each in serum), and Zn only in tissue (P<0.001)in rats with PVL compared with sham-operated rats. Besides, PVL rats exhibited reduced plasma and tissue GSH, CAT, SOD (P<0.001 for each).
