| 9 | The role of active case finding in reducing patient incurred catastrophic costs for tuberculosis in Nepal显示文摘Background:The World Health Organization(WHO)End TB Strategy has established a milestone to reduce the number of tuberculosis(TB)-affected households facing catastrophic costs to zero by 2020.The role of active case finding(ACF)in reducing patient costs has not been determined globally.This study therefore aimed to compare costs incurred by TB patients diagnosed through ACF and passive case finding(PCF),and to determine the prevalence and intensity of patient-incurred catastrophic costs in Nepal.Methods:The study was conducted in two districts of Nepal:Bardiya and Pyuthan(Province No.5)between June and August 2018.One hundred patients were included in this study in a 1:1 ratio(PCF:ACF,25 consecutive ACF and 25 consecutive PCF patients in each district).The WHO TB patient costing tool was applied to collect information from patients or a member of their family regarding indirect and direct medical and non-medical costs.Catastrophic costs were calculated based on the proportion of patients with total costs exceeding 20%of their annual household income.The intensity of catastrophic costs was calculated using the positive overshoot method.The chi-square and Wilcoxon-Mann-Whitney tests were used to compare proportions and costs.Meanwhile,the Mantel Haenszel test was performed to assess the association between catastrophic costs and type of diagnosis.Results:Ninety-nine patients were interviewed(50 ACF and 49 PCF).Patients diagnosed through ACF incurred lower costs during the pre-treatment period(direct medical:USD 14 vs USD 32,P=0.001;direct non-medical:USD 3 vs USD 10,P=0.004;indirect,time loss:USD 4 vs USD 13,P<0.001).The cost of the pre-treatment and intensive phases combined was also lower for direct medical(USD 15 vs USD 34,P=0.002)and non-medical(USD 30 vs USD 54,P=0.022)costs among ACF patients.The prevalence of catastrophic direct costs was lower for ACF patients for all thresholds.A lower intensity of catastrophic costs was also documented for ACF patients,although the difference was not statistically significant.Conclusions:ACF can reduce patient-incurred costs substantially,contributing to the End TB Strategy target.Other synergistic policies,such as social protection,will also need to be implemented to reduce catastrophic costs to zero among TB-affected households. | Suman Chandra Gurung Kritika Dixit Bhola Rai Maxine Caws Puskar Raj Paudel Raghu Dhital Shraddha Acharya Gangaram Budhathoki Deepak Malla Jens W.Levy Job van Rest Knut Lonnroth Kerri Viney Andrew Ramsay Tom Wingfield Buddha Basnyat Anil Thapa Bertie Squire Duolao Wang Gokul Mishra Kashim Shah Anil Shrestha Noemia Teixeira de Siqueira-Filha | 2019 | Infectious Diseases of Poverty2019,8,6: | 1 |

| 11 | Teenage pregnancy and the sociodemographic attributes as a major contributor: Findings from an urban referral center in Nepal显示文摘BACKGROUND Teenage pregnancy is a challenging issue worldwide.Yet,despite the increased health risk and socioeconomic impact of teenage pregnancy,the numbers remain high in Nepal.AIM To determine the prevalence and sociodemographic factors associated with teenage pregnancy in Nepal.METHODS A hospital-based cross-sectional study was conducted in the Paropakar Maternity and Women’s Hospital from April to August 2017.A total of 1359 mothers were assessed and interviewed regarding their reproductive history and sociodemographic attributes.Ethical approval from the Nepal Health Research Council,permission from the concerned hospital,and consent from study subjects were obtained.RESULTS Of the 5526 deliveries,679(12.3%)were teenage pregnancy deliveries.The majority(85.1%)of mothers(out of 1359)were married at<20 years of age,with a mean age of marriage of 17.57 years(±1.994,range:12-25 years).Marriage age was a significant determinant of teenage pregnancy(odds ratio[OR]2.423[2.262-2.596];P<0.001).Likewise,a love marriage(OR:2.018[1.585-2.570];P<0.001)and first teenage pregnancy(OR:3.622[3.265-4.017];P<0.001)were significant determinants for subsequent teen pregnancies.Knowledge of family planning methods(OR:0.474;0.288-0.779;P=0.003)and use of any methods of family planning utilization(OR:0.345;0.248-0.479;P=0.000)significantly lowered the chance of teenage pregnancy.CONCLUSION Teenage pregnancy occupies a substantial proportion of total deliveries in the Nepalese maternity hospital.In addition,early pregnancy was significantly associated with age and type of marriage,education level,knowledge,and practice of contraceptive methods. | Puja Thapa Pukar Thapa Dhan Bahadur Shrestha Pravash Budhathoki Binod Karki Bimita Mahat | 2021 | World Journal of Obstetrics and Gynecology2021,10,2: | 1 |

| 16 | New-onset diabetes in COVID-19 and clinical outcomes: A systematic review and meta-analysis显示文摘BACKGROUND Diabetes mellitus(DM)is associated with adverse clinical outcomes and high mortality in patients with coronavirus disease 2019(COVID-19).The relationship between diabetes and COVID-19 is known to be bidirectional.AIM To analyze the rate of new-onset diabetes in COVID-19 patients and compare the clinical outcomes of new-onset diabetes,pre-existing diabetes,hyperglycemic,and non-diabetes among COVID-19 patients.METHODS We used the Meta-analysis of Observational Studies in Epidemiology statement for the present meta-analysis.Online databases were searched for all peerreviewed articles published until November 6,2020.Articles were screened using Covidence and data extracted.Further analysis was done using comprehensive meta-analysis.Among the 128 studies detected after thorough database searching,seven were included in the quantitative analysis.The proportion was reported with 95%confidence interval(CI)and heterogeneity was assessed using I2.RESULTS Analysis showed that 19.70%(CI:10.93-32.91)of COVID-19 patients had associated DM,and 25.23%(CI:19.07-32.58)had associated hyperglycemia.The overall mortality rate was 15.36%(CI:12.57-18.68)of all COVID-19 cases,irrespective of their DM status.The mortality rate was 9.26%among non-diabetic patients,10.59%among patients with COVID-19 associated hyperglycemia,16.03%among known DM patients,and 24.96%among COVID-19 associated DM patients.The overall occurrence of adverse events was 20.52%(CI:14.21-28.70)among COVID-19 patients in the included studies,15.29%among non-diabetic patients,20.41%among patients with COVID-19 associated hyperglycemia,20.69%among known DM patients,and 45.85%among new-onset DM.Metaregression showed an increasing rate of mortality among new hyperglycemic patients,known diabetics,and new-onset DM patients in comparison to those without diabetes.CONCLUSION A significantly higher rate of new onset DM and hyperglycemia was observed.Higher mortality rates and adverse events were seen in patients with new-onset DM and hyperglycemia than in the non-diabetic population. | Dhan Bahadur Shrestha Pravash Budhathoki Sumit Raut Sugat Adhikari Prinska Ghimire Sabin Thapaliya Ali A Rabaan Bibodh Jung Karki | 2021 | World Journal of Virology2021,10,5: | 0 |
