1丹参多酚酸盐预防大鼠术后肠粘连的机制显示文摘目的:研究丹参多酚酸盐预防大鼠术后肠粘连的作用并探讨其机制。方法:雄性SD大鼠50只,随机分为5组:正常对照组、模型组和丹参多酚酸盐低、中、高剂量组。除正常对照组外,其余大鼠均制备肠粘连模型。正常对照组和模型组每天腹腔注射葡萄糖;丹参多酚酸盐低、中、高剂量组每天腹腔注射丹参多酚酸盐,连续注射8d。各组于术后第8天取血,肉眼观察肠粘连的程度分级;取盲肠与腹壁粘连组织做病理观察;酶联免疫吸附法测定细胞因子白细胞介素-1β(interleukin-1β,IL-1β)、白细胞介素-4(interleukin-4,IL-4)及肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)水平。结果:丹参多酚酸盐能显著减轻大鼠的肠粘连程度,明显降低肠粘连大鼠IL-1β和TNF-α的表达水平,抑制纤维结缔组织的增生,但对IL-4的表达水平几乎没有影响。结论:丹参多酚酸盐可减轻大鼠肠粘连程度,降低IL-1β和TNF-α的表达水平,抑制纤维结缔组织的增生,这可能是其预防肠粘连的机制。隋新兵 张勤 裘华森 周济春 顾锡冬 陆周翔 李超 晋丽君 王刚 2007中西医结合学报2007,5,5:22
2大黄酸精氨酸预防大鼠实验性肠粘连抗炎机制的研究显示文摘目的研究大黄酸精氨酸预防大鼠实验性肠粘连的作用并探讨其抗炎机制。方法取雄性SD大鼠60只,随机分为6组:正常对照组、模型组、地塞米松组和大黄酸精氨酸低、中、高剂量组。除正常对照组外,其余各组大鼠均制备肠粘连模型。正常对照组和模型组腹腔注射生理盐水,地塞米松组腹腔注射地塞米松磷酸钠(10mg·kg-1),大黄酸精氨酸低、中、高剂量组腹腔注射大黄酸精氨酸(7.5,15,30mg·kg-1),连续给药1周。各组于术后第8天取血,ELISA法测定血清IL-1β、IL-4及TNF-α含量,肉眼观察肠粘连的程度并分级;同时取盲肠与腹壁切口组织测定羟脯氨酸(hydroxyproline,Hyp)含量,并做病理观察。结果大黄酸精氨酸能显著减轻肠粘连大鼠肠粘连的程度,明显降低肠粘连大鼠IL-1β和TNF-α的表达水平,抑制纤维结缔组织的增生,但对IL-4的表达水平几乎没有影响。结论大黄酸精氨酸可通过抑制炎症因子的过度表达,减轻炎症反应而有效预防术后肠粘连的形成。尹家乐 张爱香 王毓炜 张洪泉 2010中药新药与临床药理2010,21,1:22
3Treatment of pediatric refractory Crohn’s disease with thalidomide显示文摘AIM: To assess the efficacy and tolerability of thalidomide in pediatric Crohn's disease (CD). METHODS: Six patients with refractory CD received thalidomide at an initial dose of 2 mg/kg per day for one month, then increased to 3 mg/kg per day or decreased to 1 mg/kg per day, and again further reduced to 0.5 mg/kg per day, according to the individual patient's response to the drug. RESULTS: Remission was achieved within three months. Dramatic clinical improvement was demonstrated after thalidomide treatment. Endoscopic and pathological improvements were also observed after thalidomide treatment, which was well tolerated by all patients. 4Therapeutic and Immunoregulatory Effect of GATA-Binding Protein-3/T-Box Expressed in T-Cells Ratio of Astragalus Polysaccharides on 2,4,6-Trinitrobenzene Sulfonic Acid-Induced Colitis in Rats显示文摘Objective: To analyze the immunological characteristics of 2,4,6-trinitrobenzene sulfonic acid(TNBS)-induced colitis model and examine the therapeutic effects and mechanisms of Astragalus polysaccharides(APS) treatment. Methods: Thirty-two male specific pathogen free Spragne-Dawley rats were randomly equally assigned to four groups: control, TNBS, APS and prednisone groups. Experimental colitis was induced by enema administration of TNBS. Then rats were treated with APS(0.5 g·kg^(-1)·day^(-1), once daily) or prednisone(1.0 mg·kg^(-1)·day^(-1), once daily) by gavage for 14 days. Macroscopic lesion and histological damage were determined, and activity of myeloperoxidase(MPO) was measured in the colonic tissues. Expressions of T-box expressed in T-cells(T-bet) and GATA-binding protein-3(GATA-3) were determined by immunohistochemistry analysis and western blot. Results: Both macroscopic lesion and histological colonic damage induced by TNBS were reduced by APS and prednisone treatment. These were accompanied by significant attenuation of MPO activity(P=0.03). TNBS intervention enhanced the expression of both GATA-3 and T-bet, but the expression of T-bet was significantly enhanced than that of GATA-3, resulting in significant reduction of GATA-3/T-bet ratio(P=0.025). APS administration enhanced the expression of T-bet(P=0.04) and GATA-3(P=0.019) in comparison to TNBS group, and resulting in an up-regulated GATA-3/T-bet ratio. Prednisone treatment inhibited both expressions; however it also resulted in up-regulation of the GATA-3/T-bet ratio. Conclusions: These results demonstrated that APS exerted a beneficial immune regulatory effect on experimental colitis. 5脂多糖致炎时间对小鼠血液免疫与肠道组织形态的影响显示文摘本试验旨在通过给小鼠腹腔注射脂多糖(LPS),检测随着时间的推移小鼠血液细胞因子水平和肠道组织形态的变化。25只雄性ICR小鼠经腹腔注射LPS(5 mg/kg BW),在不同致炎时间[LPS注射前(0 h)以及注射后3、6、12、24、48和72 h]采血制备血清,通过酶联免疫吸附试验(ELISA)方法检测小鼠血清肠碱性磷酸酶(IAP)活性及炎性细胞因子肿瘤坏死因子-α(TNF-α)、白细胞介素(IL)-1β、IL-4、IL-6和IL-8的水平;同时采集小鼠十二指肠、空肠、回肠和盲肠样品,苏木精-伊红(HE)染色制作组织切片,观察各肠段病理学变化,并测量各肠段绒毛高度(VH)和隐窝深度(CD),计算VH/CD(V/C)。结果显示:血清IAP活性,促炎细胞因子TNF-α、IL-6和IL-8以及抗炎细胞因子IL-4的水平在LPS注射后6 h时达到高峰,随后下降;而血清促炎细胞因子IL-1β的水平在LPS注射后6 h时略有升高,在注射后48 h时达高峰。与注射前(0 h)相比,LPS致炎小鼠在LPS注射后6 h肠道病理变化比较明显,小鼠肠黏膜损伤明显,上皮脱落、绒毛破裂、黏膜萎缩、水肿、且绒毛较短;随着时间的推移,肠黏膜在LPS注射后48 h时已开始缓慢恢复。与注射前(0 h)相比,LPS致炎小鼠十二指肠和空肠的V/C均在注射后6 h时出现显著或极显著下降(P<0.05或P<0.01),但在注射后72 h时已无显著差异(P>0.05)。由此得出,LPS(5 mg/kg BW)腹腔注射后6 h时小鼠血清促炎性细胞因子水平达到高峰,肠黏膜损伤明显,之后逐渐恢复正常。贾军峰 王梦竹 崔一喆 王秋菊 武瑞 2018动物营养学报2018,30,9:11
6Rheumatic manifestations of inflammatory bowel disease显示文摘This article reviews the literature concerning rheu-matic manifestations of inflammatory bowel disease (IBD),including common immune-mediated pathways,frequency,clinical course and therapy. Musculoskel-etal complications are frequent and well-recognized manifestations in IBD,and affect up to 33% of pa-tients with IBD. The strong link between the bowel and the osteo-articular system is suggested by many clinical and experimental observations,notably in HLA-B27 transgenic rats. The autoimmune pathogenic mechanisms shared by IBD and spondyloarthropathies include genetic susceptibility to abnormal antigen pre-sentation,aberrant recognition of self,the presence of autoantibodies against specific antigens shared by the colon and other extra-colonic tissues,and increased intestinal permeability. The response against microor-ganisms may have an important role through molecular mimicry and other mechanisms. Rheumatic mani-festations of IBD have been divided into peripheral arthritis,and axial involvement,including sacroiliitis,with or without spondylitis,similar to idiopathic anky-losing spondylitis. Other periarticular features can oc-cur,including enthesopathy,tendonitis,clubbing,peri-ostitis,and granulomatous lesions of joints and bones.Osteoporosis and osteomalacia secondary to IBD and iatrogenic complications can also occur. The manage-ment of the rheumatic manifestations of IBD consists of physical therapy in combination with local injection of corticosteroids and nonsteroidal anti-inflammatory drugs; caution is in order however,because of their possible harmful effects on intestinal integrity,perme-ability,and even on gut inflammation. Sulfasalazine,methotrexate,azathioprine,cyclosporine and lefluno-mide should be used for selected indications. 9Aberrant activation of nuclear factor of activated T cell 2 in lamina propria mononuclear cells in ulcerative colitis显示文摘AIM:To investigate the role of nuclear factor of activated T cell 2(NFAT2),the major NFAT protein in peripheral T cells,in sustained T cell activation and intractable inflammation in human ulcerative colitis(UC). METHODS:We used two-dimensional gel-electrophoresis, immunohistochemistry,double immunohistochemical staining,and confocal microscopy to inspect the expression of NFAT2 in 107,15,48 and 5 cases of UC, Crohn's disease(CD),non-specific colitis,and 5 healthy individuals,respectively. RESULTS:Up-regulation with profound nucleo- translocation/activation of NFAT2 of lamina propria mononuclear cells(LPMC)of colonic mucosa was found specifically in the affected colonic mucosa from patients with UC,as compared to CD or NC(P<0.001,Kruskal- Wallis test).Nucleo-translocation/activation of NFAT2 primarily occurred in CD8+T,but was less prominent in CD4+T cells or CD20+B cells.It was strongly associated with the disease activity,including endoscopic stage (τ=0.2145,P=0.0281)and histologic grade(τ=0.4167, P<0.001). 10溃疡性结肠炎术后早期并发症的预防及处理显示文摘目的总结分析溃疡性结肠炎外科治疗术后并发症的发生原因及处理经验。方法回顾性分析1995年1月至2005年12月手术治疗26例溃疡性结肠炎的临床资料。因结肠出血、肠穿孔、中毒性巨结肠和吻合口瘘等并发症入院病人19例,内科治疗无效而无并发症入院病人7例。其中急诊手术12例,择期手术14例;全结肠直肠切除术11例,结肠部分切除和结肠单纯造口术15例。结果11例(18例次)病人术后出现并发症,并发症发生率为42.3%,急诊手术后并发症发生率较高,主要为切口并发症和吻合口瘘。4例吻合口瘘病人2例保守治疗痊愈,2例再次手术,共有2例病人死亡。结论全结肠直肠切除术是治疗溃疡性结肠炎的有效手段,合理掌握手术时机和采取合理的手术方式能够降低术后并发症的发生率。对于常见的吻合口瘘并发症,通过持续骶前灌洗、通畅引流以及有效的贮袋引流减压等保守治疗,可使吻合口瘘闭合。李孟彬 王为忠 张洪伟 陈冬利 刘小南 李纪鹏 季刚 2007中国实用外科杂志2007,27,3:4

12肠三叶因子对炎症性肠病小鼠细胞凋亡的调节机制显示文摘目的探讨肠三叶因子(ITF)对小鼠炎症性肠病(IBD)肠上皮细胞及脾淋巴细胞凋亡的调节作用,分析ITF对IBD的保护作用机制。方法 48只小鼠随机分为3组(n=16):三硝基苯磺酸(TNBS)组,用TNBS建立IBD模型后每只小鼠给予9 g.L-1盐水0.1 mL腹腔注射;基因重组肠三叶因子(rITF)组,建立IBD模型后每只小鼠给以rITF 0.1 mL腹腔注射;乙醇对照组,未造模型小鼠每只给予9 g.L-1盐水0.1 mL腹腔注射。各组均连续5 d注射,第6天评估疾病活动度指数(DAI),处死动物后取其结肠组织及脾脏组织,测定其结肠组织髓过氧化物酶(MPO)活性、应用原位末端标记试剂盒及流式细胞技术检测其肠上皮细胞、脾淋巴细胞凋亡情况及脾淋巴细胞Fas表达情况。结果与TNBS组小鼠相比,rITF组小鼠在死亡只数、大便性状、血便情况、体质量下降情况等临床症状及肠道局部炎症均明显减轻,MPO值显著降低(P<0.05),肠上皮细胞凋亡指数显著下降(P<0.01);乙醇对照组IBD小鼠脾淋巴细胞的凋亡明显不足,且Fas表达下调,但TNBS组与rITF组间凋亡率无显著性差异。结论 ITF对IBD小鼠具有保护作用,并且可能通过调节IBD小鼠肠上皮细胞凋亡水平保护肠上皮细胞,但ITF对脾淋巴细胞的凋亡无调节作用。滕旭 许玲芬 孙梅 吴捷 刘璐 2011实用儿科临床杂志2011,26,13:4
13Amaranthus roxburghianus root extract in combination with piperine as a potential treatment of ulcerative colitis in mice显示文摘OBJECTIVE:The present work was undertaken to determine the effects of Amaranthus roxburghianus Nevski.(Amaranthaceae) root alone and in combination with piperine in treating ulcerative colitis(UC) in mice. METHODS:Swiss albino mice were divided into seven groups(n = 6).Standard group received prednisolone(5 mg/kg,intraperitoneally).Treatment groups received hydroalcoholic extract of roots of A.roxburghianus(50 and 100 mg/kg,per oral) and a combination of hydroalcoholic extract of roots of A.roxburghianus(50 and 100 mg/kg,per oral) and piperine(5 mg/kg,per oral).Ulcer index,colitis severity,myeloperoxidase(MPO),malondialdehyde and glutathione were estimated from blood and tissue.Column chromatography of the extract was done and purified fractions were analyzed by gas chromatography-mass spectroscopy(GC-MS). RESULTS:Treatment with the combination of hydroalcoholic extract of A.roxburghianus and piperine showed minimal ulceration,hemorrhage,necrosis and leucocyte infiltration by histopathological observation.Acetic acid increased MPO levels in blood and colon tissue to 355 U/mL and 385 U/mg,respectively.The combination of hydroalcoholic extract of A.roxburghianus(100 mg/kg) and piperine(5 mg/kg) significantly decreased MPO in blood and tissue to 182 U/mL and 193 U/mg,respectively(P < 0.05).Similarly,this combination significantly reduced malondialdehyde levels and increased glutathione levels in blood and tissue. Various phytoconstituents were detected by GC-MS. 15A20 inhibits lipopolysaccharide-induced inflammation in enterocytes显示文摘AIM To examine the role of A20 in the regulation of intestinal epithelial cells(IECs) inflammation.METHODS Using gene transfection,both stable overexpression and knockdown A20-expressed HT-29 cell lines were established.Accordingly,the cells were divided into the following groups:The control group,the A20 overexpression group,the A20 knockdown group and the respective controls.A20 was stimulated with lipo-polysaccharide(LPS) in a dose- and time-dependent manner and was detected using western blotting and real-time polymerase chain reaction(PCR) analyses.Immunofluorescence and western blotting analyses were performed to investigate the role of A20 in the regulation of nuclear factor(NF)-κB activation and translocation into the nucleus.ELISA and real-time PCR were performed to examine A20 in regulating the release of the following inflammatory cytokines:Tumor necrosis factor(TNF)-a,interleukin(IL)-1b,IL-6 and IL-8.RESULTS The expression of A20 in IECs was inducible.When intestinal epithelial cells were subjected to the stimulation of LPS,the expression of A20 was increased,and the expression of A20 was induced in a dose-and timedependent manner.The expression of A20 was very low in HT-29 cells without LPS stimulation but rapidly increased and was maintained at a high level 2-4 h after stimulationwith LPS.These levels gradually declined with a change in time-course,and the expression of A20 increased with increasing LPS stimulation.Western blotting and immunofluorescence revealed that overexpression of A20 can inhibit NF-κB activation and its translocation to the nucleus.The overexpression of A20 can reduce the levels of proinflammatory cytokines involved in the pathophysiology of inflammatory bowel disease.There was no significant difference in the expression of IL-8 mR NA in the control group,A20 overexpression group or A20 knockdown group without LPS stimulation(P > 0.05);however,while after 2 h,4 h and 8 h stimulation with LPS,the expression of IL-8 in the A20 overexpression group was lower than the control group and the A20 knockdown group(P < 0.05 or P < 0.01).The expression of TNF-a was different at different time points after 8 h of LPS stimulation(F = 31.33,DF = 5,P < 0.001),and the expression of TNF-a increased as the LPS stimulation time increased.Upon LPS stimulation,lower levels of TNF-a were detected in the A20 overexpression cell lines(P < 0.05).There were no significant differences in the induction of IL-6 and IL-1b among the control group,A20 overexpression group and A20 knockdown group(P > 0.05).CONCLUSION A20 plays an important role in limiting inflammation by inhibiting LPS-induced NF-κB responses in the gut luminal.A20 may be a potential therapeutic tool for inflammatory diseases.Cui-Fang Zheng Jie-Ru Shi Ying Huang Sheng-Nan Wang 2016World Journal of Gastrointestinal Pharmacology and Therapeutics2016,7,4:1

17FT3、FT4、FT3/FT4、TSH与克罗恩病维得利珠单抗治疗后临床活动度变化关系的研究显示文摘目的:探究血清游离三碘甲状腺原氨酸(FT3)、游离四碘甲状腺原氨酸(FT4)、FT3/FT4、促甲状腺激素(TSH)对克罗恩病(CD)患者维得利珠单抗(VDZ)治疗后临床应答的预测价值。方法:纳入2020年5月至2022年5月我院接受VDZ治疗且疗程超过3个月的CD患者54例,对照组89例。收集对照组及CD患者使用VDZ治疗前、治疗3个月后FT3、FT4、FT3/FT4、TSH等甲状腺功能指标数据,用CD活动指数(CDAI)评估CD疾病活动度。比较CD患者与对照组甲状腺功能指标差异。根据VDZ治疗前后CDAI差值(ΔCDAI)是否≥100分分为应答组及无应答组,比较两组患者VDZ治疗前后甲状腺功能指标变化及其差值(ΔFT3、ΔFT4、ΔFT3/FT4、ΔTSH)与ΔCDAI相关性;用受试者工作特征(ROC)曲线分析上述差值对VDZ临床应答的预测价值。结果:CD患者治疗前FT3、FT3/FT4低于对照组(P<0.05);VDZ治疗3个月后FT3、FT3/FT4较前提高,FT4降低(P<0.05)。应答组FT3、FT4、FT3/FT4、CDAI在VDZ治疗前后自身比较,差异均有统计学意义(P<0.05)。VDZ治疗前后ΔFT3、ΔFT3/FT4与ΔCDAI间呈负相关(P<0.05),ΔFT4、ΔTSH与ΔCDAI无直线相关关系(P>0.05)。ΔFT3、ΔFT3/FT4预测VDZ治疗临床应答潜能的ROC曲线下面积(AUC)分别为0.837、0.827,灵敏度为82.9%、80.5%,特异度均为81.8%。结论:血清游离甲状腺功能指标变化尤其是FT3、FT3/FT4变化与CD患者VDZ治疗后临床活动指数变化呈负相关,并对其临床应答有一定预测价值。王艳 陈延方 陈璐 黄小莉 刘诗琦 陈洪 2023东南大学学报(医学版)2023,42,6:0
18腹腔镜、开腹胃癌根治术对腹腔粘连的影响显示文摘目的比较腹腔镜与开腹胃癌根治术术后对腹腔粘连的影响。方法将31例准备行胃癌根治术的患者随机分为腹腔镜组(16例)和开腹组(15例),对比分析两组患者血清中细胞因子IL-6,IL-1及TNF-α在术前1d、术后第1天、术后第3天、术后第7天的水平变化;同时对比分析这3种细胞因子在腹腔引流液中术后第1天、术后第3天、术后第7天的水平变化。结果腹腔镜组患者平均手术耗时为(200.31±42.96)min/例,明显长于开腹组患者(150.57±41.88)min/例。在这一前提下,术前1d,术后第1天、第3天、第7天等统一时间点测得的血清以及腹腔引流液中IL-6,IL-1及TNF-α的含量明显低于开腹手术组患者(P<0.05)。讨论传统的开腹胃癌根治术会导致较高的腹腔内粘连发生率。而腹腔镜胃癌根治术与前者相比,在术后减轻腹腔内粘连方面具有明显优势,在患者的术后预后方面充分体现出'微创'价值。谢盼盼 菅凤国 孙宏杰 黄东力 贾鲁卿 2016潍坊医学院学报2016,0,5:0
