1Pathogenesis and clinical management of Helicobacter pylori gastric infection显示文摘Helicobacter pylori(H.pylori)is a gram-negative bacterium that infects approximately 4.4 billion individuals worldwide.However,its prevalence varies among different geographic areas,and is influenced by several factors.The infection can be acquired by means of oral-oral or fecal-oral transmission,and the pathogen possesses various mechanisms that improve its capacity of mobility,adherence and manipulation of the gastric microenvironment,making possible the colonization of an organ with a highly acidic lumen.In addition,H.pylori presents a large variety of virulence factors that improve its pathogenicity,of which we highlight cytotoxin associated antigen A,vacuolating cytotoxin,duodenal ulcer promoting gene A protein,outer inflammatory protein and gamma-glutamyl transpeptidase.The host immune system,mainly by means of a Th1-polarized response,also plays a crucial role in the infection course.Although most H.pylori-positive individuals remain asymptomatic,the infection predisposes the development of various clinical conditions as peptic ulcers,gastric adenocarcinomas and mucosa-associated lymphoid tissue lymphomas.Invasive and non-invasive diagnostic methods,each of them with their related advantages and limitations,have been applied in H.pylori detection.Moreover,bacterial resistance to antimicrobial therapy is a major challenge in the treatment of this infection,and new therapy alternatives are being tested to improve H.pylori eradication.Last but not least,the development of effective vaccines against H.pylori infection have been the aim of several research studies.Breno Bittencourt de Brito Filipe Ant?nio Fran?a da Silva Aline Silva Soares Vinícius Afonso Pereira Maria Luísa Cordeiro Santos Mariana Miranda Sampaio Pedro Henrique Moreira Neves Fabrício Freire de Melo 2019World Journal of Gastroenterology2019,25,37:84
2Myocardial reperfusion injury and oxidative stress: Therapeutic opportunities显示文摘Acute myocardial infarction(AMI) is the leading cause of death worldwide. Its associated mortality, morbidity and complications have significantly decreased with the development of interventional cardiology and percutaneous coronary angioplasty(PCA) treatment, which quick-ly and effectively restore the blood flow to the area previously subjected to ischemia. Paradoxi-cally, the restoration of blood flow to the ischemic zone leads to a massive production of reactive oxygen species(ROS) which generate rapid and severe damage to biomolecules, generating a phenomenon called myocardial reperfusion injury(MRI). In the clinical setting, MRI is associated with multiple complications such as lethal reperfusion, no-reflow, myocardial stunning, and reperfusion arrhythmias. Despite significant advances in the understanding of the mechanisms accounting for the myocardial ischemia reperfusion injury, it remains an unsolved problem. Although promising results have been obtained in experimental studies(mainly in animal models), these benefits have not been translated into clinical settings. Thus, clinical trials have failed to find benefits from any therapy to prevent MRI. There is major evidence with respect to the contribution of oxidative stress to MRI in cardiovascular diseases. The lack- of consistency between basic studies and clinical trials is not solely based on the diversity inherent in epidemiology but is also a result of the methodological weak-nesses of some studies. It is quite possible that pharmacological issues, such as doses, active ingredients, bioavailability, routes of administration, co-therapies, startup time of the drug intervention,and its continuity may also have some responsibility for the lack- of consistency between different studies. Furthermore, the administration of high ascorbate doses prior to reperfusion appears to be a safe and rational therapy against the development of oxidative damage associated with myocardial reperfusion. 3Helicobacter pylori infection: Beyond gastric manifestations显示文摘Helicobacter pylori(H.pylori)is a bacterium that infects more than a half of world’s population.Although it is mainly related to the development of gastroduodenal diseases,several studies have shown that such infection may also influence the development and severity of various extragastric diseases.According to the current evidence,whereas this bacterium is a risk factor for some of these manifestations,it might play a protective role in other pathological conditions.In that context,when considered the gastrointestinal tract,H.pylori positivity have been related to Inflammatory Bowel Disease,Gastroesophageal Reflux Disease,Non-Alcoholic Fatty Liver Disease,Hepatic Carcinoma,Cholelithiasis,and Cholecystitis.Moreover,lower serum levels of iron and vitamin B12 have been found in patients with H.pylori infection,leading to the emergence of anemias in a portion of them.With regards to neurological manifestations,a growing number of studies have associated that bacterium with multiple sclerosis,Alzheimer’s disease,Parkinson’s disease,and Guillain-Barrésyndrome.Interestingly,the risk of developing cardiovascular disorders,such as atherosclerosis,is also influenced by the infection.Besides that,the H.pylori-associated inflammation may also lead to increased insulin resistance,leading to a higher risk of diabetes mellitus among infected individuals.Finally,the occurrence of dermatological and ophthalmic disorders have also been related to that microorganism.In this sense,this minireview aims to gather the main studies associating H.pylori infection with extragastric conditions,and also to explore the main mechanisms that may explain the role of H.pylori in those diseases.Maria Luisa Cordeiro Santos Breno Bittencourt de Brito Filipe Antonio França da Silva Mariana Miranda Sampaio Hanna Santos Marques Natalia Oliveira e Silva Dulciene Maria de Magalhaes Queiroz Fabricio Freire de Melo 2020World Journal of Gastroenterology2020,26,28:34
4Essential hypertension and oxidative stress:New insights显示文摘Essential hypertension is a highly prevalent pathological condition that is considered as one of the most relevant cardiovascular risk factors and is an important cause of morbidity and mortality around the world. Despite the fact that mechanisms underlying hypertension are not yet fully elucidated,a large amount of evidence shows that oxidative stress plays a central role in its pathophysiology. Oxidative stress can be defined as an imbalance between oxidant agents,such as superoxide anion,and antioxidant molecules,and leads to a decrease in nitric oxide bioavailability,which is the main factor responsible for maintaining the vascular tone. Several vasoconstrictor peptides,such as angiotensin Ⅱ,endothelin-1 and urotensin Ⅱ,act through their receptors to stimulate the production of reactive oxygen species,by activating enzymes like NADPH oxidase andxanthine oxidase. The knowledge of the mechanism described above has allowed generating new therapeutic strategies against hypertension based on the use of antioxidants agents,including vitamin C and E,N-Acetylcysteine,polyphenols and selenium,among others. These substances have different therapeutic targets,but all represent antioxidant reinforcement. Several clinical trials using antioxidants have been made. 5Video capsule endoscopy vs double-balloon enteroscopy in the diagnosis of small bowel bleeding:A systematic review and metaanalysis显示文摘AIM To compare the diagnostic accuracy of video capsule endoscopy(VCE) and double-balloon enteroscopy(DBE) in cases of obscure gastrointestinal bleeding(OGIB) of vascular origin.METHODS MEDLINE(via PubMed), LILACS(via BVS) and Cochrane/CENTRAL virtual databases were searched for studies dated before 2017. We identified prospective and retrospective studies, including observational, cohort, single-blinded and multicenter studies, comparing VCE and DBE for the diagnosis of OGIB, and data of all the vascular sources of bleeding were collected. All patients were subjected to the same gold standard method. Relevant data were then extracted from each included study using a standardized extraction form. We calculated study variables(sensitivity, specificity, prevalence, positive and negative predictive values and accuracy) and performed a meta-analysis using Meta-Disc software.RESULTS In the per-patient analysis, 17 studies(1477 lesions) were included. We identified3150 exams(1722 VCE and 1428 DBE) in 2043 patients and identified 2248 sources of bleeding, 1467 of which were from vascular lesions. Of these lesions, 864(58.5%) were diagnosed by VCE, and 613(41.5%) were diagnosed by DBE. The pretest probability for bleeding of vascular origin was 54.34%. The sensitivity of DBE was 84%(95%CI: 0.82-0.86; heterogeneity: 78.00%), and the specificity was92%(95%CI: 0.89-0.94; heterogeneity: 92.0%). For DBE, the positive likelihood ratio was 11.29(95%CI: 4.83-26.40; heterogeneity: 91.6%), and the negative likelihood ratio was 0.20(95%CI: 0.15-0.27; heterogeneity: 67.3%). Performing DBE after CE increased the diagnostic yield of vascular lesion by 7%, from 83% to90%.CONCLUSION The diagnostic accuracy of detecting small bowel bleeding from a vascular source is increased with the use of an isolated video capsule endoscope compared with isolated DBE. 6Role of polymorphisms in genes that encode cytokines and Helicobacter pylori virulence factors in gastric carcinogenesis显示文摘The Helicobacter pylori(H. pylori) infection is a determinant factor in gastric cancer(GC) development. However, the infection outcomes are variable and depend on both host and bacterial characteristics. Some host cytokines such as interleukin(IL)-1β, IL-1 Ra, IL-8, IL-10 and tumor necrosis factor-α play important roles in the host immune system response to the pathogen, in the development of gastric mucosal lesions and in cell malignant transformation. Therefore, these host factors are crucial in neoplastic processes. Certain polymorphisms in genes that encode these cytokines have been associated with an increased risk of GC. On the other hand, various virulence factors found in distinct H. pylori bacterial strains, including cytotoxinassociated antigen A, vacuolating cytotoxin, duodenal ulcer promoting gene A protein, outer inflammatory protein and blood group antigen binding adhesin, have been associated with the pathogenesis of different gastric diseases. The virulent factors mentioned above allow the successful infection by the bacterium and play crucial roles in gastric mucosa lesions, including malignant transformation. Moreover, the role of host polymorphisms and bacterial virulence factors in gastric carcinogenesis seems to vary among different countries and populations. 7Short-term complementary and alternative medicine on quality of life in women with fibromyalgia显示文摘BACKGROUND:Fibromyalgia(FMS)is a syndrome characterized by chronic widespread musculoskeletal pain,whose etiology is not completely understood.Different therapeutic approaches have been used with inconsistent results.This observation does not invalidate the continued search for alternative treatments aimed at improving quality of life(Qo L)in FMS.OBJECTIVE:This study compared three classical traditional Chinese medicine(TCM)therapies:acupuncture(AC),electroacupuncture(EAC)and moxibustion(MX)in the management of pain and promotion of Qo L in FMS patients.DESIGN,SETTING,PARTICIPANTS AND INTERVENTIONS:A preliminary,group-assigned,comparative study enrolled 30 women,mean age(46.90±9.24)years(range 20–60 years),who met the 1990 American College of Rheumatology criteria for FMS diagnosis and a pain-pressure threshold(PPT)< 4 kg/cm^2.The study was conducted in a teaching tertiary-care medical institution from May 2010 through April 2012.AC,EAC and MX were delivered for 30 min,once a week,for 8 weeks,bilaterally at Neiguan(PC6),Hegu(LI4),Yanglingquan(GB34),Sanyinjiao(SP6)and Taichong(LR3)acupoints.MAIN OUTCOME MEASURES:Each week,immediately before treatment and after treatment,subjects were tested for PPTs,Wong-Baker Faces Pain Scale(WBFPS;for pain intensity)and Medical Outcomes Study 36-item Short Form Health Survey(SF-36:for Qo L).RESULTS:There was no signifi cant improvement in pain or reduction of tender points in any of the groups studied,at the end of the 8th session.Signifi cant improvement of Qo L was perceived in vitality(after AC treatment)and in mental health(after EAC and MX treatments).CONCLUSION:TCM therapies(AC,EAC and MX)promoted an improvement in the Qo L in two areas(vitality and mental health)in FMS women.Further large-scale clinical trials are required to confi rm this effect.Paulo Araujo Dias Andre Brito Bastos Guimaraes Andrea de Oliveira Albuquerque Karoline Lucas de Oliveira Maria Luzete Costa Cavalcante Sergio Botelho Guimaraes 2016Journal of Integrative Medicine2016,14,1:7
8On the Misunderstanding of the Ziegler-Nichols's Formulae Usage显示文摘In the Ziegler-Nichols' s method of reaction curve,the proportional gain should be calculated as an inverse relation of the plant steady-state gain. One of the reasons behind this is to avoid an excessively high loop gain, which can jeopardize many required characteristics of the closed loop. However, many reports, scientific papers and books have been neglecting such gain compensation in the tuning formulae.This brief presents a comprehensive discussion about such uncompensated tuning rules. The main paper finding is that either the stability margin or the disturbance rejection is reduced in this case. A theoretical analysis is performed to obtain the main result. 10Nephrotoxicity in cancer treatment:An overview显示文摘Anticancer drug nephrotoxicity is an important and increasing adverse drug event that limits the efficacy of cancer treatment.The kidney is an important elimination pathway for many antineoplastic drugs and their metabolites,which occurs by glomerular filtration and tubular secretion.Chemotherapeutic agents,both conventional cytotoxic agents and molecularly targeted agents,can affect any segment of the nephron including its microvasculature,leading to many clinical manifestations such as proteinuria,hypertension,electrolyte disturbances,glomerulopathy,acute and chronic interstitial nephritis,acute kidney injury and at times chronic kidney disease.The clinician should be alert to recognize several factors that may maximize renal dysfunction and contribute to the increased incidence of nephrotoxicity associated with these drugs,such as intravascular volume depletion,the associated use of nonchemotherapeutic nephrotoxic drugs(analgesics,antibiotics,proton pump inhibitors,and bonetargeted therapies),radiographic ionic contrast media or radiation therapy,urinary tract obstruction,and intrinsic renal disease.Identification of patients at higher risk for nephrotoxicity may allow the prevention or at least reduction in the development and severity of this adverse effect.Therefore,the aim of this brief review is to provide currently available evidences on oncologic drug-related nephrotoxicity.Maria Luísa Cordeiro Santos Breno Bittencourt de Brito Filipe Antonio FranÇa da Silva Anelise Costa dos Santos Botelho Fabrício Freire de Melo 2020World Journal of Clinical Oncology2020,11,4:4

13Intelligent geospatial maritime risk analytics using the Discrete Global Grid System显示文摘Each year,accidents involving ships result in significant loss of life,environmental pollution and economic losses.The promotion of navigation safety through risk reduction requires methods to assess the spatial distribution of the relative likelihood of occurrence.Yet,such methods necessitate the integration of large volumes of heterogenous datasets which are not well suited to traditional data structures.This paper proposes the use of the Discrete Global Grid System(DGGS)as an efficient and advantageous structure to integrate vessel traffic,metocean,bathymetric,infrastructure and other relevant maritime datasets to predict the occurrence of ship groundings.Massive and heterogenous datasets are well suited for machine learning algorithms and this paper develops a spatial maritime risk model based on a DGGS utilising such an approach.A Random Forest algorithm is developed to predict the frequency and spatial distribution of groundings while achieving an R2 of 0.55 and a mean squared error of 0.002.The resulting risk maps are useful for decision-makers in planning the allocation of mitigation measures,targeted to regions with the highest risk.Further work is identified to expand the applications and insights which could be achieved through establishing a DGGS as a global maritime spatial data structure.Andrew Rawson Zoheir Sabeur Mario Brito 2022Big Earth Data2022,6,3:3

19Extramedullary plasmocytoma associated with a massive deposit of amyloid in the duodenum显示文摘We report a rare case of extramedullary plasmocytoma associated with a massive deposit of amyloid in the duodenum.A 72-year-old Japanese man was admitted to our hospital presenting with a 3-mo history of epigastric pain, vomiting and weight loss.On computed tomography(CT) a wall thickening of the fourth part of the duodenum was observed.Multiple biopsies obtained from the lesion showed inflltration of plasma cells and lymphocytes, but they were not conclusive.The patient underwent resection of the lesion and, on histopathological examination, the lesion consisted of a dense and diffuse inflltrate of plasma cells and a few admixed lymphocytes with reactive follicles extending to the muscular propria.An extensive deposition of amyloid was also observed.Immunohistochemical stains revealed that a few plasmacytoid cells showed λ light chain staining, though most were κ light chain positive.These cells also were positive for CD138 and CD56 but negative for CD20 and CD79.The flndings were consistent with extramedullary plasmocytomaassociated with a massive deposit of amyloid in duodenum.A subsequent workup for multiple myeloma was completely negative.The patient showed no signs of local recurrence or dissemination of the disease after 12 mo follow-up.Because of the association of plasmocytoma and amyloidosis, the patient must be followed up because of the possible systemic involvement of the neoplasm and amyloidosis in future.Fabiana Pirani Carneiro Maria de Nazareth Machado Sobreira Lívia Bravo Maia Alesso Cervantes Sartorelli Luiz Eduardo de Almeida Prado Franceschi Mauro Brito Brando Bárbara Wosnjuk Calaca Fernando Silva Lustosa Joo Vieira Lopes 2009World Journal of Gastroenterology2009,15,28:2
20Architecting Fault Tolerance with Exception Handling: Verification and Validation显示文摘When building dependable systems by integrating untrusted software components that were not originally designed to interact with each other, it is likely the occurrence of architectural mismatches related to assumptions in their failure behaviour. These mismatches, if not prevented during system design, have to be tolerated during runtime. This paper presents an architectural abstraction based on exception handling for structuring fault-tolerant software systems. This abstraction comprises several components and connectors that promote an existing untrusted software element into an idealised fault-tolerant architectural element. Moreover, it is considered in the context of a rigorous software development approach based on formal methods for representing the structure and behaviour of the software architecture. The proposed approach relies on a formal specification and verification for analysing exception propagation, and verifying important dependability properties, such as deadlock freedom, and scenarios of architectural reconfiguration. The formal models are automatically generated using model transformation from UML diagrams: component diagram representing the system structure, and sequence diagrams representing the system behaviour. Finally, the formal models are also used for generating unit and integration test cases that are used for assessing the correctness of the source code.
