| 1 | Twist基因蛋白表达在甲状腺乳头状癌诊断与鉴别诊断中的意义显示文摘目的探讨Twist基因蛋白在甲状腺乳头状癌中的表达状况,寻找更特异的标记物,用于甲状腺乳头状癌(PTC)与滤泡状腺瘤(FA)和良性乳头病变(BPL)的鉴别诊断。方法以50例PTC为研究组,以48例FA和47例BPL作对照组。在自制组织芯片上行免疫组织化学(SP法)标记,检测Twist基因蛋白和HBME-1,并以CK19进行对照。结果3种指标(Twist、HBME-1、CK19)阳性表达率:在PTC组分别为100%(48/48)、94.0%(47/50)和78.0%(39/50);在FA组分别为0、6.7%(3/45)和0;在BPL组分别为7.0%(3/43)、2.1%(1/47)和0。PTC组分别与FA和BPL组比较差异均有统计学意义(P=0.000)。3种指标在鉴别诊断甲状腺良、恶性病变的灵敏度分别为:100%、96.4%和97.7%,特异度分别为:94.0%、95.7%和95.1%,准确度分别为:78.0%、100%和91.9%。结论Twist可应用于辅助诊断PTC,并可应用在对FA或BPL的鉴别诊断。 | 何春年 Lili He Jin Q Cheng 陈士超 赵焕芬 翟金萍 张静 | 2008 | 中华病理学杂志2008，37，1： | 14 |

| 3 | Pulmonary infection control window in treatment of severe respiratory failure of chronic obstructive pulmonary diseases: a prospective, randomized controlled, multi-centred study显示文摘Background Early withdraw from invasive mechanical ventilation (MV) followed by noninvasive MV is a new strategy for changing modes of treatment. This study was conducted to estimate the feasibility and the efficacy of early extubation and sequential noninvasive MV commenced at beginning of pulmonary infection control window in patients with exacerbated hypercapnic respiratory failure caused by chronic obstructive pulmonary diseases (COPD). Methods A prospective, randomized controlled study was conducted in eleven teaching hospitals' respiratory or medical intensive care units in China. Ninety intubated COPD patients with severe hypercapnic respiratory failure triggered by pulmonary infection (pneumonia or purulent bronchitis) were involved in the study. When the pulmonary infection had been controlled by antibiotics and comprehensive therapy, the 'pulmonary infection control window (PIC window)' has been reached. Each case was randomly assigned to study group (extubation and noninvasive MV via facial mask immediately) or control group (invasive MV was received continuously after PIC window by using conventional weaning technique).Results Study group (n=47) and control group (n=43) had similar clinical characteristics initially and at the time of PIC window. Compared with control group, study group had shorter duration of invasive MV [(6.4±4.4) days vs (11.3±6.2) days, P=0.000], lower rate of ventilator associated pneumonia (VAP) (3/47 vs 12/43, P=0.014), fewer days in ICU [(12±8) days vs (16±11) days, P=0.047] and lower hospital mortality (1/47 vs 7/43, P=0.025).Conclusions In COPD patients requiring intubation and invasive MV for hypercapnic respiratory failure, which is exacerbated by pulmonary infection, early extubation followed by noninvasive MV initiated at the start of PIC window may decrease significantly the duration of invasive MV, the risk of VAP and hospital mortality. | Wang, C Zhan, QY Cao, ZX Wei, LQ Cheng, ZZ Liu, S Zhang, JL Chen, RC Luo, Q Niu, SF Zhu, L Wu, DW Fang, BM Wu, TH Wang, CZ Liu, YN | 2005 | Chinese Medical Journal2005，，19： | 6 |

Its thermal and transmission properties are reported. The thermal decomposition is characterized by using the X ray powder diffraction (XRPD) and infrared (IR) spectroscopy at room temperature. | 5 | Anti-N-methyl-D-aspartate receptor encephalitis that aggravates after acinetobacter baumannii pneumonia:A case report显示文摘We report an atypical case of anti-N-methyl-D-aspartate receptor encephalitis(ANMDARE). A 27-year-old man diagnosed with ANMDARE received immunotherapy and had a good recovery. However, within one month, he developed severe status epilepticus and decreased level of conscience with new hyperpyrexia and dyspnea, and was admitted to the emergency intensive care unit. Acinetobacter baumanii were found in the sputum culture; and anti-NMDAR antibodies were positive(titer: 1/80) in the cerebrospinal fluid. Repeated immunotherapy was administered with antibacterial agents, and the patient recovered except for mild psychiatric sequelae. This is the first report of ANMDARE that aggravates after acinetobacter baumannii pneumonia.
