1Intraoperative ultrasound assistance in resection of intracranial meningiomas显示文摘Objective: Intracranial meningiomas, especially those located at anterior and middle skull base, are difficult to be completely resected due to their complicated anatomy structures and adjacent vessels. It's essential to locate the tumor and its vessels precisely during operation to reduce the risk of neurological deficits. The purpose of this study was to evaluate intraoperative ultrasonography in displaying intracranial meningioma and its surrounding arteries, and evaluate its potential to improve surgical precision and minimize surgical trauma. Methods: Between December 2011 and January 2013, 20 patients with anterior and middle skull base meningioma underwent surgery with the assistance of intraoperative ultrasonography in the Neurosurgery Department of Shanghai Huashan Hospital. There were 7 male and 13 female patients, aged from 31 to 66 years old. Their sonographic features were analyzed and the advantages of intraoperative ultrasonography were discussed. Results: The border of the meningioma and its adjacent vessels could be exhibited on intraoperative ultrasonography. The sonographic visualization allowed the neurosurgeon to choose an appropriate approach before the operation. In addition, intraoperative ultrasonography could inform neurosurgeons about the location of the tumor, its relation to the surrounding arteries during the operation, thus these essential arteries could be protected carefully. Conclusions: Intraoperative ultrasonography is a useful intraoperative technique. 2硬脑膜占位病变的MRI诊断显示文摘硬脑膜占位病变多种多样,影像学表现也呈多元化,熟悉其MRI表现对帮助诊断有重要作用。硬脑膜占位病变中以脑膜瘤最为常见,不同组织学类型脑膜瘤虽有共性影像学表现,但组织学类型不同、分级不同也会影响其影像学表现,熟悉各自的影像学表现和差异,可在一定程度上推测脑膜瘤的类型和分级。硬脑膜间质肿瘤和其他相关占位病变比较少见,其影像学表现与脑膜瘤有一定差异,在进行颅内脑外肿瘤鉴别诊断时,应考虑到此类占位病变的可能性。李海婷 史大鹏 2016放射学实践2016,31,1:3
3侧脑室三角区脑膜瘤显微外科手术疗效分析显示文摘目的分析侧脑室三角区脑膜瘤患者采取显微外科手术(神经导航辅助)治疗的效果。方法选取2014-01—2017-12下同到大连市中心医院治疗的50例侧脑室三角区脑膜瘤患者,将患者随机分为2组。观察组25例,在神经导航辅助下进行肿瘤切除手术,对照组25例,采取常规肿瘤切除手术。结果治疗后观察组视野缺损者明显少于对照组(P<0.05);观察组术后暂时出现失语症2例,对照组8例,6个月后随访已恢复正常,差异无统计学意义(P>0.05);术后2周、3个月,观察组KPS评分明显优于对照组(P<0.05)。结论神经导航辅助下手术能够有效保护视野,降低术后功能障碍发生率。韩鹏 于军 李旭琴 刘乐 胡英杰 罗可 2019中国实用神经疾病杂志2019,22,10:0
4骨化型脊膜瘤的诊断与治疗显示文摘目的脊膜瘤是常见的原发性椎管内良性肿瘤,约占椎管内肿瘤的25%。骨化型脊膜瘤(OSM)是分化型脊膜瘤的罕见亚型。OSM的特点是在肿瘤内部存在可变数目、由间质分化的骨针状体。一般病例依据临床表现、MRI表现可初步诊断,根据病理可确诊。OSM由于其独特的分化形式,使邻近蛛网膜发生骨化,导致此天然屏障消失,增加手术切除难度。本文主要通过对OSM进行总结,旨在为临床上对该病的诊治提供帮助。周华康 曹悦 金凯 赵刚 卞冰阳 2018中国微侵袭神经外科杂志2018,23,12:0

6术后复发的软骨化生型脑膜瘤1例显示文摘患者女性,32岁,汉族。3年前因突发抽搐2天入院治疗行左侧矢状窦旁脑膜瘤切除,术后病理检查为化生型脑膜瘤,患者术后服用预防癫痫发作药物并定期复查,近2年来患者无不适;3天前患者无明显诱因出现右手不自主抖动。头部CT示:顶部大脑镰可见不规则软组织密度影,边界模糊,密度不均,内见不规则钙化,病灶周边见水肿带影,中线右移。术中发现:第1次手术见肿瘤破坏硬脑膜并侵蚀颅骨,肿瘤呈灰白色,膨胀性生长,质硬,包膜完整,血供丰富。第2次手术硬脑膜表面即见肿瘤,呈灰白色,质硬,血供丰富,有包膜,包膜与周围脑组织明显粘连,骨瓣被肿瘤侵蚀。阴丽娜 李道明 张静 2019临床与实验病理学杂志2019,35,9:0
