| 1 | Dietary approach and gut microbiota modulation for chronic hepatic encephalopathy in cirrhosis显示文摘Hepatic encephalopathy(HE)is a common and serious neuropsychiatric complication of cirrhosis,acute liver failure,and porto-systemic shunting.HE largely contributes to the morbidity of patients with liver disease,severely affecting the quality of life of both patients and their relatives and being associated with poor prognosis.Its presentation is largely variable,manifesting with a broad spectrum of cognitive abnormalities ranging from subtle cognitive impairment to coma.The pathogenesis of HE is complex and has historically been linked with hyperammonemia.However,in the last years,it has become evident that the interplay of multiple actors,such as intestinal dysbiosis,gut hyperpermeability,and neuroinflammation,is of crucial importance in its genesis.Therefore,HE can be considered a result of a dysregulated gut-liverbrain axis function,where cognitive impairment can be reversed or prevented by the beneficial effects induced by“gut-centric”therapies,such as non-absorbable disaccharides,non-absorbable antibiotics,probiotics,prebiotics,and fecal microbiota transplantation.In this context dietary modifications,by modulating the intestinal milieu,can also provide significant benefit to cirrhotic patients with HE.This review will provide a comprehensive insight into the mechanisms responsible for gut-liver-brain axis dysregulation leading to HE in cirrhosis.Furthermore,it will explore the currently available therapies and the most promising future treatments for the management of patients with HE,with a special focus on the dietary approach. | Daniela Campion Ilaria Giovo Paola Ponzo Giorgio M Saracco Federico Balzola Carlo Alessandria | 2019 | World Journal of Hepatology2019，11，6： | 8 |

| 3 | Split liver transplantation in adults显示文摘Split liver transplantation(SLT),while widely accepted in pediatrics,remains underutilized in adults. Advancements in surgical techniques and donor-recipient matching,however,have allowed expansion of SLT from utilization of the right trisegment graft to now include use of the hemiliver graft as well. Despite less favorable outcomes in the early experience,better outcomes have been reported by experienced centers and have further validated the feasibility of SLT. Importantly,more than two decades of experience have identified key requirements for successful SLT in adults. When these requirements are met,SLT can achieve outcomes equivalent to those achieved with other types of liver transplantation for adults. However,substantial challenges,such as surgical techniques,logistics,and ethics,persist as ongoing barriers to further expansion of this highly complex procedure. | 7 | Prognostic criteria for postoperative mortality in 170 patients undergoing major right hepatectomy显示文摘BACKGROUND: Postoperative hepatic failure is a dreadful complication after major hepatectomy and carries high morbidity and mortality rates. In this study, we assessed the accuracy of the 50/50 criteria (bilirubin >2.9 mg/dL and international normalized ratio >1.7 on postoperative day 5) and the Mullen criteria (bilirubin peak >7 mg/dL on postoperative days 1-7) in predicting death from hepatic failure in patients undergoing right hepatectomy only. In addition, we identified prognostic factors linked to intra-hospital morbidity and mortality in these patients. METHODS: One hundred seventy consecutive patients underwent major right hepatectomy at a tertiary medical center from 2000 to 2008. Nineteen (11.2%) patients suffered from liver cirrhosis. Univariate and multivariate analyses were performed to identify predictors of intra-hospital mortality, morbidity and death from hepatic failure. RESULTS: The intra-hospital mortality was 6.5% (11/170). Of the six patients who died from hepatic failure, one was positive for the 50/50 criteria, but all six patients were positive for the Mullen criteria. Multivariate analysis showed that male gender, hepatitis C (HCV), hepatocellular carcinoma, postoperative bilirubin >7 mg/dL and ALT<188 U/L on postoperative day 1 were predictive of death from hepatic failure in the postoperative period. Age >65 years, HCV, reoperation, andrenal failure were significant predictors of overall intra-hospital mortality on multivariate analysis. CONCLUSIONS: The Mullen criteria were more accurate than the 50/50 criteria in predicting death from hepatic failure in patients undergoing right hepatectomy.
