| 1 | Diagnostic value of PIVKA-Ⅱ and alpha-fetoprotein in hepatitis B virus-associated hepatocellular carcinoma显示文摘AIM: To determine the cutoff values and to compare the diagnostic role of alpha-fetoprotein(AFP) and prothrombin induced by vitamin K absence-Ⅱ(PIVKA-Ⅱ) in chronic hepatitis B(CHB).METHODS: A total of 1255 patients with CHB, including 157 patients with hepatocellular carcinoma(HCC), 879 with non-cirrhotic CHB and 219 with cirrhosis without HCC, were retrospectively enrolled. The areas under the receiver operating characteristic(AUROC) curves of PIVKA-Ⅱ, AFP and their combination were calculated and compared.RESULTS: The optimal cutoff values for PIVKA-Ⅱ and AFP were 40 m AU/m L and 10 ng/m L, respectively, for the differentiation of HCC from nonmalignant CHB. The sensitivity and specificity were 73.9% and 89.7%, respectively, for PIVKA-Ⅱ and 67.5% and 90.3% for AFP, respectively. The AUROC curves of both PIVKA-Ⅱ and AFP were not significantly different(0.854 vs 0.853, P = 0.965) for the differentiation of HCC from nonmalignant CHB, whereas the AUROC of PIVKA-Ⅱ was significantly better than that of AFP in patients with cirrhosis(0.870 vs 0.812, P = 0.042). | 2 | Limited endoscopic sphincterotomy plus large balloon dilation for choledocholithiasis with periampullary diverticula显示文摘AIM: To investigate the effectiveness and safety of limited endoscopic sphincterotomy (EST) plus large balloon dilation (LBD) for removing choledocholithiasis in patients with periampullary diverticula (PAD). METHODS: A total of 139 patients with common bile duct (CBD) stones were treated with LBD (10-20 mm balloon diameter) after limited EST. Of this total, 73 patients had PAD and 66 patients did not have PAD (controls). The results of stone removal and complications were retrospectively evaluated. RESULTS: There were no significant differences between the PAD and the control groups in overall successful stone removal (94.5% vs 93.9%), stone removal in first session (69.9% vs 81.8%), mechanical lithotripsy (12.3% vs 13.6%), and complications (11.0% vs 7.6%). Clinical outcomes were also similar between the types of PAD, but the rate of stone removal in first session and the number of sessions were significantly lower and more frequent, respectively, in type B PAD (papilla located near the diverticulum) than controls [23/38 (60.5%) vs 54/66 (81.8%), P = 0.021; and 1 (1-2) vs 1 (1-3), P = 0.037, respectively] and the frequency of pancreatitis was significantly higher in type A PAD (papilla located inside or in the margin of the diverticulum) than in controls (16.1% vs 3.0%, P = 0.047). CONCLUSION: Limited EST plus LBD was an effective and safe procedure for removing choledocholithiasis in patients with PAD. | 4 | Piglet gut microbial shifts early in life:causes and effects显示文摘The gut microbiome has long been known to play fundamentally important roles in the animal health and the well-being of its host. As such, the establishment and maintenance of a beneficial gut microbiota early in life is crucial in pigs, since early gut colonizers are pivotal in the establishment of permanent microbial community structures affecting the health and growth performance of pigs later in life. Emphasizing this importance of early gut colonizers, it is critical to understand the factors impacting the establishment of the piglet gut microbiome at weaning. Factors include, among others, diet, in-feed antibiotics, probiotics and prebiotic administration. The impact of these factors on establishment of the gut microbiome of piglets at weaning includes effects on piglet gut microbial diversity, structure, and succession. In this review, we thoroughly reviewed the most recent findings on the piglet gut microbiome shifts as influenced by weaning, and how these microbiome changes brought about by various factors that have been shown to affect the development of microbiota in piglets. | 6 | Magnesium citrate with a single dose of sodium phosphate for colonoscopy bowel preparation显示文摘AIM:To evaluate the efficacy and acceptability of magnesium citrate and a single dose of oral sodium phosphate(45 mL) solution for morning colonoscopy bowel preparation. METHODS:A total of 159 patients were randomly assigned to receive two split doses of 90 mg of sodium phosphate(GroupⅠ,n=79) or magnesium citrate(250 mL,the day before the procedure) followed by 45 mL of sodium phosphate(the day of procedure,GroupⅡ,n= 80) .The quality of bowel cleansing and the acceptability of each regimen were compared,including the satisfaction,taste,willing to repeat and adverse effects of each regimen. RESULTS:The quality of bowel cleansing of GroupⅡ was as good as that of GroupⅠ(An Aronchick scale score of good or excellent:70.9%vs 81.0%,respectively,P=0.34;the Ottawa system score:4.4±2.6 vs 3.8 ±3.0,respectively,P=0.76) .There was no statisticallysignificant difference between both groups with regard to acceptability,including the satisfaction,taste and willingness to repeat the regimen.A significantly greater number of older patients(over 65 years old) in Group Ⅱgraded the overall satisfaction as satisfactory(48.1% vs 78.1%,respectively;GroupⅠvs GroupⅡ,P=0.01) . There were no significant adverse reactions. | 7 | Hepatic resection vs percutaneous radiofrequency ablation of hepatocellular carcinoma abutting right diaphragm显示文摘BACKGROUND It is usually difficult to adequately conduct percutaneous ultrasound-guided radiofrequency(RF) ablation for hepatocellular carcinomas(HCCs) abutting the diaphragm. Our hypothesis was that the subphrenic location of HCC could have an effect on the long-term therapeutic outcomes after hepatic resection and RF ablation.AIM To compare the long-term therapeutic outcomes of hepatic resection and percutaneous RF ablation for HCCs abutting the diaphragm.METHODS A total of 143 Child-Pugh class A patients who had undergone hepatic resection(n = 80) or percutaneous ultrasound-guided RF ablation(n = 63) for an HCC(≤ 3 cm) abutting the right diaphragm were included. Cumulative local tumor progression(LTP), cumulative intrahepatic distant recurrence(IDR), disease-free survival(DFS), and overall survival(OS) rates were estimated. Prognostic factors for DFS and OS were analyzed. Complications were evaluated.RESULTS The cumulative IDR rate, DFS rate, and OS rate for the hepatic resection group and RF ablation group at 5 years were '35.9% vs 65.8%', '64.1% vs 18.3%', and'88.4% vs 68.7%', respectively. Hepatic resection was an independent prognostic factor for DFS(P ≤ 0.001; hazard ratio, 0.352; 95%CI: 0.205, 0.605; with RF ablation as the reference category); however, treatment modality was not an independent prognostic factor for OS. The LTP rate was 46.6% at 5 years for the RF ablation group. The major complication rate was not significantly different between the groups(P = 0.630). | 8 | Predicting tumor response after preoperative chemoradiation using clinical parameters in rectal cancer显示文摘AIM: To evaluate the clinical parameters and identify a better method of predicting pathological complete response (pCR). METHODS: We enrolled 249 patients from a database of 544 consecutive rectal cancer patients who underwent surgical resection after preoperative chemoradiation therapy (PCRT). A retrospective review of morphological characteristics was then performed to collect data regarding rectal examination findings. A scoring model to predict pCR was then created. To validate the ability of the scoring model to predict complete regression.RESULTS: Seventy patients (12.9%) achieved a pCR. A multivariate analysis found that pre-CRT movability (P = 0.024), post-CRT size (P = 0.018), post-CRT morphology (P = 0.023), and gross change (P = 0.009) were independent predictors of pCR. The accuracy of the scoring model was 76.8% for predicting pCR with the threshold set at 4.5. In the validation set, the accuracy was 86.7%. CONCLUSION: Gross changes and morphological findings are important predictors of pathological response. | 9 | Roles of hepatocyte nuclear factors in hepatitis B virus infection显示文摘Approximately 350 million people are estimated to be persistently infected with hepatitis B virus(HBV) worldwide. HBV maintains persistent infection by employing covalently closed circular DNA(ccc DNA), a template for all HBV RNAs. Chronic hepatitis B(CHB) patients are currently treated with nucleos(t)ide analogs such as lamivudine, adefovir, entecavir, and tenofovir. However, these treatments rarely cure CHB because they are unable to inhibit ccc DNA transcription and inhibit only a late stage in the HBV life cycle(the reverse transcription step in the nucleocapsid). Therefore, an understanding of the factors regulating ccc DNA transcription is required to stop this process. Among numerous factors, hepatocyte nuclear factors(HNFs) play the most important roles in ccc DNA transcription, especially in the generation of viral genomic RNA, a template for HBV replication. Therefore, proper control of HNF function could lead to the inhibition of HBV replication. In this review, we summarize and discuss the current understanding of the roles of HNFs in the HBV life cycle and the upstream factors that regulate HNFs. | 10 | Mitochondria as therapeutic targets for cancer stem cells显示文摘Cancer stem cells(CSCs) are maintained by theirsomatic stem cells and are responsible for tumor initiation, chemoresistance, and metastasis. Evidence for the CSCs existence has been reported for a number of human cancers. The CSC mitochondria have been shown recently to be an important target for cancer treatment, but clinical significance of CSCs and their mitochondria properties remain unclear. Mitochondriatargeted agents are considerably more effective compared to other agents in triggering apoptosis of CSCs, as well as general cancer cells, via mitochondrial dysfunction. Mitochondrial metabolism is altered in cancer cells because of their reliance on glycolytic intermediates, which are normally destined for oxidative phosphorylation. Therefore, inhibiting cancer-specific modifications in mitochondrial metabolism, increasing reactive oxygen species production, or stimulating mitochondrial permeabilization transition could be promising new therapeutic strategies to activate cell death in CSCs as well, as in general cancer cells. This review analyzed mitochondrial function and its potential as a therapeutic target to induce cell death in CSCs. | 11 | Cryptogenic multifocal ulcerous stenosing enteritis: Radiologic features and clinical behavior显示文摘AIM To investigate the characteristic radiologic findings of cryptogenic multifocal ulcerous stenosing enteritis(CMUSE) which can be differentiated from other similar bowel disease and to assess their clinical behavior.METHODS Twenty pathologically and clinically confirmed CMUSE patients(males:females = 8:12; mean age: 40.4 years) between March 2002 and August 2015 from seven academic centers in South Korea were retrospectively reviewed. We evaluated small bowel series(SBS; n = 25), computed tomography(CT) enterography(n = 21), magnetic resonance(MR) enterography(n = 2), and abdominopelvic CT(n = 18) images, focusing on enteric and perienteric manifestations. Any change in radiologic features during followup period was recorded. We evaluated clinical data including presenting symptoms, laboratory finding and presence of relapse from electronic medical records. Histopathologic findings were also evaluated. RESULTS The main symptoms were abdominal pain(n = 12) and anemia(n = 10). All patients showed small bowel strictures(n = 52, mean: 2.6 per patient) on initial CT/MR, located in the ileum(n = 47) or jejunum(n = 5). Strictures showed short-length(mean: 10.44 mm) and circumferential bowel wall thickening(mean: 5.56 mm) with layered enhancement(n = 48) that were also noted on initial SBS(n = 36) with shallow ulcers(n = 10). Some ulcerative lesions or wall thickening progressed into strictures on follow-up SBS/CT, and some strictures revealed recurrent ulceration on followup SBS. There were no penetrating disease features like fistula or abscess and no gastrointestinal tract involvement except the small bowel. Nine patients experienced disease recurrence(median relapse-free period: 32 mo) even post-operatively. | 12 | Radial growth response of Pinus densiflora and Quercus spp.to topographic and climatic factors in South Korea显示文摘Aims This study aimed to develop radial growth models and to predict the potential spatial distribution of Pinus densiflora(Japanese red pine)and Quercus spp.(Oaks)in South Korea,considering topographic and climatic factors.Methods We used a dataset of diameter at breast height and radial growth estimates of individual trees,topographic and climatic factors in systematic sample plots distributed over the whole of South Korea.On the basis that radial growth is attributed primarily to tree age,we developed a radial growth model employing tree age as an explanatory variable.We estimated standard growth(SG),defined as radial growth of the tree at age 30,to eliminate the influence of tree age on radial growth.In addition,SG estimates including the Topographic Wetness Index,temperature and precipitation were calculated by the Generalized Additive Model.Important Findings As a result of variogram analysis of SG,we found spatial autocorrelation between SG,topographic and climatic factors.Incremental temperature had negative impacts on radial growth of P.densiflora and positive impacts on that of Quercus spp.Precipitation was associated with positive effects on both tree species.Based on the model,we found that radial growth of P.densiflora would be more vulnerable than that of Quercus spp.to climatic factors.Through simulation with the radial growth model,it was predicted that P.densiflora stands would be gradually replaced with Quercus spp.stands in eastern coastal and southern regions of South Korea in the future.The models developed in this study will be helpful for understanding the impact of climatic factors on tree growth and for predicting changes in distribution of P.densiflora and Quercus spp.due to climate change in South Korea. | Jae Gyun Byun Woo Kyun Lee Moonil Kim Doo Ahn Kwak Hanbin Kwak Taejin Park Woo Hyuk Byun Yowhan Son Jung Kee Choi Young Jin Lee Joachim Saborowski Dong Jun Chung Jin Hyun Jung | 2013 | Journal of Plant Ecology2013,6,5: | 3 |
| 13 | Paraaortic lymph node metastasis in patients with intra-abdominal malignancies:CT vs PET显示文摘AIM:To compare the diagnostic accuracy of computed tomography(CT)and positron emission tomography (PET)for the preoperative detection of paraaortic lymph node(PAN)metastasis in patients with intraabdominal malignancies. METHODS:Sixty-six patients with intra-abdominal malignancies who underwent both CT and PET before lymphadenectomy were included in this study. Histopathologically,13 patients had metastatic PAN, while 53 had non-metastatic PAN.The CT criteria for metastasis were:short diameter of>8 mm,lobular or irregular shape,and/or combined ancillary findings, including necrosis,conglomeration,vessel encasement, and infiltration.The PET criterion was positive fluorodeoxyglucose(FDG)uptake.The sensitivity, specificity,positive predictive value(PPV),negative predictive value(NPV),and accuracy of both modalities were compared with the pathologic findings,and the false positive and false negative cases with both CT and PET were analyzed.RESULTS:The sensitivity,specificity,PPV,NPV,and accuracy of CT were 61.5%,84.9%,50%,90%and 80.3%,respectively.For PET,the percentages were 46.2%,100%,100%,88.3%,and 89.4%.Additionally, there were 8 false positive CT cases(8/53,15.1%) and zero false positive PET cases.Of the 13 metastatic PANs,there were 5 false negative CT scans(38.5%) and 7(53.9%)false negative PET scans. | 14 | Fatty liver is an independent risk factor for gallbladder polyps显示文摘BACKGROUND Gallbladder polyps(GBPs)are known to be associated with obesity and metabolic diseases.However,to date,the relationship between GBPs and abnormal body fat distribution,such as fatty liver,visceral obesity,or sarcopenia,has not yet been established.AIM To evaluate whether GBPs are associated with fatty liver,visceral obesity,or sarcopenia.METHODS We retrospectively reviewed the medical records of subjects who underwent various laboratory tests,body composition measurement with a non-invasive body composition analyzer,and abdominal ultrasonography during health checkups.A total of 1405 subjects with GBPs were compared with 2810 age-and sex-matched controls.RESULTS The mean age of the subjects was 46.8±11.7 years,and 63.8%were male.According to multiple logistic regression analysis,the presence of fatty liver[odds ratio(OR)1.413;95%confidence interval(CI)1.218-1.638;P<0.001]was an independent risk factor for GBP,together with low levels of alanine aminotransferase(OR 0.993;95%CI 0.989-0.996;P<0.001).Additionally,fatty liver showed both independent(OR 1.629;95%CI,1.335-1.988;P<0.001)and dosedependent(moderate to severe fatty liver;OR 2.137;95%CI,1.662-2.749;P<0.001)relationship with large GBPs(≥5 mm).The presence of sarcopenia and high visceral fat area were not significantly associated with GBPs.CONCLUSION Fatty liver was found to be closely associated with GBPs irrespective of sarcopenia and visceral obesity. | Dong-Won Ahn Ji Bong Jeong Jinwoo Kang Su Hwan Kim Ji Won Kim Byeong Gwan Kim Kook Lae Lee Sohee Oh Soon Ho Yoon Sang Joon Park Doo Hee Lee | 2020 | World Journal of Gastroenterology2020,26,44: | 3 |
| 15 | Potentiating activity of rhein in targeting of resistance genes in methicillin-resistant Staphylococcus aureus显示文摘Objective: To investigate the synergistic effect between rhein(RHE) and oxacillin against Staphylococcus aureus(MRSA) at the gene level. Method: A minimum inhibitory concentration and checkerboard dilution test were conducted to evaluate antibacterial activity. Reverse transcriptase polymerase chain reaction was conducted to investigate the gene expressions. Results: RHE exhibited a minimum inhibitory concentration of 62.5-250.0 μg/mL against various MRSA strains and the reference strain, respectively. As revealed by the checkerboard assay, a combination of RHE and oxacillin exhibited synergistic or partially synergistic effects against MRSA strains. RHE decreased the expressions of mecA/blaZ in a dose-dependent manner. RHE also decreased the expressions of the regulator genes mecI/blaI and mecR1/blaR1. | 18 | Rapid species identification of pathogenic bacteria from a minute quantity exploiting three-dimensional quantitative phase imaging and artificial neural network显示文摘The healthcare industry is in dire need of rapid microbial identification techniques for treating microbial infections.Microbial infections are a major healthcare issue worldwide,as these widespread diseases often develop into deadly symptoms.While studies have shown that an early appropriate antibiotic treatment significantly reduces the mortality of an infection,this effective treatment is difficult to practice.The main obstacle to early appropriate antibiotic treatments is the long turnaround time of the routine microbial identification,which includes time-consuming sample growth.Here,we propose a microscopy-based framework that identifies the pathogen from single to few cells.Our framework obtains and exploits the morphology of the limited sample by incorporating three-dimensional quantitative phase imaging and an artificial neural network.We demonstrate the identification of 19 bacterial species that cause bloodstream infections,achieving an accuracy of 82.5%from an individual bacterial cell or cluster.This performance,comparable to that of the gold standard mass spectroscopy under a sufficient amount of sample,underpins the effectiveness of our framework in clinical applications.Furthermore,our accuracy increases with multiple measurements,reaching 99.9%with seven different measurements of cells or clusters.We believe that our framework can serve as a beneficial advisory tool for clinicians during the initial treatment of infections. | Geon Kim Daewoong Ahn Minhee Kang Jinho Park DongHun Ryu YoungJu Jo Jinyeop Song Jea Sung Ryu Gunho Choi Hyun Jung Chung Kyuseok Kim Doo Ryeon Chung In Young Yoo Hee Jae Huh Hyun-seok Min Nam Yong Lee YongKeun Park | 2022 | Light(Science & Applications)2022,11,7: | 2 |
