| 1 | 重度脑室内出血尿激酶溶栓单双管引流效率比较显示文摘目的:在尿激酶脑室内溶栓( IVF)治疗下,比较重度脑室内出血( IVH)患者在进行单管或双管脑室外引流( EVD)时的引流效率。方法选择2010-01~2013-06符合纳入标准的47例重度IVH患者随机分为单侧脑室外引流组(单管引流组)和双侧脑室外引流组(双管引流组),各组患者均接受同样的脑室内尿激酶溶栓方案,剂量为2万U q 12 h,双管引流组左右侧药物注入选择为随机;记录并比较脑室内溶栓前及溶栓后1、3、5、7 d时的平均每日脑脊液( CSF )引流量、颅内压(ICP)、GCS评分及ICU监护时间、脑室外引流管放置时间(IVC放置时间)、IVH体积、Graeb评分、颅内感染率,并随访比较发病后6个月时的GOS评分及病死率等。结果两组患者的基线IVH体积、初次Graeb评分、GCS评分、年龄及性别例数比较差异均无统计学意义( P>0.05),具有可比性。两组ICU监护时间和IVC放置时间比较差异无统计学意义(P>0.05),平均每日CSF引流量比较差异有统计学意义( P<0.05),双管引流组平均每日CSF引流量更多。两组溶栓前及溶栓后1、3、5、7 d时IVH体积下降程度比较差异有统计学意义,双管引流组优于单管引流组(P<0.05),而Graeb评分、ICP、GCS两组比较差异无统计学意义(P>0.05);两组引流期间均未发现颅内感染。随访比较6个月时的GOS评分(t=12.63,P=0.000)和病死率(χ2=7.57, P=0.006)发现,双管引流组明显优于单管引流组。结论对于严重、体积大的IVH,相同IVF治疗条件下,放置双管EVD引流较单管清除血肿效率更高,并能改善预后和降低病死率,且不增加颅内感染率和延长IVC放置时间及ICU监护时间。 | 杜波 单爱军 王进 钟贤良 | 2014 | 中国急救医学2014,34,4: | 21 |
| 2 | Endoscopic Surgery versus External Ventricular Drainage Surgery for Severe Intraventricular Hemorrhage显示文摘The efficacy and applied value of endoscopic hematoma evacuation vs. external ventricular drainage (EVD) in the treatment of severe ventficular hemorrhage (IVH) were explored and compared.From Jan.2015 to Dec.2016,the clinical data of 42 cases of IVH were retrospectively analyzed,including 18 patients undergoing endoscopic hematoma evacuation (group A),and 24patients receiving EVD (group B).The hematoma clearance rate was calculated by 3D Slicer software,and complications and outcomes were compared between the two groups.There were no significant differences in age,sex and Graeb score between groups A and B (P>0.05).The hematoma clearance rate was 70.81%±27.64% in group A and 48.72%±36.58% in group B with a statistically significant difference (P<0.05).The operative time in groups A and B was 72.45±25.26 min and 28.54±15.27min,respectively (P<0.05).The Glasgow Coma Scale (GCS) score increased from 9.28±2.72 at baseline to 11.83±2.91 at 1 week postoperatively in group A,and from 8.25±2.62 at baseline to 10.79±4.12 at 1 week postoperatively in group B (P<0.05).The length of hospital stay was 12.67±5.97 days in group A and 17.33±8.91 days in group B with a statistically significant difference (P<0.05).The GOS scores at 6 months after surgery were 3.83±1.12 in group A,and 2.75±1.23 in group B (P<0.05). | 3 | Current status of laparoscopic and robotic ventral mesh rectopexy for external and internal rectal prolapse显示文摘External and internal rectal prolapse with their affiliated rectocele and enterocele, are associated with debilitating symptoms such as obstructed defecation, pelvic pain and faecal incontinence. Since perineal procedures are associated with a higher recurrence rate, an abdominal approach is commonly preferred. Despite the description of greater than three hundred different procedures, thus far no clear superiority of one surgical technique has been demonstrated. Ventral mesh rectopexy(VMR) is a relatively new and promising technique to correct rectal prolapse. In contrast to the abdominal procedures of past decades, VMR avoids posterolateral rectal mobilisation and thereby minimizes the risk of postoperative constipation. Because of a perceived acceptable recurrence rate, good functional results and low mesh-related morbidity in the short to medium term, VMR has been popularized in the past decade. Laparoscopic or robotic-assisted VMR is now being progressively performed internationally and several articles and guidelines propose the procedure as the treatment of choice for rectal prolapse. | 5 | Damage control orthopaedics: State of the art显示文摘Damage control orthopaedics(DCO) originally consisted of the provisional immobilisation of long bone-mainly femur-fractures in order to achieve the advantages of early treatment and to minimise the risk of complications, such as major pain, fat embolism, clotting, pathological inflammatory response, severe haemorrhage triggering the lethal triad, and the traumatic effects of major surgery on a patient who is already traumatised(the 'second hit' effect). In recent years, new locations have been added to the DCO concept, such as injuries to the pelvis, spine and upper limbs. Nonetheless, this concept has not yet been validated in well-designed prospective studies, and much controversy remains.Indeed, some researchers believe the indiscriminate application of DCO might be harmful and produce substantial and unnecessary expense. In this respect, too,normalised parameters associated with the acid-base system have been proposed,under a concept termed early appropriate care, in the view that this would enable patients to receive major surgical procedures in an approach offering the advantages of early total care together with the apparent safety of DCO. | 6 | Efficacy and safety of external application of Traditional Chinese Medicine for the treatment of acute gouty arthritis: a systematic review and Meta-analysis显示文摘OBJECTIVE:To evaluate the clinical efficacy and safety of external application of Traditional Chinese Medicine (EATCM) on acute gouty arthritis (AGA).METHODS:Six electronic data bases were retrieved from their inception to march 2017.15 randomized controlled clinical trials (RCTs) were included.The quality of the literatures was assessed according to Cochrane review criteria by using RevMan5.3 software;and related data was counted using Stata14.0 software.RESULTS:There was no significant difference in C-reactiveprotein (CRP) between experimental intervention group and control group [(WMD =1.093,95% C/(-2.514,4.701);P =0.553)].In terms of overall efficacy,the EATCM's treatment group was significantly superior to control group [(OR =3.692;95% CI (2.408,5.661);P <0.001)].The EATCM was better than western medicine treatments or other interventions in reducing the adverse reactions [(OR =0.135;95% CI (0.067,0.274);P < 0.001)].All these funnel plots showed unlikelihood of publishing bias.But due to excessive heterogeneity,the statistical results of serum uric acid and visual analogue scale and CRP between the two groups became uncertain.CONCLUSION:The results indicate that EATCM may have greater overall efficacy with fewer adverse drug reactions,although the evidence is weak owing to the low methodological quality and the small number of the included trials. | Huang Xinxing Zhu Zonghua Wu Guang-hui Min Peng Fei Peng Zhu Tao Wang Xuezong Xiao Lianbo | 2019 | Journal of Traditional Chinese Medicine2019,39,3: | 12 |
| 7 | 漂移克里金方法在雷达和雨量计联合估测降水中的应用显示文摘文中介绍了一种新的融合雷达和雨量计数据开展定量估测降水研究的空间信息统计学方法—Kriging with external drift(KED)方法。该方法能很好地融合高精度、低时空分辨率的雨量计数据和低精度、高时空分辨率的雷达数据进行插值。通过变异函数描述降水场的空间结构信息,能够充分利用数据间的空间相关性,来改进估测精度和提高处理速度。利用其优良的数学特性,以期在定量估测降水业务研究上进行新的探索和尝试。选用湖南省有代表意义的3次降水过程资料,通过雷达直接估测降水(RAD)、变分校准(VAR)以及KED3种方法,分别与雨量计测量值进行对比分析,选用代表站进行交叉验证结果均表明:RAD的均方差、绝对误差、相对误差最大,VAR次之,而KED最小。KED估测的结果与雨量计测量降水最为接近,估测效果最好;3种方法与雨量计实测值计算一定范围的误差频率,KED估测值具有最小的均方差和最小的标准差,且误差分布相对集中在0值附近,斜度和峰度最佳,试验证明该方法不仅能提高降水估测精度,且优于其他方法,VAR均方差次之,RAD均方差效果相对较差。联合雷达、雨量计估测降水的实质是把雷达估测值与雨量计测量的结果相融合,以雨量计来校准雷达估测值,保留了雷达探测到降水的中、小尺度精细特征。校准后的雨量场数值接近雨量计测值,而且能够准确反映雷达测得的降水分布形式。 | 黄小玉 陈媛 熊毅 陈波 夏正龙 廖玉芳 | 2009 | 气象学报2009,67,2: | 12 |

| 11 | Quantifying the thermal damping effect in underground vertical shafts using the nonlinear autoregressive with external input(NARX) algorithm显示文摘As air descends the intake shaft, its infrastructure, lining and the strata will emit heat during the night when the intake air is cool and, on the contrary, will absorb heat during the day when the temperature of the air becomes greater than that of the strata. This cyclic phenomenon, also known as the 'thermal damping effect' will continue throughout the year reducing the effect of surface air temperature variation. The objective of this paper is to quantify the thermal damping effect in vertical underground airways. A nonlinear autoregressive time series with external input(NARX) algorithm was used as a novel method to predict the dry-bulb temperature(Td) at the bottom of intake shafts as a function of surface air temperature. Analyses demonstrated that the artificial neural network(ANN) model could accurately predict the temperature at the bottom of a shaft. Furthermore, an attempt was made to quantify typical 'damping coefficient' for both production and ventilation shafts through simple linear regression models. METHODS:Fifty eyes with naive CNV-AMD with pretreatment best-corrected visual acuity (BCVA) better than 20/200 and treated with IVB monotherapy were evaluated. Several variables including age, sex, pre-treatment BCVA, CNV type and lesion size on fluorescein angiogram as well as SD-OCT parameters including pre-treatment central macular thickness (CMT), inner-segment/outer-segment (IS/OS) junction integrity, and external limiting membrane (ELM) integrity were analyzed to predict visual outcome.RESULTS:On univariate regression, pretreatment ELM damage was associated with less visual improvement after treatment (P =0.0145). However, ELM damage predicted only 10% of the visual outcome. On multivariate regression, pretreatment BCVA, IS/OS junction, and ELM integrity on SD-OCT were the significant predictors for the treatment effect and together predicted 37% of visual improvement. A total of 22 eyes of 21 patients [1000 centistokes(c St): 17 eyes, 5700 c St:5 eyes] were included in this study. All patients underwent active silicone oil removal via the entire lumen of a 23-gauge microcannula with suction pressure of a650-700 mm Hg vacuum using an external vacuum pump. A tubing adaptor from the Total Plus Pak誖(Alcon,Fort Worth, USA) was used to join the microcannula and silicone vacuum tube connected to an external vacuum pump. Main outcome measures were mean removal time,changes of intraocular pressure(IOP) and visual acuity,and intraoperative and postoperative complications.RESULTS: Mean removal time(min) was 1.49±0.43 for1000 c St and 7. 12 ± 1. 27 for 5700 c St. The IOP was18.57±7.48 mm Hg at baseline, 11.68 ±4.55 mm Hg at day1 postoperatively(P <0.001), and 15.95±4.92, 16.82±3.81,17.41 ±3.50, and 17.09 ±3.01 mm Hg after one week, one month, three months, and six months, respectively. | 17 | Effect of cream, prepared with Tripterygium wilfordii Hook F and other four medicinals, on joint pain and swelling in patients with rheumatoid arthritis: a double-blinded, randomized, placebo controlled clinical trial显示文摘OBJECTIVE: To investigate the effectiveness a cream onjoint pain and swelling in patients with rheumatoid arthritis(RA). The cream, topically used, in was prepared with Tripterygium wilfordii Hook F(TwHF), Mangxiao(Nalrii Sulfas), Chuanxiong(Rhizoma Chuanxiong), stir-frying with liquid adjuvant Ruxiang(Olibanum), and stir-frying with liquid adjuvant Moyao(Myrrh).METHODS: Patients were 1∶1 randomized to addon TwHF cream twice a day or placebo for 4 weeks.The primary endpoint was achievement rate of20% improvement in American College of Rheuma-tology criteria(ACR20) at week 4. Secondary endpoints were ACR50, 28-joint count Disease Activity Score(DAS28) improvement and safety profiles.Statistical analyses were performed using intention to treat analysis(ITT) set.RESULTS: A total of 70 active RA patients were enrolled. At week 4, the ACR20 was 34.3%(12/35) in TwHF cream group and 11.4%(4/35) in placebo group(P = 0.015). Similarly, a higher ACR50 responder proportion was seen in TwHF cream group with 17.1%(6/35) comparing to it in placebo group with 2.9%(1/35)(P = 0.046). The TwHF cream group also had more improvement than the placebo group on DAS28-ESR(1.1 vs 0.5, P = 0.001), DAS28-CRP(1.4 vs 0.7, P = 0.001), tender joint count(5.5 vs2.6, P = 0.018), swollen joint count(3.5 vs 1.6, P =0.003) and Physician's global assessment(25.8 vs13.0, P = 0.002), as well as C-reactive protein(11.2 vs 2.7, P = 0.048).
