| 1 | Metabolic syndrome as a risk factor for gallstone disease显示文摘AIM: To establish an association between the presence of metabolic syndrome and the development of gallstone disease.METHOIDS: We carried out a cross-sectional study in a check-up unit in a university hospital in Mexico City. We enrolled 245 subjects, comprising 65 subjects with gallstones (36 women, 29 men) and 180 controls (79women and 101 men without gallstones). Body mass index, waist circumference, blood pressure, plasma insulin, and serum lipids and lipoproteins levels were measured. Insulin resistance was calculated by homeostasis model assessment. Unconditional logistic regressionanalysis (univariate and multivariate) was used to calculate the risk of gallstone disease associated with the presence of at least three of the criteria (Adult Treatment Panel Ⅲ). Analyses were adjusted for age and sex.RESULTS: Among 245 subjects, metabolic syndrome was present in 40% of gallstone disease subjects, compared with 17.2% of the controls, adjusted by age and gender (odds ratio (OR) = 2.79; 95%CI, 1.46-5.33; P = 0.002),a dose-dependent effect was observed with each component of metabolic syndrome (OR = 2.36, 95%CI, 0.72-7.71;P = 0.16 with one component and OR = 5.54, 95%CI,1.35-22.74; P = 0.02 with four components of metabolic syndrome). Homeostasis model assessment was significantly associated with gallstone disease (adjusted OR = 2.25;95%CI, 1.08-4.69; P = 0.03).CONCLUSION: We conclude that as for cardiovascular disease and diabetes mellitus, gallstone disease appears to be strongly associated with metabolic syndrome. | Nahum Méndez-Sánchez Norberto C. | 2 | Polymorphisms of DNA repair genes XRCC1 and XRCC3, interaction with environmental exposure and risk of chronic gastritis and gastric cancer显示文摘AIM: To evaluate the association between polymorphisms XRCC1 Arg194Trp and Arg399Gln and XRCC3Thr241Met and the risk for chronic gastritis and gastric cancer, in a Southeastern Brazilian population.METHODS: Genotyping by PCR-RFLP was carried out on 202 patients with chronic gastritis (CG) and 160 patients with gastric cancer (GC), matched to 202 (C1) and 150(C2) controls, respectively.RESULTS: No differences were observed among the studied grou ps with regard to the genotype distribution of XRCC1 codons 194 and 399 and of XRCC3 codon 241. However, the combined analyses of the three variant alleles (194Trp, 399Gln and 241Met) showed an increased risk for chronic gastritis when compared to the GC group. Moreover, an interaction between the polymorphic alleles and demographic and environmental factors was observed in the CG and GC groups. XRCC1 194Trp was associated with smoking in the CG group,while the variant alleles XRCC1 399Gln and XRCC3241Met were related with gender, smoking, drinking and H pylori infection in the CG and GC groups.CONCLUSION: Our results showed no evidence of a rela-tionship between the polymorphisms XRCC1Arg194Trp and Arg399Gln and XRCC3 Thr241Met and the risk of chronic gastritis and gastric cancer in the Brazilian population, but the combined effect of these variants may interact to increase the risk for chronic gastritis,considered a premalignant lesion. | 3 | How important is donor age in liver transplantation?显示文摘The age of liver donors has been increasing in the past several years because of a donor shortage. In the United States, 33% of donors are age 50 years or older, as are more than 50% in some European countries. The impact of donor age on liver transplantation(LT) has been analyzed in several studies with contradictory conclusions. Nevertheless, recent analyses of the largest databases demonstrate that having an older donor is a risk factor for graft failure. Donor age is included as a risk factor in the more relevant graft survival scores, such as the Donor Risk Index, donor age and Model for End-stage Liver Disease, Survival Outcomes Following Liver Transplantation, and the Balance of Risk. The use of old donors is related to an increased rate of biliary complications and hepatitis C virus-related graft failure. Although liver function does not seem to be significantly affected by age, the incidence of several liver diseases increases with age, and the capacity of the liver to manage or overcome liver diseases or external injuries decreases. | 4 | Emerging concepts in liver graft preservation显示文摘The urgent need to expand the donor pool in order to attend to the growing demand for liver transplantation has obliged physicians to consider the use of suboptimal liver grafts and also to redefine the preservation strategies. This review examines the different methods of liver graft preservation, focusing on the latest advances in both static cold storage and machine perfusion(MP). The new strategies for static cold storage are mainly designed to increase the fatty liver graft preservation via the supplementation of commercial organ preservation solutions with additives. In this paper we stress the importance of carrying out effective graft washout after static cold preservation, and present a detailed discussion of the future perspectives for dynamic graft preservation using MP at different temperatures(hypothermia at 4 ℃, normothermia a t 3 7 ℃ and subnormothermia at 20 ℃- 2 5 ℃). Finally, we highlight some emerging applications of regenerative medicine in liver graft preservation. | 5 | Melt Infiltration Ability and Microstructural Evolution of Fe40Al/ TiC Composites System显示文摘Pressureless melt infiltration is an economic route f or preparation of high-density ceramic/melt composites. In this study, the Fe40 Al iron aluminide intermetallic, a low cost material of excellent oxidation and corrosion resistance, was used as binder for fabricating Fe40Al/TiC composites b y pressureless melt infiltration. The wetting ability of liquid Fe40Al in porous TiC pre-form was studied by in-situ monitoring the melting and infiltration p rocess. The infiltration ability was investigated by observing the distance of l iquid Fe40Al intrusion in porous TiC pre-forms at different infiltration temper atures and times by using optical microscope. Porous TiC per-forms with density of 60%~88%TD (theoretical density), prepared under pre-defined sintering temp e rature cycles, were used for fabricating Fe40Al/TiC composites in the range of 1 2%~40% metal content by volume. Almost full dense Fe40Al/TiC composites were su c cessfully fabricated by this technique. Liquid Fe40Al exhibited excellent infilt ration ability, the distance of complete intrusion of liquid Fe40Al in the TiC s intered pre-form with density of 88%TD was over 7 mm after 5 min at the inf iltration temperature of 1 450 ℃. Microstructural observation by SEM and TEM also showed that liquid Fe40Al filled the very narrow gaps among TiC particles, the interfaces of TiC particles and F e40Al plastic ligaments being metallurgical bonded. TEM revealed that high densi ty of dislocations formed in Fe40Al ligaments during solidification, which favor the mechanical properties. Ti decomposed from TiC particles and dissolved into Fe40Al during infiltration. According to the compositional analysis of TEM-EDS, the concentration of Ti in Fe40Al ranges at 1at%~4at% depending on composite f a bricating conditions and the distance from the measuring point to the closest Ti C particles. XRD analysis indicated that the composites were composed of two pha ses, the original TiC and Fe 0.4Al 0.6 intermetallic.
