| 1 | 急性脑梗死静脉溶栓后早期神经功能恶化与血清白蛋白水平的关系研究显示文摘目的探讨急性脑梗死静脉溶栓后早期神经功能恶化(END)与血清白蛋白水平的关系。方法收集2017年1月至2018年8月发病4.5 h内接受阿替普酶静脉溶栓治疗患者的临床资料。根据是否出现END,分为恶化组和非恶化组,分析血清白蛋白水平与END发生的相关性。结果共纳入148例患者,其中恶化组39例,非恶化组109例。与非恶化组相比,恶化组总蛋白水平、白蛋白水平和淋巴细胞计数较低,且差异有统计学意义(P<0.05)。恶化组的中性粒细胞计数、红细胞分布宽度、国际标准化比值、纤维蛋白(原)降解产物以及入院时美国国立卫生研究院卒中量表(NIHSS)评分高于非恶化组,差异有统计学意义(P<0.05)。恶化组多伴有心房颤动,且两组在血管狭窄程度及梗死部位方面比较差异也有统计学意义(P<0.05)。多因素Logistic回归分析显示,血清白蛋白(OR=0.873,95%CI 0.768~0.993)、入院时NIHSS评分(OR=1.120,95%CI 1.008~1.245)、中性粒细胞计数(OR=1.271,95%CI 1.048~1.542),以及心源性卒中和责任血管狭窄程度均可能影响急性脑梗死静脉溶栓的短期预后。出院时改良Rankin量表(mRS)评分(平均14 d)与白蛋白水平呈显著负相关(r=-0.205,P=0.013)。结论相比于非恶化患者,恶化患者的白蛋白水平较低。高水平的血清白蛋白是急性缺血性脑卒中静脉溶栓后END的保护因素。血清白蛋白水平与患者的短期预后存在相关性,白蛋白水平越低,mRS评分越高,预后越差。 | 钱新宇 吕金峰 Bilal Muhammad 杜波 耿德勤 | 2019 | 中国医师进修杂志2019,42,4: | 33 |
| 2 | Indoleamine 2,3-dioxygenase: As a potential prognostic marker and immunotherapeutic target for hepatocellular carcinoma显示文摘Tumor cells induce an immunosuppressive microen-vironment which leads towards tumor immune escape. Understanding the intricacy of immunomodulation by tumor cells is essential for immunotherapy. Indoleamine 2,3-dioxygenase(IDO) is an immunosuppressive enzyme which mediates tumor immune escape in various cancers including hepatocellular carcinoma(HCC). IDO up-regulation in HCC may lead to recruitment of regulatory T-cells into tumor microenvironment and therefore inhibit local immune responses and promote metastasis. HCC associated fibroblasts stimulate natural killer cells dysfunction through prostaglandin E2 and subsequently IDO promotes favorable condition for tumor metastasis. IDO up-regulation induces immuno-suppression and may enhance the risk of hepatitis C virus and hepatitis B virus induced HCC. Therefore, IDO inhibitors as adjuvant therapeutic agents may have clinical implications in HCC. | 7 | Assessment of sunflower germplasm for phytoremediation of lead-polluted soil and production of seed oil and seed meal for human and animal consumption显示文摘Phytoremediation is a valuable technology for mitigating soil contamination in agricultural lands,but phytoremediation without economic revenue is unfeasible for land owners and farmers.The use of crops with high biomass and bioenergy for phytoremediation is a unique strategy to derive supplementary benefits along with remediation activities.Sunflower(Helianthus annuus L.)is a high-biomass crop that can be used for the phytoremediation of polluted lands with additional advantages(biomass and oil).In this study,40 germplasms of sunflower were screened in field conditions for phytoremediation with the possibility for oil and meal production.The study was carried out to the physiological maturity stage.All studied germplasms mopped up substantial concentrations of Pb,with maximum amounts in shoot>root>seed respectively.The phytoextraction efficiency of the germplasm was assessed in terms of the Transfer factor(TF),Metal removal efficiency(MRE)and Metal extraction ratio(MER).Among all assessed criteria,GP.8585 was found to be most appropriate for restoring moderately Pb-contaminated soil accompanied with providing high biomass and high yield production.The Pb content in the oil of GP.8585 was below the Food safety standard of China,with 59.5% oleic acid and 32.1% linoleic acid.Moreover,amino acid analysis in meal illustrated significant differences among essential and nonessential amino acids.Glutamic acid was found in the highest percentage(22.4%),whereas cysteine in the lowest percentage(1.3%).Therefore,its efficient phytoextraction ability and good quality edible oil and meal production makes GP.8585 the most convenient sunflower germplasm for phytoremediation of moderately Pb-contaminated soil,with fringe benefits to farmers and landowners. | Afsheen Zehra Zulfiqar Ali Sahito Wenbin Tong Lin Tang Yasir Hamid Muhammad Bilal Khan Zarina Ali Beena Naqui Xiaoe Yang | 2020 | Journal of Environmental Sciences2020,32,1: | 10 |

| 11 | Anatomy of a eustatic event during the Turonian (Late Cretaceous) hot greenhouse climate显示文摘Sequence stratigraphic studies consider relative change in sea level(as regulated by eustasy,local tectonics and sediment supply)as the main builder of the stratigraphic record.Eustasy has generally been considered as a consequence of the growth and decay of continental ice sheets that would explain large,rapid changes in sea level,even during periods of relative global climatic warmth.However,such a mechanism has become increasingly difficult to envision during times of extreme global warmth such as the Turonian,when the equator-to-pole temperature gradient was very low and the presence of polar ice seems improbable.This paper investigates the timing and extent of sea level falls during the late Cenomanian through Turonian,especially the largest of those events,sequence boundary KTu4,which occurred during the middle to late Turonian peak of the Cretaceous hot greenhouse climate.We conclude that the amplitude of the widespread third-order sea level fall in the middle Turonian that is centered at^91.8 Ma varies at different locations depending on the influence of dynamic topography on local tectonics and regional climatic conditions.Ice volume variations seem unlikely as a mechanism for controlling sea level at this time.However,this causal factor cannot be ruled out completely since Antarctic highlands(if they existed in the Late Cretaceous)could sequester enough water as ice to cause eustatic falls.To ascertain this requires detailed tomographic imaging of Antarctica,followed by geodynamic modeling,to determine whether high plateaus could have existed to accumulate ephemeral ice sheets.Other mechanisms for sea level change,such as transference between ground water(a small amplitude shorter time scale effect)and the ocean and entrainment and release of water from the mantle to the oceanic reservoir(a potentially large amplitude and longer time scale process),are intriguing and need to be explored further to prove their efficacy at third-order time scales. | Bilal U. | 12 | Evaluation and management of acute pancreatitis显示文摘Acute pancreatitis(AP) is one of the most common gastrointestinal causes for hospi-talization in the United States. In 2015, AP accounted for approximately390000 hospitalizations. The burden of AP is only expected to increase over time.Despite recent advances in medicine, pancreatitis continues to be associated with a substantial morbidity and mortality. The most common cause of AP is gallstones, followed closely by alcohol use. The diagnosis of pancreatitis is established with any two of three following criteria:(1) Abdominal pain consistent with that of AP;(2) Serum amylase and/or lipase greater than three times the upper limit of normal; and(3) Characteristics findings seen in crosssectional abdominal imaging. Multiple criteria and scoring systems have been established for assessing severity of AP. The cornerstones of management include aggressive intravenous hydration, appropriate nutrition and pain management.Endoscopic retrograde cholangiopancreatography and surgery are important aspects in management of acute gallstone pancreatitis. | 13 | Does urodynamics predict voiding after benign prostatic hyperplasia surgery in patients with detrusor underactivity?显示文摘Objective:We sought to determine if urodynamic study(UDS)predicted voiding outcomes in men with detrusor underactivity(DU)and benign prostatic enlargement(BPE)who underwent photovaporization of the prostate(PVP).Methods:Between September 2010 and July 2015,106 male patients with BPE and DU were identified.All patients underwent PVP.Urinary retention was noted by the preoperative necessity for an indwelling or intermittent catheter.Data collection included comorbidities,quality of life(QoL)scores,prostate volume,prostate-specific antigen(PSA),UDS and perioperative outcomes.UDS parameters included volume at first desire to void,volume at first urge to void,volume of severe urge,volume at capacity,compliance,detrusor contractions,maximum urinary flow rate(Qmax),and postvoid residual(PVR).Results:A total of 106 men were included in this analysis,who had urinary retention with a Foley catheter or clean intermittent catheterization(CIC)at the time of surgery.At baseline we found patients who voided had a detrusor pressure at Qmax(Pdet@Qmax)of 10.05±6.45 cmH2O compared to 16.78±12.17 cmH2O in those who did not void(p=0.071).Postoperatively,96(90.6%,mean age 76.9±26.2 years)of patients voided successfully while 10(9.4%,mean age 80.52±9.61 years)of patients remained in urinary retention.Mean baseline Qmax was 4.895±5.452 mL/s and 2.900±3.356 mL/s(p=0.087)in those who voided and did not respectively.PVR was 319.23±330.62 mL in those who voided and 276.88263.27 mL(p=0.344)in those who did not void.No UDS parameter predicted who would void postoperatively or improvements in QoL.Conclusions:The patients with DU and BPE might be able to successfully void after undergoing PVP regardless of UDS findings.All men who voided had improved international prostate symptom score and QoL scores compared to baseline and these parameters were durable up to 12 months. | Dominique Thomas Kevin C.Zorn Nadir Zaidi Stephanie Ashley Chen Yiye Zhang Alexis Te Bilal Chughtai | 2019 | Asian Journal of Urology2019,6,3: | 8 |
| 14 | Development of polyvinylpyrrolidone/paclitaxel self-assemblies for breast cancer显示文摘The goal of this investigation was to develop and demonstrate a polymer/paclitaxel selfassembly(PTX-SA) formulation. Polymer/PTX-SAs were screened based on smaller size of formulation using dynamic light scattering analysis. Additionally, fluorescence microscopy and flow cytometry studies exhibited that polyvinylpyrrolidone(PVP)-based PTX-SAs(PVP/PTX-SAs) had superior cellular internalization capability in MCF7 and MDA-MB-231 breast cancer cells. The optimized PVP/PTXSAs exhibited less toxicity to human red blood cells indicating a suitable formulation for reducing systemic toxicity. The formation of PVP and PTX self-assemblies was confirmed using fluorescence quenching and transmission electron microscopy which indicated that the PVP/PTX-SAs were spherical in shape with an average size range of 53.81 nm as detected by transmission electron microscopy(TEM).FTIR spectral analysis demonstrates incorporation of polymer and paclitaxel functional groups in PVP/PTX-SAs. Both proliferation(MTS) and clonogenic(colony formation) assays were used to validate superior anticancer activity of PVP/PTX-SAs in breast cancer cells over paclitaxel. Such superior anticancer activity was also demonstrated by downregulation of the expression of pro-survival protein(Bcl-x L), upregulation of apoptosis-associated proteins(Bid, Bax, cleaved caspase 7, and cleaved PARP)and β-tubulin stabilization. The effect of the gap G D between the cylinder surface and the wall on the flow behavior was studied. For the case of G D ≤ 0.25, the periodicity in the flow is attributed to both the outer shear layer instability and the oscillating frequency. | 19 | 荧光介孔二氧化硅负载丙硫菌唑纳米颗粒的制备及性能研究显示文摘近年来,丙硫菌唑作为一种新颖的广谱杀菌剂备受关注,但由于其代谢产物对操作人员存在健康风险,导致其在中国的使用和登记受到限制。将丙硫菌唑制备为具有缓慢释放、可提高农药稳定性等特点的缓释制剂可有效改善其带来的健康风险问题。本研究采用纳米荧光介孔二氧化硅制备了丙硫菌唑缓释纳米颗粒,通过扫描电镜(SEM)、透射电镜(TEM)、傅里叶变换红外光谱(FTIR)、X-射线光电子能谱(XPS)、Brunauer-Emmett-Teller(BET)比表面积分析、热重分析(TGA)、荧光光谱以及高效液相色谱仪等手段对其结构与特性进行表征。采用透析袋法探讨了丙硫菌唑在释放介质中的释放行为;利用菌丝生长速率法测定了该载药颗粒对小麦赤霉病原菌的杀菌活性;通过激光共聚焦研究了荧光二氧化硅在小麦赤霉病菌菌丝体以及小麦中的传输情况。结果表明:所制备的载药纳米颗粒外观形貌较为规整,平均粒径为139 nm,其最大载药量可达27.14%;该纳米载药颗粒具有缓释性能,释放曲线符合Ritger-Peppas方程;载药颗粒与丙硫菌唑原药相比具有相同的毒力效果。此外,激光共聚焦结果表明,该荧光二氧化硅纳米颗粒可以在菌丝体以及小麦植株根部吸收和传导。 | 许春丽 Muhammad BILAL 徐博 冉刚超 赵鹏跃 曹冲 李凤敏 曹立冬 黄啟良 | 2020 | 农药学学报2020,22,2: | 6 |
| 20 | 帕博利珠单抗联合以铂类为基础的化疗治疗非小细胞肺癌合并稳定性脑转移患者的结局:KEYNOTE-021、-189和-407研究的汇总分析显示文摘背景与目的此项探索性分析回顾评价了晚期非小细胞肺癌(non-small cell lung cancer,NSCLC)患者的结局,旨在确定基线时合并脑转移是否会影响一线应用帕博利珠单抗联合化疗(pembrolizumab plus chemotherapy,PC)比对单用化疗的疗效。患者和方法对KEYNOTE-021队列G(非鳞癌)、KEYNOTE-189(非鳞癌)和KEYNOTE-407(鳞癌)三项研究晚期NSCLC患者的数据进行汇总分析。患者接受含铂两药化疗和/或35个周期帕博利珠单抗治疗(每3周200 mg)。所有研究均纳入了经治或初治的稳定性脑转移患者(KEYNOTE-189与KEYNOTE-407研究纳入初治脑转移患者)。经治的脑转移患者已处于病情稳定的状态≥2周(KEYNOTE-021队列G患者≥4周),无新发或脑转移病灶扩大的证据且入组前至少3天以上未使用激素。初治的无症状脑转移患者需定期接受脑部影像学检查。结果共纳入1298例患者,其中171例有基线脑转移,1127例无脑转移。两组的中位随访时间(范围)在数据截止时分别为10.9(0.1-35.1)个月和11.0(0.1-34.9)个月。合并脑转移和无脑转移患者的总生存期[0.48(95%CI:0.32-0.70)和0.63(95%CI:0.53-0.75)]和无进展生存期[0.44(95%CI:0.31-0.62)和0.55(95%CI:0.48-0.63)]的风险比(PC/化疗)相似。合并脑转移的患者中,PC组和单用化疗组患者的中位总生存期分别为18.8个月和7.6个月,中位无进展生存期分别为6.9个月和4.1个月。无论是否合并脑转移,PC组患者的客观缓解率都高于单用化疗组,且缓解持续时间有显著延长。合并脑转移的患者中,PC组与单用化疗组的治疗相关不良事件发生率分别为88.2%和82.8%;而无脑转移的患者中,两组治疗相关不良事件的发生率分别为94.5%和90.6%。结论无论是否合并脑转移,与单用化疗相比,帕博利珠单抗联合以铂类为基础的组织学特异性化疗可改善所有PD-L1亚组的临床结局,其中包括PD-L1肿瘤比例评分<1%的患者,并且在晚期NSCLC患者中安全性良好。该方案是晚期NSCLC初治患者的标准方案,并可用于合并稳定性脑转移的患者。 | 周清(翻译校对) Steven F Powell Delvys Rodríguez-Abreu Corey J Langer Ali Tafreshi Luis Paz-Ares Hans-Georg Kopp Jeronimo Rodríguez-Cid Dariusz M Kowalski Ying Cheng Takayasu Kurata Mark M Awad Jinaxin Lin Bin Zhao M Catherine Pietanza Bilal Piperdi Marina C Garassino | 2022 | 中国肺癌杂志2022,25,1: | 6 |
