| 1 | 成人退变性腰椎侧凸的分型及手术方法选择显示文摘[目的]探讨一种对手术治疗具有指导意义的成人退变性腰椎侧凸的临床分型。[方法]回顾性分析1994~2004年10年间手术治疗的成人退变性腰椎侧凸患者168例,根据站立位脊柱全长X线平片上椎体的旋转、前后或侧方移位程度以及冠状面和矢状面躯干的平衡分为三型。Ⅰ型:很小或没有旋转;Ⅱ型:明显旋转和移位;Ⅲ型:重度旋转和移位,伴有冠状面和/或矢状面躯干的不平衡(C2铅垂线偏移〉4cm)。再结合患者的临床症状和体征(腰痛及神经根性疼痛)分为三个亚型。A型:腰痛,不伴有神经根性痛;B型:来自腰骶部代偿侧凸的神经根性痛,伴或不伴腰痛;C型:来自主侧凸的神经根性痛,伴或不伴腰痛。[结果]ⅠA型3例,IB型28例,IC型6例;ⅡA型21例,ⅡB型45例,ⅡC型39例;ⅢA型12例,ⅢB型6例,ⅢC型8例。其中IA型以后外侧椎间融合或经椎间孔椎间融合为主;IB、IC型的手术方法以单纯椎管减压为主;ⅡA、ⅡB、ⅡC型以后路椎管减压加后外侧融合术为主;ⅢA、ⅢB、ⅢC以前路椎间融合加后路器械矫形、椎管减压及后外侧融合术为主。[结论]此分型简便实用,尤其对手术方法的选择具有指导意义。 | 刘洪 米川 Ploumis A Transfeldt E Denis F | 2010 | 中国矫形外科杂志2010，18，1： | 13 |
| 2 | Ilizarov bone transport combined with the Masquelet technique for bone defects of various etiologies (preliminary results)显示文摘BACKGROUND The Ilizarov bone transport(IBT)and the Masquelet induced membrane technique(IMT)have specific merits and shortcomings,but numerous studies have shown their efficacy in the management of extensive long-bone defects of various etiologies,including congenital deficiencies.Combining their strong benefits seems a promising strategy to enhance bone regeneration and reduce the risk of refractures in the management of post-traumatic and congenital defects and nonunion that failed to respond to other treatments.AIM To combine IBT and IMT for the management of severe tibial defects and pseudarthrosis,and present preliminary results of this technological solution.METHODS Seven adults with post-traumatic tibial defects(subgroup A)and nine children(subgroup B)with congenital pseudarthrosis of the tibia(CPT)were treated with the combination of IMT and IBT after the failure of previous treatments.The mean number of previous surgeries was 2.0±0.2 in subgroup A and 3.3±0.7 in subgroup B.Step 1 included Ilizarov frame placement and spacer introduction into the defect to generate the induced membrane which remained in the interfragmental gap after spacer removal.Step 2 was an osteotomy and bone transport of the fragment through the tunnel in the induced membrane,its compression and docking for consolidation without grafting.The outcomes were retrospectively studied after a mean follow-up of 20.8±2.7 mo in subgroup A and 25.3±2.3 mo in subgroup B.RESULTS The“true defect”after resection was 13.3±1.7%in subgroup A and 31.0±3.0%in subgroup B relative to the contralateral limb.Upon completion of treatment,defects were filled by 75.4±10.6%and 34.6±4.2%,respectively.Total duration of external fixation was 397±9.2 and 270.1±16.3 d,including spacer retention time of 42.4±4.5 and 55.8±6.6 d,in subgroups A and B,respectively.Bone infection was not observed.Postoperative complications were several cases of pin-tract infection and regenerate deformity in both subgroups.Ischemic regeneration was observed in two cases of subgroup B.Complications were corrected during the course of treatment.Bone union was achieved in all patients of subgroup A and in seven patients of subgroup B.One non-united CPT case was further treated with the Ilizarov compression method only and achieved union.After a follow-up period of two to three years,refractures occurred in four cases of united CPT.CONCLUSION The combination of IMT and IBT provides good outcomes in post-traumatic tibial defects after previous treatment failure but external fixation is longer due to spacer retention.Refractures may occur in severe CPT. | Dmitry Y Borzunov Sergey N Kolchin Denis S Mokhovikov Tatiana A Malkova | 2022 | World Journal of Orthopedics2022，13，3： | 6 |
| 3 | Contribution of functional KIR3DL1 to ankylosing spondylitis显示文摘Increasing evidence points to a role for killer immunoglobulin-like receptors(KIRs)in the development of autoimmune diseases.In particular,a positive association of KIR3DS1(activating receptor)and a negative association of KIR3DL1(inhibitory receptor)alleles with ankylosing spondylitis(AS)have been reported by several groups.However,none of the studies analyzed these associations in the context of functionality of polymorphic KIR3DL1.To better understand how the KIR3DL1/3DS1 genes determine susceptibility to AS,we analyzed the frequencies of alleles and genotypes encoding functional(KIR3DL1*F)and non-functional(KIR3DL1*004)receptors.We genotyped 83 AS patients and 107 human leukocyte antigen(HLA)-B27-positive healthy controls from the Russian Caucasian population using a two-stage sequence-specific primer PCR,which distinguishes KIR3DS1,KIR3DL1*F and KIR3DL1*004 alleles.For the patients carrying two functional KIR3DL1 alleles,those alleles were additionally genotyped to identify KIR3DL1*005 and KIR3DL1*007 alleles,which are functional but are expressed at low levels.KIR3DL1 was negatively associated with AS at the expense of KIR3DL1*F but not of KIR3DL1*004.This finding indicates that the inhibitory KIR3DL1 receptor protects against the development of AS and is not simply a passive counterpart of the segregating KIR3DS1 allele encoding the activating receptor.However,analysis of genotype frequencies indicates that the presence of KIR3DS1 is a more important factor for AS susceptibility than the absence of KIR3DL1*F.The activation of either natural killer(NK)or T cells via the KIR3DS1 receptor can be one of the critical events in AS development,while the presence of the functional KIR3DL1 receptor has a protective effect.Nevertheless,even individuals with a genotype that carried two inhibitory KIR3DL1 alleles expressed at high levels could develop AS. | Ivan V Zvyagin Ilgar Z Mamedov Olga V Britanova Dmitriy B Staroverov Evgeni L Nasonov Anna G Bochkova Anna V Chkalina Alexei A Kotlobay Dmitriy O Korostin Denis V Rebrikov Sergey Lukyanov Yuri B Lebedev Dmitriy M Chudakov | 2010 | Cellular & Molecular Immunology2010，7，6： | 5 |
