| 2 | Behavior of noncircular tunnels excavated in stratified rock masses-Case of underground coal mines显示文摘The amount of tunnels excavated along stratified/sedimentary rock masses in Quangninh coal mine area,Vietnam, is gradually increasing. Rock mass in Quangninh is characterized by beddings between rock layers. The behavior of stratified rock masses surrounding the tunnels depends on both the intact rock and the beddings between rock layers. The main characteristics of stratified rock masses that need to be considered are their heterogeneity and anisotropy. Depending on the dip angle of rock layers, movements and failure zones developed surrounding the tunnels can be asymmetrical over the vertical axis of tunnel. This asymmetry causes adverse behaviors of the tunnel structures. The objective of this study is to highlight convergences and yielded zones developed in rock masses surrounding noncircular tunnels in Quangninh coal mine area using a finite element method. The presence of bedding joints is explicitly simulated. The numerical results indicated that with the increase in dip angle of bedding joints, the stress asymmetry over the tunnel vertical axis increases. It gradually leads to an asymmetry of the failure zone surrounding the tunnel. An increase of rock mass quality means a decrease of rock mass sensitivity to the discontinuities. | 3 | Relationship between skin fold thickness and insulin resistance in the essential hypertensive patients in Vietnam显示文摘Previous studies reported a close relationship between obesity and insulin resistance in the essential hypertensive patients. Objective In this study, we examined the relationship between the skin fold thickness and insulin resistance then developed a formula to estimate the insulin resistance index according to the skin fold thickness in the essential hypertensive patients. Subjects and Methods Medical records of 80 patients (37 males, 43 females) were reviewed and the data were tabulated. Anthropometric indexes (including height, weight, waist circumference, hip circumference, and skins fold thickness at 5 fatty difference points on the Erdheim diagram), fasting plasma glucose and insulin concentration were recorded. The mean age was 57.0±9.2 years. The insulin resistance index was calculated following the Homeostasis Model Assessment (HOMA) formula. Results Compared with the group with BMI<23kg/m2, the group with BMI≥23 kg/m2 had higher fasting insulin concentration (8.85 ± 4.97 pmol/L vs 15.60 ± 8.70 pmol/L, P<0.001 ) and higher insulin resistance index in (2.15±1,24 vs 3.76±2.22, P<0.001). No significant difference in fasting plasma insulin concentration,insulin resistance index between male and female was observed (P>0.05). There was a positive correlation between skin fold thickness and the fasting insulin concentration and insulin resistance index. The skin fold thickness at point A8 had the best coefficient correlated with fasting plasma insulin(r=0.79, P<0.001) and insulin resistance index (r= 0.79, P < 0.001). | 4 | Significance of inferior wall ischemia in non-dominant right coronary artery anatomy显示文摘AIM To investigate the relationship of inferior wall ischemia on myocardial perfusion imaging in patients with nondominant right coronary artery anatomy.METHODS This was a retrospective observational analysis of consecutive patients who presented to the emergency department with primary complaint of chest pain.Only patients who underwent single photon emission computed tomography(SPECT)myocardial perfusion imaging(MPI)were included.Patients who showed a reversible defect on SPECT MPI and had coronary angiography during the same hospitalization was analyzed.Patients with prior history of coronary artery disease(CAD)including history of percutaneous coronary intervention and coronary artery bypass graft surgerys were excluded.True positive and false positive results were identified on the basis of hemodynamically significant CAD on coronary angiography,in the same territory as identified on SPECT MPI.Coronary artery dominance was determined on coronary angiography.Patients were divided into group 1 and group 2.Group1 included patients with non-dominant right coronary artery(RCA)(left dominant and codominant).Group2 included patients with dominant RCA anatomy.Demographics,baseline characteristics and positive predictive value(PPV)were analyzed for the two groups.RESULTS The mean age of the study cohort was 57.6 years.Sixtyone point seven percent of the patients were males.The prevalence of self-reported diabetes mellitus,hypertension and dyslipidemia was 36%,71.9%and 53.9%respectively.A comparison of baseline characteristics between the two groups showed that patients with a non-dominant RCA were more likely to be men.For inferior wall ischemia on SPECT MPI,patients in study group 2 had a significantly higher PPV,32/42(76.1%),compared to patients in group 1,in which only 3 out of the 29 patients(10.3%)had true positive results(P value<0.001 Z test).The difference remained statistically significant even when only patients with left dominant coronary system(without co-dominant)were compared to patients with right dominant system(32/40,76.1%in right dominant group,3/19,15.8%in left dominant group,P value<0.001 Z test).There was no significant difference in mean hospital stay,re-hospitalization,and in-hospital mortality between the two groups.CONCLUSION The positive predictive value of SPECT MPI for inferior wall ischemia is affected by coronary artery dominance.More studies are needed to explain this phenomenon. | Ali Osama Malik Oliver Abela Subodh Devabhaktuni Arhama Aftab Malik Gayle Allenback Chowdhury H Ahsan Sanjay Malhotra Jimmy Diep | 2017 | World Journal of Cardiology2017,9,3: | 2 |
