| 1 | Embolization of splenorenal shunt associated to portal vein thrombosis and hepatic encephalopathy显示文摘Hepatic encephalopathy(HE)is a cognitive disturbance characterized by neuropsychiatric alterations.It occurs in acute and chronic hepatic disease and also in patients with portosystemic shunts.The presence of these portosystemic shunts allows the passage of nitrogenous substances from the intestines through systemic veins without liver depuration.Therefore,the embolization of these shunts has been performed tocontrol HE manifestations,but the presence of portal vein thrombosis is considered a contraindication.In this presentation we show a cirrhotic patient with severe HE and portal vein thrombosis who was submitted to embolization of a large portosystemic shunt.Case report:a 57 years-old cirrhotic patient who had been hospitalized many times for persistent HE and hepatic coma,even without precipitant factors.She had a wide portosystemic shunt and also portal vein thrombosis.The abdominal angiography confirmed the splenorenal shunt and showed other shunts.The larger shunt was embolized through placement of microcoils,and the patient had no recurrence of overt HE.There was a little increase of esophageal and gastric varices,but no endoscopic treatment was needed.Since portosystemic shunts are frequent causes of recurrent HE in cirrhotic patients,portal vein thrombosis should be considered a relative contraindication to perform a shunt embolization.However,in particular cases with many shunts and severe HE,we found that one of these shunts can be safely embolized and this procedure can be sufficient to obtain a good HE recovery.In conclusion,we reported a case of persistent HE due to a wide portosystemic shunt associated with portal vein thrombosis.As the patient had other shunts,she was successfully treated by embolization of the larger shunt. | Letícia de Campos Franzoni Fábio Cardoso de Carvalho Rafael Gomes de Almeida Garzon Fábio da Silva Yamashiro Laís Augusti Lívia Alves Amaral Santos Mariana de Souza Dorna Júlio Pinheiro Baima Talles Bazeia Lima Carlos Antonio Caramori Giovanni Faria Silva Fernando Gomes Romeiro | 2014 | World Journal of Gastroenterology2014,20,42: | 6 |
| 2 | Nutritional assessment in cirrhotic patients with hepatic encephalopathy显示文摘Hepatic encephalopathy(HE) is one of the worst complications of liver disease and can be greatly influenced by nutritional status. Ammonia metabolism, inflammation and muscle wasting are relevant processes in HE pathophysiology. Malnutrition worsens the prognosis in HE, requiring early assessment of nutritional status of these patients. Body composition changes induced by liver disease and limitations superimposed by HE hamper the proper accomplishment of exams in this population, but evidence is growing that assessment of muscle mass and muscle function is mandatory due to the role of skeletal muscles in ammonia metabolism. | 3 | Phenolic composition and inhibitory activity of Mangifera indica and Mucuna urens seeds extracts against key enzymes linked to the pathology and complications of type 2 diabetes显示文摘Objective:To investigate the phenolic compounds composition and the inhibitory activity of Mangifera indica(M.indica) and Mucuna urens(M.urens) seeds extracts against some key enzymes(-amylase,-glucosidasc and aldose reductase) implicated in the pathology and complications of type 2 diabetes in vitro.Methods:Reverse phase chromatographic quantification of the major flavonoids and phenolic acids in the seeds extracts was carried out using high performance liquid chromatography coupled with diode array detection.The inhibitory activities of the seeds extracts against-amylase and-glucosidase were estimated using soluble starch and p-nitrophenylglucopyranosidc as their respective substrates.Inhibition of aldose reductase activity by the extracts was assayed using partially purified lens homogenate of normal male rat as source of enzyme;inhibition of Fe^(2+)-induced lipid peroxidation by extracts was tested in rat pancreas homogenate.Results:The chromatography result revealed that extracts of both seeds had appreciable levels of some major flavonoids and phenolic acids of pharmacological importance,including gallic acid,chlorogenic acid,caffeic acid,ellagic acid,catechin,rutin,quercitrin.quercetin and kaempferol.Extracts of both seeds effectively inhibited-amylase,-glucosidase and aldose reductase activities in a dose-dependent manner,having inhibitory preference for these enzymes in the order of aldose reductase>-glucosidase>-amylase.With lower half-maximal inhibitory concentrations(IC50)against-amylase,-glucosidase,and aldose reductase,M.indica had stronger inhibitory potency against these enzymes than M.urens.Extracts of both seeds also inhibited Fe2+-induced lipid peroxidation in a dose-dependent pattern,with M.indica being more potent than M.urens.Conclusions:The results obtained provide support for a possible use of M.indica and M.urens seeds in managing hyperglycemia and preventing the complications associated with it in type 2diabetes. | Emmanuel Anyachukwu Irondi Ganiyu Oboh Afolabi Akintunde Akindahunsi Aline Augusti Boligon Margareth Linde Athayde | 2014 | Asian Pacific Journal of Tropical Biomedicine2014,4,11: | 4 |
| 4 | Phenolics extract of Tetrapleura tetraptera fruit inhibits xanthine oxidase and Fe^2+-induced lipid peroxidation in the kidney,liver,and lungs tissues of rats in vitro显示文摘The phenolics composition and inhibitory activity of Tetrapleura tetraptera fruit extract on xanthine oxidase(XO)and Fe2+-induced lipid peroxidation in the kidney,liver and lungs tissues of rats were evaluated in vitro.Phenolics(flavonoids and phenolic acids)were quantified using reverse-phase high performance liquid chromatrography coupled with diode array detection(HPLC-DAD).The XO and Fe2+-induced lipid peroxidation inhibitory abilities of the extract were evaluated using spectrophotometric methods.The extract contained some flavonoids and phenolic acids,including catechin,epicatechin,rutin,quercetin,luteolin,apigenin;gallic,chlorogenic,caffeic and ellagic acids that are beneficial to health.The extract inhibited XO in the kidney,liver and lungs tissues in a dose-dependent manner.The half-maximal inhibitory concentrations(IC50)of the extract on XO from the tissues varied significantly(P<0.05),and were in the order of liver>kidney>lungs.The extract also inhibited Fe2+-induced lipid peroxidation in a dose-dependent pattern,having IC50 in the order of liver>lungs>kidney.T.tetraptera fruit extract could be a promising nutraceutical for preventing and managing hyperuricaemia and the associated disease conditions,due to its ability to inhibit XO;this bioactivity is attributable to combined effect of its flavonoids and phenolic acids. | Emmanuel Anyachukwu Irondi Ganiyu Oboh Samson Olalekan Agboola Aline Augusti Boligon Margareth Linde Athayde | 2016 | Food Science and Human Wellness2016,5,1: | 3 |
