| 2 | Cancer immunotherapy in clinical practice—the past,present,and future显示文摘Considerable progress has been made in the field of cancer immunotherapy in recent years. This has been made possible in large part by the identification of new immune-based cellular targets and the development of novel approaches aimed at stimulating the immune system. The role played by the immunosuppressive microenvironment in the development of tumors has been established. The success of checkpoint-inhibiting antibodies and cancer vaccines has marked the beginning of a new era in cancer treatment. This review highlights the clinically relevant principles of cancer immunology and various immunotherapeutic approaches that have either already entered mainstream oncologic practice or are currently in the process of being evaluated in clinical trials. | 4 | Safe laparoscopic cholecystectomy:Adoption of universal culture of safety in cholecystectomy显示文摘The incidence of biliary injury after laparoscopic cholecystectomy(LC) has shown a declining trend though it may still be twice that as with open cholecystectomy. Major biliary or vasculobiliary injury is associated with significant morbidity. As prevention is the best strategy, the concept of a culture of safe cholecystectomy has been recently introduced to educate surgeons and apprise them of basic tenets of safe performance of LC. Various aspects of safe cholecystectomy include:(1) thorough knowledge of relevant anatomy, various anatomical landmarks, and anatomical variations;(2) an understanding of the mechanisms involved in biliary/vascular injury, the most important being the misidentification injury;(3) identification of various preoperative and intraoperative predictors of difficult cholecystectomy;(4) proper gallbladder retraction;(5) safe use of various energy devices;(6) understanding the critical view of safety, including its doublet view and documentation;(7) awareness of various error traps(e.g., fundus first technique);(8) use of various bailout strategies(e.g., subtotal cholecystectomy) in difficult gallbladder cases;(9) use of intraoperative imaging techniques(e.g., intraoperative cholangiogram) to ascertain correct anatomy; and(10) understanding the concept of time-out.Surgeons should be facile with these aspects of this culture of safety in cholecystectomy in an attempt to reduce the incidence of biliary/vascular injury during LC. | Vishal Gupta Gaurav Jain | 2019 | World Journal of Gastrointestinal Surgery2019,11,2: | 15 |

The malignant potential of GISTs is variable ranging from small lesions with a benign behaviour to fatal sarcomas. The majo-rity of the tumours stain positively for the CD-117(KIT) and discovered on GIST-1(DOG-1 or anoctamin 1) expression, and they are characterized by the presence of a driver kinase-activating mutation in either KIT or platelet-derived growth factor receptor α. Although surgery is the primary modality of treatment, almost half of the patients have disease recurrence following surgery, which highlights the need for an effective adjuvant therapy. Traditionally, GISTs are considered chemotherapy and radiotherapy resistant. With the advent of targeted therapy(tyrosine kinase inhibitors), there has been a paradigm shift in the management of GISTs in the last decade. | 15 | Virtual ballistic impact testing of Kevlar soft armor:Predictive and validated finite element modeling of the V0-V100 probabilistic penetration response显示文摘This works presents the first fully validated and predictive capability to model the V_0-V_(100) probabilistic penetration response of a woven fabric using a yarn-level fabric finite element model. The V_0-V_(100) curve describes the probability of complete fabric penetration as a function of projectile impact velocity. The exemplar case considered in this paper comprises of a single-layer, fully-clamped, plain-weave Kevlar fabric impacted at the center by a 17-gr, 0.22 cal FSP or fragment-simulating projectile. Each warp and fill yarn in the fabric is individually modeled using 3 D finite elements and the virtual fabric microstructure is validated in detail against the experimental fabric microstructure. Material and testing sources of statistical variability including yarn strength and modulus, inter-yarn friction, precise projectile impact location, and projectile rotation are mapped into the finite element model. A series of impact simulations at varying projectile impact velocities is executed using LS-DYNA on the fabric models, with each model comprising unique mappings. The impact velocities together with the outcomes(penetration, nonpenetration) are used to generate the numerical V_0-V_(100) curve which is then validated against the experimental V_0-V_(100) curve. The numerical Vi-Vrdata(impact, residual velocities) is also validated against the experimental Vi-Vrdata. | 17 | Incremental value of magnetic resonance neurography of Lumbosacral plexus over non-contributory lumbar spine magnetic resonance imaging in radiculopathy: A prospective study显示文摘AIM: To test the incremental value of 3T magnetic resonance neurography(MRN) in a series of unilateral radiculopathy patients with non-contributory magnetic resonance imaging(MRI).METHODS: Ten subjects(3 men,7 women; mean age54 year and range 22-74 year) with unilateral lumbar radiculopathy and with previous non-contributory lumbar spine MRI underwent lumbosacral(LS) plexus MRN over a period of one year. Lumbar spine MRI performed as part of the MRN LS protocol as well as bilateral L4-S1 nerves,sciatic,femoral and lateral femoral cutaneous nerves were evaluated in each subject for neuropathy findings on both anatomic(nerve signal,course and caliber alterations) and diffusion tensor imaging(DTI)tensor maps(nerve signal and caliber alterations).Minimum fractional anisotropy(FA) and mean apparent diffusion coeffcient(ADC) of L4-S2 nerve roots,sciatic and femoral nerves were recorded.RESULTS: All anatomic studies and 80% of DTI imaging received a good-excellent imaging quality grading. In a blinded evaluation,all 10 examinations demonstrated neural and/or neuromuscular abnormality corresponding to the site of radiculopathy. A number of contributory neuropathy findings including double crush syndrome were observed. On DTI tensor maps,nerve signal and caliber alterations were more conspicuous. | 20 | Effects of ply orientation and material on the ballistic impact behavior of multilayer plain-weave aramid fabric targets显示文摘Virtual testing of fabric armor provides an efficient and inexpensive means of systematically studying the influence of various architectural and material parameters on the ballistic impact behavior of woven fabrics, before actual laboratory prototypes are woven and destructively tested. In this finite element study, the combined effects of individual ply orientations and material properties on the impact performance of multi-layered, non-stitched woven aramid fabrics are studied using 2-and 4-sided clamping configurations. Individual ply orientations of 0°, ±15°, ±30°, and ±45° are considered along with three levels of inter-yarn friction coefficient. Functionally graded fabric targets are also considered wherein the yarn stiffness progressively increases or decreases through the target thickness while keeping the yarn strain energy density constant and with all other material and architectural parameters unchanged for consistency. For each target configuration, one non-penetrating and one penetrating impact velocity is chosen. The impact performance is evaluated by the time taken to arrest the projectile and the backface deformation for the non-penetrating impacts, and by the residual velocity for the penetrating impact tests. All deterministic impact simulations are performed using LS-DYNA. 2-sided clamped targets and lower inter-yarn frictional levels generally resulted in better impact performance.The functionally graded targets generally showed either similar or inferior impact performance than the baseline fabric target configurations for the non-penetrating shots.
