1Do age-associated changes of voltage-gated sodium channel isoforms expressed in the mammalian heart predispose the elderly to atrial fibrillation?显示文摘Atrial fibrillation(AF)is the most common cardiac arrhythmia worldwide.The prevalence of the disease increases with age,strongly implying an age-related process underlying the pathology.At a time when people are living longer than ever before,an exponential increase in disease prevalence is predicted worldwide.Hence unraveling the underlying mechanics of the disease is paramount for the development of innovative treatment and prevention strategies.The role of voltage-gated sodium channels is fundamental in cardiac electrophysiology and may provide novel insights into the arrhythmogenesis of AF.Na_v1.5 is the predominant cardiac isoform,responsible for the action potential upstroke.Recent studies have demonstrated that Na_v1.8(an isoform predominantly expressed within the peripheral nervous system)is responsible for cellular arrhythmogenesis through the enhancement of pro-arrhythmogenic currents.Animal studies have shown a decline in Na_v1.5 leading to a diminished action potential upstroke during phase 0.Furthermore,the study of human tissue demonstrates an inverse expression of sodium channel isoforms;reduction of Na_v1.5 and increase of Na_v1.8 in both heart failure and ventricular hypertrophy.This strongly suggests that the expression of voltage-gated sodium channels play a crucial role in the development of arrhythmias in the diseased heart.Targeting aberrant sodium currents has led to novel therapeutic approaches in tackling AF and continues to be an area of emerging research.This review will explore how voltage-gated sodium channels may predispose the elderly heart to AF through the examination of laboratory and clinical based evidence.Emmanuel Isaac Stephanie M Cooper Sandra A Jones Mahmoud Loubani 2020World Journal of Cardiology2020,12,4:3
2Differential effects of atrial and brain natriuretic peptides on human pulmonary artery:An in vitro study显示文摘BACKGROUND The prevalence of cardiovascular diseases,especially heart failure,continues to rise worldwide.In heart failure,increasing levels of circulating atrial natriuretic peptide(ANP)and brain natriuretic peptide(BNP)are associated with a worsening of heart failure and a poor prognosis.AIM To test whether a high concentration of BNP would inhibit relaxation to ANP.METHODS Pulmonary arteries were dissected from disease-free areas of lung resection,as well as pulmonary artery rings of internal diameter 2.5–3.5 mm and 2 mm long,were prepared.Pulmonary artery rings were mounted in a multiwire myograph,and a basal tension of 1.61gf was applied.After equilibration for 60 min,rings were pre-constricted with 11.21μmol/L PGF2α(EC80),and concentration response curves were constructed to vasodilators by cumulative addition to the myograph chambers.RESULTS Although both ANP and BNP were found to vasodilate the pulmonary vessels,ANP is more potent than BNP.pEC50 of ANP and BNP were 8.96±0.21 and 7.54±0.18,respectively,and the maximum efficacy(Emax)for ANP and BNP was-2.03 gf and-0.24 gf,respectively.After addition of BNP,the Emax of ANP reduced from-0.96gf to-0.675gf(P=0.28).CONCLUSION BNP could be acting as a partial agonist in small human pulmonary arteries,and inhibits relaxation to ANP.Elevated levels of circulating BNP could be responsible for the worsening of decompensated heart failure.This finding could also explain the disappointing results seen in clinical trials of ANP and BNP analogues for the treatment of heart failure.Azar Hussain Robert T Bennett Zaheer Tahir Emmanuel Isaac Mubarak A Chaudhry Syed S Qadri Mahmoud Loubani Alyn H Morice 2019World Journal of Cardiology2019,11,10:1

4Communities and service providers address access to perinatal care in postconflict Northern Uganda:socialising evidence for participatory action显示文摘Objectives Describe participatory codesign of interventions to improve access to perinatal care services in Northern Uganda.Study design Mixed-methods participatory research to codesign increased access to perinatal care.Fuzzy cognitive mapping,focus groups and a household survey identified and documented the extent of obstructions to access.Deliberative dialogue focused stakeholder discussions of this evidence to address the obstacles to access.Most significant change stories explored the participant experience of this process.Setting Three parishes in Nwoya district in the Gulu region,Northern Uganda.Participants Purposively sampled groups of women,men,female youth,male youth,community health workers,traditional midwives and service providers.Each of seven stakeholder categories included 5-8 participants in each of three parishes.Results Stakeholders identified several obstructions to accessing perinatal care:lack of savings in preparation for childbirth in facility costs,lack of male support and poor service provider attitudes.They suggested joining saving groups,practising saving money and income generation to address the short-term financial shortfall.They recommended increasing spousal awareness of perinatal care and they proposed improving service provider attitudes.Participants described their own improved care-seeking behaviour and patient-provider relationships as short-term gains of the codesign.Conclusion Participatory service improvement is feasible and acceptable in postconflict settings like Northern Uganda.Engaging communities in identifying perinatal service delivery issues and reflecting on local evidence about these issues generate workable community-led solutions and increases trust between community members and service providers.Loubna Belaid Pamela Atim Eunice Atim Emmanuel Ochola Martin Ogwang Pontius Bayo Janet Oola Isaac Wonyima Okello Ivan Sarmiento Laura Rojas-Rozo Kate Zinszer Christina Zarowsky Neil Andersson 2021Family Medicine and Community Health2021,9,2:0
