| 1 | Appropriate treatment of acute sigmoid volvulus in the emergency setting显示文摘AIM:To investigate an appropriate strategy for the treatment of patients with acute sigmoid volvulus in the emergency setting.METHODS:A retrospective review of 28 patients with acute sigmoid volvulus treated in the Department of Colorectal Surgery,Changhai Hospital,Shanghai from January 2001 to July 2012 was performed.Following the diagnosis of acute sigmoid volvulus,an initial colonoscopic approach was adopted if there was no evidence of diffuse peritonitis.RESULTS:Of the 28 patients with acute sigmoid volvulus,19(67.9%)were male and 9(32.1%)were female.Their mean age was 63.1 ± 22.9 years(range,21-93 years).Six(21.4%)patients had a history of abdominal surgery,and 17(60.7%)patients had a history of constipation.Abdominal radiography or computed tomography was performed in all patients.Colonoscopic detorsion was performed in all 28 patients with a success rate of 92.8%(26/28).Emergency surgery was required in the other two patients.Of the 26 successfully treated patients,seven(26.9%)had recurrent volvulus.CONCLUSION:Colonoscopy is the primary emergency treatment of choice in uncomplicated acute sigmoid volvulus.Emergency surgery is only for patients in whom nonoperative treatment is unsuccessful,or in those with peritonitis. | Zheng Lou En-Da Yu Wei Zhang Rong-Gui Meng Li-Qiang Hao Chuan-Gang Fu | 2013 | World Journal of Gastroenterology2013，19，30： | 2 |
| 2 | 回肠双造口在结肠扭转手术中的应用显示文摘目的介绍回肠双造口在结肠扭转手术中的应用,并比较此法与其他方法的不同。方法将我院胃肠外科2005年-2012年收治的结肠扭转患者39例分为2组,其中一组行一期吻合手术,另一组行一期吻合后加做回肠保护性双造口手术,比较两组在手术时间、切口感染,术后腹腔出血、术后吻合口漏、总住院时间发生率和死亡率之间的不同。结果两组在术后腹腔出血、死亡率和术后切口感染方面比较差异无统计学意义(P>0.05),而加做回肠造口后吻合口漏明显下降,但手术时间(127.76±11.86)min和总住院时间(20.00±1.97)d明显延长。结论在结肠扭转时切除扩张扭转的部分结肠并一期吻合后加做回肠造口虽然耗费时间,但手术更安全。 | 王晓龙 马克强 | 2013 | 湖南中医药大学学报2013，33，10： | 0 |
