| 1 | Inflammatory status in chronic renal failure: The role of homocysteinemia and pro-inflammatory cytokines显示文摘AIM: To evaluate determinants of inflammatory markers in chronic renal failure patients according to the level of glomerular filtration rate. METHODS: One hundred fifty four patients(Age; 44 ± 06 years, male/female; 66/88) with chronic renal failure(CRF) were divided into 6 groups according to the National Kidney Foundation(NKF) classification. They included 28 primary stage renal failure patients(CRF 1), 28 moderate stage renal failure patients(CRF 2),28 severe stage renal failure patients(CRF 3), 18 endstage renal failure patients(CRF 4), 40 hemodialysis(HD) patients, and 12 peritoneal dialysis(PD) patients. Tumor necrosis factor-α(TNF-α), interleukin-1β(IL-1β), interleukin-6(IL-6) and C-reactive protein(CRP) were analyzed by immunosorbent assay kit(ELISA)(Cayman Chemical's ACETM EIA kit). Immunoassay methods were used for total homocysteine(tH cy)(fluorescence polarization immunoanalysis HPLC, Perkin Emer 200 series), transferrin(MININEPHTM human transferin kit: ZK070.R), ferritin(ADVIA Centaur) and fibrinogen analysis(ACL 200). Differences between groups were performed using SPSS 20.0 and data are expressed as the mean ± SD.RESULTS: Results showed that in comparison with CRF 1 group and other groups, TNF-α and IL-6 levels were respectively more elevated in HD(16.38 ± 5.52 pg/mL vs 0.39 ± 0.03 pg/mL, 11.05 ± 3.59 pg/mL vs 8.20 ± 0.22 pg/mL, P < 0.001) and PD(14.04 ± 3.40 pg/m L vs 0.39 ± 0.03 pg/m L, 10.15 ± 1.66 pg/m L vs 8.20 ± 0.22 pg/m L, P < 0.001). IL-1β levels were increased in HD(9.63 ± 3.50 pg/m L vs 3.24 ± 0.10 pg/mL, P < 0.001) and CRF 4(7.76 ± 0.66 pg/mL vs 3.24 ± 0.10 pg/m L, P < 0.001) patients than in CRF 1 and in the other groups. Plasma t Hcy levels were higher in HD(32.27 ± 12.08 μmol/L) and PD(28.37 ± 4.98 μmol/L) patients compared to the other groups of CRF(P < 0.001). The serum CRP level was significantly increased in HD(18.17 ± 6.38 mg/L) and PD(17.97 ± 4.85 mg/L) patients compared to the other groups of CRF patients(P < 0.001). The plasma fibrinogen level was more elevated in HD(6.86 ± 1.06 g/L) and CRF 4(6.05 ± 0.57 g/L) than in the other groups(P < 0.001). Furthermore; the ferritin level was higher in HD(169.90 ± 62.16 ng/m L) and PD(90.08 ± 22.09 ng/m L) patients compared to the other groups of CRF(P < 0.001). The serum transferrin value was significantly decreased especially in PD(1.78 ± 0.21 g/L) compared to the other groups(P < 0.001). We found a negative correlationbetween glomerular filtration rate(GFR), TNF-α levels(r =-0.75, P < 0.001), and t Hcy levels(r =-0.68, P < 0.001). We observed a positive correlation between GFR and transferrin levels(r = 0.60, P < 0.001). CONCLUSION: CRF was associated with elevated inflammatory markers. | 2 | Bartter and Gitelman syndromes:Spectrum of clinical manifestations caused by different mutations显示文摘Bartter and Gitelman syndromes(BS and GS) are inherited disorders resulting in defects in renal tubularhandling of sodium,potassium and chloride.Previously considered as genotypic and phenotypic heterogeneous diseases,recent evidence suggests that they constitute a spectrum of disease caused by different genetic mutations with the molecular defects of chloride reabsorption originating at different sites of the nephron in each condition.Although they share some characteristic metabolic abnormalities such as hypokalemia,metabolic alkalosis,hyperplasia of the juxtaglomerular apparatus with hyperreninemia,hyperaldosteronism,the clinical and laboratory manifestations may not always allow distinction between them.Diuretics tests,measuring the changes in urinary fractional excretion of chloride from baseline after administration of either hydrochlorothiazide or furosemide show very little change(< 2.3%) in the fractional excretion of chloride from baseline in GS when compared with BS,except when BS is associated with KCNJ1 mutations where a good response to both diuretics exists.The diuretic test is not recommended for infants or young children with suspected BS because of a higher risk of volume depletion in such children.Clinical symptoms and biochemical markers of GS and classic form of BS(type III) may overlap and thus genetic analysis may specify the real cause of symptoms.However,although genetic analysis is available,its use remains limited because of limited availability,large gene dimensions,lack of hot-spot mutations,heavy workup time and costs involved.Furthermore,considerable overlap exists between the different genotypes and phenotypes.Although BS and GS usually have distinct presentations and are associated with specific gene mutations,there remains considerable overlap between their phenotypes and genotypes.Thus,they are better described as a spectrum of clinical manifestations caused by different gene mutations. | Amar Al Shibli Hassib Narchi | 2015 | World Journal of Methodology2015,5,2: | 25 |

| 5 | Biliary brush cytology:Factors associated with positive yields on biliary brush cytology显示文摘AIM: To evaluate the yield of brushing biliary strictures and the factors associated with a positive result in biliary strictures. METHODS: Data on all consecutive patients (01/02 -10/05) who were identified to have a biliary stricture and who underwent biliary brush cytology were collected. The yield of positive biliary brush cytology was evaluated and compared to results with the gold standard for diagnosis (defi ned as either defi nitive surgical histology or clinical course). Additionally,associated factors of positive results including stricture location,gender,age,mass size,length of stricture,and dilatation prior to brushing cytology were assessed. RESULTS: From 199 patients who had brushing cytology samples (10 patients were excluded due to lack of gold standard diagnosis),77 patients had positive brushing cytology (yield 41%). Variables associated with positive cytology brushing on initial endoscopic retrograde cholangiography were age 1.02 (1.00-1.05),mass size > 1 cm 2.22 (1.01-4.89) and length of stricture > 1 cm 3.49 (1.18-10.2). The sensitivity of biliary brushing was 61%,its specifi city 98%,the positive predictive value reached 99%,and the negative predictive value was 57%. CONCLUSION: Our results revealed a 41% positive yield from brushing cytology. The sensitivity of biliary brushing cytology in our center was 61% and the specif icity was 98%. | 7 | Imaging and radiological interventions in extra-hepatic portal vein obstruction显示文摘Extrahepatic portal vein obstruction(EHPVO) is a primary vascular condition characterized by chronic long standing blockage and cavernous transformation of portal vein with or without additional involvement of intrahepatic branches, splenic or superior mesenteric vein. Patients generally present in childhood with multiple episodes of variceal bleed and EHPVO is the predominant cause of paediatric portal hypertension(PHT) in developing countries. It is a pre-hepatic type of PHT in which liver functions and morphology are preserved till late. Characteristic imaging findings include multiple parabiliary venous collaterals which form to bypass the obstructed portal vein with resultant changes in biliary tree termed portal biliopathy or portal cavernoma cholangiopathy. Ultrasound with Doppler, computed tomography, magnetic resonance cholangiography and magnetic resonance portovenography are non-invasive techniques which can provide a comprehensive analysis of degree and extent of EHPVO, collaterals and bile duct abnormalities. These can also be used to assess in surgical planning as well screening for shunt patency in post-operative patients. The multitude of changes and complications seen in EHPVO can be addressed by various radiological interventional procedures. | 8 | An international program on Silk Road Disaster Risk Reduction--a Belt and Road initiative (2016-2020)显示文摘Belt and Road Initiative(BRI) is a Chinese national strategy which calls for cooperative economic, political and cultural exchange at the global level along the ancient Silk Road. The overwhelming natural hazards located along the belt and road bring great challenges to the success of BRI. In this framework, a 5-year international program was launched to address issues related to hazards assessment and disaster risk reduction(DRR). The first workshop of this program was held in Beijing with international experts from over 15 countries. Risk conditions on Belt and Road Countries(BRCs) have been shared and science and technology advancements on DRR have been disseminated during the workshop. Under this program, six task forces have been setup to carry out collaborative research works and three prioritized study areas have been established. This workshop announced the launching of this program which involved partners from different countries including Pakistan, Nepal, Russia, Italy, United Kingdom, Sri Lanka and Tajikistan. The program adopted the objectives of Sendai Framework for Disaster Risk Reduction 2015-2030 and United Nation Sustainable Development Goals 2030 and was implemented to assess disaster risk in BRCs and to propose suitable measures for disaster control which can be appropriate both for an individual country and for specific sites. | 9 | Endoscopic ultrasound staging for early esophageal cancer: Are we denying patients neoadjuvant chemo-radiation?显示文摘AIM To evaluate the accuracy of endoscopic ultrasound(EUS) in early esophageal cancer(EC) performed in a highvolume tertiary cancer center. METHODS A retrospective review of patients undergoing esophagectomy was performed and patients with c T1 N0 and c T2 N0 esophageal cancer by EUS were evaluated. Patient demographics, tumor characteristics, and treatment were reviewed. EUS staging was compared to surgical pathology to determine accuracy of EUS. Descriptive statistics was used to describe the cohort. Student's t test and Fisher's exact test or χ~2 test was used to compare variables. Logistic regression analysis was used to determine if clinical variables such as tumor location and tumor histology were associated with EUS accuracy.RESULTS Between 2000 and 2015, 139 patients with clinical stage Ⅰ or Ⅱ?A esophageal cancer undergoing esophagectomy were identified. There were 25(18%) female and 114(82%) male patients. The tumor location included the middle third of the esophagus in 11(8%) and lower third and gastroesophageal junction in 128(92%) patients. Ninety-three percent of patients had adenocarcinoma. Preoperative EUS matched the final surgical pathology in 73/139 patients for a concordance rate of 53%. Twenty-nine patients(21%) were under-staged by EUS; of those, 19(14%) had unrecognized nodal disease. Positron emission tomography(PET) was used in addition to EUS for clinical staging in 62/139 patients. Occult nodal disease was only found in 4 of 62 patients(6%) in whom both EUS and PET were negative for nodal involvement. CONCLUSION EUS is less accurate in early EC and endoscopic mucosal resection might be useful in certain settings. | 10 | Landslide susceptibility assessment of the region affected by the 25 April 2015 Gorkha earthquake of Nepal显示文摘Nepal was hit by a 7.8 magnitude earthquake on 25^(th) April,2015.The main shock and many large aftershocks generated a large number of coseismic landslips in central Nepal.We have developed a landslide susceptibility map of the affected region based on the coseismic landslides collected from remotely sensed data and fieldwork,using bivariate statistical model with different landslide causative factors.From the investigation,it is observed that most of the coseismic landslides are independent of previous landslides.Out of 3,716 mapped landslides,we used 80% of them to develop a susceptibility map and the remaining 20% were taken for validating the model.A total of 11 different landslide-influencing parameters were considered.These include slope gradient,slope aspect,plan curvature,elevation,relative relief,Peak Ground Acceleration(PGA),distance from epicenters of the mainshock and major aftershocks,lithology,distance of the landslide from the fault,fold,and drainage line.The success rate of 87.66% and the prediction rate of86.87% indicate that the model is in good agreement between the developed susceptibility map and theexisting landslides data.PGA,lithology,slope angle and elevation have played a major role in triggering the coseismic mass movements.This susceptibility map can be used for relocating the people in the affected regions as well as for future land development. | Amar Deep Regmi Megh Raj Dhital ZHANG Jian-qiang SU Li-jun CHEN Xiao-qing | 2016 | Journal of Mountain Science2016,13,11: | 7 |
| 11 | Smear positive pulmonary tuberculosis among diabetic patients at the Dessie referral hospital, Northeast Ethiopia显示文摘Background:Tuberculosis(TB)is an infectious disease which is still a major cause of morbidity and mortality throughout the world.People with diabetes mellitus(DM)have a three times higher risk of developing active TB than people without diabetes.However,there is not enough credible information on the burden of pulmonary tuberculosis(PTB)among DM patients in Ethiopia,in general,and in the city of Dessie,in particular.Therefore,this study aims to determine the prevalence and associated risk factors of smear positive PTB among diabetic patients at a referral hospital in Dessie.Methods:A cross-sectional study was conducted from February 2012 to April 2012.Patient demographic characteristics were collected using a pre-tested standard questionnaire format.Spot-morning-spot sputum specimens were collected from the study participants and examined for acid-fast bacilli using direct microscopy by the Ziehl-Neelsen staining technique.Data was entered and analyzed using the SPSS version 16 statistical software and p-value<0.05 was considered as statistically significant.Results:Out of 225 TB suspected diabetic patients,52%were males and 48%were females.Their ages ranged from 12 to 82 years,with a mean age of 47.2 years.Urban residence(AOR:5.5;95%CI:1.07-28.20),history of TB(AOR:13.4;95%CI:2.74-65.73),contact with TB patients in the family(AOR:9.4;95%CI:1.822-48.50),and long duration of DM(AOR:8.89;95%CI:1.88-58.12)were independently associated with the development of active TB in people living with DM.Conclusions:The prevalence of smear positive PTB was 6.2%in TB suspected diabetic patients,which is higher compared with the general population(0.39%).Patients with a previous history of contact with TB patients,as well as those who had prolonged diabetes,were more prone to have PTB.Therefore,screening of diabetic patients for PTB infection during follow-up is necessary. | Hiwot Amare Aschalew Gelaw Belay Anagaw Baye Gelaw | 2013 | Infectious Diseases of Poverty2013,2,1: | 7 |

| 13 | Embolization of splenorenal shunt associated to portal vein thrombosis and hepatic encephalopathy显示文摘Hepatic encephalopathy(HE)is a cognitive disturbance characterized by neuropsychiatric alterations.It occurs in acute and chronic hepatic disease and also in patients with portosystemic shunts.The presence of these portosystemic shunts allows the passage of nitrogenous substances from the intestines through systemic veins without liver depuration.Therefore,the embolization of these shunts has been performed tocontrol HE manifestations,but the presence of portal vein thrombosis is considered a contraindication.In this presentation we show a cirrhotic patient with severe HE and portal vein thrombosis who was submitted to embolization of a large portosystemic shunt.Case report:a 57 years-old cirrhotic patient who had been hospitalized many times for persistent HE and hepatic coma,even without precipitant factors.She had a wide portosystemic shunt and also portal vein thrombosis.The abdominal angiography confirmed the splenorenal shunt and showed other shunts.The larger shunt was embolized through placement of microcoils,and the patient had no recurrence of overt HE.There was a little increase of esophageal and gastric varices,but no endoscopic treatment was needed.Since portosystemic shunts are frequent causes of recurrent HE in cirrhotic patients,portal vein thrombosis should be considered a relative contraindication to perform a shunt embolization.However,in particular cases with many shunts and severe HE,we found that one of these shunts can be safely embolized and this procedure can be sufficient to obtain a good HE recovery.In conclusion,we reported a case of persistent HE due to a wide portosystemic shunt associated with portal vein thrombosis.As the patient had other shunts,she was successfully treated by embolization of the larger shunt. | Letícia de Campos Franzoni Fábio Cardoso de Carvalho Rafael Gomes de Almeida Garzon Fábio da Silva Yamashiro Laís Augusti Lívia Alves Amaral Santos Mariana de Souza Dorna Júlio Pinheiro Baima Talles Bazeia Lima Carlos Antonio Caramori Giovanni Faria Silva Fernando Gomes Romeiro | 2014 | World Journal of Gastroenterology2014,20,42: | 6 |

| 15 | Landslide Susceptibility Mapping along Bhalubang–Shiwapur Area of Mid-Western Nepal Using Frequency Ratio and Conditional Probability Models显示文摘Roads constructed in fragile Siwaliks are prone to large number of instabilities. Bhalubang–Shiwapur section of Mahendra Highway lying in Western Nepal is one of them. To understand the landslide causative factor and to predict future occurrence of the landslides, landslide susceptibility mapping(LSM) of this region was carried out using frequency ratio(FR) and weights-of-evidence(W of E) models. These models are easy to apply and give good results. For this, landslide inventory map of the area was prepared based on the aerial photo interpretation, from previously published/unpublished reposts, and detailed field survey using GPS. About 332 landslides were identified and mapped, among which 226(70%) were randomly selected for model training and the remaining 106(30%) were used for validation purpose. A spatial database was constructed from topographic, geological, and land cover maps. The reclassified maps based on the weight values of frequency ratio and weights-of-evidence were applied to get final susceptibility maps. The resultant landslide susceptibility maps were verified andcompared with the training data, as well as with the validation data. From the analysis, it is seen that both the models were equally capable of predicting landslide susceptibility of the region(W of E model(success rate = 83.39%, prediction rate = 79.59%); FR model(success rate = 83.31%, prediction rate = 78.58%)). In addition, it was observed that the distance from highway and lithology, followed by distance from drainage, slope curvature, and slope gradient played major role in the formation of landsides. | 16 | CT patterns of nodal disease in pediatric chest tuberculosis显示文摘AIM:To highlight various patterns of nodal involvement and post treatment changes in pediatric chest tuberculosis based on contrast enhanced computed tomography(CECT) scans of chest.METHODS:This was a retrospective study consisting of 91 patients aged less than 17 years,who attended Paediatrics OPD of All India Institute of Medical Sciences with clinically diagnosed tuberculosis or with chest radiographs suggestive of chest tuberculosis.These patients had an initial chest radiograph as well as CECT of the chest and follow up imaging after 6 mo,and in some cases 9 mo,of completion of anti-tubercular treatment(ATT).CECT of these patients was reviewed for the location and extent of nodal involvement along with determination of site,size,enhancement pattern and calcification.RESULTS:Enlargement of mediastinal or hilar lymph nodes was found in 88/91 patients(96.7%),with the most common locations being paratracheal(84.1%),and subcarinal(76.1%).The most common pattern of enhancement was found to be inhomogenous.The nodes were conglomerate in 56.8% and discrete in 43.2%.In addition,perinodal fat was obscured in 84.1% of patients.In the post-treatment scan,there was 87.4% reduction in the size of the nodes.All nodes post-treatment were discrete and homogenous with perinodal fat present.Calcification was found both pre-and post-treatment,but there was an increase in incidence after treatment(41.7%).There was hence a reduction in size,change in enhancement pattern,and appearance of perinodal fat with treatment.CONCLUSION:Tubercular nodes have varied appearance and enhancement pattern.Conglomeration and obscuration of perinodal fat suggest activity.In residual nodes decision to continue ATT requires clinical correlation. | Amar Mukund Rashmi Khurana Ashu S Bhalla Arun K Gupta Sushil K Kabra | 2011 | World Journal of Radiology2011,3,1: | 5 |

METHODS: Retrospective analysis of contrast enhanced ultrasound (CEUS) examinations over 24 mo for diagnosis, surveillance, characterization and follow up of 50 patients in the context of HCC was performed. The source and indication of referrals, change in referral rate, accuracy and usefulness of CEUS in a tertiary liver center equipped with a 64 slice dual energy computer tomography (CT) and 3 tesla magnetic resonance imaging (MRI) were studied. Sonovue (BR1, Bracco, Italy, a second generation contrast agent) was used for contrast US studies. Contrast enhanced CT/MRI or both were performed in all patients. The findings were taken as a baseline reference and correlation was done with respect to contrast US. Contrast enhanced MRI was performed using hepatocyte specific gadobenate dimeglumine (Gd-BOPTA). Iomeron (400 mg; w/v) was used for dynamic CT examinations. RESULTS: About 20 (40%) of the examinations were referred from clinicians for characterization of a mass from previous imaging. About 15 (30%) were performed for surveillance in chronic liver disease; 5 (10%) examinations were performed for monitoring lesions after radiofrequency ablation (RFA); 3 (6%) were post trans-arterial chemoembolization (TACE) assessments and 3 (6%) were patients with h/o iodinated contrast allergy. About 2(4%) were performed on hemodynamically unstable patients in the intensive care with raised alpha fetoprotein and 2(4%) patients were claustrophobic. The number of patients referred from clinicians steadily increased from 12 in the first 12 mo of the study to 38 in the last 12 mo. CEUS was able to diagnose 88% of positive cases of HCC as per reference standards. In the surveillance group, specificity was 53.3% vs 100% by CT/MRI. Post RFA and TACE specificity of lesion characterization by CEUS was 100% in single/large mass assessment, similar to CT/MRI. For non HCC lesions such as regenerative and dysplastic nodules, the specificity was 50% vs 90% by CT/MRI. The positive role of CEUS in imaging spectrum of HCC included a provisional urgent diagnosis of an incidentally detected mass. It further led to a decrease in time for further management. A confident diagnosis on CEUS was possible in cases of characterization of an indeterminate mass, in situations where the patient was unfit for CT/MRI, was allergic to iodinated contrast or had claustrophobia, etc.CEUS was also cost effective, radiation free and an easy modality for monitoring post RFA or TACE lesions. | 19 | Bottom hole pressure estimation using hybridization neural networks and grey wolves optimization显示文摘An effective design and optimum production strategies of a well depend on the accurate prediction of its bottom hole pressure(BHP)which may be calculated or determined by several methods.However,it is not practical technically or economically to apply for a well test or to deploy a permanent pressure gauge in the bottom hole to predict the BHP.Consequently,several correlations and mechanistic models based on the knownsurfacemeasurementshave beendeveloped.Unfortunately,all these tools(correlations&mechanistic models)are limited to some conditions and intervals of application.Therefore,establish a global model that ensures a large coverage of conditions with a reduced cost and high accuracy becomes a necessity.In this study,we propose new models for estimating bottom hole pressure of vertical wells with multiphase flow.First,Artificial Neural Network(ANN)based on back propagation training(BP-ANN)with 12 neurons in its hidden layer is established using trial and error.The next methods correspond to optimized or evolved neural networks(optimize the weights and thresholds of the neural networks)with Grey Wolves Optimization(GWO),and then its accuracy to reach the global optima is compared with 2 other naturally inspired algorithms which are the most used in the optimization field:Genetic Algorithm(GA)and Particle Swarms Optimization(PSO).The models were developed and tested using 100 field data collected from Algerian fields and covering a wide range of variables.The obtained results demonstrate the superiority of the hybridization ANN-GWO compared with the 2 other hybridizations or with the BP learning alone.Furthermore,the evolved neural networks with these global optimization algorithms are strongly shown to be highly effective to improve the performance of the neural networks to estimate flowing BHP over existing approaches and correlations. | Menad Nait Amar Nourddine Zeraibi Kheireddine Redouane | 2018 | Petroleum2018,4,4: | 4 |
