| 1 | Management of afferent loop obstruction: Reoperation or endoscopic and percutaneous interventions?显示文摘Afferent loop obstruction is a purely mechanical complication that infrequently occurs following construction of a gastrojejunostomy. The operations most commonly associated with this complication are gastrectomy with Billroth Ⅱ or Roux-en-Y reconstruction, and pancreaticoduodenectomy with conventional loop or Roux-en-Y reconstruction. Etiology of afferent loop obstruction includes:(1) entrapment, compression and kinking by postoperative adhesions;(2) internal herniation, volvulus and intussusception;(3) stenosis due to ulceration at the gastrojejunostomy site and radiation enteritis of the afferent loop;(4) cancer recurrence; and(5) enteroliths, bezoars and foreign bodies. Acute afferent loop obstruction is associated with complete obstruction of the afferent loop and represents a surgical emergency, whereas chronic afferent loop obstruction is associated with partial obstruction. Abdominal multiple detector computed tomography is the diagnostic study of choice. CT appearance of the obstructed afferent loop consists of a C-shaped, fluidfilled tubular mass located in the midline between the abdominal aorta and the superior mesenteric artery with valvulae conniventes projecting into the lumen. The cornerstone of treatment is surgery. Surgery includes:(1) adhesiolysis and reconstruction for benign causes; and(2) by-pass or excision and reconstruction for malignant causes. However, endoscopic enteral stenting, transhepatic percutaneous enteral stenting and direct percutaneous tube enterostomy have the principal role in management of malignant and radiation-induced obstruction. | 3 | Role of diaphragm in pancreaticopleural fistula显示文摘A pancreatic pleural effusion may result from a pancreatopleural fistula. We herein discuss two interesting issues in a similar case report of a pleural effusion caused after splenectomy, which was recently published in the World Journal of Gastroenterology . Pancreatic exudate passes directly through a natural hiatus in the diaphragm or by direct penetration through the dome of the diaphragm from a neighboring subdiaphragmatic collection. The diaphragmatic lymphatic 'stomata' does not contribute to the formation of such a pleural effusion, as it is inaccurately mentioned in that report. A strictly conservative approach is recommended in that article as the management of choice. | 5 | Heat Transfer and Performance Characteristics of Axial Cooling Fans with Downstream Guide Vanes显示文摘This study examines experimentally the effect of stators on the performance and heat transfer characteristics of small axial cooling fans. A single fan impeller, followed by nine stator blades in the case of a complete stage, was used for all the experimental configurations. Performance measurements were carried out in a constant speed stage performance test rig while the transient liquid crystal technique was used for the heat transfer measurements. Full surface heat transfer coefficient distributions were obtained by recording the temperature history of liquid crystals on a target plate. The experimental data indicated that the results are highly affected by the flow conditions at the fan outlet. Stators can be beneficial in terms of pressure drop and efficiency, and thus more economical operation, as well as, in the local heat transfer distribution at the wake of the stator blades if the fan is installed very close to the cooling object.
