| 1 | 中国妇女妊娠前后单纯服用叶酸对神经管畸形的预防效果显示文摘目的 评价妇女在妊娠前后服用单纯 40 0 μg叶酸增补剂对胎 /婴儿神经管畸形 (NTDs)的预防效果。方法 1993~ 1995年在中国北方的NTDs高发地区和南方的NTDs低发地区妇女增补叶酸的推广项目中 ,共募集从孕前或孕后任何时间开始服药的妇女 130 142名 ,未服药的妇女 1176 89名 ;设计的服药方法从婚检时开始到孕满 3个月为止 ,每天服用单纯叶酸片 40 0 μg ;最后对妇女的分娩结局进行监测并进行预防效果的对比评价研究。结果 服药组妇女生育的胎婴儿中共发现 10 2例NTDs ,对照组胎婴儿中共发现 173例NTDs。末次月经前募集的未服药妇女在孕 2 0周以后分娩的胎婴中NTDs发生率 ,北方为 4 8‰ (16 / 3 318) ,南方为 1 0‰ (2 8/ 2 82 6 5 ) ,而妊娠前后服药妇女组则分别为1 0‰ (13/ 13 0 12 )和 0 6‰ (34/ 5 86 38)。与末次月经前募集的未服药妇女组相比 ,北方服药妇女NTDs发生率明显降低 ,其中依从性大于 80 %的服药组妇女预防率达 85 % (95 %CI为 6 2 %~ 94% ) ,南方地区服叶酸的预防率为 41% (95 %CI为 3 %~ 6 4% )。结论 妇女在妊娠前后每天服用单纯叶酸 40 0 | 李竹 RobertJ Berry 李松 J David Erickson 王红 Cynthia A Moore 赵平 Joseph Mulinare 洪世欣 Lee-Yang C Wong 呼和牧人 Jacqueline Gindler 郝玲 Adolfo Correa 朱慧萍 Elaine Gunter | 2000 | 中华医学杂志2000，80，7： | 185 |
| 2 | Pitfalls in histoacryl glue injection therapy for oesophageal, gastric and ectopic varices:A review显示文摘Histoacryl glue is used increasingly for the treatment of gastric and ectopic varices, and there is experience in its use for oesophageal varices. It is an effective treatment, yet numerous reports of complications have accumulated. This review of the literature describes the technique, explores circulatory and vascular consideration unique to portal hypertension and categorises the complications into: 'Embolisation', 'local venous thrombosis', 'fistulisation and extravascular injection', 'ulceration, erosion and extrusion', and 'nidus of infection'. | 3 | Efficacy of thioguanine treatment in inflammatory bowel disease: A systematic review显示文摘AIM To critically assess the available literature regarding the efficacy of thioguanine treatment in inflammatory bowel disease(IBD) patients, irrespective of the(hepato-) toxicity profile.METHODS A systematic literature search of the MEDLINE database using Pub Med was performed using the keywords 'thioguanine', '6-TG', 'thioguanine', 'inflammatory bowel disease', 'IBD', 'Crohn's disease', 'Ulcerative colitis' and 'effectiveness' in order to identify relevant articles published in English starting from 2000. Reference lists of the included articles were crosschecked for missing articles. Reviewed manuscripts concerning the effectiveness of thioguanine treatment in IBD were reviewed by the authors and the data were extracted. Data were subsequently analyzed with descriptive statistics. Due to the lack of standardized outcomes, a formal meta-analysis was not performed.RESULTS A total of 11 applicable studies were found that involved the effectiveness of thioguanine therapy in IBD. Eight studies were conducted in a prospectivemanner, in the remaining three studies, data was collected retrospectively. In total, 353 IBD-patients(225 patients with Crohn's disease, 119 with ulcerative colitis and nine with unclassified IBD) with prior azathioprine/mercaptopurine resistance and/or intolerance(n = 321) or de novo thioguanine administration(n = 32) were included for analysis, of which 228(65%) had clinical improvement on thioguanine therapy, based on standard IBD questionnaires, biochemical parameters or global physician assessments. Short-term results were based on 268 treatment years(median follow-up 9 mo, range 3-22 mo) with a median daily dose of 20 mg(range 10-80 mg). Discontinuation, mostly due to adverse events, was reported in 72 patients(20%). | 5 | Benefits of the use of blood conservation in scoliosis surgery显示文摘AIM To investigate whether autologous blood transfusion(ABT) drains and intra-operative cell salvage reduced donor blood transfusion requirements during scoliosis surgery.METHODS Retrospective data collection on transfusion requirements of patients undergoing scoliosis surgery is between January 2006 and March 2010. There were three distinct phases of transfusion practice over this time: Group A received 'traditional treatment' with allogeneic red cell transfusion(ARCT) in response to an intra- or postoperative anaemia(Hb < 8 g/d L or a symptomatic anaemia); Group B received intra-operative cell salvage in addition to 'traditional treatment'. In group C,ABT wound drains were used together with both intra-operative cell salvage and 'traditional treatment'.RESULTS Data from 97 procedures on 77 patients,there was no difference in mean preoperative haemoglobin levels between the groups(A: 13.1 g/d L; B: 13.49 g/d L; C: 13.66 g/d L). Allogeneic red cell transfusion was required for 22 of the 37 procedures(59%) in group A,17 of 30(57%) in group B and 16 of 30(53%) in group C. There was an overall 6% reduction in the proportion of patients requiring an ARCT between groups A and C but this was not statistically significant(χ2 = 0.398). Patientsin group C received fewer units(mean 2.19) than group B(mean 2.94)(P = 0.984) and significantly fewer than those in group A(mean 3.82)(P = 0.0322).
