| 1 | 重新审视中国高龄老人血压与死亡风险之间的关系:以社区为基础的纵向前瞻性队列研究显示文摘目的探讨中国高龄老人血压与3年全因死亡风险和死因别死亡风险之间的关系。研究设计以社区为基础的纵向前瞻性队列研究。研究地点2011和2014年在中国22个省份开展的两轮中国老年健康影响因素跟踪调查。研究对象4658名高龄老人(平均年龄为92.1岁)。主要结局指标随访3年全因死亡和死因别死亡。结果经过3年的随访,共计1977人死亡,收缩压(SBP)、平均动脉压(MAP)、脉压差(PP)与死亡风险间呈U型关系,SBP、MAP和PP在143.5、101和66mmHg时死亡风险最低。对混杂因素进行调整后,仅SBP与死亡风险的U型关系存在,风险最低点为129mmHg。与129mmHg的人群相比,SBP低于107mmHg时,死亡风险随SBP的升高而降低[风险比(HR)从1.47(1.01~2.17)到1.08(1.01~1.17)1;SBP高于154mmHg时,死亡风险随SBP的升高而增加[HR从1.08(1.01~1.17)到1.27(1.02~1.58)]。在死因别分析中,与中等水平SBP(107~154mmHg)相比,高水平的SBP(〉154mmHg)与心血管疾病(CVD)死亡风险升高相关联[调整HR1.51(1.12~2.02)];低水平SBP(〈107mmHg)与非CVD死亡风险升高相关联[调整HR1.58(1.26~1.98)]。在敏感性分析与亚组分析中血压与死亡风险的U型关系保持不变。结论研究在中国高龄老人中发现SBP与3年全因死亡风险呈U型关系,高SBP意味着更高的CVD死亡风险,低SBP意味着更高的非CVD死亡风险。这提示高龄老人目前的血压管理理念有待重新评估,可能需要针对这一特殊人群制定特定的血压指南。 | 吕跃斌 高翔 殷召雪 陈华帅 罗杰斯 Melanie Sereny Brasher Virginia Byers Kraus 李湉湉 曾毅 施小明 张希如(译) 毛琛(校) | 2018 | 英国医学杂志中文版2018，21，8： | 17 |

| 3 | All-silicon carrier accumulation modulator based on a lateral metal-oxide-semiconductor capacitor显示文摘In silicon photonics, the carrier depletion scheme has been the most commonly used mechanism for demonstrating high-speed electro-optic modulation. However, in terms of phase modulation efficiency, carrieraccumulation-based devices potentially offer almost an order of magnitude improvement over those based on carrier depletion. Previously reported accumulation modulator designs only considered vertical metal-oxidesemiconductor(MOS) capacitors, which imposes serious restrictions on the design flexibility and integratability with other photonic components. In this work, for the first time to our knowledge, we report experimental demonstration of an all-silicon accumulation phase modulator based on a lateral MOS capacitor. Although fatty acid oxidation disorders, lysosomal and glycogen storage disorders and organic acidurias are well-known to be associated with cardiomyopathies, emerging reports suggest that mitochondrial dysfunction and congenital disorders of glycosylation may also account for a proportion of cardiomyopathies. | 8 | Deliberate release:Plague-A review显示文摘Yersinia pestis is the causative agent of plague and is considered one of the most likely pathogens to be used as a bioweapon.In humans,plague is a severe clinical infection that can rapidly progress with a high mortality despite antibiotic therapy.Therefore,early treatment of Y.pestis infection is crucial.This review provides an overview of its clinical manifestations,diagnosis,treatment,prophylaxis,and protection requirements for the use of clinicians.We discuss the likelihood of a deliberate release of plague and the feasibility of obtaining,isolating,culturing,transporting and dispersing plague in the context of an attack aimed at a westernized country.The current threat status and the medical and public health responses are reviewed.We also provide a brief review of the potential prehospital treatment strategy and vaccination against Y.pestis.Further,we discuss the plausibility of antibiotic resistant plague bacterium,F1-negative Y.pestis,and also the possibility of a plague mimic along with potential strategies of defense against these.An extensive literature search on the MEDLINE,EMBASE,and Web of Science databases was conducted to collate papers relevant to plague and its deliberate release.Our review concluded that the deliberate release of plague is feasible but unlikely to occur,and that a robust public health response and early treatment would rapidly halt the transmission of plague in the population.Front-line clinicians should be aware of the potential of a deliberate release of plague and prepared to instigate early isolation of patients.Moreover,front-line clinicians should be weary of the possibility of suicide attackers and mindful of the early escalation to public health organizations. | Issmaeel Ansari Gareth Grier Mark Byers | 2020 | Journal of Biosafety and Biosecurity2020，2，1： | 2 |
