| 1 | Prediabetes diagnosis and treatment: A review显示文摘Prediabetes is an intermediate state of hyperglycemia with glycemic parameters above normal but below the diabetes threshold. While, the diagnostic criteria of prediabetes are not uniform across various international professional organizations, it remains a state of high risk for developing diabetes with yearly conversion rate of 5%-10%. Observational evidence suggests as association between prediabetes and complications of diabetes such early nephropathy, small fiber neuropathy, early retinopathy and risk of macrovascular disease. Several studies have shown efficacy of lifestyle interventions with regards to diabetes prevention with a relative risk reduction of 40%-70% in adults with prediabetes. While there is increasing evidence to prove the efficacy of pharmacotherapy in prevention of diabetes in adults with prediabetes, pharmaceutical treatment options other than metformin are associated with adverse effects that limit their use for prediabetes. There are no reports of systematic evaluation of health outcomes related to prediabetes in children. The effects of pharmacotherapy of prediabetes on growth and pubertal development in children remains unknown. Secondary intervention with pharmacotherapy with metformin is advocated for high-risk individuals but criteria for such consideration benefit of early intervention, long term cost effectiveness of such interventions and the end point of therapy remain unclear. Pharmacotherapy must be used with caution in children with prediabetes. Prediabetes is a condition defined as having blood glucose levels above normal but below the defined threshold of diabetes. It is considered to be an at risk state, with high chances of developing diabetes. While, prediabetes is commonly an asymptomatic condition, there is always presence of prediabetes before the onset of diabetes. The elevation of blood sugar is a continuum and hence prediabetes can not be considered an entirely benign condition. | 2 | Inverted internal limiting membrane flap technique for very large macular hole显示文摘AIM: To assess the anatomical and visual outcome of idiopathic macular holes greater than 1000 μm using the inverted internal limiting membrane flap technique.METHODS: This retrospective case series included 5 eyes of 5 patients with idiopathic macular hole with base diameter greater than 1000 μm who underwent inverted internal limiting membrane flap technique along with standard 23 G pars plans vitrectomy with posterior hyaloid detachment and fluid gas exchange with 12%-14% perfluoropropane(C3F8). Preoperative and postoperative visual acuity and spectral domain optical coherence tomography images were evaluated. The main outcome measures were visual outcome and macular hole closure.RESULTS: Mean age was 63.2±8.4y with all 5 subjects being females. Mean duration of symptoms was 11±14 mo with a mean postoperative follow up of 13.2 ±13 mo. The mean base diameter of the macular holes was 1420±84.8 μm(1280-1480 μm). Type 1 closure was achieved in four out of five patients, while one patient had type 2 closure using the inverted internal limiting membrane(ILM) flap technique. Median baseline BCVA was 0.79 log MAR(Snellen's equivalent 20/120) and median final BCVA0.6 log MAR(Snellen's equivalent 20/80) with mean visual improvement of approximately three lines improvement. | 4 | Risk factors for colonoscopic perforation: A population-based study of 80118 cases显示文摘AIM: To assess the incidence and risk factors associated with colonic perforation due to colonoscopy. METHODS: This was a retrospective cross-sectional study. Patients were retrospectively eligible for inclusion if they were 18 years and older and had an inpatient or outpatient colonoscopy procedure code in any facility within the Geisinger Health System during the period from January 1, 2002 to August 25, 2010. Data are presented as median and inter-quartile range, for continuous variables, and as frequency and percentage for categorical variables. Baseline comparisons across those with and without a perforation were made using the two-sample t -test and Pearson's χ2 test, as appropriate.RESULTS: A total of 50 perforations were diagnosed out of 80118 colonoscopies, which corresponded to an incidence of 0.06% (95%CI: 0.05-0.08) or a rate of 6.2 per 10000 colonoscopies. All possible risk factors associated with colonic perforation with a P -value < 0.1 were checked for inclusion in a multivariable logbinomial regression model predicting 7-d colonic perforation. The final model resulted in the following risk factors which were significantly associated with risk of colonic perforation: age, gender, body mass index, albumin level, intensive care unit (ICU) patients, inpatient setting, and abdominal pain and Crohn's disease as indications for colonoscopy. CONCLUSION: The cumulative 7 d incidence of colonic perforation in this cohort was 0.06%. Advanced age and female gender were significantly more likely to have perforation. Increasing albumin and BMI resulted in decreased risk of colonic perforation. Having a colonoscopy indication of abdominal pain or Crohn's disease resulted in a higher risk of colonic perforation. Colonoscopies performed in inpatients and particularly the ICU setting had substantially greater odds of perforation. | 5 | A comprehensive review on hybrid electric vehicles: architectures and components显示文摘The rapid consumption of fossil fuel and increased environmental damage caused by it have given a strong impetus to the growth and development of fuelefficient vehicles. Hybrid electric vehicles (HEVs) have evolved from their inchoate state and are proving to be a promising solution to the serious existential problem posed to the planet earth. Not only do HEVs provide better fuel economy and lower emissions satisfying environmental legislations, but also they dampen the effect of rising fuel prices on consumers. HEVs combine the drive powers of an internal combustion engine and an electrical machine. The main components of HEVs are energy storage system, motor, bidirectional converter and maximum power point trackers (MPPT, in case of solar-powered HEVs). The performance of HEVs greatly depends on these components and its architecture. This paper presents an extensive review on essential components used in HEVs such as their architectures with advantages and disadvantages, choice of bidirectional converter to obtain high efficiency, combining ultracapacitor with battery to extend the battery life, traction motors’ role and their suitability for a particular application. Inclusion of photovoltaic cell in HEVs is a fairly new concept and has been discussed in detail. | 6 | Correlation of transient elastography with hepatic venous pressure gradient in patients with cirrhotic portal hypertension: A study of 326 patients from India显示文摘AIM To study the diagnostic accuracy of transient elastography(TE) for detecting clinically significant portal hypertension(CSPH) in Indian patients with cirrhotic portal hypertension.METHODS This retrospective study was conducted at the Institute of Liver, Gastroenterology, and Panceatico-Biliary Sciences, Sir Ganga Ram Hospital, New Delhi, on consecutive patients with cirrhosis greater than 15 years of age who underwent hepatic venous pressure gradient(HVPG) and TE from July 2011 to May 2016. Correlation between HVPG and TE was analyzed using the Spearman's correlation test. Receiver operating characteristic ( ROC) curves were prepared for determining the utility of TE in predicting various stages of portal hypertension. The best cut-off value of TE forthe diagnosis of CSPH was obtained using the Youden index.RESULTS The study included 326 patients [median age 52(range 16-90) years; 81% males]. The most common etiology of cirrhosis was cryptogenic(45%) followed by alcohol(34%). The median HVPG was 16.0(range 1.5 to 30.5) mm Hg. Eighty-five percent of patients had CSPH. A significant positive correlation was noted between TE and HVPG(rho 0.361, P < 0.001). The area under ROC curve for TE in predicting CSPH was 0.740(95%CI: 0.662-0.818)(P < 0.01). A cut-off value of TE of 21.6 k Pa best predicted CSPH with a positive predictive value(PPV) of 93%.CONCLUSION TE has a fair positive correlation with HVPG; thus, TE can be used as a non-invasive modality to assess the degree of portal hypertension. | 7 | Longest duration of retention of video capsule: A case report and literature review显示文摘Video capsule endoscopy (VCE) is a safe innovative tool for investigating obscure gastrointestinal bleeding, Crohn's disease and other small bowel pathologies. The capsule is usually excreted with faeces within 24-48 h. Retention of capsule rarely occurs, and it usually depends on the indication of VCE. The longest reported case of capsule retention in the literature is 2.5 years. Surgical approach is considered effective to retrieve the retained capsule. We present a case of asymptomatic retention of capsule for four and half years in a 49-year-old man who underwent VCE to explore the cause of obscure gastrointestinal bleeding. It was successfully retrieved endoscopically. | 10 | Pyogenic liver abscess caused by Fusobacterium in a 21-year-old immunocompetent male显示文摘A 21-year-old male with no significant past medical history, presented with right upper quadrant(RUQ) abdominal pain along with fevers and chills. Lab work revealed leukocytosis, anemia, and slightly elevated alkaline phosphatase. Viral serology for hepatitis B virus, hepatitis C virus, and human immunodeficiency virus were negative and he was immunocompetent.Computed tomography imaging revealed hepatic abscesses, the largest measuring 9.5 cm. Empiric antibiotics were started and percutaneous drains were placed in the abscesses. Anaerobic cultures from the abscesses grew Fusobacterium nucleatum. This is a gram negative anaerobic bacteria; a normal flora of the oral cavity. Fusobacterium is most commonly seen in Lemiere's disease, which is translocation of oral bacteria to the internal jugular vein causing a thrombophlebitis and subsequent spread of abscesses. Our patient did not have Lemiere's, and is the first case described of fusobacterium pyogenic liver abscess in a young immunocompetent male with good oral hygiene. This case was complicated by sepsis, empyema, and subsequent abscesses located outside the liver. These abscesses' have the propensity to flare abruptly and can be fatal. This case not only illustrates fusobacterium as a rare entity for pyogenic liver abscess, but also the need for urgent diagnosis and treatment. It is incumbent on physicians to diagnose and drain any suspicious hepatic lesions. While uncommon, such infections may develop without any overt source and can progress rapidly. | 11 | Role of Joshi's external stabilization system with percutaneous screw fixation in high-energy tibial condylar fractures associated with severe soft tissue injuries显示文摘Purpose: The treatment of high-energy tibial condylar fractures which are associated with severe soft tissue injuries remains contentious and challenging. In this study, we assessed the results of Joshi's external stabilization system (JESS) by using the principle of ligamentotaxis and percutaneous screw fixation for managing high-energy tibial condylar fractures associated with severe soft tissue injuries. Methods: Between June 2008 and June 2010, 25 consecutive patients who were 17-71 years (mean, 39.7), underwent the JESS fixation for high-energy tibial condylar fractures associated with severe soft tissue injuries. Out of 25 patients, 2 were lost during follow-up and in 1 case early removal of frame was done, leaving 22 cases for final follow-up. Among them, 11 had poor skin condition with abrasions and blisters and 2 were open injuries (Gustilo-Anderson grade I & 11). The injury mechanisms were motor vehicle accidents (n - 19), fall from a height (n = 2) and assault (n - 1). The fractures were classified according to Schatzker classification system. Results: There were 7 type-V, 14 type-Vl and 1 type-IV Schatzker's tibial plateau fractures. The average interval between the injury and surgery was 6.8 days (range 2-13). The average hospital stay was 13 days (range, 7-22). The average interval between the surgery and full weight bearing was 13.6 weeks (range 11-20). The average range of knee flexion was 121° (range 105°-135°). The normal extension of the knee was observed in 20 patients, and an extensor lag of 5°-8° was noted in 2 patients. The complications included superficial pin tract infections (n 4) with no knee stiffness. Conclusion: JESS with lag screw fixation combines the benefit of traction, external fixation, and limited internal fixation, at the same time as allowing the ease of access to the soft tissue for wound checks, pin care, dressing changes, measurement of compartment pressure, and the monitoring of the neurovascular status. | 12 | Protective effect of diosmin against diabetic neuropathy in experimental rats显示文摘OBJECTIVE:The present study was undertaken to evaluate the effect of diosmin in diabetic neuropathy in type 2 diabetic rats.METHODS:Type 2 diabetes was induced in male Sprague-Dawley rats by single intraperitoneal injection of streptozotocin(35 mg/kg) and high-fat diet.Four weeks after the confirmation of diabetes,diabetic rats were treated with diosmin(50 and 100 mg/kg,p.o.) for next 4 weeks.Rats were evaluated for biochemical,behavioral and oxidative stress parameters.Eddy's hot plate and tail immersion test were performed on 6th,7th,8th,9th and 10th weeks of experiment to assess thermal hyperalgesia and cold allodynia respectively.Further,the walking function test was performed for assessing the motor responses at the end of the treatment schedule.RESULTS:Rats were fed with high-fat diet throughout the experiment schedule and administration of low-dose streptozotocin induced significant elevation in blood glucose level and insulin resistance which was confirmed by oral glucose tolerance test.Treatment with diosmin at doses of 50 and 100 mg/kg significantly restored the reduced body weight,elevated blood sugar and lipid profiles.Further the dose-dependent improvement was observed in thermal hyperalgesia,cold allodynia and walking function in diabetic rats treated with diosmin.Elevated levels of malondialdehyde,and nitric oxide and decreased glutathione levels and superoxide dismutase activity in diabetic rats were restored significantly after the 4 weeks of diosmin treatment.CONCLUSION:Diosmin has shown beneficial effect in preventing the progression of early diabetic neuropathy in rats. | Dilpesh Jain Manish Kumar Bansal Rahul Dalvi Aman Upganlawar Rahul Somani | 2014 | Journal of Integrative Medicine2014,12,1: | 4 |

| 15 | In Silico Characterization and Homology Modeling of Thylakoidbound Ascorbate Peroxidase from a Drought Tolerant Wheat Cultivar显示文摘Ascorbate peroxidase,a haem protein (EC 1.11.1.11),efficiently scavenges hydrogen peroxide (H2O2) in cytosol and chloroplasts of plants.In this study,a fulllength coding sequence of thylakoid-bound ascorbate peroxidase cDNA (TatAPX) was cloned from a drought tolerant wheat cultivar C306.Homology modeling of the TatAPX protein was performed by using the template crystal structure of chloroplastic ascorbate peroxidase from tobacco plant (PDB: 1IYN).The model structure was further refined by molecular mechanics and dynamic methods using various tools such as PROCHECK,ProSA and Verify3D.The predicted model was then tested for docking with H2O2,the substrate for TatAPX enzyme.The results revealed that Arg233 and Glu255 in the predicted active site of the enzyme are two important amino acid residues responsible for strong hydrogen bonding affinity with H2O2,which might play an important role in scavenging of H2O2 from the plant system. | A. | 16 | A secondary bile acid from microbiota metabolism attenuates ileitis and bile acid reduction in subclinical necrotic enteritis in chickens显示文摘Background:Clostridium perfringens-induced chicken necrotic enteritis(NE)is responsible for substantial economic losses worldwide annually.Recently,as a result of antibiotic growth promoter prohibition,the prevalence of NE in chickens has reemerged.This study was aimed to reduce NE through titrating dietary deoxycholic acid(DCA)as an effective antimicrobial alternative.Materials and methods:Day-old broiler chicks were assigned to six groups and fed diets supplemented with 0(basal diet),0.8,1.0 and 1.5 g/kg(on top of basal diet)DCA.The birds were challenged with Eimeria maxima(20,000 oocysts/bird)at d 18 and C.perfringens(109 CFU/bird per day)at d 23,24,and 25 to induce NE.The birds were sacrificed at d 26 when ileal tissue and digesta were collected for analyzing histopathology,mRNA accumulation and C.perfringens colonization by real-time PCR,targeted metabolomics of bile acids,fluorescence in situ hybridization(FISH),or terminal deoxynucleotidyl transferase dUTP nick end labeling(TUNEL)assay.Results:At the cellular level,birds infected with E.maxima and C.perfringens developed subclinical NE and showed shortening villi,crypt hyperplasia and immune cell infiltration in ileum.Dietary DCA alleviated the NE-induced ileal inflammation in a dose-dependent manner compared to NE control birds.Consistent with the increased histopathological scores,subclinical NE birds suffered body weight gain reduction compared to the uninfected birds,an effect attenuated with increased doses of dietary DCA.At the molecular level,the highest dose of DCA at 1.5 g/kg reduced C.perfringens luminal colonization compared to NE birds using PCR and FISH.Furthermore,the dietary DCA reduced subclinical NE-induced intestinal inflammatory gene expression and cell apoptosis using PCR and TUNEL assays.Upon further examining ileal bile acid pool through targeted metabolomics,subclinical NE reduced the total bile acid level in ileal digesta compared to uninfected birds.Notably,dietary DCA increased total bile acid and DCA levels in a dose-dependent manner compared to NE birds.Conclusion:These results indicate that DCA attenuates NE-induced intestinal inflammation and bile acid reduction and could be an effective antimicrobial alternative against the intestinal disease. | Mohit Bansal Ying Fu Bilal Alrubaye Mussie Abraha Ayidh Almansour Anamika Gupta Rohana Liyanage Hong Wang Billy Hargis Xiaolun Sun | 2020 | Journal of Animal Science and Biotechnology2020,11,3: | 3 |
| 17 | Origin and geochemical characterization of the glauconites in the Upper Cretaceous Lameta Formation, Narmada Basin, central India显示文摘This study presents geochemical characteristics of glauconites in estuarine deposits within the Maastrichtian Lameta Formation in central India. Resting conformably over the Bagh Group, the Lameta Formation consists of ~4-5 m thick arenaceous, argillaceous and calcareous green sandstones underlying the Deccan Traps. The sandstone is friable, medium-to coarse-grained, well-sorted and thoroughly crossstratified, and contains marine fossils. Detailed petrography, spectroscopy and mineral chemistry indicates unique chemical composition of glauconite with high K_2O, MgO, Al_2O_3 and moderate TFe_2O_3. Glauconite is formed by the replacement of K-feldspars, initially as stringers in the cleavages and fractures of feldspars. Incipient glauconite subsequently evolves fully, appearing as pellets. Fully-evolved glauconite pellets often leave tiny relics of K-feldspar. XRD exhibits characteristic peak of 10A from basal(001)reflection of glauconite, indicating the 'evolved' character. The K_2O content of glauconites in the Lameta Formation varies from 5.51% to 8.29%, corroborating the 'evolved' to 'highly-evolved' maturation stage.The TFe_2O_3 content of glauconite varies from 12.56% to 18.90%. The PASS-normalized-REE patterns of glauconite exhibit a 'hat-shape' confirming the authigenic origin of glauconites. The slightly-negative to slightly-positive Ce anomaly value and the moderate TFe_2O_3 content of glauconite agree well with a suboxic,estuarine condition. The replacement of K-feldspar by the glauconite contributes towards the high K_2O content. | 18 | Imaging-based algorithmic approach to gallbladder wall thickening显示文摘Gallbladder(GB) wall thickening is a frequent finding caused by a spectrum of conditions. It is observed in many extracholecystic as well as intrinsic GB conditions. GB wall thickening can either be diffuse or focal. Diffuse wall thickening is a secondary occurrence in both extrinsic and intrinsic pathologies of GB, whereas, focal wall thickening is mostly associated with intrinsic GB pathologies. In the absence of specific clinical features, accurate etiological diagnosis can be challenging. The survival rate in GB carcinoma(GBC) can be improved if it is diagnosed at an early stage, especially when the tumor is confined to the wall. The pattern of wall thickening in GBC is often confused with benign diseases, especially chronic cholecystitis, xanthogranulomatous cholecystitis, and adenomyomatosis. Early recognition and differentiation of these conditions can improve the prognosis. In this minireview, the authors describe the patterns of abnormalities on various imaging modalities(conventional as well as advanced) for the diagnosis of GB wall thickening.
