1Exercise guidelines for gestational diabetes mellitus显示文摘The prevalence of gestational diabetes mellitus(GDM)is increasing worldwide.This disease has many detrimental consequences for the woman,the unborn foetus and child.The management of GDM aims to mediate the effects of hyperglycaemia by controlling blood glucose levels.Along with pharmacology and dietary interventions,exercise has a powerful potential to assist with blood glucose control.Due to the uncertainty of risks and benefits of exercise during pregnancy,women tend to avoid exercise.However,under adequate supervision exercise is both safe and beneficial in the treatment of GDM.Therefore it is vital that exercise is incorporated into the continuum of care for women with GDM.Medical doctors should be able to refer to competently informed exercise professionals to aid in GDM treatment.It is important that exercise treatment is informed by research.Hence,the development of evidence-based guidelines is important to inform practice.Currently there are no guidelines for exercise in GDM.This review aims to assess the efficacy of exercise for the management of GDM in order to establish an exercise prescription guideline specific to the condition.It is recommended that women with GDM should do both aerobic and resistance exercise at a moderate intensity,a minimum of three times a week for 30-60min each time.Cliantha Padayachee Jeff S Coombes 2015World Journal of Diabetes2015,6,8:31
2Health benefits of traditional Chinese sports and physical activity for older adults: A systematic review of evidence显示文摘Background: Traditional Chinese sports and physical activities(PAs) have a long history and are practiced by millions of Chinese. However,relatively few systematic reviews of the scientific evidence for their health benefits, especially for older Chinese adults, have been undertaken.Evidence acquisition: Between January and March 2016, a systematic search was conducted using the CNKI and Pub Med databases to identify studies published between 2000 and 2015. Studies were selected for review if they were designed specifically to evaluate the health benefits of traditional Chinese sports and PAs in adults aged 50 years and older in the Mainland of China. The studies included observational, uncontrolled,and randomized and controlled designs. Papers published without an English title or abstract were excluded.Evidence synthesis: The initial search identified a total of 229 studies. After removing duplicates and studies that did not meet the inclusion/exclusion criteria, 95 studies were selected for review. Special attention was given to studies of the most commonly practiced activities: Tai Ji Quan,Qigong, and Yangko exercises. A positive association between these types of exercise and health benefits was noted for healthy older adults and those with chronic diseases. Evidence on other types of activities was less clear due to the limited number of studies conducted.Conclusion: There is promising evidence that traditional Chinese sports and PAs provide many health benefits for older Chinese adults. 3Fundamental motor skills,screen-time,and physical activity in preschoolers显示文摘Purpose: To examine the associations among preschoolers fundamental motor skills, screen-time, physical activity(PA), and sedentary behavior(SB).Methods: Children ages 3à4 years were enrolled in a prospective observational trial of PA. Trained assessors conducted the Test of Gross Motor Development-3 rd edition(TGMD-3), and the Movement Assessment Battery for Children-2 nd edition, and parent-reported child screen-time and sociodemographic information. Children wore an accelerometer for 7 days to examine SB and total PA(TPA). TPA was further characterized as moderateto-vigorous PA(MVPA) or vigorous PA(VPA). Mixed linear models were calculated, controlling for age(for TGMD-3), sex, household income, and accelerometer wear time(for accelerometry models), with childcare center as a random effect. The primary analysis reported on the cross-sectional baseline data of 126 children with complete fundamental motor skill and screen-time data; a subanalysis included 88 children with complete accelerometry data.Results: Children were 3.4 § 0.5 years of age(54% girls; 46% white, 42% African American, 12% other). A total of 48% lived in households at or below the federal poverty level. Children engaged in 5.1 § 3.6 h/day of screen-time. Children's screen-time was inversely related to the Movement Assessment Battery for Children-2 nd edition, manual dexterity skills percentile(b(SE) = ?1.7(0.8), p = 0.049). In the accelerometry subsample,children engaged in 5.9 § 0.9 h/day of TPA of which 1.7 § 0.6 h/day was MVPA. Boys engaged in more MVPA and VPA and less SB compared with girls(all p < 0.05). A higher TGMD-3, total score(b(SE) = 0.4(0.2), p = 0.017) and locomotor score(b(SE) = 0.7(0.3), p = 0.018) were associated with more VPA but not with TPA or MVPA. Screen-time and television in the bedroom were not related to SB, TPA, MVPA, or VPA.Conclusion: Children's motor skills were positively related to VPA but inversely related to screen-time. 4Prescribing physical activity to prevent and manage gestational diabetes显示文摘Gestational diabetes mellitus(GDM)is the most prevalent metabolic disorder during pregnancy.Women diagnosed with GDM have a substantially greater risk of developing type 2 diabetes within 5-10 years after delivery,and the risk is increased by excess body weight.Uncontrolled hyperglycemia during pregnancy is potentially harmful to both mother and fetus,resulting in a greater need for Caesarian-section deliveries,delivery of larger infants with more excess body fat,a greater risk of infant death and stillbirth,and an elevated risk of infant hypoglycemia immediately after birth.Fortunately,engaging in physical activity prior to and during pregnancy may lower the risk of developing GDM.Pregnant women should also be advised how to safely increase their physical activity during pregnancy and the postpartum period.An initial approach to becoming more physically active can simply be to encourage women to incorporate more unstructured physical activity into daily living,both before and during pregnancy.Giving women an appropriate exercise prescription can encourage them to participate in physical activity safely and effectively throughout pregnancy to prevent and/or manage GDM.Engaging in 30 min of moderate intensity physical activity on most,if not all,days of the week has been adopted as a recommendation for all pregnant women.Sheri R Colberg Kristin Castorino Lois Jovanovi 2013World Journal of Diabetes2013,4,6:12
5Associations between individual and environmental factors and habitual physical activity among older Chinese adults:A social–ecological perspective显示文摘Purpose: To examine, within a social–ecological framework, associations between multifaceted individual and environmental factors and habitual physical activity(HPA) among older Chinese adults.Methods: Through a mix of qualitative and quantitative methods, a survey instrument assessing various factors underlying 3 social–ecological dimensions of intrapersonal, interpersonal, and community and environmental resources was developed. Using a cross-sectional design, older adults(n = 1580, aged 67 ± 7 years) recruited from 10 communities in Shandong province completed the social–ecological survey of HPA. 6Effects of exergaming on motor skill competence,perceived competence,and physical activity in preschool children显示文摘Background: Few school settings offer opportunities for preschool children to engage in structured physical activity, and only a few studies have been conducted examining exergaming's effectiveness on health outcomes in this age group. This study's purpose, therefore, was to examine a school-based exergaming intervention's effect on preschool children's perceived competence(PC), motor skill competence(MSC), and physical activity versus usual care(recess), as well as to examine gender differences for these outcomes.Methods: A total of 65 preschool children from 2 underserved urban schools were assigned to 1 of 2 conditions, with the school as the experimental unit:(1) usual care recess group(8 weeks of 100 min of recess/week(5 days £ 20 min)) and(2) exergaming intervention group(8 weeks of100 min of exergaming/week(5 days £ 20 min) at school). All children underwent identical assessments of PC, MSC, and moderate-to-vigorous physical activity(MVPA) at baseline and at the end of the 8 th week.Results: A significant Group £ Time effect was observed for MVPA, F(1, 52) = 4.37, p = 0.04, h2 p= 0.04, but not for PC, F(1, 52) = 0.83, p = 0.37,h2 p= 0.02, or MSC, F(1, 52) = 0.02, p = 0.88, h2 p= 0.00. Specifically, the intervention children displayed significantly greater increased MVPA after8 weeks than the comparison children. Additionally, there was a significant time effect for MSC, F(1, 52) = 15.61, p < 0.01, h2 p= 0.23, and gender effect for MVPA, F(1, 52) = 5.06, p = 0.02, h2 p= 0.09. Although all preschoolers' MSC improved across time, boys demonstrated greater MVPA than girls at both time points.Conclusion: Exergaming showed a positive effect in promoting preschool children's MVPA at school and has the potential to enhance PC and MSC. 7Physical activity and health in the presence of China's economic growth:Meeting the public health challenges of the aging population显示文摘Three decades of rapid economic development in China have not only benefited millions of Chinese by improving their living standards but have also dramatically increased the number of people who are part of the country's aging population. However, economic growth has not been accompanied by sufficient attention given to important public health issues, including an increase in the incidence of chronic diseases and a decline in physical activity(PA) that comes with an aging population. The rapid growth in China's older population will soon exert an impact on the nation's economy, population health status, and health behaviors, and will increase stress on its healthcare system. This review article provides a broad perspective on the impact of rapid economic development, industrialization, and urbanization on health-related behaviors, with a specific focus on PA among older adults. 8Physical rehabilitation for lung transplant candidates and recipients: An evidence-informed clinical approach显示文摘Physical rehabilitation of lung transplant candidates and recipients plays an important in optimizing physical function prior to transplant and facilitating recovery of function post-transplant. As medical and surgical interventions in lung transplantation have evolved over time, there has been a demographic shift of individuals undergoing lung transplantation including older individuals, those with multiple co-morbidites, and candidates with respiratory failure requiring bridging to transplantation. These changes have an impact on the rehabilitation needs of lung transplant candidates and recipients. This review provides a practical approach to rehabilitation based on research and clinical practice at our transplant centre. It focuses on functional assessment and exercise prescription during an uncomplicated and complicated clinical course in the pre-transplant, early and late post-transplant periods. 9Effect of Physical Activity on Hospital Service Use and Expenditures of Patients with Coronary Heart Disease: Results from Dongfeng-Tongji Cohort Study in China显示文摘The intervention of behaviors, including physical activity (PA), has become a strategy for many hospitals dealing with patients with chronic diseases. Given the limited evidence available about PA and healthcare use with chronic diseases, this study explored the association between different levels of PA and annual hospital service use and expenditure for inpatients with coronary heart disease (CHD) in China. We analyzed PA information from the first follow-up survey (2013) of the Dongfeng-Tongji cohort study of 1460 CHD inpatients. We examined factors such as PA exercise volume and years of PA and their associations with the number of inpatient visits, number of hospital days, and inpatient costs and total medical costs. We found that the number of hospital days and the number of inpatient visits were negatively associated with intensity of PA level. Similarly, total inpatient and outpatient costs declined when the PA exercise volume levels increased. Furthermore, there were also significant associations between the number of hospital days, inpatient costs or total medical costs and levels of PA years. This study provides the first empirical evidence about the effects of the intensity and years of PA on hospital service use and expenditure of CHD in China. 10Associations between the physical environment and park-based physical activity:A systematic review显示文摘Background:With an increase in the evidence for the associations between park-based physical activity(PA)and physical environments(especially park and neighborhood environments),researchers face an important challenge in interpreting and summarizing the evidence to develop environ?mental change interventions.An updated review is needed to better inform policymaking and environmental interventions.The current study aimed to systematically review the research on the associations of park-based PA with park and neighborhood environmental characteristics.Methods:We targeted English peer-reviewed articles from 5 electronic databases using keywords related to park-based PA,park environments,and neighborhood environments.Of the 4071 identified papers,25 studies published between 2008 and 2016 met all the eligibility criteria and were included in this review.Results:The characteristics of physical environment that received consistent support included paths/trails,lighting,and incivilities(e.g.,broken glasses and litter).Mixed findings were revealed for 6 park environmental factors(unspecified active facilities,playgrounds and skating areas,fit?ness stations,picnic areas,greenness,and park size)and 2 neighborhood environmental factors(park density and park proximity).Conclusion:It can be concluded that paths/trails,lighting,and incivilities are 3 key physical environmental attributes of park-based PA.Given the inconsistent findings on park and neighborhood environmental factors,more robust designs such as prospective investigation are required.Ru Zhang Hagen Wulff Yanping Duan Petra Wagner 2019Journal of Sport and Health Science2019,8,5:7
11Treatment approach to type 2 diabetes:Past,present and future显示文摘Type 2 diabetes mellitus(DM) is a lifelong metabolic disease, characterized by hyperglycaemia which gradually leads to the development and progression of vascular complications. It is recognized as a global burden disease, with substantial consequences on human health(fatality) as well as on health-care system costs. This review focuses on the topic of historical discovery and understanding the complexity of the disease in the field of pathophysiology, as well as development of the pharmacotherapy beyond insulin. The complex interplay of insulin secretion and insulin resistance developed from previously known 'ominous triumvirate' to 'ominous octet' indicate the implication of multiple organs in glucose metabolism. The pharmacological approach has progressed from biguanides to a wide spectrum of medications that seem to provide a beneficial effect on the cardiovascular system. Despite this, we are still not achieving the target treatment goals. Thus, the future should bring novel antidiabetic drug classes capable of acting on several levels simultaneously. 12Physical activity among older Chinese adults living in urban and rural areas: A review显示文摘With an increase in rural-to-urban migration, a rapidly aging population, and the rising risk of developing noncommunicable diseases in China,it is important to understand the epidemiology of physical activity(PA) and health in the context of disease prevention and population health.Despite its public health importance, there is a significant lack of knowledge about PA in older Chinese adults that may hamper primary prevention efforts of health promotion in an increasingly aging population. To fill this gap, this article presents a narrative review of PA in the older Chinese adult population with a special focus on residential settings(i.e., urban and rural). Using existing studies, the review examines overall PA patterns and their correlates and discusses public health implications and future research. 13Validity and Reliability of Chinese Physical Activity Questionnaire for Children Aged 10-17 Years显示文摘Objective This study is aimed to report the development,the reliability and validity of the Chinese Children Physical Activity Questionnaire(CCPAQ)which was designed for the assessment of physical activity pattern in young population in China.Methods The CCPAQ was administered for two times in 119 children(mean age 13.1±2.4 years;boys 47%)to examine reliability by using intraclass correlation coefficients.Validity was determined in 106 participants by agreement with the CCPAQ measures and the objective method,the ActiGraph accelerometer.Data on physical activity patter n including time spe nt on d iff ere nt intensi ties and total physical activity,sedentary behavior as well as physical activity energy expenditure were used to assess the validity with Spearman's correlation coefficient and the Bland-Altman plots.Results The reliability coefficient of the CCPAQ ranged from 0.63-0.93(Intra-class correlation coefficient).Spearman's correlation coefficient for validity of time spent on total physical activity and sedentary behavior were all 0.32(P<0.001),and for physical activity energy expenditure was 0.58(P<0.001).Time spent on moderate-to-vigorous physical activity and light physical activity showed a relatively low correlation with the accelerometer(rho=0.20,P=0.040;rho=0.19,P=0.054).Conclusion The CCPAQ appears to be a promising and feasible method to assess physical activity pattern in Chinese children.YANG Xi JAGO Russell ZHAI Yi YANG Zhen Yu WANG Yu Ying SI Xiang WANG Jun GAO Jian Fen CHEN Jing Ron YU Ying Jie ZHAO Wen Hua 2019Biomedical and Environmental Sciences2019,32,9:6
14The effect of the CHAMP intervention on fundamental motor skills and outdoor physical activity in preschoolers显示文摘Purpose: Physical activity(PA) and fundamental motor skills are important components of current and future trajectories of health in young children. This study examined the effects of a 5-week motor skill intervention on preschoolers' motor skill competence and their PA behaviors while participating in the motor skill intervention or outdoor free play(recess).Methods: A total of 102 preschoolers served as participants and were part of a motor skill intervention group(n = 64) or a control/outdoor free play group(n = 38). Children's motor skills were assessed before and after the intervention using the Test of Gross Motor Development-3 rd edition. PA during the motor skill intervention and outdoor free play was assessed using accelerometers both immediately before the start(baseline, Week 0 or Week 1) and end(late, Week 5 or Week 6) of the intervention.Results: All children significantly improved their motor skills from baseline to late assessment(p < 0.05). Children in the motor skill intervention demonstrated greater rates of change(p < 0.001) and scored higher on all motor skills at the late assessment compared with the control group(p < 0.001). There was no effect of group(control vs. intervention), but there was a significant effect of sex on children's PA during outdoor free play at baseline(p < 0.05). Similarly, there was no effect of group on PA during days with the movement program(intervention vs. outdoor free play) at either time point, but boys were more active than girls at the late assessment(p < 0.05). Last, children in the intervention engaged in more PA while participating in the intervention toward the end of the intervention than at the beginning.Conclusion: The 5-week motor skill intervention was effective at improving preschoolers' motor skills and rates of change in motor skills were higher for children who completed the intervention compared with children in the control group. 15Physical activity after total knee arthroplasty: A critical review显示文摘Total knee arthroplasty(TKA) is the most commonly performed elective surgery in the United States. TKA typically improves functional performance and reduces pain associated with knee osteoarthritis. Little is known about the influence of TKA on overall physical activity levels. Physical activity, defined as 'any bodily movement produced by skeletal muscles that results in energy expenditure', confers many health benefits but typically decreases with endstage osteoarthritis. The purpose of this review is to describe the potential benefits(metabolic, functional, and orthopedic) of physical activity to patients undergoing TKA, present results from recent studies aimed to determine the effect of TKA on physical activity, and discuss potential sources of variability and conflicting results for physical activity outcomes. Several studies utilizing self-reported outcomes indicate that patients perceive themselves to be more physically active after TKA than they were before surgery. Accelerometry-based outcomes indicate that physical activity for patients after TKA remains at or below pre-surgical levels. Several different factors likely contributed to these variable results, including the use of different instruments, duration of follow-up, and characteristics of the subjects studied. Comparison to norms, however, suggests that daily physical activity for patients following TKA may fall short of healthy agematched controls. 16Exercise in patients coping with breast cancer: An overview显示文摘Breast cancer is the most common type of cancer in women, but fortunately has high survival rates. Many studies have been performed to investigate the effects of exercise in patients diagnosed with breast cancer. There is evidence that exercise after the diagnosis of breast cancer improves mortality, morbidity, health related quality of life, fatigue, physical functioning, muscle strength, and emotional wellbeing. Based on scientific data, breast cancer patients should be recommended to participate in rehabilitation programs including aerobic and strength training. 17Daily physical activity and type 2 diabetes: A review显示文摘Physical activity improves glycemic control and reduces the risk of cardiovascular disease(CVD) and mortality in patients with type 2 diabetes(T2D). Moderate to vigorous physical activity is recommended to manage T2D; however, patients with T2D can be physically weak, making it difficult to engage in the recommended levels of physical activity. Daily physical activity includes various activities performed during both occupational and leisure time such as walking, gardening, and housework that type 2 diabetic patients should be able to perform without considerable physical burden. This review focuses on the association between daily physical activity and T2D. Walking was the most common form of daily physical activity, with numerous studies demonstrating its beneficial effects on reducing the risk of T2D, CVD, and mortality. Walking for at least 30 min per day was shown to reduce the risk of T2D by approximately 50%. Additionally, walking was associated with a reduction in mortality. In contrast, evidence was extremely limited regarding other daily physical activities such as gardening and housework in patients with T2D. Recent studies have suggested daily physical activity, including non-exercise activity thermogenesis, to be favorably associated with metabolic risks and mortality. 19Associations between cardiac arrhythmia,incident disability in activities of daily living and physical performance:the ILSA study显示文摘Background Cardiac arrhythmias are common conditions in older people.Unfortunately,there is limited literature on associations between cardiac arrhythmias and physical performance or disability.We therefore aimed to prospectively investigate associations between cardiac arrhythmias and changes in disability and physical performance during 8 years of follow-up,using data from the Italian Longitudinal Study on Aging(ILSA).Methods Cardiac arrhythmias diagnosis was posed through a screening phase,confirmed by a physician.The onset of disability in activities of daily living(ADL)and the changes in several physical performance tests during follow-up were considered as outcomes.Fully-adjusted and propensity-score Cox Proportional Hazard models and mixed models were used for exploring associations between cardiac arrhythmia and the outcomes of interest.Results The prevalence of cardiac arrhythmia at baseline was 23.3%.People reporting cardiac arrhythmia at the baseline were significantly older,more frequently male,smokers and reported a higher presence of all medical conditions investigated(hypertension,heart failure,angina,myocardial infarction,diabetes,stroke),but no difference in dementia,Parkinsonism,cognitive or mood disorder.Cardiac arrhythmia at baseline was significantly associated with the incidence of disability in ADL(HR=1.23;95%:CI:1.01–1.50;P=0.0478 in propensity score analyses;HR=1.28;95%CI:1.01–1.61;P=0.0401 in fully adjusted models).Cardiac arrhythmia at baseline was also associated with a significant worsening in balance test(P=0.0436).Conclusions The presence of cardiac arrhythmia at baseline was associated with a significant higher risk of disability and of worsening in some physical performance tests,particularly those relating to balance.Screening and frequently assessing physical performance in older people affected by cardiac arrhythmia can be important to prevent a loss of physical performance,with further,potential,complications of medical management.Marianna Noale Nicola Veronese Lee Smith Andrea Ungar Stefano Fumagalli Stefania Maggi the Italian Longitudinal Study on Aging Working Group 2020Journal of Geriatric Cardiology2020,17,3:3
20Land-walking vs.water-walking interventions in older adults:Effects on aerobic fitness显示文摘Background:Low cardiorespiratory fitness is an independent predictor of all-cause and cardiovascular mortality,and interventions that increase fitness reduce risk.Water-walking decreases musculoskeletal impact and risk of falls in older individuals,but it is unclear whether water-walking improves aerobic fitness in the same way as weight-dependent land-walking.This randomized controlled trial involved 3 intervention groups—a no-exercise control group(CG),a land-walking(LW)group,and a water-walking(WW)group—to investigate the comparative impacts of LWandWWto CG on fitness.Methods:Both exercise groups attended individually tailored,center-based,intensity-matched 3×weekly sessions for 24 weeks,which progressed to 150 min of exercise per week.This was followed by a 24-week no-intervention period.Maximal graded exercise tests were performed on a treadmill at Weeks 0,24,and 48.Results:Maximal oxygen uptake increased from Week 0 to Week 24 in both exercise groups(0.57±0.62 mL/kg/min,0.03±0.04 L/min for LW;0.93±0.75 mL/kg/min,0.06±0.06 L/min for WW,mean±SE)compared to the CG(-1.75±0.78 mL/kg/min,-0.16±0.05 L/min)(group×time,p<0.05).Time to exhaustion increased significantly following LW only(123.4±25.5 s),which was significantly greater(p=0.001)than the CG(24.3±18.5 s).By Week 48,the training-induced adaptations in the exercise groups returned to near baseline levels.Conclusion:Our study supports current physical-activity recommendations that 150 min/week of moderate-intensity exercise produces improvements in fitness in previously sedentary older individuals.Also,LW andWW elicit similar improvements in fitness if conducted at the same relative intensities.Exercise-na?ve older individuals can benefit from the lower impact forces and decreased risk of falls associated withWWwithout compromising improvements in cardiorespiratory fitness.Andrew Haynes Louise H.Naylor Howard H.Carter Angela L.Spence Elisa Robey Kay L.Cox Barbara A.Maslen Nicola T.Lautenschlager Nicola D.Ridgers Daniel J.Green 2020Journal of Sport and Health Science2020,9,3:2
