1Biliary strictures complicating living donor liver transplantation:Problems,novel insights and solutions显示文摘Biliary stricture complicating living donor liver transplantation(LDLT) is a relatively common complication, occurring in most transplant centres across the world. Cases of biliary strictures are more common in LDLT than in deceased donor liver transplantation. Endoscopic management is the mainstay for biliary strictures complicating LDLT and includes endoscopic retrograde cholangiography, sphincterotomy and stent placement(with or without balloon dilatation). The efficacy and safety profiles as well as outcomes of endoscopic management of biliary strictures complicating LDLT is an area that needs to be viewed in isolation, owing to its unique set of problems and attending complications; as such, it merits a tailored approach, which is yet to be well established. The diagnostic criteria applied to these strictures are not uniform and are over-reliant on imaging studies showing an anastomotic narrowing. It has to be kept in mind that in the setting of LDLT, a subjective anastomotic narrowing is present in most cases due to a mismatch in ductal diameters. However, whether this narrowing results in a functionally significant narrowing is a question that needs further study. In addition, wide variation in the endotherapy protocols practised in most centres makes it difficult to interpret the results and hampers our understanding of this topic. The outcome definition for endotherapy is also heterogenous and needs to be standardised to allow for comparison of data in this regard and establish a clinical practice guideline. There have been multiple studies in this area in the last 2 years, with novel findings that have provided solutions to some of these issues. This review endeavours to incorporate these new findings into the wider understanding of endotherapy for biliary strictures complicating LDLT, with specific emphasis on diagnosis of strictures in the LDLT setting, endotherapy protocols and outcome definitions. 15Optimum Profiles of Endwall Contouring for Enhanced Net Heat Flux Reduction and Aerodynamic Performance显示文摘Successfully utilized non-axisymmetric endwalls to enhance turbine efficiencies(aerodynamic and turbine inlet temperatures)by controlling the characteristics of the secondary flow in a blade passage.This is accomplished by steady-state numerical hydrodynamics and deep knowledge of the field of flow.Because of the interaction between mainstream and purge flow contributing supplementary losses in the stage,non-axisymmetric endwalls are highly susceptible to the inception of purge flow exit compared to the flat and any advantage rapidly vanishes.The conclusions reveal that the supreme endwall pattern could yield a lowering of the gross pressure loss at the design stage and is related to the size of the top-loss location being productively lowered.This has led to diminished global thermal exchange lowered in the passage of the vane alone.The reverse flow adjacent to the suction side corner of the endwall is migrated farther from the vane surface,as the deviated pressure spread on the endwall accelerates the flow and progresses the reverse flow core still downstream.The depleted association between the tornado-like vortex and the corner vortex adjacent to the suction side corner of the endwall is the dominant mechanism of control in the contoured end wall.In this publication,we show that the non-axisymmetric endwall contouring by selective numerical shape change method at most prominent locations is advantageous in lowering the thermal load in turbines to augment the net heat flux reduction as well as the aerodynamic performance using multi-objective optimization.Arjun K S Tide P S Biju N 2024Journal of Harbin Institute of Technology(New Series)2024,31,2:0
16Geospatial approach to elucidate anomalies in the hierarchical organization of drainage network in Kuttiyadi River Basin, Southern India显示文摘An assessment of anomalies in the hierarchical organization of the drainage network in the Kuttiyadi River Basin(KuRB),Kerala,has been performed by considering various morphometric parameters such as bifurcation index(R),hierarchical anomaly index(Δa),hierarchical anomaly density(ga),and stream gradient index(SL)in a geographical information system(GIS)platform.Further,a digital elevation model(DEM)of the area has been generated from Cartosat stereo pair data at 2.5-m resolution.The computed quantitative information about drainage characteristics reveals the highest drainage anomaly is observed in sub-watersheds(SW)III and IV.It is observed that neo-tectonic activity caused the development of younger stage drainage patterns of structural controls in the sub-watersheds of this river basin.The tectonic activityinduced diffusion,high energy fluvial erosion,and anthropogenic interferences altered the hierarchical organization of the drainage network of the sub-watersheds in mature to old stages of geomorphic evolution.The results of finding validated with asymmetry factor and ratio of the hierarchical index(Δa)with hierarchical anomaly number(A),bifurcation index(R),direct bifurcation ratio(Rdb),stream gradient index(SL),and denudation index(logTu).From the denudation index analysis,the sediment yield of the river basin is identified as 0.67 t·km^(−2)·yr^(−1).Moreover,the asymmetric factor(AF)in the KuRB shows the imprints of Paleo−Neo Proterozoic crustal tilting toward a NNW−SSE direction.Thulasi Veedu SWETHA Girish GOPINATH Arun BHADRAN Arjun P 2022Frontiers of Earth Science2022,16,3:0
