| 1 | Extragastric manifestations of Helicobacter pylori infection: Possible role of bacterium in liver and pancreas diseases显示文摘Helicobacter pylori(H. pylori) is an ancient microorganism that has co-evolved with humans for over 60000 years. This bacterium typically colonizes the human stomach and it is currently recognized as the most common infectious pathogen of the gastroduodenal tract. Although its chronic infection is associated with gastritis, peptic ulcer, dysplasia, neoplasia, MALT lymphoma and gastric adenocarcinoma, it has been suggested the possible association of H. pylori infection with several extragastric effects including hepatobiliary and pancreatic diseases. Since a microorganism resembling H. pylori was detected in samples from patients with hepatobiliary disorders, several reports have been discussed the possible role of bacteria in hepatic diseases as hepatocellular carcinoma, cirrhosis and hepatic encephalopathy, nonalcoholic fatty liver disease and fibrosis. Additionally, studies have reported the possible association between H. pylori infection and pancreatic diseases, especially because it has been suggested that this infection could change the pancreatic physiology. Some of them have related a possible association between the microorganism and pancreatic cancer. H. pylori infection has also been suggested to play a role in the acute and chronic pancreatitis pathogenesis, autoimmune pancreatitis, diabetes mellitus and metabolic syndrome. | 3 | 慢性丙型肝炎患者幽门螺杆菌感染及其相关因素分析显示文摘目的:探讨幽门螺杆菌(H pylori)在慢性丙型肝炎中的作用.方法:采用H pylori流行病学诊断标准检测血清anti-H pylori-IgG,来判定是否存在H pylori感染,实时荧光定量聚合酶链反应(FQ-PCR)方法检测血清中HCV RNA的含量,聚合酶链反应-微板核酸杂交-酶联免疫测定法检测血清中HCV基因型.结果:H pylori感染率在慢性丙型肝炎组、丙型肝炎肝硬化组和合并肝癌组明显高于健康对照组(55.5%、76.5%、78.6%vs 43.4%,P<0.05),且随着病变程度的加重,H pylori感染率增加.不同HCV载量的慢性丙型肝炎患者H pylori感染率均增加.基因1a型、1b型、2a型和2b型患者H pylori感染率分别为59.5%、66.7%、65.0%和59.2%,各基因型之间比较无显著性差异.结论:慢性丙型肝炎患者幽门螺杆茵感染率明显增加,H pylori在慢性丙型肝炎向肝硬化和肝癌的发展过程中可能与HCV发挥协同作用. | 季尚玮 张永贵 王江滨 | 2008 | 世界华人消化杂志2008,16,5: | 6 |

| 7 | 原发性肝癌组织中螺杆菌属16SrRNA基因的检测及意义显示文摘目的探讨原发性肝癌(HCC)组织中螺杆菌感染情况。方法选取经病理诊断的28例HCC患者的肝癌组织为实验组,22例非肝癌肝病患者肝组织和25例胃癌患者胃癌组织作对照组。采用聚合酶链反应(PCR)扩增螺杆菌属16S rRNA基因、幽门螺杆菌相关功能基因空泡毒素基因(vacA)和细胞毒素相关基因(cagA),16S rRNA的扩增产物经Southern杂交确认,并将PCR产物进行测序及同源性比较。结果28例HCC标本中有17例检出螺杆菌属16S rRNA基因,阳性率为60.7%;25例胃癌标本有18例检出螺杆菌16S rRNA,阳性率为72.0%;其他肝病组未扩增出16S rRNA基因。16S rRNA PCR产物经Southern杂交证实为幽门螺杆菌。序列测定表明,肝癌和胃癌组织中的螺杆菌16S rRNA序列与幽门螺杆菌序列有97.8%的同源性。肝癌组相关功能基因有3例cagA基因阳性,胃癌组有2例cagA基因阳性,均未扩增出vacA基因。提示,幽门螺杆菌菌株的基因型多为Ⅱ型,而少数为Ⅰ型。结论HCC患者肝组织中存在螺杆菌感染且感染率较高。螺杆菌感染与HCC可能存在相关性。 | 宣世英 李宁 战淑慧 强新 | 2006 | 中华传染病杂志2006,24,6: | 1 |
| 8 | Significance of hepatitis B virus surface antigen, hepatitis C virus expression in hepatocellular carcinoma and pericarcinomatous tissues显示文摘AIM: To investigate the correlation between hepatitis B virus surface antigen (HBsAg), hepatitis C virus (HCV) expression in hepatocellular carcinoma (HCC), the HAI score of the noncancerous region of the liver and the serum Alpha fetoprotein (AFP) level.METHODS: The patterns of HBsAg and HCV in 100 cases of HCC and their surrounding liver tissues were studied on paraffin-embedded sections with immuno-histochemistry, the histological status was determined by one pathologist and one surgeon simultaneously using the hepatitis activity index (HAI) score, and AFP was detected by radioimmunity. The study included 100 consecutive patients who underwent curative resection for HCC. Based on HBsAg and HCV expression, the patients were classified into 4 groups: patients positive for HBsAg (HBsAg group), patients positive for HCV (HCV group), patients negative for both HCV and HBsAg (NBNC group) and patients positive for both HBsAg and HCV (BC group).RESULTS: The BC group had significantly higher HAI scores than the other three groups. (BC > HCV > HBsAg > NBNC). HBV and HCV virus infection was positively correlated with HAI (rs = 0.39, P = 0.0001). The positive rate of AFP (85.7%) and the value of AFP (541.2 ng/mL) in the group with HBV and HCV co-infection were the highest among the four groups. The positive rate (53.3%) of AFP and the value of AFP ( 53.3 ng/mL) in the group with none-infection of HBV and HCV were the lowest. HBV and HCV virus infection was positively correlated with AFP(rs = 0.38, P = 0.0001). CONCLUSION: The AFP increase in patients with liver cancer was positively correlated with the infection of HBV and HCV. The serum AFP elevation by the infection of HBV and HCV is one of mechanisms which lead to hepatocarcinogenesis, and the antivirus intervening treatment of hepatitis is significant for the prognosis of liver cancer. From our Spearman’s rank correlation analysis, we can conclude that the severity of virally induced inflammation is correlated with HBsAg and HCV expression in HCC tissues and noncancerous tissues.
