1Clinical importance of aspirin and clopidogrel resistance显示文摘Aspirin and clopidogrel are important components of medical therapy for patients with acute coronary syndromes, for those who received coronary artery stents and in the secondary prevention of ischaemic stroke. Despite their use, a significant number of patients experience recurrent adverse ischaemic events. Interindividual variability of platelet aggregation in response to these antiplatelet agents may be an explanation for some of these recurrent events, and small trials have linked 'aspirin and/or clopidogrel resistance', as measured by platelet function tests, to adverse events. We systematically reviewed all available evidence on the prevalence of aspirin/clopidogrel resistance, their possible risk factors and their association with clinical outcomes. We also identified articles showing possible treatments. After analyzing the data on different laboratory methods, we found that aspirin/clopidogrel resistance seems to be associated with poor clinical outcomes and there is currently no standardized or widely accepted definition of clopidogrel resistance. 3Biliary cysts:Etiology,diagnosis and management显示文摘Biliary cysts(BC) are rare dilatations of different parts of a biliary tract.They account for approximately 1% of all benign biliary diseases.BC occur the most frequently in Asian and female populations.They are an important problem for pediatricians,gastroenterologists,radiologists and surgeons.Clinical presentation and management depend on the BC type.Cholangiocarcinoma is the most serious and dangerous BC complication.The other complications associated with BC involve cholelithiasis and hepatolithiasis,cholangitis,acute and chronic pancreatitis,portal hypertension,liver fibrosis and secondary liver cirrhosis and spontaneous cyst perforation.Different BC classifications have been described in the literature.Todani classification dividing BC into five types is the most useful in clinical practice.The early diagnosis and proper treatment are very important,because BC are associated with a risk of carcinogenesis.A malignancy risk increases with the age.Radiological investigations(ultrasonography,computed tomography,endoscopic retrograde cholangiopancreatography and magnetic resonance cholangiopancreatography) play an important role in BC diagnostics.Currently,prenatal diagnosis using ultrasonography is possible.It allows to differentiate biliary disorders in fetals and to perform the early surgical treatment that improves results.In most patients,total cyst excision with Roux-Y hepaticojejunostomy is the treatment of choice.Surgical treat-ment of BC is associated with high success rate and low morbidity and mortality.The early treatment is associated with a lower number of complications.Patients following BC surgery require permanent and careful postoperative observation using laboratory and imaging investigations because of possibility of biliary anastomosis stricture and biliary cancer in tissue remnant.Beata Jabońska 2012World Journal of Gastroenterology2012,18,35:26
4Iatrogenic bile duct injuries:Etiology,diagnosis and management显示文摘Iatrogenic bile duct injuries (IBDI) remain an important problem in gastrointestinal surgery. They are most frequently caused by laparoscopic cholecystectomy which is one of the commonest surgical procedures in the world. The early and proper diagnosis of IBDI is very important for surgeons and gastroenterologists, because unrecognized IBDI lead to serious complications such as biliary cirrhosis, hepatic failure and death. Laboratory and radiological investigations play an important role in the diagnosis of biliary injuries. There are many classifications of IBDI. The most popular and simple classification of IBDI is the Bismuth scale. Endoscopic techniques are recommended for initial treatment of IBDI. When endoscopic treatment is not effective, surgical management is considered. Different surgical reconstructions are performed in patients with IBDI. According to the literature, Roux- en-Y hepaticojejunostomy is the most frequent surgical reconstruction and recommended by most authors. In the opinion of some authors, a more physiological and equally effective type of reconstruction is end- to-end ductal anastomosis. Long term results are the most important in the assessment of the effectiveness of IBDI treatment. There are a few classifications for the long term results in patients treated for IBDI; the Terblanche scale, based on clinical biliary symptoms, is regarded as the most useful classification. 5Bacteriophages and Lysins in Biofilm Control显示文摘To formulate the optimal strategy of combatting bacterial biofilms,in this review we update current knowledge on the growing problem of biofilm formation and its resistance to antibiotics which has spurred the search for new strategies to deal with this complication.Based on recent findings,the role of bacteriophages in the prevention and elimination of biofilm-related infections has been emphasized.In vitro,ex vivo and in vivo biofilm treatment models with single bacteriophages or phage cocktails have been compared.A combined use of bacteriophages with antibiotics in vitro or in vivo confirms earlier reports of the synergistic effect of these agents in improving biofilm removal.Furthermore,studies on the application of phage-derived lysins in vitro,ex vivo or in vivo against biofilm-related infections are encouraging.The strategy of combined use of phage and antibiotics seems to be different from using lysins and antibiotics.These findings suggest that phages and lysins alone or in combination with antibiotics may be an efficient weapon against biofilm formation in vivo and ex vivo,which could be useful in formulating novel strategies to combat bacterial infections.Those findings proved to be relevant in the prevention and destruction of biofilms occurring during urinary tract infections,orthopedic implant-related infections,periodontal and peri-implant infections.In conclusion,it appears that most efficient strategy of eliminating biofilms involves phages or lysins in combination with antibiotics,but the optimal scheme of their administration requires further studies.Marzanna Lusiak-Szelachowska Beata Weber-Dabrowska Andrzej Gorski 2020Virologica Sinica2020,35,2:11
6Epithelioid hemangioendothelioma of the liver as a rare indication for liver transplantation显示文摘AIM: To investigate the indications and outcomes of liver transplantation for hepatic epithelioid hemangioendothelioma(HEHE). METHODS: Between 1989 and August 2013, in the Department of General, Transplant, and Liver Surgery, Medical University of Warsaw, 1306 orthotopic liver transplantations(OLTx) were performed, including 72 retransplantations. Unresectable HEHE was an indication for OLTx in 10 patients(0.8% of primary OLTx), the mean age of the patients was 40.5 ± 13.3 years(range 23-65 years), and the male-to-female ratio was 2:8. Kaplan-Meier survival analysis in HEHE, hepatocellular carcinoma(HCC), and other OLTx recipients groups was performed. The differences in mortality were compared using the χ2 test. A P-value < 0.05 indicated statistical significance.RESULTS: No concomitant liver disease was found in any patient. There was no neoadjuvant chemotherapy or radiotherapy. Liver function test results were normal in most of the patients. The levels of alpha-fetoprotein, carcinoembryonic antigen, and carbohydrate antigen 19-9 were normal. In immunohistochemical staining, the neoplastic cells were positive for factor Ⅷ-related antigen, CD31, and CD34, which are endothelial cell markers, and negative for cytokeratin 19, cytokeratin 7, and HepPar-1. Nine patients were alive without tumor recurrence. One patient died 2 mo after OLTx due to septic complications. No morbidity was observed. Maximum follow-up was 11.4 years, with a minimum of 1 mo. The cumulative survival rate at the end of followup in HEHE patients was 87.5% compared with 54.3% in the HCC group and 76.3% in the other OLTx recipients group(χ2 test = 1.784, df = 2, P = 0.409).CONCLUSION: Unresectable HEHE, without extrahepatic metastases is an excellent indication for liver transplantation. 7Intestinal epithelium, intraepithelial lymphocytes and the gut microbiota-Key players in the pathogenesis of celiac disease显示文摘Celiac disease(CD) is a chronic immune-mediated disorder triggered by the ingestion of gluten in genetically predisposed individuals. Before activating the immune system, gluten peptides are transferred by the epithelial barrier to the mucosal lamina propria, where they are deamidated by intestinal tissue transglutaminase 2. As a result, they strongly bind to human leucocyte antigens(HLAs), especially HLA-DQ2 and HLA-DQ8, expressed on antigen-presenting cells. This induces an inflammatory response, which results in small bowel enteropathy. Although gluten is the main external trigger activating both innate and adaptive(specific) immunity, its presence in the intestinal lumen does not fully explain CD pathogenesis. It has been hypothesized that an early disruption of the gut barrier in genetically susceptible individuals, which would result in an increased intestinal permeability, could precede the onset of gluten-induced immune events. The intestinal barrier is a complex functional structure, whose functioning is dependent on intestinal microbiotahomeostasis, epithelial layer integrity, and the gutassociated lymphoid tissue with its intraepithelial lymphocytes(IELs). 12Complete Oxidation of Methane over Palladium Supported on Alumina Modified with Calcium,Lanthanum,and Cerium Ions显示文摘The activity and thermal stability of Pd/Al_2O_3 and Pd/(Al_2O_3+MO_x)(M=Ca,La,Ce) palladium catalysts in the reaction of complete oxidation of methane are presented in this study.The catalyst supports were prepared by sol-gel method and they were dried either conventionally or with supercritical carbon dioxide.Then they were impregnated with palladium nitrate solution.The catalysts with unmodified alumina had a high surface area.The activity and thermal stability of the alumina- supported catalyst was also very high.The introduction of calcium,lanthanum,or cerium oxide into alumina support caused a decrease of the surface area in the way dependent on the support precursor drying method.These modifiers decreased the activity of palladium catalysts,and they required higher temperatures for the complete oxidation of methane than unmodified Pd/Al_2O_3.The improvement of the palladium activity by lanthanum and cerium support modifier was observed only at low temperatures of the reaction.Beata Stasinska Wojciech Gac Theophilos Ioannides Andrzej Machocki 2007Journal of Natural Gas Chemistry2007,16,4:4
13Modeling of Thermal Properties of Thermal Insulation Layered with Transparent,Opaque and Reflective Film显示文摘The popular and effective food preservation technology based on refrigeration is not sufficient for high-quality products while undergoing logistic operations(transport and retail). One of the basic factors that affects the quality of chilled and frozen food products during storage and transport is packaging. A protective function of packaging strongly depends on the material used and its composition. There are different kinds of thermal insulation used for food packaging. One of them, proposed by the authors is a multilayer structure of insulation made of rectangular air cells. The insulation can be prepared by means of plastic film featuring various properties. The paper presents how to improve an effective material designed for food freezing and transport aiming to enhance its thermal resistance through the application of different transparency, reflectivity and emissivity of the film. Mathematical model based on heat exchange equations, including conduction, convection and radiation throughout a number of parallel internal sheets of film of multilayer structures was proposed. Thermal properties depending on different transparency, reflectivity and emissivity of the film were analyzed. The model was verified experimentally showing its compatibility and obtaining a significant influence of thermal resistance according to the type of film used to make air structures, the number and thickness of its layers as well as the gaps between internal folds. 14Hepatectomy for bile duct injuries: When is it necessary?显示文摘Iatrogenic bile duct injuries(IBDI)are still a challenge for surgeons.The most frequently,they are caused by laparoscopic cholecystectomy which is one of the commonest surgical procedure in the world.Endoscopic techniques are recommended as initial treatment of IBDI.When endoscopic treatment is not effective,surgery is considered.Different surgical biliary reconstructions are performed in most patients in IBDI.Roux-Y hepaticojejunostomy is the commonest biliary reconstruction for IBDI.In some patients with complex IBDI,hepatectomy is required.Recently,Liet al analyzed the factors that had led to hepatectomy for patients with IBDI after laparoscopic cholecystectomy(LC).Authors concluded that hepatectomy might be necessary to manage early or late complications after LC.The study showed that proximal IBDI(involving hepatic confluence)and IBDI associated with vascular injuries were the two independent risk factors of hepatectomy in this series.Authors distinguished two main groups of patients that require liver resection in IBDI:those with an injury-induced liver necrosis necessitating early intervention,and those in whom liver resection is indicated for treatment of liver atrophy following long-term cholangitis.In this commentary,indications for hepatectomy in patients with IBDI are discussed.Complex biliovascular injuries as indications for hepatectomy are presented.Short-and long-term results in patients following liver resection for IBDI are also discussed.Hepatectomy is not a standard procedure in surgical treatment of IBDI,but in some complex injuries it should be considered.Beata Jablońska 2013World Journal of Gastroenterology2013,19,38:4

17Role of cyclooxygenase-2 gene polymorphisms in pancreatic carcinogenesis显示文摘AIM:To evaluate the clinical significance of-765G/C and-1195G/A cyclooxygenase-2 (COX-2) gene polymorphisms in patients with pancreatic cancer (PC).METHODS:The study included 201 patients:85 with PC and 116 healthy controls.-765G/C and-1195G/A COX-2 gene polymorphisms were studied in DNA isolated from blood samples.The associations of the analyzed genotypes and clinical data at diagnosis were evaluated.RESULTS:We found an increased frequency of the homozygous-1195AA COX-2 genotype in patients with PC (53.7%) compared with the control group (21%) (P < 0.01).In contrast,the distribution of genotypeand allele frequencies of the-765G/C COX-2 polymorphism in the PC patients were not different from those in control groups.A correlation between presence of homozygous-1195AA COX-2 genotype and tumor size > 3 cm was observed (P < 0.05).Analyzed polymorphisms were unrelated to the patients' sex and age,nor to the presence of regional or distant metastases.CONCLUSION:These preliminary results indicate that the-1195G/A COX-2 polymorphism may play an important role in PC prognosis and carcinogenesis.Renata Talar-Wojnarowska Anita Gasiorowska Marek Olakowski Pawel Lampe Beata Smolarz Hanna Romanowicz-Makowska Ewa Malecka-Panas 2011World Journal of Gastroenterology2011,17,36:4

19Mass budgets of the Lambert, Mellor and Fisher Glaciers and basal fluxes beneath their flowbands on Amery Ice Shelf显示文摘We used in situ measurements and remote-sensing data sets to evaluate the mass budgets of the Lambert, Mellor and Fisher Glaciers and the basal melting and freezing rates beneath their flowbands on the Amery Ice Shelf. Our findings show the Lambert and Mellor Glaciers upstream of the ANARE Lambert Glacier Basin (LGB) traverse may have positive imbalances of 3.9±2.1 Gt a-1 and 2.1±2.4 Gt a-1, respectively, while the Fisher Glacier is approximately in balance. The upstream region as a whole has a positive imbalance of 5.9±4.9 Gt a-1. The three same glaciers downstream of the ANARE LGB traverse line are in negative imbalance, where the whole downstream region has a negative imbalance of -8.5±5.8 Gt a-1. Overall the mass budgets of the Lambert, Mellor, and Fisher Glaciers are close to bal-ance, and the collective three-glacier system is also nearly in balance with a mass budget of -2.6±6.5 Gt a-1. The significant positive imbalances for the interior basin upstream of the ice-movement stations established in the early 1970s (GL line) reported previously are possibly due to an overestimate of the total accumulation and an underestimate of the ice flux through the GL line. The mean melting rate is -23.0±3.5 m ice a-1 near the southern grounding line, which decreases rapidly downstream, and transitions to refreezing at around 300 km from the southern extremity of the Amery Ice Shelf. Freezing rates along the flowbands are around 0.5±0.1 to 1.5±0.2 m ice a-1. The per-centage of ice lost from the interior by basal melting beneath the flowbands is about 80%±5%. The total basal melting and refreezing beneath the three flowbands is 50.3±7.5 Gt ice a-1 and 7.0±1.1 Gt ice a-1, respectively. We find a much larger total basal melting and net melting than the results for the whole Amery Ice Shelf derived from previous modeling and oceanographic measurements.Kenneth C. JEZEK Beata M. 20Pancreatic intraductal papillary mucinous neoplasms:Current diagnosis and management显示文摘Intraductal papillary mucinous neoplasms(IPMNs)represent approximately 1%of all pancreatic neoplasms and 25%of cystic neoplasms.They are divided into three types:main duct-IPMN(MD-IPPMN),branch duct-IPMN(BD-IPMN),and mixed type-IPMN.In this review,diagnostics,including clinical presentation and radiological investigations,were described.Magnetic resonance imaging is the most useful for most IPMNs.Management depends on the type and radiological features of IPMNs.Surgery is recommended for MD-IPMN.For BD-IPMN,management involves surgery or surveillance depending on the tumor size,cyst growth rate,solid components,main duct dilatation,high-grade dysplasia in cytology,the presence of symptoms(jaundice,new-onset diabetes,pancreatitis),and CA 19.9 serum level.The patient’s age and comorbidities should also be taken into consideration.Currently,there are different guidelines regarding the diagnosis and management of IPMNs.In this review,the following guidelines were presented:Sendai International Association of Pancreatology guidelines(2006),American Gastroenterological Association guidelines,revised international consensus Fukuoka guidelines(2012),revised international consensus Fukuoka guidelines(2017),and European evidence-based guidelines according to the European Study Group on Cystic Tumours of the Pancreas(2018).The Verona Evidence-Based Meeting 2020 was also presented and discussed.Beata Jabłońska PawełSzmigiel Sławomir Mrowiec 2021World Journal of Gastrointestinal Oncology2021,13,12:3
