| 1 | Efficacy and safety of vildagliptin in clinical practice-results of the PROVIL-study显示文摘AIM:To investigate efficacy and safety of vildagliptin compared to other oral antidiabetics in clinical practice in Germany.METHODS:In this prospective,open,observational study,patients with type 2 diabetes mellitus(T2DM) previously on oral monotherapy were selected by their treating physician to receive either vildagliptin addon to metformin(cohort 1),vildagliptin+metformin single-pill combination(SPC)(cohort 2)or another dual combination therapy with oral antidiabetic drugs(OADs)(cohort 3).According to routine clinical practice,interim examinations occurred every 3 mo:at baseline,after approximately 3 mo and after approximately 6 mo.Parameters documented in the study included demographic and diagnostic data,history of T2DM,data on diabetes control,vital signs,relevant prior and concomitant medication and disease history.Efficacy was assessed by changes in HbA1c and fasting plasma glucose(FPG)3 mo and 6 mo after initiation of dual combination therapy.Safety was assessed by adverseevent reporting and measurement of specific laboratory values(serum creatinine,total bilirubin,alanine aminotransferase,aspartate aminotransferase,creatine kinase).RESULTS:Between October 2009 and January 2011,a total of 3881 patients were enrolled in this study.Since 47 patients were withdrawn due to protocol violations,3834 patients were included in the statistical analysis.There were no relevant differences between the three cohorts concerning age,body weight and body mass index.Average diabetes duration was approximately 6 years and mean HbA1c was between 7.6%and 7.9% at baseline.Antidiabetic treatment was recorded in 3648 patients.Patients were treated with vildagliptin add-on to metformin(n=603),vildagliptin+metformin(SPC)(n =2198),and other oral OADs including combinations of metformin with sulfonylurea(n=370),with glitazones(n =123),other dipeptidyl peptidase-4 inhibitors(n=99).After 6 mo of treatment,the absolute decrease in HbA1c(mean±SE)was significantly more pronounced in patients receiving vildagliptin add-on to metformin(-0.9% ±0.04%)and vildagliptin+metformin(SPC)(-0.9%± 0.03%)than in patients receiving other OADs(-0.6% ±0.04%;P<0.0001).In addition,significant cohort differences were observed for the improvement in FPG after 6 mo treatment(vildagliptin add-on to metformin:-291 mg/L±18.3 mg/L;vildagliptin+metformin(SPC):-305 mg/L±9.6 mg/L;other antidiabetic drugs:-209 mg/L±14.0 mg/L for(P<0.0001).Moderate decreases in body weight(absolute difference between last control and baseline:mean±SE)were observed for patients in all cohorts(vildagliptin add-on to metformin:-1.4 kg ±0.17 kg;vildagliptin+metformin(SPC):-1.7 kg± 0.09 kg;other OADs:?0.8 kg±0.13 kg).No significant differences in adverse events(AEs)and other safety measures were observed between the cohorts.When performing an additional analysis by age(patients<65 years vs patients≥65 years),there was no relevant difference in the most common AEs between the two age groups and the AE profile was similar to that of the overall patient population.CONCLUSION:Clinical practice confirms that vildagliptin is an effective and well-tolerated treatment in combination with metformin in T2DM patients. | Matthias Blüher Ira Kurz Simone Dannenmaier Markus Dworak | 2012 | World Journal of Diabetes2012,3,9: | 9 |

| 3 | Hepatitis B or C viral infection and risk of pancreatic cancer: A meta-analysis of observational studies显示文摘AIM: To investigate if there is an association between hepatitis B virus (HBV) or hepatitis C virus (HCV) infection and the risk of pancreatic cancer. METHODS: All relevant studies published before 11 October, 2012 were identified by a systematic search of MEDLINE, EMBASE, BIOSIS Previews and the Cochrane Library databases and with cross-referencing. The observational studies that reported RR or OR estimates with 95%CIs for the association between HBV or HCV and pancreatic cancer were included. A random-effects model was used to summarize meta-analytic estimates. The Newcastle-Ottawa quality assessment scale was applied to assess the quality of the methodology in the included studies. RESULTS: A total of 8 eligible studies were selected for meta-analysis. Overall, chronic hepatitis B and inactive hepatitis B surface antigen (HBsAg) carrier state (HBsAg positive) had a significantly increased risk of pancreatic cancer with OR of 1.20 (95%CI: 1.01-1.39), especially in the Chinese population (OR = 1.30, 95%CI: 1.05-1.56). Past exposure to HBV (possible occult HBV infection) had an increased OR of pancreatic cancer risk (OR = 1.24, 95%CI: 1.05-1.42), especially among those patients without natural immunity [anti hepatitis B core (HBc) positive/hepatitis B surface antibody (anti HBs) negative], with OR of 1.67 (95%CI: 1.13-2.22). However, past exposure to HBV with natural immunity (anti-HBc positive/anti-HBs positive) had no association with pancreatic cancer development, with OR 0.98 (95%CI: 0.80-1.16), nor did the HBV active replication (hepatitis B e antigen positive status), with OR 0.98 (95%CI: 0.27-1.68). The risk of pancreatic cancer among anti-HBs positive patients was significantly lower than among anti-HBs negative patients (OR = 0.54, 95%CI: 0.46-0.62). Past exposure to HCV also resulted in an increased risk of pancreatic cancer (OR = 1.26, 95%CI: 1.03-1.50). Significant between-study heterogeneity was observed. Evidence of publication bias for HBV/HCV infection-pancreatic cancer association was not found. CONCLUSION: Chronic HBV and HCV infection increases pancreatic cancer risk. | 5 | EVALUATION OF THE PARAMETERIZATION SCHEMES AND NUDGING TECHNIQUES IN GRAPES FOR WARM SECTOR TORRENTIAL RAINS USING SURFACE OBSERVATIONS显示文摘Sensitivities of parameterization schemes were conducted based on the Global/Regional Assimilation and Prediction System(GRAPES) model. Surface observations were used to evaluate the simulations and to improve the model’s ability to simulate the extreme precipitation over southern China on 20 July 2016. The results showed that GRAPES captured the large-scale precipitation over southern China but failed to predict the extreme precipitation over Xinyi. The model showed a systematic cold biases by adopting different parameterization schemes. In particular, the ECMWF analyses data showed a strong cold bias over Guangdong province and Guangxi region. Observational nudging results showed that the surface temperature could largely help to alleviate the cold bias. The alleviation in the warm sector accounted for main improvement by the nudging scheme, and the RMSE was reduced by 1.56 degree from 3.25 degree to 1.69 degree by 1-h simulation and with 1.3 degree alleviation by 2-h simulation. Of 306 eligible study participants, interviews were completed in197(64%). Participants were asked about sociodemographic variables, disease and treatment characteristics, their desire for fertility, and their use of Western or Chinese Medicine.RESULTS: The proportions of women who planned to use Western or Chinese Medicine to help achieve pregnancy were 17.3% and 14.7%, respectively. The result of binary logistic regression showed that current employment and lack of children were factors predictive of the use of Western Medicine. Younger age and the use of Chinese Medicine before treatment were predictors of using Chinese Medicine. While the desire for fertility preservation was related to the use of Western Medicine,the desire for pregnancy was related to the use of Chinese Medicine.CONCLUSION: Various characteristics guided the women's decisions to use Western or Chinese Medicine. | 11 | International normalized ratio and Model for End-stage Liver Disease score predict short-term outcome in cirrhotic patients after the resolution of hepatic encephalopathy显示文摘BACKGROUND In patients with cirrhosis,hepatic encephalopathy(HE)indicates a poor prognosis despite the use of artificial liver and liver transplantation,presenting as frequent hospitalizations and increased mortality rate.AIM To determine predictors of early readmission and mid-term mortality in cirrhotic patients discharged after the resolution of HE.METHODS From January to February 2018,213 patients were enrolled in this observational study assessing all the successive patients with cirrhosis discharged from Department of Gastroenterology and Department of Infectious and Liver Diseases,Second Affiliated Hospital of Chongqing Medical University after the resolution of HE.The patients were followed for 6 mo.For each subject,demographic,clinical,and laboratory variables were assessed at the time of diagnosis of HE,during hospital stay,at discharge,and during follow-up.The primary endpoints were incidence of early readmission and mid-term mortality.RESULTS During follow-up,65(31%)patients experienced an early readmission.International normalized ratio(INR)[odds ratio(OR)=2.40;P=0.003)at discharge independently predicted early readmission.The incidence of early readmission was significantly higher in patients with an INR>1.62 at discharge than in those with an INR≤1.62(44%vs 19%;P<0.001).Model for End-stage Liver Disease(MELD)score(OR=1.11;P=0.048)at discharge proved to be an independent predictor of early readmission caused by HE.Hemoglobin(OR=0.97;P=0.005)at discharge proved to be an independent predictor of non-early readmission.During 6 months of follow-up,34(16%)patients died.Artificial liver use(hazard ratio=6.67;P=0.021)during the first hospitalization independently predicted mid-term mortality.CONCLUSION INR could be applied to identify fragile cirrhotic patients,MELD score could be used to predict early relapse of HE,and anemia is a potential target for preventing early readmission. | Xiao-Peng Hu Jian Gao | 2019 | World Journal of Gastroenterology2019,25,26: | 2 |
| 12 | Ultra-low noise L-band cryogenic astronomical receiver for the FAST telescope显示文摘This paper presents an ultra-low noise L-band radio astronomical cryogenic receiver for the Fivehundred-meter Aperture Spherical radio Telescope(FAST) telescope. The development of key low noise microwave parts of coupling low noise amplifier(Coupling-LNA) and conical quad-ridge orthogonal mode transducers(OMT) and reasonable system integration achieve outstanding performance of receiver. It covers the frequency range of 1.2 GHz to 1.8 GHz. Novel cryogenic Coupling-LNAs with low noise, large return loss, high dynamic range and the function of coupling calibration signals are developed for the proposed receiver. Amplification and coupling function circuits are integrated as a single Coupling-LNA with full noise temperature of 4 K at the physical temperature of 15 K. Its return loss is more than 18 d B, and output1 d B compression power is +5 d Bm. A cryogenic dewar is fabricated to provide 55 K and 15 K cryogenic environment for OMT and Coupling-LNAs, respectively. The receiver's system noise temperature is below9 K referred to feed aperture plane. In order to compare the as- similation capability of Monte Carlo H ∞ filter with that of the ensemble Kalman filter (EnKF) in solving practical problems caused by temporal correlation or spatial correlation of observational errors, two numerical experiments are performed by using Lorenz (1963) system and shallow-water equations re- spectively. The result is that the assimilation capability of the new method is better than that of EnKF method. | 17 | Constraints on brane inflation after Planck 2015: Impacts of the latest local measurement of the Hubble constant显示文摘We investigate the observational constraints on three typical brane inflation models by considering the latest local measurement of the Hubble constant in the global fit. We also employ other observational data, including the Planck 2015 CMB data, the BICEP2/Keck Array B-mode data, and the baryon acoustic oscillations data, in our analysis. Previous studies have shown that the addition of the latest local H_0 measurement favors a larger spectral index, and can exert a significant influence on the model selection of inflation. In this work, we investigate its impacts on the status of brane inflation models. We find that, when the direct H_0 measurement is considered, the prototype model of brane inflation is still in good agreement with the current observational data within the 2σ level range. For the KKLMMT model, the consideration of the H_0 measurement allows the range of the parameter β to be amplified to O(10^(-2)), which slightly alleviates the fine-tuning problem. For the IR DBI model, the addition of the H_0 measurement does not provide a better fit. These results show that the consideration of the new H_0 prior can exert a considerable influence on the brane inflation models. The seeing in summer and autumn is better than that in winter and spring. The overall seeing is divided into the boundary layer seeing and free atmosphere seeing to investigate the climatic phenomena or meteorological events that might lead to seasonal variation in the seeing. The overall seeing was measured by the solar difference image motion monitor(SDIMM). The boundary layer seeing is calculated from the temperature difference between air and water. The analysis results show that the seasonal variation in seeing is caused by the alternation of subtropical high and westerly jet. The decrease of seeing in winter and spring at FSO is probably related to the westerly jet. A complete analysis of the seeing at FSO is given in this paper. The analysed results show that there is obviously a quasi-biennial oscillation (QBO) in the interannual variations of the amplitudes of total ozone. Generally, the amplitudes of wavenumber 1 and 2 during the westerly of the equatorial QBO are larger than those during the easterly. In the early winter, the amplitude of wavenumber 1 during the easterly phase is larger, and in the late winter, it is larger during the westerly phase.
