1The Uplift of the Longmenshan Thrust Belt and Subsidence of the West Sichuan Foreland Basin显示文摘Based on fission track dating of apatite, and measurement of vitrinite reflectance of rock samples from the Longmenshan (Longmen Mountain)area and the West Sichuan foreland basin and computer modelling it is concluded that (l)the Songpan-Garze fold belt has uplifted at least by 3-4 km with an uplift rate of no less than 0.3-0.4 mm/a since 10 Ma B.P.; (2) the Longmenshan thrust nappe belt has uplifted at least by 5-6 km with an uplift rate of more than 0.5- 0.6 mm /a since 10 Ma B.P.; (3) the Longmenshan detachment belt has uplifted by 1 - 2 km at a rate of 0.016-0.032 mm/a since 60 Ma B.P.; (4) the West Sichuan foreland basin has uplifted by 1.7-3 km at a rate of 0.028-0.05 mm/a since 60 Ma B.P.; (5) the uplift rate of the area on the west side of the Beichuan-Yingxiu-Xiaoguanzi fault for the last 10 Ma is 40 times as much as that on its east side; (6) the uplifting of the the Songpan - Garze fold belt and the subsidence of the West Sichuan foreland basin 60 Ma ago exhibit a mirro-image correlation, i.Liu Shugen, Luo Zhili, Dai Sulan, Changlu Institute of Technology, Chengdu, Sichuan, ChinaDennis Arne Dalhousie U niversity, Canadaand C.J.L. 2Submucosal tunneling and endoscopic resection of submucosal tumors at the esophagogastric junction显示文摘AIM: To evaluate the safety and efficacy of submucosal tunneling and endoscopic resection(STER) for treating submucosal tumors(SMTs).METHODS: Between August 2012 and October 2013, 21 patients with SMTs originating from the muscularis propria(MP) layer at the esophagogastric junction were treated by STER of their tumors. Key steps of the procedure include:(1) mucosal incision: a 2-cm longitudinal mucosal incision was made 5 cm proximal to the tumor;(2) submucosal tunneling: a submucosal tunnel was created 5 cm proximal to and 1 to 2 cm distal to the tumor;(3) tumor resection: the SMT was resected under direct endoscopic viewing;(4) hemostasis: while finishing the tumor resection, careful hemostasis of the MP defect and the tunnel was performed; and(5) mucosal closure: the mucosal incision site was closed by using hemostatic clips. During the operation, equipment used included a cap-fitted endoscope, an insulatedtip knife, a hook knife, hemostatic forceps, an injection needle, a snare, an endoclip, and a high-frequency generator. Carbon dioxide(CO2) insufflation was achieved by using a CO2 insufflator.RESULTS: The median age of the patients was 46.2 years(range, 35-59 years), and the majority were male(18 male vs 3 female). Complete resection rate was 100%(21/21). Eighteen lesions were resected en bloc. Mean tumor size was 23 mm(range, 10-40 mm), and mean procedure time was 62.9 min(range, 45-90 min). Pathological diagnosis of these tumors included leiomyoma(15 out of 21) and gastrointestinal stromal tumor(6 out of 21). Full-thickness MP resection was performed in 9 of 21 patients(42.9%), with mediastinal and subcutaneous emphysema occurring in all nine. At the completion of the procedure, all patients received closure of the incision with hemoclips. One patient required percutaneous drainage. The remaining 20patients required no further endoscopic or surgical intervention. There were no incidents of massive or delayed bleeding. The median follow-up period after the procedure was 6 mo(range, 2-14 mo). 4Stroke mechanisms and outcomes of isolated symptomatic basilar artery stenosis显示文摘background While diffuse atherosclerotic disease affecting the posterior circulation has been described extensively,the prevalence,natural history and angiographic characteristics of isolated symptomatic basilar artery stenosis(ISBAS)remain unknown.Methods We reviewed our prospective institutional database to identify patients with≥50% symptomatic basilar artery(BA)stenosis without significant atherosclerotic burden in the vertebral or posterior cerebral arteries.Stroke mechanism,collateral circulation,and degree and length of stenosis were analysed.The primary outcome was time from index event to new transient ischaemic attack(TIA),acute ischaemic stroke(AIS)or death.Other outcome variables included modified Rankin Scale(mRS)score on discharge and last follow-up.results Of 6369 patients with AIS/TIA,91(1.43%)had ISBAS.Seventy-three(80.2%)patients presented with AIS and 18(19.8%)with TIA.Twenty-nine(31.9%)were women and the median age was 66.8±13.6 years.The mean follow-up time was 2.7 years.The most common stroke mechanism was artery-to artery thromboembolism(45.2%),followed by perforator occlusion(28.7%)and flow-dependent/hypoperfusion(15.1%).The percentage of stenosis was lower in patients who had favourable outcome compared with those with mRS 3-6 on discharge(78.3±14.3 vs 86.9±14.5,p=0.007).Kaplan-Meier curves showed higher recurrence/death rates in patients with≥80% stenosis,mid-basilar location and poor collateral circulation.Approximately 13% of patients with ISBAS presented with complete BA occlusion.Conclusion ISBAS is an uncommon(1.43%)cause of TIA and AIS.Men in their 60s are mostly affected,and artery-to artery embolism is the most common stroke mechanism.Mid-basilar location,≥80% stenosis and poor collateral circulation are important factors associated with worse prognosis.Edgar A Samaniego Amir Shaban Santiago Ortega-Gutierrez Jorge A Roa David M Hasan Colin Derdeyn Biyue Dai Harold Adams Enrique Leira 2019Stroke & Vascular Neurology2019,4,4:10
5Biological responses of small yellow croaker(Larimichthys polyactis) to multiple stressors: a case study in the Yellow Sea, China显示文摘Temporal changes in biological characteristics of small yellow croaker Larimichthys polyactis in the Yellow Sea were examined for the period of 1960–2008. The body size and age of small yellow croaker decreased substantially, in particular, average length of fish in 2008 was reduced by ~85% than those occurring in 1985, and at that time ~93% of the total catch was dominated by one-year-old individuals. Correspondingly, growth parameters also varied significantly over the years, i.e., k(growth coefficient) and t_0(zero-length age) gradually increased from 0.26 and –0.58 year in 1960 to 0.56 and –0.25 year in 2008, respectively. Although, L∞(body length)sharply decreased from 34.21 cm in 1960 to 24.06 cm in 2008, and t_r(inflexion age) decreased from 3.78 year in1960 to 1.61 year in 2008. There was a great increase both in natural mortality coefficient and fishing mortality coefficient. However, according to the gray correlation analysis, changes in the biological characteristics of small yellow croaker were induced by different stressors ranked as: fishing vessel power>feeding grade>sea surface temperature. 7Cardiac implications of thrombotic thrombocytopenic purpura显示文摘Thrombotic thrombocytopenic purpura(TTP) is a multisystem disorder that essentially can affect any organ in the human body. The hallmark of the pathogenesis in TTP is the large von Willebrand factor multimers on plateletmediated micro-thrombi formation, leading to microvascular thrombosis.Autopsy studies showed that cardiac arrest and myocardial infarction are the most common immediate causes of death in these patients. Clinical manifestations of cardiac involvement in TTP vary dramatically, from asymptomatic elevation of cardiac biomarkers, to heart failure, MI and sudden cardiac death. There is limited knowledge about optimal cardiac evaluation and management in patients with TTP. The absence of typical cardiac symptoms,combined with complicated multi-organ involvement in TTP, may contribute to the under-utilization of cardiac evaluation and treatment. Prompt diagnosis and timely initiation of effective therapy could be critically important in selected cases. Based on our experience and this review of the literature, we developed several recommendations for focused cardiac evaluation for patients with acute TTP:(1) patients with suspected or confirmed TTP should be screened for the potential presence of cardiac involvement with detailed history and physical,electrocardiogram and cardiac enzymes;(2) clinical deterioration of TTP patients warrants immediate cardiac reevaluation;(3) TTP patients with clinical evidence of cardiac involvement should be monitored for telemetry, cardiac biomarkers and evaluated with transthoracic echocardiography. These patients require urgent targeted TTP treatment as well as cardiac-specific treatment. Aspirin therapy is indicated for all TTP patients. Since epicardial coronary artery involvement is rare, cardiac catheterization is usually not required, given the high risk for hemorrhage and kidney injury;(4) we recommend evidence-based medical therapy for ischemic symptoms and heart failure.
