1膈肌超声:一种预测机械通气患者撤机结局的新方法显示文摘目的:评价超声检查膈肌增厚和活动度对机械通气患者撤机结局的预测价值。方法:纳入54例成功通过自主呼吸试验(SBT)的患者进行研究。SBT过程中,采用超声检查评估患者的膈肌活动度、吸气末和呼气末的膈肌厚度(Tdi)以及膈肌增厚分数(DTF%);同时记录常规的撤机参数。撤机后,追踪观察患者48小时。结果:54例患者中,14例(25.9%)患者撤机失败。撤机成功患者的膈肌活动度、吸气末和呼气末Tdi以及DTF%均高于撤机失败的患者,差异有统计学意义(P<0.05)。与撤机成功相关的膈肌指标的临界值为:膈肌活动度≥10.5mm;吸气末Tdi≥21mm;呼气末Tdi≥10.5mm;DTF%≥34.2%。各指标预测撤机成功的敏感性分别为87.5%、77.5%、80.0%和90.0%;特异性分别为71.5%、86.6%、50.0%和64.3%。将膈肌活动度≥10.5mm和吸气末Tdi≥21mm两者结合,预测撤机成功的敏感性降低至64.9%,但特异性升高至100%;浅快呼吸指数(RSBI)<105用于预测撤机成功的敏感性达90.0%,但特异性仅有18.7%。结论:对通过SBT的患者,膈肌活动度和吸气末膈肌厚度的超声评估,是优质的机械通气患者撤机结局的预测指标。因此,推荐考虑使用上述指标和RSBI,以改善撤机结局。Shereen Farghaly Ali A.Hasan 2017中国护理管理2017,17,1:61

3Effect of the Arrangement of a New-Type of Turbulator Inserts on Heat Pipe Exchanger Performances显示文摘This research tests the effect of introducing turbulators of a new type into a circular tube heat exchanger under a constant and uniform longitudinal heatflux condition.A 45 mm diameter copper tube with a length of 1,350 mm is utilized with a solid disk being inserted inside the tube,which consists of three sections,each one containing two slots.The slot is cut at a 45 degree angle toward the inner tube surface,which results in diverging theflow toward the inner hot tube surface in order to enhance the heat transfer process.Air is considered as the workingfluid with Prandtl number 0.71.The Reynolds number spans the interval from 6,000–13,500,which indicates that the consideredflow is turbulent.The heat exchanger performance is studied and analyzed in terms of average Nusselt number.The experimental results show that the Nusselt number value is directly proportional to the increase of the Reynolds number,and the number of turbulators inserts.With the use of three novel turbulators,the heat transfer was about 3.15 times higher than that in the smooth tube and the friction factor was about 1.11.Ibtisam A.Hasan Wafa Maki 2023Fluid Dynamics & Materials Processing2023,19,10:0
4△-Resonances in Ground State Properties of _(20)^(40)Ca Spherical Cold Finite Nucleus at Equilibrium and under Compression显示文摘The ground state properties of the spherical nucleus ^(40)Ca have been investigated by using constrainedspherical Hartree-Fock (CSHF) approximation at equilibrium and under high radial compression in a six major shells.The effective baryon-baryon interaction that includes the Δ(1236) resonance freedom degrees to calculate nuclear propertiesis used.The nucleon-nucleon (N-N) interaction is based on Reid soft core (RSC) potential.The results of calculationsshow that much of increase in the nuclear energy generated under compression is used to create the massive A particles.The number of Δ's can be increased to about 2.1% of constituents of nucleus when nuclear density reaches about 1.34times of normal density.The single particle energy levels are calculated and their behavior under compression is alsoexamined.A good agreement has been found between current calculations and phenomenological shell model for lowlying single-particle spectra.The gap between shells is very clear and L-S coupling become stronger as increasing thestatic load on the nucleus.The results show a considerable reduction in compressibility when freedom degrees of Δ's aretaken into account.It has been found that the total nuclear radial density becomes denser in the interior and less densein the exterior region of nucleus.The surface of nucleus becomes more and more responsive to compression than outerregion.Mohammed H.E.Abu-Sei’leek Mahmoud A.Hasan 2010Communications in Theoretical Physics2010,,8:0
