| 1 | Comparative outcomes of Inferior Vena Cava filters placed at bedside using digital radiography versus conventional fluoroscopy显示文摘Purpose:To retrospectively assess the outcomes of Inferior Vena Cava(IVC)filters placed in critically ill patients in the ICU at bedside using digital radiograph(DR)guidance with previous cross-sectional imaging for planning,compared to IVC filters placed by conventional fluoroscopy(CF).Method and materials:The cohort consisted of 129 IVC filter placements;48 placed at bedside and 81 placed conventionally from July 2015 to September 2016.Patient demographics,indication,radiation exposures,access site,procedural duration,dwell time,and complications were identified by the EMR.IVC Filter positioning with measurements of tip to renal vein distance and lateral filter tilt were performed when cavograms or post placement CTs were available for review.Statistical analysis was performed using Stata IC 11.2.Results:Technical success of the procedure was 100% in both groups.Procedural duration was longer at the bedside lasting 14.5+/-10.2 versus 6.7+/-6.0 min(p<0.0001).The bedside DR group had a median radiation exposure of 25 mGy(15-35)and the CF group had mean radiation exposure of 256.94 mGy+/-158.6.There was no significant difference in distance of IVC tip to renal vein(p=0.31),mispositioning(p=0.59),degree of filter tilt(p=0.33),or rate of complications(p=0.65)between the two groups.Conclusion:IVCF placement at the bedside using DR is comparable to CF with no statistical difference in outcomes based on IVCF positioning,degree of lateral tilt or removal issues.It decreased radiation dose,but with overall increased procedural time. | John A.Walker Matthew Milam Jorge E.Lopera | 2021 | Journal of Interventional Medicine2021,4,3: | 3 |
| 2 | Emergency department rectal temperatures in over 10 years:A retrospective observational study显示文摘BACKGROUND:Fever in patients can provide an important clue to the etiology of a patient's symptoms.Non-invasive temperature sites(oral,axillary,temporal) may be insensitive due to a variety of factors.This has not been well studied in adult emergency department patients.To determine whether emergency department triage temperatures detected fever adequately when compared to a rectal temperature.METHODS:A retrospective chart review was made of 27 130 adult patients in a high volume,urban emergency department over an eight-year period who received first a non-rectal triage temperature and then a subsequent rectal temperature.RESULTS:The mean difference in temperatures between the initial temperature and the rectal temperature was 1.3 °F(P<0.001),with 25.9%of the patients having higher rectal temperatures >2°F,and 5.0%having higher rectal temperatures >4 °F.The mean difference among the patients who received oral,axillary,and temporal temperatures was 1.2 °F(PO.001),1.8 °F(PO.001),and 1.2 °F(P<0.001) respectively.About 18.1%of the patients were initially afebrile and found to be febrile by rectal temperature,with an average difference of 2.5 °F(P<0.001).These patients had a higher rate of admission(61.4%,P<0.005),and were more likely to be admitted to the hospital for a higher level of care,such as an intensive care unit,when compared with the full cohort(12.5%vs.5.8%,P<0.005).CONCLUSIONS:There are significant differences between rectal temperatures and noninvasive triage temperatures in this emergency department cohort.In almost one in five patients,fever was missed by triage temperature. | Graham A.Walker Daniel Runde Daniel M.Rolston Dan Wiener Jarone Lee | 2013 | World Journal of Emergency Medicine2013,4,2: | 2 |

| 5 | The diagnostic and prognostic utility of the dual-task tandem gait test for pediatric concussion显示文摘Background:The tandem gait test has gained interest recently for assessment of concussion recovery.The purpose of our study was to determine the prognostic and diagnostic use of the single-and dual-task tandem gait test,alongside other clinical measures,within 10 days of pediatric concussion.Methods:We assessed 126 patients post-concussion(6.3§2.3 days post-injury,mean§SD)at a pediatric sports medicine clinic and compared them to 58 healthy controls(age:15.6§1.3 years;43%female).We also compared the 31 patients with concussion who developed persistent post-concussion symptoms(PPCS)(age=14.9§2.0 years;48%female)to the 81 patients with concussion who did not develop PPCS following the initial assessment(age:14.1§3.0 years;41%female).All subjects completed a test battery,and concussion patients were monitored until they experienced concussion-symptom resolution.The test battery included tandem gait(single-task,dual-task(performing tandem gait while concurrently completing a cognitive test)conditions),modified Balance Error Scoring System(mBESS),and concussion symptom assessment(Health and Behavior Inventory).We defined PPCS as symptom resolution time>28 days post-concussion for the concussion group.Measurement outcomes included tandem gait time(single-and dual-task),dual-task cognitive accuracy,mBESS errors(single/double/tandem stances),and symptom severity.Results:The concussion group completed the single-task(mean difference=9.1 s,95%confidential interval(95%CI):6.1-12.1)and dual-task(mean difference=12.7 s,95%CI:8.716.8)tandem gait test more slowly than the control group.Compared to those who recovered within 28 days of concussion,the PPCS group had slower dual-task tandem gait test times(mean difference=7.9 s,95%CI:2.0-13.9),made more tandem-stance mBESS errors(mean difference=1.3 errors,95%CI:0.2-2.3),and reported more severe symptoms(mean difference=26.6 Health and Behavior Inventory rating,95%CI:21.1-32.6).Conclusion:Worse dual-task tandem gait test time and mBESS tandem stance performance predicted PPCS in pediatric patients evaluated within 10 days of concussion.Tandem gait assessments may provide valuable information augmenting common clinical practices for concussion management. | Katie A.Van Deventer Corrine N.Seehusen Gregory A.Walker Julie C.Wilson David R.Howell | 2021 | Journal of Sport and Health Science2021,10,2: | 0 |
| 6 | Immunosuppressive,antimicrobial and insecticidal activities of inhibitor cystine knot peptides produced by teratocytes of the endoparasitoid wasp Cotesia flavipes(Hymenoptera:Braconidae)显示文摘Teratocytes are specialized cells released by parasitoid wasps into their hosts.They are known for producing regulatory molecules that aid the development of imma-ture parasitoids.We have recently reported the primary structures of cystine-rich peptides,including some containing inhibitor cystine knot(ICK)motifs,produced by teratocytes of the parasitoid Cotesia flavipes(Hymenoptera:Braconidae).ICKs are known for their stability and diverse biological functions.In this study,we produced four putative ICK peptides from the teratocytes of C.Jlavipes using solid-phase peptide synthesis or recom-binant expression in E.coli,and investigated their functions on host immune modulation as well their potential to impair the development of two lepidopterans after ingestion of the peptides.In addition,the peptides were assayed against pathogens and human cells.The peptides did not influence total hemocyte count but suppressed cellular immunity,detectable as a reduction of hemocyte encapsulation(CtICK-I,CftICK-II,CtICK-I)and spread indexes(CftICK-IV)in the host.None of the peptides influenced the activi-ties of prophenoloxidase and phenoloxidase in the hemolymph of larval Diatraea sacchar-alis(Lepidoptera:Crambidae).CftICK-I and CftICK-II with previously unknown function showed antifungal activity against Candida albicans but were non-toxic to human cells.CtICK-I,CftICK-II,and CftICK-II increased larval mortality and reduced leaf consump-tion of D.saccharalis,a permissive host for C.flavipes.The CftICK-III also increased larval mortality and reduced leaf consumption of Spodoptera frugiperda(Lepidoptera:Noctuidae),a non-permissive host for C.flavipes.This study highlights biological func-tions and biotechnological potential of ICK peptides from the teratocytes of C.flavipes. | Ciro P.G.Pinto Andrew A.Walker Glenn F.King Guilherme D.Rossi | 2023 | Insect Science2023,30,4: | 0 |
