1Novel DPY19L2 variants in globozoospermic patients and the overcoming this male infertility显示文摘Globozoospermia has been reported to be a rare but severe causation of male infertility,which results from the failure of acrosome biogenesis and sperm head shaping.Variants of dpy-19-like 2(DPY19L2)are highly related to globozoospermia,but related investigations have been mainly performed in patients from Western countries.Here,we performed a screening of DPY19L2 variants in a cohort of Chinese globozoospermic patients and found that five of nine patients carried DPY19L2 deletions and the other four patients contained novel DPY19L2 point mutations,as revealed by whole-exome sequencing.Patient 3(P3)contained a heterozygous variant(c.2126+5G>A),P6 contained a homozygous nonsense mutation(c.1720C>T,p.Arg574*),P8 contained compound heterozygous variants(c.H82-1184delATCf p.Leu394_Ser395deIinsPhe;c.368A>T,p.Hisl23Arg),and P9 contained a heterozygous variant(c.H82-1184delATCTTI frameshift).We also reported intracytoplasmic sperm injection(ICSI)outcomes in the related patients,finding that ICSI followed by assisted oocyte activation(AOA)with calcium ionophore achieved high rates of live births.In summary,the infertility of these patients results from DPY19L2 dysfunction and can be treated by ICSI together with AOA.Yong-Liang Shang Fu-Xi Zhu Jie Yan Liang Chen Wen-Hao Tang Sai Xiao Wei-Ke Mo Zhi-Guo Zhang Xiao-Jin He Jie Qiao Yun-Xia Cao Wei Li 2019Asian Journal of Andrology2019,21,2:9

3Imaging in male-factor obstructive infertility显示文摘The main purpose of imaging evaluation in male infertility is to identify and treat correctable causes of infertility, such as obstruction of the seminal tract. Various imaging modalities are available to evaluate men with obstructive infertility including scrotal ultrasonography, transrectal ultrasound (TRUS), vasography, magnetic resonance imaging, seminal vesicle aspiration, seminal tract washout, and seminal vesiculography. To date the most reliable and accurate diagnostic technique for obstructive infertility is unclear. In this review article, we report the role of these modalities in diagnosis of obstructive infertility. Scrotal sonography is the initial modality, and if patient results indicate non obstructive azoospermia as varicocele or testicular pathology they will be treated according to standard protocols for management of these pathologies. If the patient findings indicate proximal obstructive azoospermia, they can be managed by vasoepididymostomy. If the scrotal ultrasound is normal, TRUS is the second imaging modality. Accordingly, they are classified into patients with criteria of obstructive infertility without urogenital cysts where TRUS-guided aspiration and seminal vesiculography can be performed and transurethral resection of the ejaculatory ducts (TURED) will be the management of choice. In patients with urogenital cyst, TRUS-guided cyst aspiration and opacification are performed. Ifthe cyst is communicating with the seminal tract, management will be transurethral incision of the cyst. If the cyst is not in communication, the obstruction may be relieved after cyst aspiration. If the obstruction is not relieved, TURED will be the management of choice. Sperm harvested during aspiration may be stored and used in assisted reproduction techniques. If the results of TRUS are inconclusive or doubtful, endorectal magnetic resonance imaging should be performed to serve as a 'detailed map' for guiding corrective operative interventions.Ragab H Donkol 2010World Journal of Radiology2010,2,5:5

5Altered microRNA profiles of testicular biopsies from patients with nonobstructive azoospermia显示文摘Many studies have shown that microRNAs(miRNAs)play vital roles during the spermatogenesis.However,little is known about the altered miRNA profiles of testicular tissues in nonobstructive azoospermia(NOA).Using microarray technology,the miRNA expression profiles of testicular biopsies from patients with NOA and of normal testicular tissues were determined.Bioinformatics analyses were conducted to predict the enriched biological processes and functions of identified miRNAs.The microarray data were validated by quantitative reverse transcriptase polymerase chain reaction(qRT-PCR),the results of which were then validated with a larger sample size.Correlations between the miRNA expression levels and clinical characteristics were analyzed.Receiver operating characteristic(ROC)curve analysis was used to evaluate the diagnostic ability of miRNAs for azoospermia.Hierarchical clustering showed that 129 miRNAs were significantly differentially expressed between the NOA and control groups.Bioinformatics analysis indicated that the differentially expressed miRNAs were involved in spermatogenesis,cell cycle,and mitotic prometaphase.In the subsequent qRT-PCR assays,the selected miRNA expression levels were consistent with the microarray results,and similar validated results were obtained with a larger sample size.Some clinical characteristics were significantly associated with the expression of certain miRNAs.In particular,we identified a combination of two miRNAs(miR-10b-3p and miR-34b-5p)that could serve as a predictive biomarker of azoospermia.This study provides altered miRNA profiles of testicular biopsies from NOA patients and examines the roles of miRNAs in spermatogenesis.These profiles may be useful for predicting and diagnosing the presence of testicular sperm in individuals with azoospermia.Hai-Tao Zhang Zhe Zhang Kai Hong Wen-Hao Tang De-Feng Liu Jia-Ming Mao Yu-Zhuo Yang Hao-Cheng Lin Hui Jiang 2020Asian Journal of Andrology2020,22,1:4
6Effect of ketotifen fumarate on experimental autoimmune orchitis and torsion of the spermatic cord显示文摘The aim of this work was to study effects of ketotifen fumarate(KF)on prevention of tissue damage in testes of rats with experimental autoimmune orchitis(EAO)and on the contralateral testis in a model of prolonged testicular cord torsion(TCT).Rats with EAO or TCT were injected intraperitoneally once daily with KF or saline solution(vehicle group).Incidence and severity of testicular damage were evaluated by histopathology using an EAO score or a Johnsen score.Mast cells(MC)were identified by histochemistry and quantified.In EAO model,KF significantly reduced severity of histopathological testicular damage compared to rats in the vehicle group.KF also reduced the number of testicular MC compared to vehicle group.Similarly,in TCT model,multifocal damage of the contralateral testis was observed 30 days after testicular torsion characterized by sloughing of the germinal epithelium,seminiferous tubule atrophy,and interstitial edema.Focal signs of inflammation and fibrosis of seminiferous tubular walls were also observed.In contrast,sections of contralateral testis of rats injected with KF and killed 30 days after surgery showed normal histological features.A significant decrease in the number of MC was observed in rats treated with KF compared to untreated animals.In conclusion,we demonstrated that treatment with KF reduced testicular inflammatory process and MC infiltrates in both EAO and TCT models.The results suggest a promising treatment for infertile male patients with testicular pathologies associated with inflammation and germ cell loss.Diego Moreno Cristian M Sobarzo Livia Lustig Marcelo G Rodriguez Pena Vanesa Anabella Guazzone 2020Asian Journal of Andrology2020,22,1:3

8Leptin and its actions on reproduction in males显示文摘Leptin, an adipocyte-derived hormone, serves numerous physiological functions in the body, particularly during puberty and reproduction. The exact mechanism by which leptin activates the gonadotropin-releasing hormone (GnRH) neurons to trigger puberty and reproduction remains unclear. Given the widespread distribution of leptin receptors in the body, both central and peripheral mechanisms involving the hypothalamic-pituitary-gonadal axis have been hypothesized. Leptin is necessary for normal reproductive function, but when present in excess, it can have detrimental effects on the male reproductive system. Human and animal studies point to leptin as a link between infertility and obesity, a suggestion that is corroborated by findings of low sperm count, increased sperm abnormalities, oxidative stress, and increased leptin levels in obese men. In addition, daily leptin administration to normal-weight rats has been shown to result in similar abnormalities in sperm parameters. The major pathways causing these abnormalities remain unidentified;however, these adverse effects have been attributed to leptin-induced increased oxidative stress because they are prevented by concurrently administering melatonin. 9A comparison of the relative efficiency of ICSI and extended culture with epididymal sperm versus testicular sperm in patients with obstructive azoospermia显示文摘This is a retrospective cohort study comparing blastocyst transfer outcomes following intracytoplasmic sperm injection utilizing epididymal versus testicular sperm for men with obstructive azoospermia.All cases at a single center between 2012 and 2016 were included.Operative approach was selected at the surgeon’s discretion and included microepididymal sperm aspiration or testicular sperm extraction.Blastocyst culture was exclusively utilized prior to transfer.The primary outcome was live birth rate.Secondary outcomes included fertilization rate,blastulation rate,euploidy rate,and implantation rate.A mixed effects model was performed.Seventy-six microepididymal sperm aspiration cases and 93 testicular sperm extraction cases were analyzed.The live birth rate was equivalent(48.6%vs 50.5%,P=0.77).However,on mixed effects model,epididymal sperm resulted in a greater likelihood of fertilization(adjusted OR:1.37,95%CI:1.05–1.81,P=0.02)and produced a higher blastulation rate(adjusted OR:1.41,95%CI:1.1–1.85,P=0.01).As a result,the epididymal sperm group had more supernumerary blastocysts available(4.3 vs 3,P<0.05).The euploidy rate was no different.Pregnancy rates were no different through the first transfer cycle.However,intracytoplasmic sperm injection following microepididymal sperm aspiration resulted in a greater number of usable blastocysts per patient.Thus,the true benefit of epididymal sperm may only be demonstrated via a comparison of cumulative pregnancy rates after multiple transfers from one cohort.Scott J Morin Brent M Hanson Caroline R Juneau Shelby A Neal Jessica N Landis Richard T Scott Jr James M Hotaling 2020Asian Journal of Andrology2020,22,2:2
10Multi-center evaluation of oxidation-reduction potential by the MiOXSYS in males with abnormal semen显示文摘According to the World Health Organization(WHO),oxidative stress(OS)is a significant contributor to male infertility.SeminalOS can be measured by a number of assays,all of which are either costly or time sensitive and/or require large semen volume andcomplex instrumentation.One less expensive alternative is to quantify the oxidation-reduction potential(ORP)with the MiOXSYS.In this international multi-center study,we assessed whether ORP levels measured by the MiOXSYS could distinguish semensamples that fall within the 2010 WHO normal reference values from those that do not.Semen samples were collected from 2092patients in 9 countries;ORP was normalized to sperm concentration(mV/10^6 sperm/ml).Only those samples with a concentration>1×10^6 sperm ml1 were in eluded.The results showed that 199 samples fell within the WHO no rmal refere nee range while theremaining 1893 samples did not meet one or more of the criteria.ORP was negatively correlated with all semen parameters(P <0.01)except volume.The area under the curve for ORP was 0.765.The ORP cut-off value(1.34 mV/10^6 sperm/ml)was able todifferentiate specimens with abnormal semen parameters with 98.1%sensitivity,40.6%specificity,94.7%positive predictivevalue(PPV)and 66.6%negative predictive value(NPV).When used as an adjunct to traditional semen analysis,ORP levels mayhelp identify altered functional status of spermatozoa caused by OS in cases of idiopathic male infertility and in male partners ofcouples sufferi ng recurre nt pregna ncy loss,and thereby directi ng these men to relevant medical therapies and lifestyle modificati ons.Ashok Agarwal Manesh Kumar Panner Selvam Mohamed Arafa Hiroshi Okada Sheryl Homa Aideen Killeen Basak Balaban Ramadan Saleh Abdullah Armagan Shubhadeep Roychoudhury Suresh Sikka 2019Asian Journal of Andrology2019,21,6:2
11EMC10 governs male fertility via maintaining sperm ion balance显示文摘Infertility is a severe public health problem worldwide that prevails up to 15% in reproductive-age couples,and male infertility accounts for half of total infertility.Studies on genetically modified animal models have identifed lots of genes involved in the pathogenesis of male infertility.The underlying causes,however,remain largely unclear.In this study,we provide evidence that EMCIO,one subunit of endoplasmic reticulum (ER)membrane protein complex (EMC),is required for male fertility.EMC10 is significantly decreased in spermatozoa from patients with asthenozoospermia and positively associated with human sperm motility. Male mice lacking Emc10 gene are completely sterile.Emc10-null spermatozoa exhibit multiple defects including abnormal morphology,decreased motility,impaired capacitation,and impotency of acrosome reaction,thereby which are incapable of fertilizing intact or ZP-free oocytes.However,intracytoplasmic sperm injection could rescue this defect caused by EMC10 deletion. 13Evaluation of gonadotropireplacement therapy in male patients with hypogonadotropic hypogonadism显示文摘Hypogonadotropic hypogonadism(HH)is a rare disease in which medical treatment has a high success rate to achieve fertility.This study aimed to an a lyze the efficacy of horm one replacement therapy and determi ne predictive factors for successful spermatogenesis and spontaneous pregnancy in patients with idiopathic HH.A total of 112 patients with low testosterone(T),luteinizing hormone(LH)and follicle-stimulating hormone(FSH),and normal prolactin levels were diagnosed with HH and administered LH and FSH analogs as hormone replacement therapy.During treatment,96(85.7%)patients had sperm present in ejaculate samples.Among these patients,72 were married and wanted a child.Of these 72 patients,48(66.7%)of couples had pregnancies from natural conception.After initiation of treatment,the mean time for the appearance of sperm in semen was 9.48 months.There were no significant differences between baseline FSH,T,and LH levels;however,older age,larger testicular size,and low rate of undescended testes were favorable factors for successful spermatogenesis.Larger testicular size and older age were also the main predictive factors for natural conception.We found that patients with undescended testes had a younger age,smaller testes,and lower T levels compared with patients exhibit!ng descended testes.The rate of sperm found in the ejaculate was not significantly decreased in patients with undescended compared with descended testis(73.7%vs 87.6%,P=0.261).The medical approach for males with HH and azoospermia provides a successful treatment modality in regard to successful spermatogenesis and achievement of pregnancy.Mazhar Ortac Muhammed Hidir Emre Salabas Abubekir Boyuk Caner Bese Yasar Pazir Ates Kadioglu 2019Asian Journal of Andrology2019,21,6:1
14Abnormal expression of cen-trosome protein (centrin) in spermatozoa of male human infertility显示文摘To study the relations between male infertility and centrosome protein (centrin) and the functions of centrin in spermatogenesis, the matured spermatozoa of 10 normal male people and 18 male infertility patients were stained by immunofluorescence labeling antibody against centrin. The results showed that two fluorescence signal dots appeared in the normal male spermatozoa and were located at the base of flagellum. They are proximal centriole and distal centriole. However, in some spermatozoa of the male infertility, centrin protein was located abnormally at the base of flagellum and its staining signals were spread, the normal proximal and distal centrioles were confused and could not be recognized separately. These results suggest that abnormality of centrosome protein may be related to male infertility. 15Infertility in men with inflammatory bowel disease显示文摘Inflammatory bowel disease(IBD) predominantly affects young adults. Fertility-related issues are therefore im-portant in the m-anagem-ent of patients with IBD. However, relatively m-odest attention has been paid to reproductive issues faced by m-en with IBD. To investigate the effects of IBD and its treatm-ent on m-ale fertility, we reviewed the current literature using a system-atic search for published studies. A PubM ed search were perform-ed using the m-ain search term-s 'IBD AND m-ale infertility', 'Crohn's disease AND m-ale infertility', 'ulcerative colitis AND m-ale infertility'. References in review articles were used if relevant. We noted that active inflammation, poor nutrition, alcohol use, sm-oking, m-edications, and surgery m-ay cause infertility in m-en with IBD. In surgery such as proctocolectom-y with ileal pouch-anal anastom-osis, rectal incision seem-s to be associated with sexual dysfunction. Of the m-edications used for IBD, sulfasalazine reversibly reduces m-ale fertility. No other m-edications appear to affect m-ale fertility significantly, although sm-all studies suggested som-e adverse effects. There are lim-ited data on the effects of drugs for IBD on m-ale fertility and pregnancy outcom-es; however, patients should be inform-ed of the possible effects of paternal drug exposure. 16Genetics and male infertility显示文摘The goal of this review is to explain the requirement for understanding the genetic structure of infertility arising from male factor and to discuss the essentials of these genetic elements(2). The majority of the population is affected by this disorder caused by male factor infertility(1); but the etiologies are still unknown. After the primary genetic structure in infertile phenotypes is searched, an evaluation can be made. Thus the reasons causing infertility can be discovered and patients can benefit from effective therapies(1). Publications about male infertility within the recent 10 years in the Pubmed database were discussed(1). There are some approachments for describing the function of specificgenes, but no adequate study is present to be useful for diagnosing and treating male infertility(1). Male fertility and fertility in offspring of males are considerably affected by the exact transition of epigenetic information(1). When the genetic factors playing a role in male infertility were analysed, significant steps will be taken for treating patients and determining the reasons of idiopathic infertility(1). 17Hypogonadism is independently associated with varicocele repair-in a contemporary cohort of men in the USA显示文摘We aimed to identify demographic and clinical predictors of varicocele repair in a contemporary cohort of men in the USA.We queried the 2009-2015 MarketScan Database using relevant ICD9,ICD10,and CPT codes to identify all 18-45 year olds with varicoceles.Differences in age,area of residence,clinical characteristics,and medical management between men who did and did not undergo varicocelectomy (open,laparoscopic,or microsurgical)during the study period were compared using unpaired t-tests and Chi-squared tests for continuous and categorical variables,respectively.Multivariable logistic regression analysis was used to evaluate age,semen analyses,and serum hormone assessment as predictors of varicocele repair.SAS version 9.4 was used for all statistical analyses.Significance was set at P <0.05.Approximately 40%of men with varicoceles underwent repair, primarily through an open approach.Men who underwent repair were more likely to have a diagnosis of male infertility (15.5%vs 7.9%,P <0.001)and male hypogonadism (3.4% vs 0.9%)and were more likely to complete semen analyses (36.1%vs 12.2%, P <0.001)and serum testosterone evaluation (42.5%vs 18.8%,P <0.001).In multivariable regression models,the strongest predictors of varicocele repair were semen analysis (OR =2.78,95%Ch 2.56-3.02),age 18-25 years (OR =2.66,95%CI. 2.36-2.98),and serum testosterone evaluation (OR =1.67,95%CI.1.51-1.86).Although male infertility remains the most important indication for varicocele repair,male hypogonadism is emerging as an independent predictor of varicocelectomy,which may represent a change in the clinical management of varicoceles in the USA.Cailey Guercio Dattatraya Patil Akanksha Mehta 2019Asian Journal of Andrology2019,21,1:0
18Compound heterozygous mutations in PMFBP1 cause acephalic spermatozoa syndrome:A case report显示文摘BACKGROUND Acephalic spermatozoa syndrome(ASS)is an extremely rare form of severe teratozoospermia,where in most of the sperm either appear to lack heads or have disconnected or poorly connected heads and tails.CASE SUMMARY We reported the case of a male patient with secondary infertility whose sperm showed typical ASS upon morphological analysis.Whole-exome sequencing was performed on the patient’s peripheral blood,which revealed two heterozygous variants of the PMFBP1 gene:PMFBP1c.414+1G>T(p.?)and PMFBP1c.393del(p.C132Afs*3).CONCLUSION It is speculated that the compound homozygous mutation of PMFBP1 may be the cause of ASS.We conducted a literature review in order to provide the basis for genetic counseling and clinical diagnosis of patients with ASS.Tian-Qin Deng Yu-Li Xie Jiang-Bo Pu Jiang Xuan Xue-Mei Li 2022World Journal of Clinical Cases2022,10,34:0
19Intracytoplasmic sperm injection in the treatment of male infertility due to obstructive or non-obstructive azoospermia显示文摘Objective: To evaluate the effects of intracytoplasmic sperm injection (ICSI) ontreatment of infertility due to obstructive and non-obstructive azoospermia..Methods: A retrospective analysis of fertilization, cleavage, embryo implantationand pregnancy rates was done in 158 ICSI cycles including 112 obstructive azoospermiaand 46 non-obstructive azoospermia. Ovarian hyperstimulation and ICSI procedureswere performed by conventional protocol. The sperm was collected by percutaneous epi-didymal sperm aspiration (PESA) or testicular sperm extraction (TESE).Results:The fertilization rate (73.1% vs. 67.0%), cleavage rate (88.6% vs. 86.3%), embryo implantation rate (20.7% vs. 11.4%), clinical pregnancy rate per trans-fer cycle (35.7% vs. 19.6%) were obtained for obstructive and non-obstructiveazoospermia, respectively.Conclusion: The results revealed that in the cases of obstructive azoospermia, ferti-lization rate, embryo implantation rate and clinical pregnancy rate were significantlyhigher than those of non-obstructive azoospermia. 20Impact of chronological ageing on semen parameters in southern Indian men visiting infertility centre:A retrospective study显示文摘Objective:To investigate the association between age and semen parameters among male partners of subfertile couples.Methods:This retrospective study analyzed the semen of 1523 infertile men aged 26 to 50 years.Data were extracted from GarbhaGudi IVF Centre database from January 2019 to September 2020.The basic semen parameters were interpreted according to the WHO manual 2021,6th edition.Semen parameters in different age groups were compared.Results:Total and progressive motile sperms were significantly higher in the age group of 26-30 years compared to other age groups(P<0.05).Normal sperm count was significantly higher in the age group of 26-30 years compared to the age groups of 41-45 years and>46 years(P=0.001).However,sperm head defects,neck and midpiece defects,tail defects,and cytoplasmic droplets showed statistically insignificant difference in all the age groups(P>0.05).Semen viscosity showed no statistical difference in all the age groups compared to the reference age group of 26 to 30 years.Conclusions:Higher age can lead to a significant decrease in normal sperms and motility in subfertile men.Hence,male partner age should be considered as one of the major determining factors for reproductive outcomes.Muhammed Asif Asha S.Vijay Maheshwari Syed Fyzullah Usha Rani Swathi R Damodara Gowda KM 2023Asian pacific Journal of Reproduction2023,12,1:0
