| 1 | 基于IMRT时代的第八版AJCC/UICC鼻咽癌临床分期建议显示文摘目的准确的分期系统对癌症的治疗至关重要。随着癌症分期和治疗方法的演变,需要不断评价分期的适用性和改进性。方法基于第7版AJCC/UICC分期回顾性分析香港和中国大陆2个肿瘤中心收治的1609例接受调强放射治疗的首诊无转移鼻咽癌患者临床资料,所有患者治疗前均行核磁共振分期评估。结果无其他T3、T4期解剖结构受侵患者中,伴有咀嚼肌间隙(翼内肌和/或翼外肌)侵犯、椎前肌侵犯及咽旁间隙侵犯的三组患者之间0s相近。伴广泛软组织(上述侵犯结构以外的软组织)受侵患者OS与伴有颅内侵犯或颅神经侵犯相似。仅2%患者锁骨上窝以上淋巴结转移者直径〉6cm,其0s率与下颈淋巴结转移者类似。用下颈(环状软骨尾侧缘水平以下)代替锁骨上窝并不影响N分期之间的风险差异性。采用推荐的T、N分期,T。N。、T。N,期0s相近。结论经AJCC/UICC分期筹备委员会审阅后,建议第8版分期应将翼内肌/翼外肌从T。降到T:期,增加椎前肌为L期,用下颈取代锁骨上窝,将淋巴结最大直径〉6cm合并归为N,期,将T。、N,期统一归为Ⅳ。期。这些改变不仅使得相邻分期间风险差异性更好,而且使得临床实践性与全球适用性之间达到最佳平衡。 | 潘建基 Wai Tong Ng 宗井凤 Lucy L.K.Chan Brian O' Sullivan 林少俊 Henry C.K.Sze 陈韵彬 Horace C.W.Choi 郭巧娟 Wai Kuen Kan 肖友平 Xu Wei Quynh Thu Le Christine M.Glastonbury A.Dimtrios Colevas Randal S.Weber Jatin P.Shah Anne W.M.Lee | 2016 | 中华放射肿瘤学杂志2016，25，3： | 65 |

| 4 | Epinephrine injection therapy versus a combination of epinepnrine injection and endoscopic hemoclip in the treatment of bleeding ulcers显示文摘AIM: To assess the efficacy of hemoclip application in combination with epinephrine injection in the treatment of bleeding peptic ulcers and to compare the clinical outcomes between patients treated with such a combination therapy and those treated with epinephrine injection alone.METHODS: A total of 293 patients (211 males, 82females) underwent endoscopic therapy for bleeding peptic ulcers. Of these, 202 patients (152 males, 50females) received epinephrine injection therapy while 91patients (59 males, 32 females) received combination therapy. The choice of endoscopic therapy was made by the endoscopist. Hemostatic rates, rebleeding rates, need for emergency surgery and 30-d mortality were the outcome measures studied.RESULTS: Patients who received combination therapy were significantly older (mean age 66±16 years, range24-90 years) and more suffered from chronic renal failure compared to those who received epinephrine injection therapy alone (mean age 61±17 years, range 21-89 years).Failure to achieve permanent hemostasis was 4% in the group who received epinephrine injection alone and 11%in the group who received combination therapy. When the differences in age and renal function between the two treatment groups were taken into account by multivariate analysis, the rates of initial hemostasis,rebleeding rates, need for surgery and 30-d mortality for both treatment options were not significantly different.CONCLUSION: Combination therapy of epinephrine injection with endoscopic hemoclip application is an effective method of achieving hemostasis in bleeding peptic ulcer diseases. | 16 | Insights on the solubilization products after combined alkaline and ultrasonic pre-treatment of sewage sludge显示文摘This work provides insights on the solubilization products after a simultaneous combination of alkaline and ultrasonic(ALK + ULS) pre-treatment of sewage sludge.Soluble chemical oxygen demand(SCOD) increased from 1200 to 11,000 mg/L after such treatment. Organics with molecular weight around 5.6 k Da were solubilized because of the synergistic effect of ultrasound and alkali. Organics with molecular weight larger than300 k Da increased from 7.8% to 60%, 16% and 42.3% after ULS, ALK and ALK + ULS treatment, respectively. Excitation emission matrix fluorescence spectroscopy analysis identified soluble microbial product-like and humic acid-like matters as the main solubilization products. Sludge anaerobic biodegradability was significantly enhanced with the simultaneous application of ALK + ULS pre-treatment. ALK + ULS pre-treatment resulted in 37.8% biodegradability increase compared to the untreated sludge. | 18 | Structure and sequence analysis of influenza A virus nucleoprotein显示文摘Influenza A virus nucleoprotein (NP) forms homo-oligomers and multiple copies of NP wrap around genomic RNA, along with a trimeric polymerase making up ribonucleoprotein (RNP) complex. Sequence comparison of more than 2500 influenza A NP showed that this protein contains 30.1 % of polymorphic residues. NP is composed of a head and a body domain and a tail loop/ linker region. The head domain is more conserved than the body domain, as revealed from the structure-based sequence alignment. NP oligomerization is mediated by the insertion of the non-polymorphic and structurally conserved tail loop of one NP molecule to a groove of another NP. The different form of NP oligomers is due to the flexibility of the polymorphic linkers that join the tail loop to the rest of the protein. The RNA binding property of NP is known to involve the protruding element and the flexible basic loop between the head and body domains, both having high degree of primary sequence conservation. | 19 | Nitrous oxide emission by denitrifying phosphorus removal culture using polyhydroxyalkanoates as carbon source显示文摘Nitrous oxide(N2O) emission has been reported to be enhanced during denitrification when internally-stored compounds are used as carbon sources.However,negligible N2O emissions have been detected in the few studies where polyhydroxyalkanoates(PHA) were specifically used.This study investigated and compared the potential enhancement of N2O production,based on utilization of an internally-stored polymer and external carbon(acetate) by a denitrifying phosphorus removal culture.Results indicated that at relatively low chemical oxygen demand-to-nitrogen(COD/N) ratios,more nitrite was reduced to N2O in the presence of an external carbon source as compared to an internal carbon source(PHA).At relatively higher COD/N ratios,similar N2O reduction rates were obtained in all cases regardless of the type of carbon source available.N2O reduction rates were,however,generally higher in the presence of an internal carbon source.Results from the study imply that when the presence of an external carbon source is not sufficient to support denitrification,it is likely competitively utilized by different metabolic pathways of denitrifying polyphosphate accumulating organisms(DPAOs) and other ordinary denitrifiers.This study also reveals that the consumption of PHA is potentially the rate-limiting step for N2O reduction during denitrification. | Yan Zhou Melvin Lim Soekendro Harjono Wun Jern Ng | 2012 | Journal of Environmental Sciences2012，24，9： | 17 |
