1Chronic complications of spinal cord injury显示文摘Spinal cord injury(SCI) is a serious medical condition that causes functional, psychological and socioeconomic disorder. Therefore, patients with SCI experience significant impairments in various aspects of their life. The goals of rehabilitation and other treatment approaches in SCI are to improve functional level, decrease secondary morbidity and enhance health-relatedquality of life. Acute and long-term secondary medical complications are common in patients with SCI. However, chronic complications especially further negatively impact on patients' functional independence and quality of life. Therefore, prevention, early diagnosis and treatment of chronic secondary complications in patients with SCI is critical for limiting these complications, improving survival, community participation and health-related quality of life. The management of secondary chronic complications of SCI is also important for SCI specialists, families and caregivers as well as patients. In this paper, we review data about common secondary longterm complications after SCI, including respiratory complications, cardiovascular complications, urinary and bowel complications, spasticity, pain syndromes, pressure ulcers, osteoporosis and bone fractures. 2Microbial profile and antibiotic sensitivity pattern in bile cultures from endoscopic retrograde cholangiography patients显示文摘AIM:To identify the frequency of bacterial growth,the most commonly grown bacteria and their antibiotic susceptibility,and risk factors for bacterial colonization in bile collected from patients with different biliary diseases.METHODS:This prospective study was conducted between April 2010 and August 2011.Patients with various biliary disorders were included.Bile was aspirated by placing a single-use,5F,standard sphincterotome catheter into the bile duct before the injection of contrast agent during endoscopic retrograde cholangiopancreaticography (ERCP).Bile specimens were transported to the microbiology laboratory in blood culture bottles within an anaerobic transport system.Bacteria were cultured and identified according to the standard protocol used in our clinical microbiology laboratory.The susceptibilities of the organisms recovered were identified using antimicrobial disks,chosen according to the initial gram stain of the positive cultures.RESULTS:Ninety-one patients (27% male,mean age 53.7 ± 17.5 years,range:17-86 years) were included in the study.The main indication for ERCP was benign biliary disease in 79 patients and malignant disease in 12 patients.The bile culture was positive for bacterial growth in 46 out of 91 (50.5%) patients.The most frequently encountered organisms were Gram-negative bacteria including Escherichia coli (28.2%),Pseudomonas (17.3%) and Stenotrophomonas maltophilia (15.2%).There were no significant differences between patients with malignant and benign disease (58% vs 49%,P = 0.474),patients with acute cholangitis and without acute cholangitis (52.9% vs 50%,P = 0.827),patients who were empirically administered antibiotics before intervention and not administered (51.4% vs 60.7%,P = 0.384),with regard to the bacteriobilia.We observed a large covering spectrum or low resistance to meropenem,amikacin and imipenem.CONCLUSION:We did not find a significant risk factor for bacteriobilia in patients with biliary obstruction.A bile sample for microbiological analysis may become a valuable diagnostic tool as it leads to more accurate selection of antibiotics for the treatment of cholangitis.Muhsin Kaya Remzi Bestas Fatma Bacalan Ferhat Bacaksz Esma Gülsun Arslan Mehmet Ali Kaplan 2012World Journal of Gastroenterology2012,18,27:13
3Treatment with NADPH oxidase inhibitor apocynin alleviates diabetic neuropathic pain in rats显示文摘Increased reactive oxygen species by the activation of NADPH oxidase(NOX) contributes to the development of diabetic complications.Apocynin,a NOX inhibitor,increases sciatic nerve conductance and blood flow in diabetic rats.We investigated potential protective effect of apocynin in rat diabetic neuropathy and its precise mechanism of action at molecular level.Rat models of streptozotocin-induced diabetes were treated with apocynin(30 and 100 mg/kg per day,intragastrically) for 4 weeks.Mechanical hyperalgesia and allodynia were determined weekly using analgesimeter and dynamic plantar aesthesiometer.Western blot analysis and histochemistry/immunohistochemistry were performed in the lumbar spinal cord and sciatic nerve respectively.Streptozotocin injection reduced pain threshold in analgesimeter,but not in aesthesiometer.Apocynin treatment increased pain threshold dose-dependently.Western blot analysis showed an increase in catalase and NOX-p47 phox protein expression in the spinal cord.However,protein expressions of neuronal and inducible nitric oxide synthase(n NOS,i NOS),superoxide dismutase,glutathion peroxidase,nitrotyrosine,tumor necrosis factor-α,interleukin-6,interleukin-1β,aldose reductase,cyclooxygenase-2 or MAC-1(marker for increased microgliosis) in the spinal cord remained unchanged.Western blot analysis results also demonstrated that apocynin decreased NOX-p47 phox expression at both doses and catalase expression at 100 mg/kg per day.Histochemistry of diabetic sciatic nerve revealed marked degeneration.n NOS and i NOS immunoreactivities were increased,while S-100 immunoreactivity(Schwann cell marker) was decreased in sciatic nerve.Apocynin treatment reversed these changes dose-dependently.In conclusion,decreased pain threshold of diabetic rats was accompanied by increased NOX and catalase expression in the spinal cord and increased degeneration in the sciatic nerve characterized by increased NOS expression and Schwann cell loss.Apocynin treatment attenuates neuropathic pain by decelerating the increased oxidative stress-mediated pathogenesis in diabetic rats.Murat Olukman Aytül onal Fatma Gül celenk Yigit Uyankgil Türker cavusoglu Neslihan Düzenli Sibel ülker 2018Neural Regeneration Research2018,13,9:4
4Precontoured buttress plate vs reconstruction plate for acetabulum posterior wall fractures: A biomechanical study显示文摘BACKGROUND The purpose of open reduction and internal fixation of acetabulum posterior wall fractures is to restore anatomical structure and stability of the hip joint, in order to start weight bearing as soon as possible and prevent hip arthrosis; restoration of the anatomy should preserve function of the joint as well. Although 'special shaped precontoured plates' have been developed in recent years for surgical treatment of this region, studies comparing the traditional plates with the newly designed precontoured plates are lacking.AIM To evaluate the biomechanical properties of precontoured anatomic buttress and conventional curved reconstruction plates(CCRPs) for posterior wall acetabulum fracture treatment.METHODS Twelve pelvis models were created for testing plate treatment of fracture in the posterior wall of the acetabulum. These 12 pelvis models were used to create 24 hemipelvis models(experimental) by cutting from the sagittal plane and passing over the center of gravity, after which the posterior wall acetabular fractures(of similar type and size) were created. In these experimental models, the right acetabulum was fixed with a 5-hole CCRP, while the left was fixed with a precontoured anatomic buttress plate(PABP). Samples were placed through thetest device and were subjected to static load testing, with a constant testing velocity of 2 mm/min until the load reached 2.3 kN or the acetabular fixation failed. Dynamic tests were also performed with sinusoidal wave load, with a maximal load of 2.3 kN and a load ratio of 0.1.RESULTS The average stiffness values were 460.83 ± 95.47 N/mm for the PABP and 291.99± 118.58 N/mm for the 5-hole CCRP. The precontoured anatomic acetabulum buttress plates had significantly higher rigidity than the CCRPs(P = 0.022). There was a statistically significant difference between the unloaded and 2.3 kN-loaded values of AL(posterosuperior fracture line vertical to the ground surface) and CL(posteroinferior fracture line vertical to the ground surface) parameters for both the PABPs and the 5-hole CCRPs(P = 0.036 and P = 0.045, respectively).According to the static tests, the amount of total displacement was significantly less in the PABPs than in the CCRPs. Comparative analysis of the displacement in the BL(posterior wall fracture line horizontal to the ground) parameter yielded no statistically significant differences between the PABP and the 5-hole CCRPs(P= 0.261).CONCLUSION PABP provides more stable fixation in acetabulum posterior wall fractures than5-hole CCRP, allowing for proximal or distal fracture line screw application without reshaping.Güray Altun Gürsel Saka Teyfik Demir Fatma Kübra Erbay Elibol Mehmet Or?un Polat 2019World Journal of Orthopedics2019,10,5:3
5Laser-assisted dacryocystorhinostomy in nasolacrimal duct obstruction:5-year follow-up显示文摘AIM:To evaluate the long-term sucess rate of laserassisted dacryocystorhinostomy (L-DCR) in patients with nasolacrimal duct obstruction (NDO).METHODS:Forty-one eyes of forty patients aged between 21-85y (mean 56.7y) who underwent L-DCR for the treatment of NDO were included in this retrospective,non-randomized study. The follow-up time was 72mo.Functional sucess was defined as the disappearance of epiphora under normal conditions and the presence of a patent ostium on lacrimal irrigation. Anatomical success was defined as a patent lacrimal passage on syringing besides continuing epiphora. Surgical failure was defined as persistent epiphora and closed ostium.RESULTS:Twenty-seven of 40 patients (67.5%) were female and 13 of 40 patients (32.5%) were male. The NDO was right-sided in 17 (42.5%) patients and left-sided in 22(55%) patients whereas 1 (2.5%) patient had undergone bilateral surgery. In 11 (27.5%) patients there were additional nasal abnormalities requiring simultaneous surgical approach. The average time for L-DCR was 26.50±4.9min(16-39min) and the average total amount of laser energy used was 287±27.9 J (239-367 J). At the 5y follow-up, anatomical sucess rate was 75.0%(30 patients) and functional success rate was 65.0%(26 patients), whereas surgical failure was seen in 25%(10 patients). Revision of surgery was performed in 10 cases (25.0%); failure of revision surgery was seen in 2 cases(5.0%).CONCLUSION: Transcanalicular L-DCR is a reliable and fast procedure in the treatment of NDO. It can be alternative to external DCR which is accepted as the gold standard currently. 9Effects of moxifloxacin exposure on the conjunctival flora and antibiotic resistance profile following repeated intravitreal injections显示文摘AIM:To evaluate the effects of moxifloxacin exposure on the conjunctival flora and antibiotic resistance profile following repeated intravitreal injections.METHODS:Seventy-two eyes of 36 patients [36 eyes in control group, 36 eyes in intravitreal injection(IVI) group]were enrolled in the study. All the eyes had at least one IVI and had diabetic macular edema(DME) or age-related macular degeneration(ARMD). Moxifloxacin was prescribed to all the patients four times a day for five days following injection. Conjunctival cultures were obtained from the lower fornix via standardized technique with every possible effort made to minimize contamination from the lids, lashes, or skin. Before the application of any ophthalmic medication, conjunctival cultures were obtained from both eyes using sterile cotton culture. An automated microbiology system was used to identify the growing bacteria and determine antibiotic sensitivity.RESULTS:The bacterial cultures were isolated from 72 eyes of 36 patients, sixteen of whom patients(44.4%)were male and twenty(55.6%) were female. Average age was 68.4 ±9.0(range 50-86). The average number of injections before taking cultures was 3.1+1.0. Forty-eight(66.7%) of 72 eyes had at least one significant organism.There was no bacterial growth in 8(20.5%) of IVI eyes and in 16(44.4%) of control eyes(P =0.03). Of the bacteria isolated from culture, 53.8% of coagulase negative staphylococci(CoNS) in IVI eyes and 47.2%CoNS in control eyes. This difference between IVI eyes and control eyes about bacteria isolated from culture was not statistically significant(P =0.2). Eleven of 25 bacteria(44.0%) isolated from IVI eyes and 11(57.9%) of 19 bacteria isolated from control eyes were resistant to oxacillin. The difference in frequency of moxifloxacine resistance between two groups was not statistically significant(12.0% in IVI eyes and 21.1% in control eyes)(P =0.44). There were no cases of resistance to vancomycin, teicoplanin and linezolid.CONCLUSION:There was no difference in species of bacteria isolated from cultures, or in the frequency of resistance to antibiotics between eyes that had recurrent IVI followed by moxifloxacin exposure compared with control eyes. 12Goblet cell carcinoid of the appendix and mixed adenoneuroendocrine carcinoma: Report of three cases显示文摘Neuroendocrine neoplasms are the most common epithelial tumors among appendix tumors. Appendix tumors that are completely or partially composed of neuroendocrine cells are divided into two categories: Classic carcinoid tumors and goblet cell carcinoid tumors(GCCT). They are known to progress more aggressively than classic(neuro) endocrine tumors. In this study, three cases with acute appendicitis symptoms are presented, including their clinical and histopathological findings.
