| 1 | Deep sternal wound infection after cardiac surgery: Evidences and controversies显示文摘Despite many advances in prevention and perioperative care, deep sternal wound infection(DSWI) remains a pressing concern in cardiac surgery, with a still relevant incidence and with a considerable impact on in-hospital mortality and also on mid- and long-term survival. The permanent high impact of this complication is partially related to the increasing proportion of patients at highrisk for infection, as well as to the many patient and surgical risk factors involved in the pathogenesis of DSWI. The prophylactic antibiotic therapy is one of the most important tools in the prevention of DSWI. However, the choice of antibiotic, the dose, the duration, the adequate levels in serum and tissue, and the timing of antimicrobial prophylaxis are still controversial. The treatment of DSWI ranges from surgical revision with primary closure to surgical revision with open dressings or closed irrigation, from reconstruction with soft tissue flaps to negative pressure wound therapy(NPWT). However, to date, there have been no accepted recommendations regarding the best management of DSWI. Emerging evidence in the literature has validated the efficacy and safety of NPWT either as a single-line therapy, or as a 'bridge' prior to final surgical closure. | 3 | Incidence and risk factors for potentially suboptimal serum concentrations of vancomycin during cardiac surgery显示文摘AIM To investigate the incidence and risk factors for vancomycin concentrations less than 10 mg/L during cardiac surgery.METHODS In this prospective study, patients undergoing cardiac surgery received a single dose of 1000 mg of vancomycin. Multiple arterial samples were drawn during surgery. Exclusion criteria were hepatic dysfunction; renal dysfunction; ongoing infectious diseases; solid or hematologic tumors; severe insulin-dependent diabetes; body mass index of < 17 or > 40 kg/m^2; pregnancy or lactation; antibiotic, corticosteroid, or other immunosuppressive therapy; vancomycin or nonsteroidal anti-inflammatory drug therapy in the previous 2 wk; chemotherapy or radiation therapy in the previous 6 mo; allergy to vancomycin or cefazolin; drug abuse; cardiac surgery in the previous 6 mo; previous or scheduled organ transplantation; preoperative stay in the intensive care unit for more than 24 h; emergency procedure or lack of adequate preparation for surgery; and participation in another trial.RESULTS Over a 1-year period, 236 patients were enrolled, and a total of 1682 serum vancomycin concentrations(median 7/patient) were measured. No vancomycin levels under 10 mg/L were recorded in 122 out of 236 patients(52%), and 114 out of 236 patients(48%) were found to have at least 1 serum sample with a vancomycin level < 10 mg/L; 54 out of 236 patients(22.9%) had at least 5 serum samples with a vancomycin level lower than 10 mg/L. Vancomycin infusion was administered for 60 min in 97 out of 236 patients(41%). In 47 patients(20%), the duration of infusion was longer than 60 min, and in 92 patients(39%) the duration of infusion was shorter than 60 min. The maximum concentration and area under the concentration-time curve were significantly higher in patients with no vancomycin levels less than 10 mg/L(P < 0.001). The multivariate analysis identified female gender, body mass index(BMI) > 25 kg/m^2, and creatinine clearance above 70 mL/min as risk factors for vancomycin levels less than 10 mg/L.CONCLUSION Results of this study identified female gender, BMI > 25 kg/m^2, and creatinine clearance above 70 mL/min as risk factors for suboptimal vancomycin serum concentration during cardiac surgery; no relationship was found between infusion duration and vancomycin levels less than 10 mg/L. | 4 | Cretaceous tectonic evolution of the Neo-Tethys in Central Iran:Evidence from petrology and age of the Nain-Ashin ophiolitic basalts显示文摘The Nain and Ashin ophiolites consist of Mesozoic melange units that were emplaced in the Late Cretaceous onto the continental basement of the Central-East Iran microcontinent(CEIM).They largely consist of serpentinized peridotites slices;nonetheless,minor tectonic slices of sheeted dykes and pillow lavas-locally stratigraphically associated with radiolarian cherts-can be found in these ophiolitic melanges.Based on their whole rock geochemistry and mineral chemistry,these rocks can be divided into two geochemical groups.The sheeted dykes and most of the pillow lavas show island arc tholeiitic(IAT)affinity,whereas a few pillow lavas from the Nain ophiolites show calc-alkaline(CA)affinity.Petrogenetic modeling based on trace elements composition indicates that both IAT and CA rocks derived from partial melting of depleted mantle sources that underwent enrichment in subduction-derived components prior to melting.Petrogenetic modeling shows that these components were represented by pure aqueous fluids,or sediment melts,or a combination of both,suggesting that the studied rocks were formed in an arc-forearc tectonic setting.Our new biostratigraphic data indicate this arc-forearc setting was active in the Early Cretaceous.Previous tectonic interpretations suggested that the Nain ophiolites formed,in a Late Cretaceous backarc basin located in the south of the CEIM(the so-called Nain-Baft basin).However,recent studies showed that the CEIM underwent a counter-clockwise rotation in the Cenozoic,which displaced the Nain and Ashin ophiolites in their present day position from an original northeastward location.This evidence combined with our new data and a comparison of the chemical features of volcanic rocks from different ophiolites around the CEIM allow us to suggest that the Nain-Ashin volcanic rocks and dykes were formed in a volcanic arc that developed on the northern margin of the CEIM during the Early Cretaceous in association with the subduction,below the CEIM,of a Neo-Tethys oceanic branch that was existing between the CEIM and the southern margin of Eurasia.As a major conclusion of this paper,a new geodynamic model for the Cretaceous evolution of the CEIM and surrounding Neo-Tethyan oceanic basins is proposed. | Tahmineh Pirnia Emilio Saccani Ghodrat Torabi Marco Chiari Spela Gorican Edoardo Barbero | 2020 | Geoscience Frontiers2020,11,1: | 2 |
| 5 | Insight into the interaction of inhaled corticosteroids with human serum albumin: A spectroscopic-based study显示文摘It is well known that the safety and efficacy profile of an inhaled cortocosteroid(ICS) is influenced by the pharmacokinetic properties and associated pharmacodynamic effects of the drug. Freely circulating,protein unbound, and active ICS can cause systemic adverse effects. Therefore, a detailed investigation of drug-protein interaction could be of great interest to understand the pharmacokinetic behaviour of corticosteroids and for the design of new analogues with effective pharmacological properties. In the present work, the interaction between some corticosteroids and human serum albumin(HSA) has been studied by spectroscopic approaches. UV–Vis spectroscopy confirmed that all the investigated corticosteroids can bind to HSA forming a protein-drug complex. The intrinsic fluorescence of HSA was quenched by all the investigated drugs, which was rationalized in terms of a static quenching mechanism.
