1Gastric per-oral endoscopic myotomy: Current status and future directions显示文摘Gastroparesis, or symptomatic delayed gastric emptying in the absence of mechanical obstruction, is a challenging and increasingly identified syndrome. Medical options are limited and the only medication approved by the Food and Drug Administration for treatment of gastroparesis is metoclopramide, although other agents are frequently used off label. With this caveat, first-line treatments for gastroparesis include dietary modifications, antiemetics and promotility agents, although these therapies are limited by suboptimal efficacy and significant medication side effects. Treatment of patients that fail first-line treatments represents a significant therapeutic challenge. Recent advances in endoscopic techniques have led to the development of a promising novel endoscopic therapy for gastroparesis via endoscopic pyloromyotomy, also referred to as gastric per-oral endoscopic myotomy or per-oral endoscopic pyloromyotomy. 2Hepatitis C infection and renal cell carcinoma:A systematic review and meta-analysis显示文摘AIM To investigate the association between hepatitis C virus(HCV) infection and risk of renal cell carcinoma(RCC).METHODS A literature search was performed from inception until February 2016.Studies that reported relative risks,odd ratios,hazard ratios or standardized incidence ratio comparing the risk of RCC among HCV-infected participants vs those without HCV infection were included.Participants without HCV infection were used as comparators.Pooled odds ratios and 95%CI were calculated using a random-effect,generic inverse variance method.RESULTS Seven observational studies were with 196826 patients were included in the analysis to assess the risk of RCC in patients with HCV.A significantly increased risk of RCC among participants with HCV infection was found with a pooled RR of 1.86(95%CI:1.11-3.11).The association between RCC and HCV was marginally insignificant after a sensitivity analysis limited only to studies with adjusted analysis,with a pooled RR of 1.50(95%CI:0.93-2.42).CONCLUSION Our study demonstrated a potential association between HCV infection and RCC.Further studies of RCC surveillance in patients with HCV are required.Karn Wijarnpreecha Pitchaphon Nissaisorakarn Suthanya Sornprom Charat Thongprayoon Natanong Thamcharoen Kunlatida Maneenil Alexander J Podboy Wisit Cheungpasitporn 2016World Journal of Gastrointestinal Pathophysiology2016,7,4:0
3Machine learning better predicts colonoscopy duration显示文摘BACKGROUND The use of machine learning(ML)to predict colonoscopy procedure duration has not been examined.AIM To assess if ML and data available at the time a colonoscopy procedure is scheduled could be used to estimate procedure duration more accurately than the current practice.METHODS Total 40168 colonoscopies from the Clinical Outcomes Research Initiative database were collected.ML models predicting procedure duration were developed using data available at time of scheduling.The top performing model was compared against historical practice.Models were evaluated based on accuracy(prediction–actual time)±5,10,and 15 min.RESULTS ML outperformed historical practice with 77.1%to 68.9%,87.3%to 79.6%,and 92.1%to 86.8%accuracy at 5,10 and 15 min thresholds.CONCLUSION The use of ML to estimate colonoscopy procedure duration may lead to more accurate scheduling.Alexander Joseph Podboy David Scheinker 2020Artificial Intelligence in Gastroenterology2020,1,1:0
