| 1 | Rubber band ligation for 750 cases of symptomatic hemorrhoids out of 2200 cases显示文摘AIM: To study the results for the treatment of symptomatic hemorrhoids using rubber band ligation (RBL) method. METHODS: A retrospective study for 750 patients who came to the colorectal unit from June, 1998 to September, 2006, data was retrieved from archived fi les. RBL was performed using the Mc Gown applicator on an outpatient basis. The patients were asked to return to out-patient clinic for follow up at 2 wk, 1 mo, 6 mo and through telephone call every 6 mo for 2 years). RESULTS: After RBL, 696 patients (92.8%) were cured with no difference in outcome for second or third degree hemorrhoids (P = 0.31). Symptomatic recurrence was detected in 11.04% after 2 years. A total of 52 patients (6.93%) had 77 complications from RBL which required no hospitalization. Complications were pain, rectal bleeding and vaso-vagal symptoms(4.13%, 4.13% and 1.33% of patients, respectively). At 1 mo there were a significant improvement in mean SF-36 scores over baseline in five items, while after 2 years there were improvement in all items over baseline, but not significant. No significant manometeric changes after band ligation. CONCLUSION: RBL is a simple, safe and effective method for treating symptomatic second and third degree hemorrhoids as an out patient procedure with signifi cant improvement in quality of life. | 3 | Impact of land uses on water quality in Malaysia: a review显示文摘Land use changes in urbanization,industrialization,and agricultural processes will continue to have negative impacts on water quality at all scales.The impact of land use changes on water quality is generally studied by analyzing the relationships between land use and water quality indicators.Therefore,the purpose of this research was to review and analyze the main relationships between land use and water quality,as well as to visualize the major sources and processes of water quality pollution in Malaysia.To achieve our goal,we evaluated the significance of both land use and water quality attributes used in the past studies and correlated them to understand their relationship from another angle of view.The results revealed that 87%of the reviewed studies indicated urban land use as a major source of water pollution,while 82%indicated agricultural land use,77%indicated forest land use,and 44%indicated other land uses.However,the results of correlation analysis showed that agricultural and forest-related activities more affected water quality through their significant positive correlation with physical and chemical indicators of water quality,while urban development activities had a greater impact on water quality through altering hydrological processes such as runoff and erosion.These findings would provide decision-makers with useful information for managing water pollution processes rather than sources only. | Moriken Camara Nor Rohaizah Jamil Ahmad Fikri Bin Abdullah | 2019 | Ecological Processes2019,8,1: | 8 |
| 4 | Geotechnical investigations and remediation design for failure of tunnel portal section: a case study in northern Turkey显示文摘Mass movements are very common problems in the eastern Black Sea region of Turkey due to its climate conditions, geological, and geomorphological characteristics. High slope angle, weathering, dense rainfalls, and anthropogenic impacts are generally reported as the most important triggering factors in the region. Following the portal slope excavations in the entrance section of Cankurtaran tunnel, located in the region, where the highly weathered andesitic tuff crops out, a circular toe failure occurred. The main target of the present study is to investigate the causes and occurrence mechanism of this failure and to determine the feasible remedial measures against it using finite element method(FEM) in four stages. These stages are slope stability analyses for pre-and postexcavation cases, and remediation design assessments for slope and tunnel. The results of the FEM-SSR analyses indicated that the insufficient initial support design and weathering of the andesitic tuffs are the main factors that caused the portal failure. After installing a rock retaining wall with jet grout columns and reinforced slope benching applications, the factor of safety increased from 0.83 to 2.80. In addition toslope stability evaluation, the Rock Mass Rating(RMR), Rock Mass Quality(Q) and New Austrian Tunneling Method(NATM) systems were also utilized as empirical methods to characterize the tunnel ground and to determine the tunnel support design. The performance of the suggested empirical support design, induced stress distributions and deformations were analyzed by means of numerical modelling. Finally, it was concluded that the recommended stabilization technique was essential for the dynamic long-term stability and prevents the effects of failure. | 7 | Optimizing the value of measuring inferior vena cava diameter in shocked patients显示文摘Point-of-care ultrasound has been increasingly used in evaluating shocked patients including the measurement of inferior vena cava(IVC) diameter. Operators should standardize their technique in scanning IVC. Relativechanges are more important than absolute numbers. We advise using the longitudinal view(B mode) to evaluate the gross collapsibility, and the M mode to measure the IVC diameter. Combining the collapsibility and diameter size will increase the value of IVC measurement. This approach has been very useful in the resuscitation of shocked patients, monitoring their fluid demands, and predicting recurrence of shock. Pitfalls in measuring IVC diameter include increased intra-thoracic pressure by mechanical ventilation or increased right atrial pressure by pulmonary embolism or heart failure. The IVC diameter is not useful in cases of increased intra-abdominal pressure(abdominal compartment syndrome) or direct pressure on the IVC. | 8 | Advanced trauma life support training: How useful it is?显示文摘We have tried in a recently published systematic review(World J of Surg 2014; 38: 322-329) to study the educational value of advanced trauma life support(ATLS) courses and whether they improve survival of multiple trauma patients. This Frontier article summarizes whatwe have learned and reflects on future perspectives in this important area. Our recently published systematic review has shown that ATLS training is very useful from an educational point view. It significantly increased knowledge, and improved practical skills and the critical decision making process in managing multiple trauma patients. These positive changes were evident in a wide range of learners including undergraduate medical students and postgraduate residents from different subspecialties. In contrast, clear evidence that ATLS training reduces trauma death is lacking. It is obvious that it is almost impossible to perform randomized controlled trials to study the effect of ATLS courses on trauma mortality. Studying factors predicting trauma mortality is a very complex issue. Accordingly, trauma mortality does not depend solely on ATLS training but on other important factors, like presence of well-developed trauma systems including advanced pre-hospital care. | 10 | Radioembolisation and portal vein embolization before resection of large hepatocellular carcinoma显示文摘Resectability of hepatocellular carcinoma in patients with chronic liver disease is dramatically limited by the need to preserve sufficient remnant liver in order to avoid postoperative liver insufficiency. Preoperative treatments aimed at downsizing the tumor and promoting hypertrophy of the future remnant liver may improve resectability and reduce operative morbidity. Here we report the case of a patient with a large hepatocellular carcinoma arising from chronic liver disease. Preoperative treatment, including tumor downsizing with transarterial radioembolization and induction of future remnant liver hypertrophy with right portal vein embolization, resulted in a 53% reduction in tumor volume and compensatory hypertrophy in the contralateral liver. The patient subsequently underwent extended right hepatectomy with no postoperativesigns of liver decompensation. Pathological examination demonstrated a margin-free resection and major tumor response. | 11 | Epidemiology of isolated hand injuries in the United Arab Emirates显示文摘AIM To provide suggestions for hand injury prevention by study the demography and risk factors of casualties suffering from isolated hand injuries.METHODS All trauma patients with isolated hand injuries who were admitted to Al Ain Hospital for more than 24 h during a period of 3 years were studied. Patient demographics,location, mechanism/time of injury, and length of hospital stay were all analyzed.RESULTS Two hundred and ten patients were studied. Their mean age was 29.7 years. Males constituted 92%. Sixty-five point one percent of all cases were from the Indian subcontinent. The workplace was the most common location of injury(67.1%), followed by the home(17.1%)and road(6.2%). Machinery caused 36.2% of all injuries,followed by heavy object(20.5%) and fall(11%). Cases injured at home were young(P < 0.0001) with an associated higher incidence of females(P < 0.0001).CONCLUSION Male workers in Al Ain city are at greater risk of sustaining hand injuries, predominantly from machinery.
