1核因子-κB在大鼠实验性溃疡性结肠炎肠组织的表达及其意义显示文摘目的:观察大鼠实验性溃疡性结肠炎肠组织核因子-κB(NF-κB)及TNF-α、ICAM-1的表达,探讨NF-κB在溃疡性结肠炎发病过程中的作用.方法:采用三硝基苯磺酸(TNBS)制备大鼠溃疡性结肠炎模型,设正常组、生理盐水组及轻、重模型组共4组.采用免疫组织化学染色检测肠组织NF-κB、TNF-α和ICAM-1表达,生化方法检测MPO含量,并分析NF-κB活性与肠道病理损伤指数、MPO含量、TNF-α及ICAM-1表达的关系.结果:溃疡性结肠炎大鼠肠组织中NF-κB活性,TNF-α、ICAM-1表达及MPO含量较正常组及生理盐水组显著增高(NF-κB:52.14±9.81、30.26±10.20,60.73±13.41、45.24±10.86vs13.31±4.76、16.95±6.83,11.61±4.85、14.10±5.76;TNF-α:74.50±11.20、48.11±5.95,84.09±14.52、53.40±8.79vs16.99±5.48、20.04±6.76,10.13±1.79、16.03±6.21;ICAM-1:68.15±7.25、44.34±7.54,77.69±8.09、47.01±8.82vs15.34±4.03、17.50±6.95,10.33±2.38、13.41±4.91;MPO:1.69±0.11,0.71±0.06vs0.39±0.07,0.31±0.08;P<0.01),且在病情严重组增高尤为明显.NF-κB表达水平与TNF-α和ICAM-1阳性细胞密度、MPO含量、肠道病理损伤指数呈显著正相关关系(r分别为0.9304,0.8680,0.6865,0.9292,0.8462;P<0.001或P<0.005).结论:NF-κB在溃疡性结肠炎发病过程中可能起重要作用.李军华 于皆平 何小飞 徐细明 2003世界华人消化杂志2003,11,2:24
2活动期溃疡性结肠炎患者凝血指标变化的meta分析显示文摘目的评价活动期溃疡性结肠炎(UC)患者凝血指标变化。方法检索Pub Med、Medline、EMBASE、Web of science、Cochrane图书馆、中国学术期刊全文数据库(CNKI)、中国生物医学文献数据库(CBM)及万方数据等数据库公开发表的中英文文献,检索时间均为从建库至2015年1月。对符合条件的研究进行提取资料和质量评价后,采用Stata12.0软件进行meta分析。结果共有28项病例-对照研究文献纳入meta分析,结果显示活动期UC患者血小板(PLT)、纤维蛋白原(FIB)、D-二聚体水平较正常组显著升高,平均血小板体积(MPV)、凝血酶原时间(PT)水平较正常健康组显著降低。敏感性分析显示,结果稳定性好,发表偏倚采用Egger及Begg法显示:关于D-二聚体的文献存在一定发表偏倚,其他指标的文献均无明显发表偏倚。结论活动期UC患者存在凝血指标的异常,可能存在高凝状态。查安生 汪悦 查瑞瑶 2015细胞与分子免疫学杂志2015,31,11:20
3Abnormal function of platelets and role of angelica sinensis in patients with ulcerative colitis显示文摘AIM:To explore the abnormal function of platelets and the role of angelica sinensis injection (ASI) in patients with ulcerative colitis (UC).METHODS:In 39 patients with active UC, 25 patients with remissive UC and 30 healthy people, α-granule membrane protein (GMP-140) and thromboxane B2 (TXB2) were detected by means of ELISA, 6-keto-PGF1α was detected by radioimmunoassay, platelet count (PC) and 1min platelet aggregation rate (1min PAR) were detected by blood automatic tester and platelet aggregation tester respectively,and von Willebrand factor related antigen (vWF:Ag) was detected by the means of monoclonal-ELISA. The 64 patients with UC were divided into two therapy groups. After routine treatment and angelica sinensis injection (ASI)+routine treatment respectively for 3 weeks, all these parameters were also detected.RESULTS:The PC, 1min PAR and levels of GMP-140,TXB2, and vWF:Ag in active UC were significanrly higher than those in remissive UC and normal controls (P<0.05-0.01).Meanwhile, 1 min PAR and levels of GMP-140, TXB2,and vWF:Ag in remissive UC were still significantly higher than those in normal controls (P<0.05). Furthermore, 6-keto-PGF1-α level in active and remissive UC was remarkably lower than that in normal control (P<0.05-0.01). These parameters except 6-keto-PGF1α were significantly improved after the treatment in ASI therapy group (P<0.05-0.01),whereas they all were little changed in routine therapy group (P>0.05).CONCLUSION:Platelets can be significantly activated in UC,which might be related with vascular endothelium injury and imbalance between TXB2 and 6-keto-PGF1α in blood.ASI can significantly inhibit platelet activation, relieve vascular endothelial cell injury, and improve microcirculation in UC.Wei-GuoDong Shao-PingLiu Hai-HangZhu He-ShengLuo Jie-PingYu 2004World Journal of Gastroenterology2004,10,4:14

6Relationship between plasma D-dimer levels and clinicopathologic parameters in resectable colorectal cancer patients显示文摘AIM:To assess the clinical significance of the D-dimer levels and the relationship between plasma D-dimer levels and clinicopathologic parameters in operable colorectal cancer patients.METHODS:The plasma levels of D-dimer were measured pre- and postoperatively in 35 patients with colorectal cancer, and 30 healthy subjects served as controls by the method of quantitative enzyme-linked immunosorbent assay (ELISA).RESULTS: The mean preoperative plasma levels of D-dimer in the patients with colorectal cancer (1.06±0.24mg/L) were significantly higher than those of controls (0.33±0.12mg/L,P<0.01). The D-dimer levels were remarkably elevated on the 1st day after operation (1.22±0.55mg/L,P<0.01).On the 3rd day the level of D-dimer began to stepwise descend and on the 14^th day nearly returned to control level.The preoperative levels of D-dimer were significantly correlated with the lymph node metastasis and Dukes stage but had no association with tumor location and the degree of differentiation.A stepwise increase in the mean D-dimer levels was found with increase of the tumor stage.CONCLUSION:Hypercoagulation and higher fibrinolytic activities occur in patients with colorectal cancer. 7溃疡性结肠炎患者结肠黏膜COX-2和TNF-α的表达及其意义显示文摘目的研究溃疡性结肠炎(UC)患者结肠黏膜环氧合酶-2(COX-2)和肿瘤坏死因子-α(TNF-α)的表达以及与UC严重程度的关系。方法采用免疫组织化学染色法对38例活动期UC患者和10例对照组结肠黏膜中COX-2和TNF-α的表达进行检测。结果 UC患者结肠黏膜中COX-2和TNF-α的表达较对照组显著增高(P<0.05),且COX-2和TNF-α的表达与活动期UC患者的病情严重程度及内镜分级有关(P<0.05),COX-2与TNF-α的表达呈正相关(r=0.831,P<0.05)。结论 COX-2和TNF-α参与了UC的发病过程,并且可以反映UC的炎症活动情况。师永盛 霍丽娟 安丽婷 2011山西医科大学学报2011,42,4:8

9丹参酮联合美沙拉秦对溃疡性结肠炎患者凝血功能及组织TF、TFPI表达的影响显示文摘目的探讨丹参酮胶囊联合美沙拉秦肠溶片对溃疡性结肠炎患者凝血功能及溃疡周围黏膜组织中TF、TFPI表达的影响。方法将88例溃疡性结肠炎患者应用随机数字法分为对照组和治疗组,每组44例。对照组予以美沙拉秦肠溶片治疗,治疗组在对照组基础上加用丹参酮胶囊进行治疗,2组均治疗14d为1个疗程,共治疗2个疗程。观察2组治疗前后凝血功能指标,包括血小板计数(PLT)、血小板平均体积(MPV)、活化部分凝血酶原时间(APTT)、纤维蛋白原(FIB)、D-二聚体(D-D)、C-反应蛋白(CRP)水平的变化,观察2组治疗前后溃疡周围黏膜组织中TF、TFPI阳性率的表达情况。结果治疗前,2组患者各项凝血指标及TF、TFPI表达,差异无统计学意义(P>0.05)。治疗后,2组患者PLT、MPV、APTT、FIB、D-D、CRP均较前改善(P<0.05),且治疗组改善程度优于对照组(P<0.05)。治疗后,对照组溃疡周围黏膜组织TF阳性率为36.36%,治疗组阳性率为18.18%,差异具有统计学意义(P<0.05)。治疗后,对照组溃疡周围黏膜组织TFPI阳性率为34.09%,治疗组阳性率为13.64%,差异具有统计学意义(P<0.05)。结论丹参酮联合美沙拉秦能够显著改善溃疡性结肠炎患者的高凝状态,降低微血栓形成的风险,疗效显著,值得临床推广。何灏澜 2018中西医结合研究2018,10,5:6

11超敏C反应蛋白和D-二聚体与溃疡性结肠炎肠外表现的相关性显示文摘[目的]观察超敏反应蛋白(hs-CRP)和D-二聚体(D-DI)在合并肠外表现的溃疡性结肠炎(EIM-UC)和非合并肠外表现的溃疡性结肠炎(NEIM-UC)患者血浆中的变化,探究其相关性及临床意义。[方法]选取入院收治的溃疡性结肠炎患者96例,分为EIM-UC组(50例)和NEIM-UC组(46例),同期纳入30例健康人作为健康对照组,行血浆Hs-CRP和DDI检测。设立Cut off值对照。Cut off值对照吸光度(A)值应在0.3~0.8间;诊断效能用敏感性、特异性描述,并构建ROC曲线。[结果]EIM-UC组、NEIM-UC组的hs-CRP和D-DI水平均高于健康对照组(P<0.05),EIM-UC组的hs-CRP水平高于NEIM-UC组(P<0.05),但2组间的DDI水平比较差异无统计学意义。根据Cut off值推算hs-CRP和D-DI诊断对EIM-UC的敏感度、特异度,其中hs-CRP和Cut off值的比值有统计学差异(P<0.05),D-DI无统计学差异(P>0.05)。[结论]hs-CRP的明显升高与EIM-UC相关,暂未有足够证据证明D-DI与EIM-UC的相关性。滕晓丽 时昭红 2019临床消化病杂志2019,31,6:5

13阿魏酸钠抗大鼠乙酸性结肠炎损伤的作用显示文摘目的:研究SF对大鼠乙酸性结肠炎的作用及其机制方法:利用乙酸灌肠制备大鼠结肠炎模型.实验设正常对照组,模型对照组,SF给药组(200 mg/kg,400 mg/kg, 800 mg/kg),每天灌肠给药1次,用药7 d.实验结束后评价大鼠结肠黏膜损伤指数(CMDI)与粪便隐血实验(OBT)、检测结肠组织MDA,NO,PGE2,TXB2含量;MPO, SOD活性;COX-2,NF-κBp65表达水平及血小板聚集率. 结果:模型组大鼠CMDI,OBT评分显著增加;MDA, NO,PGE2,TXB2含量,MPO活性,COX-2与NF- κBp65表达水平及血小板聚集率显著升高;而SOD活性显著降.SF灌肠改善模型组大鼠CMDI,OBT评分;使结肠组织MDA,NO,PGE2,TXB2含量,MPO活性,血小板聚集率不同程度降低,SOD活性升高,同时下调COX-2与NF-κBp65的过度表达.SF用药呈一定量效关系. 结论:SF通过抗氧化,抑制血小板活化、花生四烯酸代谢及NF-κB表达,缓解大鼠乙酸性结肠炎炎症反应,减轻结肠黏膜损伤.刘少平 董卫国 罗和生 余保平 于皆平 2004世界华人消化杂志2004,12,1:5
14泄浊解毒方对慢性非特异性溃疡性结肠炎大鼠结肠黏膜环氧合酶-2及前列腺素E_2的影响显示文摘目的观察泄浊解毒方对慢性非特异性溃疡性结肠炎(CUC)大鼠结肠黏膜中环氧合酶-2(COX-2)、前列腺素E2(PGE2)表达的影响,探究其可能的作用机制。方法将40只雄性Wistar大鼠适应性饲养1周后,随机分为5组,即空白组、模型组、柳氮磺胺吡啶组、泄浊解毒方高剂量组、泄浊解毒方低剂量组,每组各8只。除空白组外,其余4组大鼠以三硝基苯磺酸(TNBS)联合乙醇造模法复制CUC动物模型。造模成功后第5 d,空白组及模型组大鼠予0.9%氯化钠注射液5 m L灌胃,柳氮磺胺吡啶组予柳氮磺胺吡啶混悬液0.3 g/kg灌胃,泄浊解毒方高、低剂量组分别予20.0、10.0 g/kg泄浊解毒方药液灌胃,均每日1次,连续给药14 d。用药结束后以免疫组化法测定大鼠结肠黏膜COX-2及PGE2的表达。结果模型组大鼠结肠黏膜COX-2及PGE2表达水平均较空白组明显提高(P<0.05);与模型组比较,泄浊解毒方高剂量组、低剂量组及柳氮磺胺吡啶组大鼠结肠黏膜COX-2及PGE2表达水平明显下降(P<0.05),且泄浊解毒方高剂量组低于柳氮磺胺吡啶组(P<0.05)。结论泄浊解毒方治疗CUC大鼠效果良好,抑制COX-2表达、减少PGE2生成可能是其作用机制。刘建平 李玲玲 郎晓猛 任杰 赵源 杨倩 2015河北中医2015,37,12:5
15溃疡性结肠炎血栓前状态的中西医研究现状显示文摘溃疡性结肠炎(ulcerative colitis,UC)是一种病因和发病机制尚未完全清楚、发病率逐渐升高的消化系统常见炎性肠病(inflammatory bowel disease,IBD),近20年来国内以医院为基础的调查数据估计,发现UC的患病率约为11.6/10万。随着病情的加重与迁延,伴有癌变的威胁及发生严重并发症的可能性,因而,对于UC病因及治疗的研究受到全球广泛重视。UC存在血栓前状态(prethrombot—ic status,PTS),本文结合中医学整体观念及对其“局部微癜瘕”的认识,将近20年对于UC血液高凝状态的国内外病理学及临床观察研究概况与进展、活血通络法治疗UC的文献进行整理归纳,探讨活血通络法对于UC血液高凝状态的治疗作用。刘蕊洁 叶永安 2013中国中西医结合消化杂志2013,21,5:4
16Elevated plasma von Willebrand factor levels in patients with active ulcerative colitis reflect endothelial perturbation due to systemic inflammation显示文摘AIM: To evaluate the plasma von Willebrand factor (vWF) levels in patients with ulcerative colitis (UC) and to investigate their relationship with disease activity,systemic inflammation and coagulation activation.METHODS: In 46 patients with ulcerative colitis (active in 34 patients), clinical data were gathered and plasma vWF levels, markers of inflammation (ESR, CRP, and fibrinogen) and thrombin generation (TAT, F1+2, and D-dimers) were measured at baseline and after 12 wk of treatment. Plasma vWF levels were also determined in 52 healthy controls (HC). The relationship of plasma vWF levels with disease activity, disease extent, response to therapy, acute-phase reactants (APRs) and coagulation markers (COAGs) was assessed.RESULTS: The mean plasma vWF concentrations were significantly higher in active UC patients (143.38±63.73%) than in HC (100.75±29.65%, P = 0.001)and inactive UC patients (98.92±43.6%, P = 0.031).ESR, CRP and fibrinogen mean levels were significantly higher in active UC patients than in inactive UC patients,whereas there were no significant differences in plasma levels of D-dimers, F1+2, and TAT. UC patients with raised APRs had significantly higher mean plasma vWF levels than those with normal APRs (144.3% vs 96.2%,P = 0.019), regardless of disease activity. Although the mean plasma vWF levels were higher in UC patients with raised COAGs than in those with normal COAGs,irrespective of disease activity, the difference was not significant (141.3% vs 118.2%, P = 0.216). No correlation was noted between plasma vWF levels and disease extent. After 12 wk of treatment, significant decreases of fibrinogen, ESR, F1+2, D-dimers and vWF levels were noted only in UC patients with clinical and endoscopic improvement.CONCLUSION: Our data indicate that increased plasma vWF levels correlate with active ulcerative colitis and increased acute-phase proteins. Elevated plasma vWF levels in ulcerative colitis possibly reflect an acutephase response of the perturbed endothelium due to inflammation. 17大蒜素对大鼠溃疡性结肠炎淋巴细胞凋亡及其调控蛋白的影响显示文摘目的:通过观察大鼠溃疡性结肠炎淋巴细胞凋亡及其调控蛋白Bcl-2和Bax的表达及大蒜素对其的影响,探讨大蒜素(Allitridi,Alt)对溃疡性结肠炎肠黏膜的保护作用及其机制.方法:用三硝基苯磺酸(TNBS)建立大鼠溃疡性结肠炎模型.将动物随机分为空白对照组(Normalgroup)、三硝基苯磺酸组(TNBSgroup)、三硝基苯磺酸+生理盐水组(TNBS+NSgroup)、三硝基苯磺酸+大蒜素组(TNBS+Altgroup)四组.利用DNA缺口末端标记技术(TUNEL法)和Bcl-2、Bax蛋白免疫组化染色,分别检测溃疡性结肠炎大鼠肠组织中的淋巴细胞凋亡和淋巴细胞Bcl-2和Bax的表达,生化检测一氧化氮(NO)含量,并观察肠道大体形态和组织学改变.结果:和三硝基苯磺酸组相比,三硝基苯磺酸+大蒜素组中淋巴细胞凋亡增加(2.1±1.0vs5.9±2.0,P<0.01),Bcl-2表达阳性淋巴细胞减少及一氧化氮(NO)含量下降(10.0±2.5vs31.0±6.0,197±11vs523±40,P<0.01).损伤指数明显下降(1.6±0.5vs5.8±0.7,2.1±0.6vs6.1±0.6,P<0.01).结论:大蒜素可以通过促进淋巴细胞凋亡和清除NO自由基而对TNBS诱导的溃疡性结肠炎肠黏膜有保护作用.徐细明 于皆平 何小飞 李军华 郑敏 於亮亮 2003世界华人消化杂志2003,11,5:4
18炎症性肠病与心血管病关联研究回顾与现状显示文摘炎症性肠病(inflammatory bowel disease,IBD)是一类肠道的慢性非特异性炎症性疾病,包括溃疡性结肠炎(ulcerative colitis,UC)和克罗恩病(Crohn's dis﹣ease,CD).目前 IBD的发病机制尚不清楚,普遍认为免疫、遗传、感染以及环境因素在 IBD 的发病过程中起着重要作用.炎症反应在动脉粥样硬化、血栓形成及IBD的发病机制中起着必不可少的作用.考虑到IBD是一种全身炎症状态,伴随有促炎因子水平的升高,理所当然地预料IBD患者进展性动脉粥样硬化、血栓性损伤的并发症增加[1].在一项IBD患者心血管病的回顾研究[2]中,发现IBD和静脉血栓栓塞之间的正性关联;但与冠状血管疾病相关联的证据相对稀少.本文回顾近年来国内外发表的关于IBD共患心血管病的相关文献,介绍了IBD共患心血管病的相关研究现状.陈翠连 王帆 贾钧 李国珍 2019医学新知2019,29,4:1
19溃疡性结肠炎与肺栓塞显示文摘目前肺栓塞(pulmonary embolism,PE)仍被误认为是一少见病,实际上是常见的,其发病率在心血管中居于第3位,仅次于冠心病与高血压,病死率高达20%-30%。PE虽然凶险,然而是可预防可治疗的,提高对该病的认识,采取早期预防对预后具有重要意义。众所周知,PE危险因素包括深静脉血栓形成、高龄、卧床、手术、妊娠,肿瘤等,国内外曾报道多例溃疡性结肠炎(ulcerative colitis,UC)患者发生PE病例,本文主要对两者关系及作用机制作一综述,望临床工作者提高对UC的认识。张弘宇 2016中国医学创新2016,13,7:0
